    IN HOSPITAL OUTCOMES AND ACS

    Objective: Frequency of in hospital outcomes in admitted with ACS. Methodology; This was a cross sectional study that was carried out at PIMS hospital, Islamabad during September 2015 to September 2016. In this study the cases of both genders with age more than 30 years were selected via Non probability consecutive sampling. These cases were admitted with standard AHA criteria of acute coronary syndrome bases on chest pain and ECG changes along with cardiac enzymes and were assessed for various clinical outcomes till their discharge or death. The cases with co morbid renal or hepatic failure or those that had pre hospital CPR, were excluded form this study. Results; In the present study there were total 100 cases, out of which 57 (57%) were males and 43 (43%) females. The mean age of the subjects was 54.11±10.35 years and mean duration of symptoms of ACS was 14.67±3.44 hours. The most common complication was arrhythmia noted in 41 (41%) of the cases, followed by hear failure in 15% cardiogenic shock in 13% and mortality in 4% of the cases. Conclusion; ACS can result in multiple complications during hospital stay and the most common one is arrhythmias. Key words; Acute coronary syndrome, Mortality, Shock, Arrhythmia

    Chikungunya encephalitis: Usual presentation at an unusual time. a case report

    CHIK is an arboviral infection caused by chikungunya virus transmitted by Aedes mosquito worldwide. It usually manifests as a self-limiting acute febrile illness with rash, body ache and severe polyarthralgia; in some patients, symptoms persist up to months or even years. Neurological manifestations are relatively uncommon. In recent years Pakistan and other South East Asian countries have been hit several times by its epidemics1. We report a case of a middle-aged male who presented in our hospital with acute febrile illness along with neurological manifestations. Extensive laboratory investigations were done and he was diagnosed with seropositive CHIKV. He was treated symptomatically and recovered without any complication. Although previously cases of chikungunya meningoencephalitis have been reported in Pakistan during epidemics; such cases sporadically have not been reported so far1

    Cerebral venous sinus thrombosis with positive Dengue serology

    Dengue fever is a viral febrile illness caused by dengue virus and spread by Aedes mosquito worldwide. It usually manifests as an acute fever associated with body ache, skin rash, and thrombocytopenia. Neurological manifestations are relatively uncommon but well-known. We report a case of a 25-year-old male patient who presented to our hospital with acute febrile illness, headache, and vomiting. Extensive laboratory investigations were done and he was diagnosed with cerebral venous sinus thrombosis. Dengue serology was positive. He was treated with anticoagulation and hydration with complete recovery. The treatment with anticoagulation is challenging due to accompanying thrombocytopenia. Only two previous case reports of cerebral venous sinus thrombosis with positive dengue serology have been reported in the literature

    Assessment of Average Length of Stay among Stroke Patients in a Tertiary Care Hospital

    Background: Stroke is defined as a clinical syndrome where sudden onset neurological deficit appears and remains for 24 hours or more due to vascular compromise. Stroke is the leading cause of disability worldwide and the second leading cause of death. Certain risk factors have been discussed in the literature previously which are strongly associated with stroke. However, the literature is scarce in terms of risk factors affecting the hospital stay of patients suffering from stroke. Objective: The objective of this study is to evaluate the risk factors that may affect the hospital stay in stroke patients. Materials and Methods: This study was conducted prospectively at Aga Khan University Hospital, Karachi where length of hospital stay among stroke patients were included. The length of hospital stay (LOS) was found and compared by dividing the included participants into subgroups including age (30-50 and 51-80), gender (male and female), diabetes mellitus type II (yes or no), hypertension (yes or no), duration of disease (</=2 or >2 days), smoking (yes or no), BMI, and type of stroke (ischemic or hemorrhagic). Results: We included 94 stroke patients. The LOS in hospital was significantly associated with age (P=0.01), gender (0.01), diabetes mellitus type II (P=0.04), smoking (P=0.01), BMI (P=0.01) while it was not associated significantly with hypertension (P=0.09), duration of disease (P=0.05) and type of stroke (P=0.08). Conclusion: LOS after stroke differed according to the sex, age, diabetes mellitus type II, and BMI. Hence, BMI and smoking are modifiable risk factors that could decrease the LOS while more efforts can be made to control blood sugars at optimum levels for further decreasing the LOS

Nadeem wanted to establish a society based on justice. Nadeem wanted to eliminate discrimination among classes that is rampant in our society. Nadeem always dreamt of a society where human being is loved, equality, tolerance. This article is detailed study of humanism, tolerance, harmony that can be found in Nadeem's poetry. It also reveals Nadeem's effort for spreading tolerance, harmony, love in society.

    Religious Understandings of Hospice and Palliative Care

    How do the various religious traditions understand what it means to have a good death? What might we learn from our religious neighbors about what constitutes appropriate hospice and palliative care? These questions and others will be discussed from Buddhist, Christian, Jewish, and Muslim perspectives. This program will be facilitated by Rabbi David Wirtschafter, the Jay Phillips Center\u27s visiting scholar in Jewish Studies, and grew out of an interfaith reading group sponsored by the Jay Phillips Center and facilitated by Barbara Marincel, an intern-scholar with the Jay Phillips Center and graduate student in theology at St. Catherine University. Panelists Owais Bayunus is the past president of the Islamic Center of Minnesota and currently serves as the director of its interfaith activities. He serves on the boards of the Joint Religious Legislative Coalition and the Muslim-Christian Dialogue Center at the University of St. Thomas, and he is the author of When Death Approaches, which instructs caregivers about end-of-life care for Muslim patients. Rabbi Amy Eilberg, a special consultant to the Jay Phillips Center, co-founded the Bay Area Jewish Healing Center, where she directed the Jewish Hospice Care Program, and she also co-founded and co-directed the Yedidya Center for Jewish Spiritual Direction. She is nationally known as a leader of the Jewish healing movement and in the field of Jewish spiritual direction. Joen Snyder O\u27Neal is an ordained Zen priest who received dharma transmission from Katagiri Roshi in 1989. She teaches at Compassionate Ocean Dharma Center and has written on a number of topics from a Zen perspective, including end-of-life care. Joan Olson is a board-certified chaplain at Abbott Northwestern Hospital in Minneapolis working with the Palliative Care team, a certified spiritual director through Sacred Ground Center for Spirituality, and has served as a supervisor for Sacred Ground’s spiritual director training program.     Influence of online versus traditional learning on EFL listening skills: A blended mode classroom perspective

    Online learning is becoming more and more popular than traditional learning, and the need to investigate its influence within the framework of instruction and learning domains is — in today's emerging, cutting-edge technology world — an academic trend. The current study's problem intends to explore the impact of virtual versus traditional learning on the academic achievement of EFL students, a subject that has garnered substantial attention from English Language Teaching (ELT) researchers. This study aims to clarify if EFL students' academic progress in a listening skills classroom is influenced by traditional or online learning. The present research compiled information on how EFL students performed when English language teaching took place online as opposed to in a traditional classroom environment. This study, using an experimental research design with (N = 30) pairs of students (both male and female), was held at Najran University, KSA, in the academic year 2022–2023 b y using probability (random sampling). This study used pre-and post-tests to gather data from the subjects of the study, bifurcated into controlled and experimental groups employing the two modes of teaching, viz., online and traditional. The findings of the investigation proved that the experimental group achieved better performance compared to the control group in terms of results and scores. There are no significant differences based on gender. In addition, (N = 20) teachers teaching listening skills to EFL learners participated in semi-structured interviews. The qualitative analysis enlisted flexibility, accessibility, effective communication, collaboration, monitoring of student progress, and the use of a blackboard as constructive elements, followed by maintenance costs, wastage of available resources, long-term engagement, limited face-to-face interaction, and demotivation as critical perspectives. According to the findings of this research, the author recommends further studies with more variables

    الأهداف: هدفت هذه الدّراسةُ إلى إبراز مكانة الحاكم النّيسابوري في علم الحديث الشّريف، وكونه من أوائل المصنّفين فيه، فقد حاز شرفَ السّبق، واستحق الإتمام بمصنفاته وتخصيصها بالدّراسة، وقد قام الباحثان بدراسة أحد الكتب التي تعدّ مغمورة نوعًا ما بالمقارنة مع نظرائها في المجال ذاته، مع ما فيه من قواعد مكثّفة ومركّزة في علم مصطلح الحديث، تحتاج إلى الدّراسة والتّحليل، وإبراز الجهد المميز للحاكم فيه، وبيان مكانة علم المصطلح التي يجدر بطلبة العلم أن يهتمّوا بها ويتقنوا مفرداتها، وإظهار ما فيه من مسائل وقواعد حديثية يفيد منها طلبة العلم الشّرعي بوجهٍ عامٍ، وعلم الحديث النّبوي على وجهِ الخصوص، وإفراد مسائل علم المصطلح بالدّراسة والمقارنة والتّحليل. المنهجية: وقد اعتمد الباحثان في دراستهما على: المنهجِ الاستقرائي، والمنهجِ التّحليلي، مع مقارنة المسائل والقضايا الحديثية بما ذكره الحاكم النّيسابوري في كتابه: (علوم الحديث). النتائج: ومن أهم ما خلصت إليه الدراسة، أنَّ الحاكم النّيسابوري- رحمه الله- من النّقاد الجهابذة، الذين تبوأوا مكانةً علميةً عاليةً، وصاحب التّصانيف والمؤلفات العديدة في علم الحديث الشّريف، فكان من أوائل من صنَّف في علم المصطلح، وله مؤلفاتٌ عدّة في هذا الفن ومن مؤلفاته (المدخل إلى الإكليل) محل الدراسة، وقد ضمّنه عدداً من المسائل والقضايا الحديثية المختصة وعلوم المصطلح. الخلاصة: توصي هذه الدراسة الباحثين المختصين، بعمل المزيد من البحوث التي تتناول مؤلفات الحاكم النيسابوري بالدراسة والتحليل، فالحاكم وكتبه جدير بالاهتمام، والنّهل من علمه وعلومه وخاصة كتابه: (المدخل إلى كتاب الإكليل).Objectives: This study aimed to highlight the scholarly status of Al-Ḥākim al-Naysābūrī in the science of the Noble Ḥadīth. As one of the earliest scholars to author works in this field, he gained the distinction of precedence and deserves scholarly attention through dedicated study of his writings. The researchers examined one of his works that is relatively obscure compared to others in the same discipline, despite its containing dense and concentrated rules in the science of Ḥadīth terminology. These rules require analysis and examination. The study also sought to showcase Al-Ḥākim’s distinguished effort in this work, clarify the importance of Ḥadīth terminology as a discipline that students of knowledge should give due attention to and master, and highlight the issues and terminological principles it contains, which benefit students of Islamic sciences in general and students of Ḥadīth in particular. The study further aimed to analyze, compare, and examine individual issues of Ḥadīth terminology. Methods: The researchers relied on both the inductive and analytical approaches, along with comparative analysis of Ḥadīth-related issues against what Al-Ḥākim al-Naysābūrī presented in his book ʿUlūm al-Ḥadīth (Hadith Sciences). Results: Among the most important findings of the study is that Al-Ḥākim al-Naysābūrī—may God have mercy on him—was one of the eminent critics who held a high scholarly rank. He authored numerous works in the science of the Noble Ḥadīth and was among the earliest scholars to write in the discipline of terminology. His book Al-Madkhal ilā al-Iklīl, the subject of this study, contains several specialized issues and discussions in Ḥadīth sciences and terminology. Conclusions: The study recommends that specialized researchers conduct further research examining the works of Al-Ḥākim al-Naysābūrī through study and analysis. Al-Ḥākim and his writings are worthy of scholarly attention and of drawing upon his knowledge—especially his book Al-Madkhal ilā Kitāb al-Iklīl

    Construction of partially degenerate laguerre-genocchi polynomials with their applications

    Various applications of degenerate polynomials in different areas call for the thoughtful study and research, and many extensions and variants can be found in the literature. In this paper, we introduce partially degenerate Laguerre-Genocchi polynomials and investigate their properties and identities. Furthermore, we introduce a generalized form of partially degenerate Laguerre-Genocchi polynomials and derive some interesting properties and identities. The results obtained are of general character and can be reduced to yield formulas and identities for relatively simple polynomials and numbers. © 2020 the Author(s), licensee AIMS Press

    Patellar resurfacing and kneeling ability after total knee arthroplasty: a systematic review

    Abstract Background Difficulty kneeling following total knee arthroplasty (TKA) remains highly prevalent, and has cultural, social, and occupational implications. With no clear evidence of superiority, whether or not to resurface the patella remains debatable. This systematic review examined whether resurfacing the patella (PR) or not (NPR) influences kneeling ability following TKA. Methods This systematic review was conducted by following PRISMA guidelines. Three electronic databases were searched utilizing a search strategy developed with the aid of a department librarian. Study quality was assessed using MINROS criteria. Article screening, methodological quality assessment and data extraction were performed by two independent authors, and a third senior author was consulted if consensus was not reached. Results A total of 459 records were identified, with eight studies included in the final analysis, and all deemed to be level III evidence. The average MINORS score was 16.5 for comparative studies and 10.5 for non-comparative studies. The total number of patients was 24,342, with a mean age of 67.6 years. Kneeling ability was predominantly measured as a patient-reported outcome measure (PROM), with two studies also including an objective assessment. Two studies demonstrated a statistically significant link between PR and kneeling, with one demonstrating improved kneeling ability with PR and the other reporting the opposite. Other potential factors associated with kneeling included gender, postoperative flexion, and body mass index (BMI). Re-operation rates were significantly higher in the NPR cohort whereas PR cohorts had higher Feller scores, patient-reported limp and patellar apprehension. Conclusion Despite its importance to patients, kneeling remains not only under-reported but also ill-defined in the literature, with no clear consensus regarding the optimum outcome assessment tool. Conflicting evidence remains as to whether PR influences kneeling ability, and to clarify the situation, large prospective randomized studies are required
