    GRAND FORKS, N.D.—Currently, about 50 percent of infants in the United States and in North Dakota have some alcohol exposure in early pregnancy. Most of these infants’ mothers then quit drinking. However, about 6 to 10 percent of pregnant women drink throughout their pregnancy. “Many people thinking about this issue would recognize the link between maternal drug use and developmental problems, which is a major public health issue in the United States and across the world,” said Larry Burd, PhD, a professor in the Department of Pediatrics at the University of North Dakota School of Medicine and Health Sciences, and director of the Fetal Alcohol Syndrome Center at the UND SMHS. The center provides prevention, diagnostic, and treatment services for people of all ages who have concerns related to prenatal alcohol exposure. “However, the much larger problem of prenatal alcohol exposure is a far more serious and much larger public health issue across the world and in North Dakota that is often overlooked,” Burd said. “This level of prenatal exposure is linked to increasing rates of complications during pregnancy and in the newborn.” The 2015 North Dakota Legislative Assembly created the North Dakota Task Force on Substance Exposed Newborns to study the growing problem and to produce a report for the 65th Legislative Assembly, convening in 2017. Burd was the author of the report submitted to the task force, A Report to the North Dakota Task Force on Substance Exposed Newborns. “In some cases these children will have a fetal alcohol spectrum disorder,” Burd said. “These disorders typically are lifelong disorders and extremely costly.     Seit der Einführung der §§ 284, 285 in das deutsche StGB ist die Frage, ob der Gesetzgeber die in den§§ 284, 285 StGB beschriebenen Verhaltensweisen strafrechtlich verfolgen darf, stark umstritten. Besonders stark hinterfragt wird, welches Rechtsgut durch §§ 284, 285 StGB geschützt werden soll. Die Frage erscheint insbesondere im Hinblick auf die verwaltungsakzessorische Ausgestaltung dieser Strafnormen und die Tatsache, dass der Staat stark von den Einnahmen aus dem Glücksspielmarkt profitiert, interessant. Die Arbeit stellt zunächst die zu dem von §§ 284, 285 StGB geschützten Rechtsgut üblicherweise vertretenen Ansichten dar und arbeitet sodann Kriterien heraus, die für die Beurteilung der Legitimität einer Strafnorm von Bedeutung sind, wobei unter anderem auf die jeweilige Deliktsstruktur Wert gelegt wird.     Ph.D.Introduction. Epidemiology is the science of studying health risks in populations. Many previous studies on burn injury have focussed on individual patients. The objective of this study is to determine the distribution of burn injury in eighteen defined geographical districts with the aim of identifying possible population-based determinants of risk.Methodology. Patient data comes from the first ten years in which electronic patient records were available, 2002 – 2011. A key linking field identifies the patient’s district of residence. These districts are described in Census reports. Using descriptive statistics, the eighteen districts and the risk of burn injury are characterised and compared. Using analytical statistics, the relationship between changes in burn incidence rates and changes in key characteristics of each district are examined.Results. Heterogenicity is demonstrated in district characterization and in burn incidence and occurs both within each year of study and throughout the study period. Analysis of potential determinants of burn injury is complex due to the multiple variables and the relatively few data points in the ten-year study period. The district with the highest incidence of burns, including the most vulnerable group, 0-2-year-olds, ranked lowest in monthly household income, % of people of working age in the labour force and average household size and highest in the % of elderly in the population. This association remained constant, however the risk (incidence of burn injury) is falling whilst the proportion of that district population with higher education is increasing. Conversely, the district with the lowest incidence of burn injury had the highest average monthly income and the highest proportion of the population with higher education throughout the study period.Conclusions. It is evident that risks do change and that improving levels of education and economic status within populations are associated with reduced risk of burn injury.     Reply to Rosenblum et al. and Carriquiry and Ommen: Understanding probative value, triers-of-fact, and usefulness of validity study results

    We thank Rosenblum et al. (1) and Carriquiry and Ommen (2) for their interest in our article (3) regarding the validity of cartridge-case comparisons. They make three assertions: that we misrepresent the technique's probative value, that setting trier-of-fact pretest odds = 1 is improper, and that our results may not speak to the validity of the forensic technique. We appreciate the opportunity to clarify these misunderstandings, which we address in turn. Rosenblum et al.'s belief that we misrepresent the technique’s probative value stems from their viewing trier-of-fact posttest beliefs as the measure of probative value (or, in their words, “strength of forensic evidence”). In fact, the likelihood ratio is the true measure of probative value. Guyll et al. correctly highlight the likelihood ratio as the empirical probative value measure that determines how much change is justified in trier-of-fact beliefs from any particular pretest value to a consequent posttest value. Likewise, the actual transcript (4) of the relevant testimony cited by Rosenblum et al. reveals an accurate portrayal of the technique’s probative value as quantified by the likelihood ratio by showing how much change it would justify in the trier-of-fact beliefs from an initial pretest value to a consequent posttest value.     Author attitudes to professional medical writing support

    To understand academic/clinician authors’ perceptions regarding the value of professional medical writers. An online survey of academic/clinician authors was conducted to understand the value of professional medical writer support in the development of publications (abstracts, posters and manuscripts). Responses were collected anonymously. The survey used a negative-to-positive, 6 point scale to evaluate respondents’ opinions and experiences of working with professional medical writers, and multiple choice to indicate in which areas professional medical writers added value. Responses from 76/260 authors were received (Europe, n = 57; 75.0%; North America, n = 16; 21.1%; Asia-Pacific region, n = 3; 3.9%). The majority of respondents were either clinicians (n = 45; 59.2%) or academic researchers (n = 25; 32.9%). A total of 82.9% (63/76) of respondents felt that it was acceptable to receive professional medical writer assistance with their publications, and 84.0% (63/75) valued the assistance provided. The services most valued (>50 responses) were editing and journal styling, conformity with reporting guidelines (e.g. CONSORT) and manuscript submissions. Fewer respondents (25–49 responses) valued management of timelines and co-author reviews, scientific/technical writing assistance and expert guidance on authorship requirements/good publication practice. The least valued service was the scientific expertise of the professional medical writer (3 responses). Respondents to this survey were generally accepting of medical writing assistance and valued many aspects of the role, in particular editorial support. The survey was small, however, and potentially biased towards authors with experience of working with medical communication agencies. Although many medical writers come from a scientific background and have relevant expertise, this was not perceived as a value. It would be beneficial to educate authors and journal editors regarding medical writers’ scientific expertise and role.</p

    Perivascular cell contribution to skeletal muscle repair following physiological injury

    PURPOSE: We have previously demonstrated that muscle-derived mesenchymal stem cells (MSCs; Sca-1+CD45-) accelerate Pax7+ satellite cell expansion and fiber repair during the initial stages of eccentric exercise training in mice. MSCs are a heterogeneous population of perivascular stromal cells, including but not limited to, pericytes, fibroadipogenic progenitor cells, and smooth muscle cells. Therefore, the purpose of this study was to examine the specific contribution of muscle-derived pericytes (CD146+CD45-CD31- or CD146+Lin-) to skeletal muscle repair following physiological injury. METHODS: Adult male wild type mice received bilateral intramuscular injection of pericytes 24 hr after a single bout of resistance exercise initiated by electrical stimulation of the sciatic nerve (n=5-6/group). Mice were sacrificed 4 days post-stimulation and the tibialis anterior muscles were analyzed for markers of damage (IgG+ myofibers, collagen), inflammation (CD11+ cells), and repair (fiber CSA, CLN+ fibers, myonuclei content). To determine the impact of the pericyte secretome on myoblast expansion and myotube development, CD146+Lin- pericytes were isolated from adult male wild type mouse skeletal muscle via enzymatic digestion and FACS. Following 12 days of recovery in culture, isolated pericytes were subjected to strain (10%, 1 Hz) for 1 hr using a Flexercell Strain unit. Conditioned media (CM) was collected from strained (CM-S) or unstrained (CM-NS) pericytes 24 hr later and applied to C2C12 myoblasts for up to 72 hr. RESULTS: Pericyte transplantation following electrical stimulation inhibited inflammation based on CD11b+ quantity and recovered the capillary-to-fiber ratio (P<0.05). The application of CM-S simultaneously reduced C2C12 quantity and increased myotube formation compared to CM-NS (P<0.05).
