    Learning from Medication Errors in Healthcare : How to Make Medication Error Reporting Systems Work?

    Medication errors are one of the most common incidents leading to adverse events in healthcare worldwide. Tackling these major problems requires the implementation of a systems approach to healthcare, stating that risks should be managed proactively by improving the healthcare system. One of the recommended key strategies for learning from medication errors and risk prone processes is the establishment of local and national medication error reporting (MER) systems in healthcare. This study explored national and local MER systems in different countries and what makes them work in learning from medication errors. The study also explored how continuing education in medication safety could be organised for practicing healthcare professionals. The study applied both qualitative and quantitative research methods and utilized various data sources. The study was based on the theory of Human Error and the systems approach to risk management. The study comprised of three phases. Phase I explored the existing MER systems in different countries and their development and implementation. 16 medication safety experts from different countries responded to an online-survey. A national or local MER system existed in 11 of the countries. Blaming for errors, and a lack of time, training and coordination of reporting continue to be the major barriers to reporting. Learning from errors and a non-punitive approach are essential features of a MER system. There is also a need for promoting international networking of medication safety experts and bodies for sharing information and learning from others. Several factors associated with the successful development and implementation of MER systems were also identified. Phase II assessed the inter-rater reliability of medication error classifications in a voluntary Reporting System for Safety Incidents in Health Care Organizations (HaiPro) widely used in Finland. Also medication errors (n=32 592) reported in 2007-2009 and their contributing factors were explored. The inter-rater reliability was found acceptable (κ ≥0.41) in 11 out of 42 (26%) variables (e.g., near miss or actual error) describing the reported medication errors. Thus, the medication errors reaching the acceptable level of inter-rater reliability could be pooled from different healthcare units for the exploration of medication errors at the level of all reporting organisations. The most frequently reported medication errors were: dispensing errors (33%, n=10 906); administration errors (24%, n=7 972); and documentation errors (17%, n=5 641). The most commonly reported contributing factor was deficiencies in communication and course of information related to patients medications. In Phase III educational approaches were developed for introducing medication safety for healthcare professionals as a three-day interdisciplinary course. International higher education experts in pharmacy (n=19) brainstormed four syllabi with teaching and assessment methods. Following this, a combined syllabus was developed. All four syllabi were based on constructive, problem-based learning methods and focused on understanding a systems approach in managing medication safety. Learning linked to learners practice through assignments at the workplace appeared to be the key. The study suggests that MER systems need to be improved in many countries together with their operational environments. Moreover, the operational environments of MER systems must support the functionality of these systems. The key factor for successful MER systems and learning from medication errors is having a systems approach as a theoretical context in all reporting and learning processes throughout the operational environments of MER systems. The current work also suggests that constructive problem-based learning linked to learners practice through assignments is the key when developing a course for continuing education in medication safety for healthcare professionals.Lääkehoitoihin liittyvät poikkeamat ovat yleisiä sosiaali-ja terveydenhuollossa niin Suomessa kuin muualla maailmassakin. Poikkeama voi tapahtua missä vaiheessa lääkehoitoa tahansa. Lääkäri saattaa epähuomiossa määrätä potilaalle väärän lääkkeen, hoitaja annostella lääkkeen väärällä annoksella tai farmasian ammattilainen toimittaa väärän lääkevalmisteen apteekista. Lääkehoidon turvallisuuden edistämiseksi keskeistä ei kuitenkaan ole syyllisen selvittäminen vaan se, miksi virhe tapahtui. Tavoitteena on oppia tapahtuneista poikkeamista, niihin johtaneista tekijöistä ja terveydenhuollon organisaatioiden toimimattomista prosesseista jotka edesauttavat virheiden syntymistä. Yhtenä keskeisenä lääkehoidon riskien hallinnan työvälineenä ovat lääkehoidon vaaratapahtumien raportointijärjestelmät, joiden kautta terveydenhuollon organisaatiot saavat tietoa lääkehoitoon liittyvistä poikkeamista ja riskeistä. Tässä väitöstutkimuksessa tutkittiin lääkehoidon vaaratapahtumien raportointijärjestelmiä eri maissa. Tutkimuksessa pyrittiin myös selvittämään, kuinka nämä järjestelmät saataisiin toimimaan tavalla, joka tuottaisi mahdollisimman laadukasta tietoa lääkehoidon riskien hallinnan tueksi sosiaali-ja terveydenhuollon arjessa. Lisäksi tutkittiin kuinka sosiaali-ja terveydenhuollon ammattilaisille voitaisiin järjestää täydennyskoulutusta lääkitysturvallisuuden ja riskienhallinnan osaamisen varmistamiseksi. Tutkimuksen ensimmäisessä osassa tutkittiin lääkehoidon vaaratapahtumien raportointijärjestelmiä 16 eri maassa. Kansallinen tai paikallinen (esim. yhdessä sairaalassa käytössä oleva) raportointijärjestelmä oli käytössä useimmissa tutkimukseen osallistuneista maista (n=11), joista valtaosa oli kehittyneitä maita. Raportointijärjestelmien käytön suurimpia esteitä olivat virheistä syyllistävä kulttuuri, ajanpuute, sekä raportoinnin koulutuksen ja koordinaation puute. Tutkimuksen toisessa osassa tutkittiin Suomessa laajassa käytössä olevan Sosiaali-ja terveydenhuollon vaaratapahtumien raportointijärjestelmään (HaiPro) vuosina 2007-2009 raportoituja lääkehoidon vaaratapahtumia (n=32 592), sekä aineiston laatua. Tutkimuksessa havaittiin, että vaaratapahtumailmoitusten sisällön laatua ja aineiston analysointia tulisi kehittää terveydenhuollon organisaatioissa erityisesti vaaratapahtumien myötävaikuttavien tekijöiden osalta. Yleisimmin raportoidut vaaratapahtumat liittyivät lääkkeiden jakoon (33%, n=10 906), annosteluun (24%, n=7 972); sekä kirjaamiseen (17%, n=5 641). Yleisin raportoitu poikkeamiin myötävaikuttava tekijä oli puutteet potilaan lääkehoitoon liittyvän tiedon kulussa ja kommunikaatiossa. Lääkitysturvallisuuden kannalta on keskeistä, että sosiaali-ja terveydenhuollon ammattilaiset hallitseva lääkehoidon riskienhallinnan perusteet. Väitöskirjan viimeinen osa perustui tutkimukseen, jossa kehitettiin tieteellisin menetelmin moniammatillinen lyhytkurssi tämän osaamisen varmistamiseksi. Koulutus perustuu konstruktiiviseen oppimiseen, jossa keskeisenä on lääkitysturvallisuuden järjestelmälähtöinen kehittäminen. Koulutuksessa keskeisimpänä oppimisen menetelmänä on ongelmalähtöinen oppiminen, ohjattu teoriatiedon soveltaminen käytäntöön ja työssä oppiminen koulutuksen osana. Väitöstutkimuksen perusteella lääkehoidon vaaratapahtumien raportointijärjestelmiä ja niiden toimintaympäristöjä tulee kehittää useissa maissa. Raportointijärjestelmien toimivuuden kannalta keskeisintä on, että raportointi, aineiston analysointi ja tiedon hyödyntäminen terveydenhuollon prosessien parantamiseksi perustuu järjestelmälähtöisyyteen ja poikkeamista oppimiseen. Konstruktiivinen ongelmalähtöinen oppiminen yhdistettynä osallistujien käytännön työhön ovat keskeisimmät menetelmälliset tekijät kehitettäessä koulutusta sosiaali-ja terveydenhuollon ammattilaisille lääkitysturvallisuudesta ja lääkehoidon riskienhallinnasta.ei saavutettav

    Geriatrisen lääkehoitoprosessin kehittäminen Päijät-Hämeen hyvinvointikuntayhtymässä

    Tämä tutkimus on tehty paikallisessa kontekstissa kehittäen vuodesta 2010 alkaen lääkehuoltoa ja lääkehuoltopalveluja Lahden kaupungin sairaala-apteekissa, joka yhdistyi Päijät-Hämeen keskussairaalan sairaala-apteekin kanssa vuonna 2017 muodostaen Päijät-Hämeen hyvinvointiyhtymän sairaala-apteekin. Tutkimuksen tavoitteena oli lääkehoitoprosessin ja siihen liittyvien toimintakäytäntöjen kehittäminen turvallisen ja järkevän lääkehoidon toteuttamiseksi iäkkäiden lääkehoidossa perusterveydenhuollon vuodeosastoilla ja kotihoidossa (geriatriset yksiköt). Tässä sairaala- ja terveyskeskusfarmasian erikoistumiskoulutukseen liittyvässä lisensiaattityössä kuvataan pitkäjänteistä paikallista kehittämiskokonaisuutta, joka liittyy sosiaali- ja terveyspalveluiden uudistamiseen sekä kansallisten vanhus- ja lääkepoliittisten linjausten paikalliseen toteuttamiseen. Tarkemmin on kuvattu kaksi kehittämishanketta, jotka liittyivät iäkkäiden potilaiden lääkitysten tuloarviointiin perusterveydenhuollon vuodeosastolle tullessa (tutkimus I) ja terveydentila- ja toimintakykymittareilla saadun potilastiedon hyödyntämiseen lääkehoitojen toteutuksessa (tutkimus II). Tutkimuksen I tavoitteena oli kehittää toimintamalli ja työkalu lääkityksen tuloarviointiin iäkkään potilaan tullessa perusterveydenhuollon vuodeosastolle. Tutkimusmenetelmänä käytettiin toimintatutkimusta, jossa hyödynnettiin kirjallisuushakua kansainvälisistä lääkehoitojen ajantasaistamissuosituksista sekä kehittämistyöhön osallistuneen henkilökunnan haastatteluja ja palautekeskusteluja kehitystyön eri vaiheissa. Tutkimukseen osallistuneen kahden geriatrisen osaston henkilökunta (2 lääkäriä, 3 hoitajaa ja 2 osastofarmaseuttia) koki yksimielisesti ongelmaksi ajantasaisen kotilääkityslistan puutteen. He esittivät ratkaisuksi osastofarmaseutin työtehtävien entistä merkittävämpää keskittämistä tulovaiheen lääkityslistan ajantasaistamiseen ja arviointiin. Tätä varten he kehittivät lääkityksen ajantasaistamis- ja arviointimallin, jossa osastofarmaseutti kerää potilaan taustatiedot, haastattelee potilasta, tekee lääkityksen ajantasaistamisen ja arvioinnin sekä tuottaa suositukset lääkärin päätöksentekoa varten. Työkalu kehitettiin alun perin paperiseksi dokumentointilomakkeeksi, joka sisälsi lääkityksen ajantasaistamis- ja arviointiosuuden. Myös toimintamallin kuvaus ja ohjeistus tehtiin paperisina. Myöhemmin toimintamallin kehitystä on jatkettu ja osin digitalisoitu dokumentaatiota. Tutkimuksen II tavoitteena oli tutkia, millaisia terveydentila- ja toimintakykymittareita iäkkäiden kotihoidon asiakkaiden arvioinnissa käytetään, miten mittareista saatua tietoa hyödynnetään lääkehoidon toteutuksessa ja miten mittareista saadun tiedon hyödynnettävyyttä voitaisiin tehostaa lääkehoidon seurannassa. Tutkimus toteutettiin sähköisellä kyselyllä kahden paikkakunnan kotihoidon lääkäreille (paikkakunta A: vastausprosentti 60 %, n=3, paikkakunta B: n=3, vastausprosenttia ei voitu laskea) ja hoitajille (paikkakunta A: vastausprosentti 21 %, n=50, paikkakunta B: vastausprosentti 53 %, n=65) sekä puhelinhaastatteluilla kotihoidon johdolle (paikkakunta A: n=3 ja paikkakunta B: n=2) vuonna 2017. Lääkärit hyödynsivät vaihtelevasti terveydentilamittareita arvioidessaan kotihoidon asiakkaiden lääkehoidon vaikutuksia. Eniten he hyödynsivät GFR-arvoa, verenpaineen ja verensokerin mittausten sekä INR-mittausten tuloksia. Hoitohenkilökunnan yleisimmin tuntemia ja käyttämiä mittareita olivat kognitiivisen toimintakyvyn arvioimiseen käytettävä MMSE, ravitsemustilan arvioimiseen käytetty MNA, ortostaattinen koe ja painoindeksi BMI. Vastanneet hoitajat hyödynsivät yleisesti RAI- ja RAVA-arviointien tuloksia. . Lääkärit hyödynsivät niitä vähän osittain siksi, että he luottivat hoitajien antamaan tietoon potilaasta. Hoitohenkilökunta koki RAI-arvioinnit paljon aikaa vievinä, mutta heidän mielestään oli erittäin tärkeää, että hoitajalla ja lääkärillä on yhteinen näkemys asiakkaan tilanteesta. Kotihoidon johdon kokemuksen mukaan RAI-arviointien avulla pystytään seuraaman asiakkaan vointia ja voinnin kehitystä. Päijät-Hämeessä tehty pitkäjänteinen kehittämistyö osoittaa, että sairaala-apteekkeja ja farmasian ammattilaisia tarvitaan tulevaisuudessa yhä enemmän osallistumaan lääkehoitoprosessin toteutukseen, kehittämiseen ja koordinointiin yhteistyössä eri hoitoyksiköiden kanssa. Erilaisten terveydentilaa ja toimintakykyä arvioivien mittausten hyödyntämistä voisi tehostaa lääkehoitojen yksilöllistämisessä ja vaikutusten seurannassa muuallakin kuin kotihoidossa. Lisäksi mittauksia voisi hyödyntää osana farmasian ammattilaisten tekemiä lääkehoidon arviointeja. Lääkehoidon arviointeja ja seurantaa tulisi kehittää perusterveydenhuollossa ja toteuttaa moniammatillisessa yhteistyössä. Potilaiden lääkehoidon laatu ja turvallisuus paranisivat farmasian ammattilaisten osallistuessa hoitotiimien työskentelyyn. Lisäkehittämistä tarvitaan potilastietojärjestelmissä oleviin sähköisiin lomakkeisiin, jotta ajantasaiset lääkitystiedot voidaan dokumentoida ja kaikki hoitoon osallistuvat ammattilaiset pystyvät niitä hyödyntämään eri hoitoyksiköissä.This study was conducted in a local context within Lahti City Hospital, Päijät-Häme Central Hospital and Päijät-Häme Joint Authority for Health and Wellbeing. In 2010, the development of clinical pharmacy services was started in the Lahti City Hospital Pharmacy, which merged with Päijät-Häme Central Hospital Pharmacy in 2017, forming Päijät-Häme Joint Authority for Health and Wellbeing Hospital Pharmacy, which continues the development work. This licentiate thesis in hospital pharmacy speicialization describes long-term local service development responding to national health and social services reform, and implementing national policies concerning medicines management services for older people. The aim of this study was to develop the medication management process and related practices to foster rational and safe pharmacotherapy on geriatric wards in the hospital and in home care. Two service development projects in medication reconciliation and review of geriatric patients’ medications on hospital admission (Study I) and utilization of health and functional capacity measures in the monitoring of rational pharmacotherapy in home care (Study II) have been described in detail. The objective of Study I was to develop a procedure and a tool for medication reconciliation and review of geriatric patients’ medications on hospital admission. Action research, with a literature review, and the ward care team as a development team, was employed in the collaborative development process that first identified problems in the then current admission process and, secondly, developed a procedure and a tool. The participants of the study, two physicians, three nurses and two clinical pharmacists, on two geriatric wards in primary care in Lahti were concordant regarding the problems experienced, such as a lack of an up-to-date home medication list in the then current medication process in 2012. To solve these problems, the care team recommended that clinical pharmacists should reconcile and review patients’ medications. The developed and piloted medication reconciliation and review procedure and tool guided the pharmacist to interview the patient, take the medication history, reconcile and review the medication, and make recommendations for possible medication changes. In 2013, the paper-based procedure, the tool and instructions for their use were partly converted to electronic format. Later, a new page was created for the pharmacists’ notes in the electronic medical record. The objective of Study II was to explore, which health and functional capacity measures were used, how the patient-specific information obtained was utilized in the monitoring of pharmacotherapy in older home care clients, and how the use of this information could be enhanced. The study was conducted in home care in two medium-sized cities in southern Finland in 2017. The perceptions of home care physicians (city A: n=3, response rate 60%; city B: n=3, response rate could not be calculated) and nurses (city A: n=50, response rate 21%; city B: n=65, response rate 53%) were investigated by an electronic survey. The perceptions of home care management personnel (city A: n=3 and city B: n=2) by telephone interviews were studied. Of the responding nurses, 70% were practical nurses. The physicians reported that they utilized health status measures at varying degrees to assess the effects of pharmacotherapy in older home care clients. According to the physicians, they regularly used glomerular filtration rate (GFR) measurements, blood pressure, blood glucose and INR measurements. The responding nurses recognized and utilized the Mini-Mental State Exam (MMSE), Mini Nutritional Assessment (MNA), and Body Mass Index (BMI). Of the responding nurses, many performed RAI (Resident Assessment Instrument) or RAVA (Rajala-Vaissi) functional capacity assessments, however, the physicians reported that they used these results seldom. The nurses experienced RAI assessments as quite time consuming but they considered them important in sharing information with the physicians and agreeing on a client’s health condition. According to the interviewed home care managers, RAI assessments were utilized in setting goals for care and rehabilitation plans of home care clients and for monitoring their functional capacity. The home care managers thought RAI assessments were a good way to follow up a client’s condition. In home care, a wide range of measurements are made to assess health status and functional capacity, the use of which could be enhanced in the planning and monitoring of pharmacotherapy. This use of the patient data could be extended to pharmacist-conducted collaborative medication reviews. The long-term local development of hospital pharmacy services in Päijät-Häme demonstrates that hospital pharmacies and pharmacists are increasingly needed in the future to participate collaboratively in managing, coordinating, and developing the medication management process within evolving social and health services

    Developing a medication-safety self-assessment tool for rural primary care units - a case from Finnish Lapland

    Abstract Background In rural areas, primary care faces several challenges, and medication therapy is one of the most complex processes in primary care. With a specific, proactive, medication-safety self-assessment tool designed for rural primary care units, healthcare professionals could identify development needs in their medication processes. Methods The Delphi consensus method with two Delphi rounds was used to create a medication-safety self-assessment tool for rural primary care units in Finnish Lapland. A preliminary tool was designed based on three national and international risk management tools. Statements of the preliminary tool were evaluated with a two-round Delphi panel by 12 experts in primary care and patient safety. Evaluated aspects were suitability for primary care settings, medication safety relevance, and the necessity of the statements to be included in the developed rural, primary care, medication-safety self-assessment tool. Results In the first Delphi round, a consensus of ≥ 85% on being “sufficiently important and essential” was reached on 39% of the statements (n = 118/304), of which 86% (n = 101/118) were included, and 14% (n = 17/118) were excluded from the final primary care medication- safety self-assessment tool. In the second round, 84% of the statements (n = 141/167) reached a consensus, of which 70% (n = 98/141) were excluded and 30% (n = 43/141) included in the final tool. The included 144 statements were divided into 12 thematic sub-groups: (1) Patient information, (2) Drug information, (3) Communication of drug orders and other drug information; (4) Drug labeling, packaging and nomenclature; (5) Drug storage and distribution, (6) Medication device acquisition and use, (7) Environmental factors, workflow and staffing patterns; (8) Staff competency and education, (9) Patient education, (10) Preventive risk management, 11. Learning from medication safety incidents, and 12. Electronic health record. Conclusions The developed medication-safety self-assessment tool is targeted for proactive medication risk management in rural primary care settings. While experts reached a consensus for the Primary care Medication Safety Self Assessment tool contents, adopting the tool to suit the rural primary care environments in different countries should be further investigated

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Evolution of Intravenous Medication Errors and Preventive Systemic Defenses in Hospital Settings - A Narrative Review of Recent Evidence

    Objectives Intravenous drug administration has been associated with severe medication errors in hospitals. The present narrative review is based on a systematic literature search, and aimed to describe the recent evolution in research on systemic causes and defenses in intravenous medication errors in hospitals. Methods This narrative review was based on Reason's theory of systems-based risk management. A systematic literature search covering the period from June 2016 to October 2021 was conducted on Medline (Ovid). We used the search strategy and selection criteria developed for our previous systematic reviews. The included articles were analyzed and compared to our previous reviews. Results The updated search found 435 articles. Of the 63 included articles, 16 focused on systemic causes of intravenous medication errors, and 47 on systemic defenses. A high proportion (n = 24, 38%) of the studies were conducted in the United States or Canada. Most of the studies focused on drug administration (n = 21/63, 33%) and preparation (n = 19/63, 30%). Compared to our previous review of error causes, more studies (n = 5/16, 31%) utilized research designs with a prospective risk management approach. Within articles related to systemic defenses, smart infusion pumps remained most widely studied (n = 10/47, 21%), while those related to preparation technologies (n = 7/47, 15%) had increased. Conclusions This narrative review demonstrates a growing interest in systems-based risk management for intravenous drug therapy and in introducing new technology, particularly smart infusion pumps and preparation systems, as systemic defenses. When introducing new technologies, prospective assessment and continuous monitoring of emerging safety risks should be conducted.Peer reviewe

    How to make medication error reporting systems work : Factors associated with their successful development and implementation

    This study explored factors associated with successful development and implementation of medication error reporting (MER) systems in different healthcare contexts. A descriptive online questionnaire comprising of structured and open-ended questions was responded to by 16 medication safety experts in 16 countries. The present paper describes the rich and multidimensional qualitative data from the experts’ narratives from open-ended questions. Several factors related to the national context of MER systems, i.e., the operational environment, were identified to impact successful development and implementation of these systems. The factors were: awareness of deficiencies in medication safety at local and national levels to justify the need for MER systems; gaining political will for the development and implementation actions together with international and governmental support; creating or reforming legislation and national regulations, guidelines and strategies to support MER; allocation of adequate human and financial resources; establishment of an organisation or centre to coordinate and lead MER; and extending systems approach and safety culture to all parts of the operational environment to facilitate openness on and learning from medication errors. In conclusion, operational environments of MER systems must be constructed to support functionality of these systems, and need to be improved in many countries.Peer reviewe

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
