    Mid-term safety profile evaluation of Bio-A absorbable synthetic mesh as cruroplasty reinforcement

    Purpose: The aim of the present paper is to report the results of a single institution series of hiatal hernia repair (HHR) with augmented mesh hiatoplasty focusing on safety and efficacy profile of Bio-A absorbable synthetic mesh. Materials and methods: A retrospective evaluation of prospectively maintained database showed 120 consecutive patients submitted to HHR reinforced with bio-absorbable synthetic mesh. The study populations included two groups: (A) 92 obese patients—reinforced hiatoplasty concurrent with bariatric procedure; (B) 28 non-obese patients—reinforced hiatoplasty concurrent with antireflux surgery. Symptoms assessment was made with GERD-HRQL and Rome III. The X-ray with barium swallow, the CT scan, in selected cases, and the endoscopy were used as recurrence evaluation and as endoscopic complications assessment. Only patients with a mean follow-up of 12 months were included in this study. A Cox hazard was made to evaluate factors affecting the recurrence. Results: No case of intra-peri and post-operative (mean follow-up of 41 months) complications mesh related were registered. The dysphagia-rate was 8.7% for Group A and 11% for Group B. 74% of Group A and 61% of Group B patients are actually PPIs free with median GERD-HRQL score of 4 (from 16) and 6 (from 23), respectively (difference pre-post-operative < 0.05). Recurrence rate was 5.4% in Group A and 7.1% in Group B. The Cox hazard analysis showed that the use of more than four stitches for cruroplasty represents a negative factor on recurrence (HR = 8; p < 0.05). Conclusions: This is, in our knowledge, the largest report (120 consecutive patients) with mid-term follow-up (41 months of mean FU) on bio-absorbable mesh on the hiatus in obese and non-obese patients. These results supports the use of absorbable mesh for HHR (safe profile—0% of complications rate), showing excellent recurrence rate results and good GERD symptoms control

    Setting the budget for targeted research projects

    We consider a funding competition for targeted projects. Potential participants have stochastic opportunity costs, and do not know the number of competitors. The funding agency sets a budget cap indicating the maximum funding that participants may request. We show that raising the budget cap helps to attract more participants but causes an increase in the requested funds. A higher budget cap is optimal when the preferences of researchers and the funding agency are more congruent, competition is lower, targeted projects have larger social value, the cost of public funds is smaller, or bidding preparation costs are lower

When audit intensity and outcome are unverifiable, we show that with interim-participation constraints the optimal mechanism may use only the auditor's report, disregarding the agent's information. Furthermore, the auditor obtains the asset and the agent a monetary compensation, when a high asset value is reported.     THE EFFECT OF THYROID STATE ON RESPIRATORY ACTIVITIES OF THREE RAT LIVER MITOCHONDRIAL FRACTIONS

    In this paper we report that three different rat liver mitochondrial fractions, differing in density, exhibit differential effects when the animals are made hypo- or hyperthyroid. The investigations have been performed by correlating the protein content, the succinic dehydrogenase behaviour and the respiratory features of the three fractions in different thyroid states with morphometric-stereologic analysis the electron micrographie level. The results indicate that the thyroid hormone influences both the mass and the functionality of the heavy (H) and light (L) fraction. In hypothyroid rats the H fraction increases (+43%) while the L fraction decreases (- 32%) and their respiratory activity is drastically reduced. Adenosine triphosphate (ATP) synthesis in the H fraction is also inhibited. Triiodothyronine (T3) administration to the above animals restores the values observed in control rats. At morphometric level we note in hypothyroid rats an increase in the number of mitochondria together with a concomitant increase in the average volume of a single mitochondrion. We are inclined to explain the above results through an action exerted by T3 on a hypothetical mitochondrial cycle starting with the formation of light organelles from heavy ones. © 1989

The effect of cold exposure and of PTU and PTU + T3 administration on the protein content and succinic dehydrogenase activity of three mitochondrial populations obtained from rat liver was examined. Our results indicated the following: 2. 2. Succinic dehydrogenase activity increases mainly in the light mitochondrial fraction of cold-exposed rats. 3. 3. PTU administration of cold-exposed animals does not affect the increment in enzyme activity of the heavy fraction but blocks the increment of the light fraction. 4. 4. PTU + T3 administration restores succinic dehydrogenase activity to the values prevalent in normal cold-exposed rats. 5. 5. These findings suggest that thyroid hormone may stimulate the formation of light mitochondria during cold exposure. © 1986

    LIGHT MITOCHONDRIA AND CELLULAR THERMOGENESIS

    We show that remarkable differences exist among 3 mitochondrial subpopulations with regard to oxidation-phosphorilation coupling. In the heavy fraction (sedimenting at 3,000 g) ATP synthesis is optimized while in the medium and light fractions (sedimenting at 10,000 and 27,000 g, respectively) heat production tends to be optimized as the mitochondrial size decreases. As the oxygen consumption of the L-fraction drastically increases (+150%) after fifteen days of cold exposure, such differences have remarkable implications for the mechanism of cellular thermogenesis and its regulation. © 1988 Academic Press, Inc
