    The influence of vitamin D on the clinical course of the disease in patients with pulmonary tuberculosis

    Introduction. Vitamin D insufficiency or deficiency is defined as a very common condition in the general population. Vitamin D deficiency has a positive effect on a wide range of microbial infections, such as gram-positive and gram-negative bacteria, fungi, mycobacteria, and viruses. The aim of the study. To determine the level of vitamin D in patients with susceptible pulmonary tuberculosis. Materials and methods. The randomized case-control study included 45 patients with newly diagnosed pulmonary tuberculosis (PDTB] of the lungs with preserved sensitivity and 30 practically healthy individuals (PHI) of the control group. The level of vitamin D in blood serum was examined using the VD (Vitamin D) ELISA Kit (Optics Valley Biomedical Industrial Park, Wuhan, China (430075)). Results. It was established that the average level of 2 5 (OH) D concentration in the blood serum of patients with pulmonary tuberculosis was 18.46±4.23 ng/ml, in the control group it was 28.834±5.79 ng/ml and was statistically significant (p = 0.001). Our multivariate studies showed a strong direct relationship between the presence of a specific tubercular process and vitamin D deficiency (<20 ng/ml) (OR 2.817, 95%, Cl 1.287 - 4.894, P < 0.005). Conclusions. Vitamin D deficiency was noted in 94% of patients with pulmonary tuberculosis with severe intoxication syndrome (7-10 points). In tuberculosis with moderate intoxication syndrome (4-6 points), 69.7% of patients had a vitamin D deficiency, which practically did not differ from indicators in patients with mild intoxication syndrome (1-3 points) - 67.2% of people

    Utilizarea izoniazidei sirop pentru prevenirea tuberculozei la copiiThe purpose of the study was to conduct a comparative analysis of the use of isoniazid in the form of syrup and tablet for the prevention of tuberculosis in children. Materials and methods. 114 children at risk who were on sanatorium treatment at Chernivtsi children&rsquo;s sanatorium (Ukraine) were examined for tuberculosis. For chemoprophylaxis in children the drug &ldquo;Isoniazid&rdquo; was used: 61 children received the drug in the form of syrup (group 1), 53 - in tablet form (group 2). On the basis of Institute of Pneumology &ldquo;Marius Nasta&rdquo; (Romania) 83 children enrolled to the 1 group received preventive therapy with isoniazid in the form of syrup. Results. Significantly lower levels of total protein were reported in 1 group - by 9.6% than in patients in 2 group (р&lt;0.05). Also significantly higher levels of bilirubin - by 17.2 %, ALT (alanine transaminase) - by 19 %, AST (aspartate transaminase) - by 22.4 % and thymol test - by 15.5 % were reported in 2 group compared with 1 group (p &lt;0.05), evidencing that there are preconditions for the development of toxic hepatitis when using isoniazid in tablet form. It is well established that the changes in blood chemistry were much more pronounced in children who received isoniazid tablets and were increasing up to the completion of the course of chemoprophylaxis. Conclusions. The results of complex clinical, laboratory and instrumental study allow us to recommend &ldquo;Isoniazid&rdquo; in the form of syrup 100 mg / 5 ml as highly effective drug for the prevention of tuberculosis in children.Использование сиропа изониазида для профилактики туберкулеза у дете

    The aim: To investigate the in!uence of prescribing a complex of amino acids in pathogenetic therapy in patients with pulmonary tuberculosis on liver function. Materials and methods: The study included 50 patients with drug susceptible TB and 50 patients with drug-resistant TB (multidrug-resistant and extensively drug-resistant). Results: The study included 50 patients with drug susceptible tuberculosis (TB) and 50 patients with drug-resistant TB. When comparing biochemical pa-rameters characterizing liver function in patients with drug-susceptible TB after 1 month of anti-tuberculosis therapy, it was found that patients receiving additional therapy with a complex of amino acids had a lower level of bilirubin, p<0.05. After 60 doses, patients receiving additional therapy with amino acids had signi"cantly lower bilirubin levels alanine aminotransferase (ALT) and aspartate aminotransferase (AST), p <0.05. When comparing the biochemical parameters characterizing liver function in patients with drug-resistant tuberculosis after a month of anti-tuberculosis therapy, signi"cantly higher protein level was found in the groups of patients receiving additional therapy with amino acids, as well as signi"cantly lower ALT level, AST and creatinine p<0.05. Conclusions: The additional appointment of the complex of amino acids in the pathogenetic therapy of patients with pulmonary tuberculosis makes it possible to reduce the severity of hepatotoxic reactions manifested by the main parameters (AST, ALT, total bilirubin) and to increase the protein-synthetic function of the liver, which allows us to recommend their appointment to improve the tolerance of anti-tuberculosis therapy

    Patients with newly diagnosed pulmonary tuberculosis (TB) had lower initial plasma levels of vi- tamins B1 and B12, compared to age- and sex-matched healthy persons. We did not find significant chang- es in vitamin B status and electroneuromyography during the treatment with combinations of isoniazid and first-line anti-TB drugs. During the treatment with combinations of linezolid and second-line anti-TB drugs, we identified significant decreases of plasma vi- tamin B1 and B12 levels and worsening of electroneu- romyography indices by sensory and motor fibers. The deficiency of vitamins B1 and B12 during treatment with linezolid can be considered as one of the pathogenetic mechanisms of progressive damage of peripheral nervous system in pulmonary tu- berculosis patients, which must be taken into account during the course of antitubercular therapy
