Long-term effectiveness of adjuvant Goserelin in premenopausal women with early breast cancer
No full textBACKGROUND: Systematic reviews have found that luteinizing hormone-releasing hormone (LHRH) agonists are effective in treating premenopausal women with early breast cancer. METHODS: We conducted long-term follow-up (median 12 years) of 2706 women in the Zoladex In Premenopausal Patients (ZIPP), which evaluated the LHRH agonist goserelin (3.6 mg injection every 4 weeks) and tamoxifen (20 or 40 mg daily), given for 2 years. Women were randomly assigned to receive each therapy alone, both, or neither, after primary therapy (surgery with or without radiotherapy/chemotherapy). Hazard ratios and absolute risk differences were used to assess the effect of goserelin treatment on event-free survival (breast cancer recurrence, new tumor or death), overall survival, risk of recurrence of breast cancer, and risk of dying from breast cancer, in the presence or absence of tamoxifen. RESULTS: Fifteen years after the initiation of treatment, for every 100 women not given tamoxifen, there were 13.9 (95% confidence interval [CI] = 17.5 to 19.4) fewer events among those who were treated with goserelin compared with those who were not treated with goserelin. However, among women who did take tamoxifen, there were 2.8 fewer events (95% CI = 7.7 fewer to 2.0 more) per 100 women treated with goserelin compared with those not treated with goserelin. The risk of dying from breast cancer was also reduced at 15 years: For every 100 women given goserelin, the number of breast cancer deaths was lower by 2.6 (95% CI = 6.6 fewer to 2.1 more) and 8.5 (95% CI = 2.2 to 13.7) in those who did and did not take tamoxifen, respectively, although in the former group the difference was not statistically significant. CONCLUSIONS: Two years of goserelin treatment was as effective as 2 years of tamoxifen treatment 15 years after starting therapy. In women who did not take tamoxifen, there was a large benefit of goserelin treatment on survival and recurrence, and in women who did take tamoxifen, there was a marginal potential benefit on these outcomes when goserelin was added

Geographic profiling in Nazi Berlin: fact and fiction
No full textGeographic profiling uses the locations of connected crime sites to make inferences about the probable location of the offender’s ‘anchor point’ (usually a home, but sometimes a workplace). We show how the basic ideas of the method were used in a Gestapo investigation that formed the basis of a classic German novel about domestic resistance to the Nazis during the Second World War. We use modern techniques to re-analyse this case, and show that these successfully locate the Berlin home address of Otto and Elise Hampel, who had distributed hundreds of anti-Nazi postcards, after analysing just 34 of the 214 incidents that took place before their arrest. Our study provides the first empirical evidence to support the suggestion that analysis of minor terrorism-related acts such as graffiti and theft could be used to help locate terrorist bases before more serious incidents occur

No full textThe aim of this thesis is to determine how women are treated in selectedworks of Henry de Montherlant. This is explored by examining their relationshipswith other women as well as with men. Inevitably, this leads to an analysis ofthe multifaceted area of love. Part I researches Montherlant's prose work and included in this section is the investigation of the importance of 'l'ordre male' to the author and the influence this exerts over his early prose work in the areas of tauromachy, war and sport, and where the male adherence to this concept leaves women. The 'syncretisme et alternance' which is central to Montherlant's thinking is explored in this section.Part 2 is concerned with Montherlant's theatre in which the psychological development of the main characters is of great importance. It is in this section that a comparative study is made of the influence of Mme. Elisabeth Zehrfuss' written contribution to La Reine morte. Her unpublished notes are set out in full in the Appendix. The thesis also draws on the unpublished correspondence between Henry de Montherlant and Elisabeth Zehrfuss between the years 1934 and 1945. An investigation is made as to whether or not there are any differences between the way women are treated in Montherlant's prose and in his theatre and the conclusion is drawn that there are
An economic analysis of the production of hydrogen from wind-generated electricity for use in transport applications
No full textWind-generated electricity is often considered a particularly promising option for producing hydrogen from renewable energy sources. However, the economic performances of such systems generally remain unclear because of unspecified or favorable assumptions and operating conditions. The aim of this paper is to clarify these conditions by examining how the hydrogen produced is used. The analysis that has been conducted in the framework of the HyFrance 3 project concerns hydrogen for transport applications. Different technical systems are considered such as motorway hydrogen filling stations, Hythane®-fueled buses or second-generation biofuels production, which present contrasted hydrogen use characteristics. This analysis reveals considerable variations in hydrogen production costs depending on the demand profiles concerned, with the most favorable configurations being those in which storage systems are kept to a minimum.wind power ; hydrogen production
