Bone alkaline phosphatase isoforms in chronic kidney disease : mineral and bone disorder
No full textChronic kidney disease (CKD) is associated with increased mortality and cardiovascular complications. Disturbances in mineral metabolism occur early <luring the course of CKD and several components of the CKD-mineral and bone disorder (CKD-MBD) are independent predictors of mortality. Alkaline phosphatase (ALP) is necessary for skeletal mineralization and is also involved in the process of vascular calcification. In recent years, ALP has evolved as a strong predictor of mortality in the CKD population. The significant role of ALP in the mineralization process renders it a putative target for the treatment and prevention of vascular calcification. Three circulating isoforms of bone ALP (BALP) have been identified (B/I, B 1, and B2). A fourth isoform, Blx, has been identified exclusively in serum from patients with CKD. The aim of the present thesis was to further elucidate the role ofthe BALP isoforms in CKD with respect to bone abnormalities and vascular calcification. In study I we identified the novel BALP isoform Blx in 20% of patients with mild to moderate CKD. Blx was associated with lower glomerular filtration rate and higher serum phosphate and calcium x phosphate product, which are risk factors for cardiovascular mortality in CKD. We also identified the BALP isoforms B/I, Bl and B2 as predictors of total hip bone mineral density. Study II was an experimental study, investigating the role of the BALP isoforms in phosphate induced calcification of human aortic smooth muscle cells (HASMCs). We found that the ALP expressed in HASMCs is exclusively BALP. Phosphate induced calcification of HASMCs was associated with increased BALP isoforms B/I, Blx, and B2 activities, which implies functional differences between the BALP isoforms in HASMC calcification. In study III we investigated the association of BALP isoforms in serum and histomorphometric parameters of bone in patients on chronic hemodialysis. W e identified the BALP isoform Blx as a novel marker for reduced osteoblastic activity. Study IV was a prospective cohort study of the association of serum BALP isoforms with aortic calcification and vascular stiffness in prevalent chronic dialysis patients. Blx was associated with baseline and time varying vascular stiffness, determined by pulse wave velocity, but not with calcification of the abdominal aorta. We also found an association of Blx with better event-free survival. In conclusion, these studies demonstrate that the BALP isoforms, especially isoform Blx, are involved in different aspects of CKD-MBD. This opens up for further research to identify the BALP isoforms as diagnostic markers and possible treatment targets in CKD-MBD

The reality of fiction: the ECO by Mathias Goeritz

This article covers the full biography of a building, the Experimental Museum El Eco, designed by Germanborn and Mexican émigré artist and architect Mathias Goeritz. It provides an approach intersecting the biography of the author, the history of the building, and prominent individuals of the two cultural traditions, German and Mexican, who participated in the creation of a very special and unique building: El Eco. On the one hand, the ethics of Expressionism, the interest in non-European art, the cult of primitivism and the aesthetic system of the pair of concepts abstraction-empathy, all stemming from German culture. On the other, the pantheistic religiosity of landscape, zoomorphism and anthropomorphism, the interest in masks, and the aesthetics of monumental scale, stemming from pre-Cortesian Mexican culture. Taking the stance of intertwining Mathias Goeritz parcours with those of individuals and issues from his German past and his Mexican future – highlighting the figures of Wilhelm Worringer, Paul Westheim, Luis Barragán, Edmundo O'Gorman, and Ida Rodríguez Prampolini – this article proposes a return trip from fiction to reality, following in the footsteps of the author and comparing them with the pathway of the very building El Eco
Oswald Mathias Ungers and Sol LeWitt: Variations
No full textOne of the fundamentals of Oswald Mathias Ungers work is the definition of a new unity between art and architecture. What this investigation is focused on is the identification of the intersection point between art and architecture, considering the project for the Entrance to the Kaiserthermen in Trier (20032007) as case study, revealing the system of theoretical, methodological and compositional correspondences. Plastic art is Ungers' main interlocutor: one of the author's most important references is Sol LeWitt (1928-2007), an artist he admired and collected. Setting the work of Oswald Mathias Unger and Sol LeWitt side by side, offers the opportunity to underline the shared interpretation that the two authors have of the world of forms postulated by Henri Focillon (1881-1943) in Life of Forms, according to which form, autonomous with respect to time and place, has its own universe, governed by its own laws, space and matter. According to these premises, we propose to read Trier's building through the instruments of art, following the hypothesis that in this project art embodies a creative and positive component in the definition of architecture, a cultural stimulus that guides the author in the design process
Klotho: a potential therapeutic target in aging and neurodegeneration beyond chronic kidney disease—a comprehensive review from the ERA CKD-MBD working group
No full textKlotho, a multifunctional protein, acts as a co-receptor in fibroblast growth factor 23 and exerts its impact through
various molecular pathways, including Wnt, hypoxia-inducible factor and insulin-like growth factor 1 pathways. The
physiological significance of Klotho is the regulation of vitamin D and phosphate metabolism as well as serving as a vital
component in aging and neurodegeneration. The role of Klotho in aging and neurodegeneration in particular has gained
considerable attention. In this narrative review we highlight several key insights into the molecular basis and
physiological function of Klotho and synthesize current research on the role of Klotho in neurodegeneration and aging.
Klotho deficiency was associated with cognitive impairment, reduced growth, diminished longevity and the development
of age-related diseases in vivo. Serum Klotho levels showed a decline in individuals with advanced age and those
affected by chronic kidney disease, establishing its potential diagnostic significance. Additionally, multiple medications
have been demonstrated to influence Klotho levels. Therefore, this comprehensive review suggests that Klotho could
open the door to novel interventions aimed at addressing the challenges of aging and neurodegenerative disorders
Alkaline Phosphatase : An Old Friend as Treatment Target for Cardiovascular and Mineral Bone Disorders in Chronic Kidney Disease
No full textAlkaline phosphatase (ALP) is an evolutionary conserved enzyme and widely used biomarker in clinical practice. Tissue-nonspecific alkaline phosphatase (TNALP) is one of four human isozymes that are expressed as distinct TNALP isoforms after posttranslational modifications, mainly in bone, liver, and kidney tissues. Beyond the well-known effects on bone mineralization, the bone ALP (BALP) isoforms (B/I, B1, B1x, and B2) are also involved in the pathogenesis of ectopic calcification. This narrative review summarizes the recent clinical investigations and mechanisms that link ALP and BALP to inflammation, metabolic syndrome, vascular calcification, endothelial dysfunction, fibrosis, cardiovascular disease, and mortality. The association between ALP, vitamin K, bone metabolism, and fracture risk in patients with chronic kidney disease (CKD) is also discussed. Recent advances in different pharmacological strategies are highlighted, with the potential to modulate the expression of ALP directly and indirectly in CKD-mineral and bone disorder (CKD-MBD), e.g., epigenetic modulation, phosphate binders, calcimimetics, vitamin D, and other anti-fracture treatments. We conclude that the significant evidence for ALP as a pathogenic factor and risk marker in CKD-MBD supports the inclusion of concrete treatment targets for ALP in clinical guidelines. While a target value below 120 U/L is associated with improved survival, further experimental and clinical research should explore interventional strategies with optimal risk-benefit profiles.
