Virtual Reality Environmental Enrichment Effects on Craving for Cigarette in Smokers
: Background: Preclinical studies suggested the exposure to environmental enrichment (EE) as an intervention able to prevent or reduce nicotine-taking and nicotine-seeking behaviors. Virtual reality (VR) may help to test the effects of EE in smokers in a reproducible and feasible manner. Materials and Methods: In the present study, 31 smokers (14 women) were divided into two groups: (1) exposure to a virtual EE (VR-EE) and (2) exposure to a virtual neutral environment (VR-NoEE). Cigarette craving was assessed as basal and evoked, at different timepoints during the session. Behavior activity during VR exposure, mood, and subjective measures were also collected. Results: EE exposure in VR significantly reduced craving scores from basal timepoint. This was not observed in the VR-NoEE group, which significantly increased craving compared with values at neutral scenario. When both groups were exposed to smoking-related VR scenario, the VR-EE group showed an increased craving compared with previous timepoint up to score values not different from those in the VR-NoEE group. A significant positive correlation between basal craving scores and interactive behavior with virtual smoking cues was observed in the VR-NoEE but not in the VR-EE group. Conclusion: These findings suggest that virtual EE might have an inhibitory effect in smokers on basal, but not on evoked cigarette craving. Noteworthily, the interactive activity correlation to craving scores in the VR-NoEE participants was not observed in the VR-EE group, adding further evidence that the enrichment simulation was nonetheless able to modify behavior in the smoking-related scenario

Italia – si propone di approfondire i risvolti della condanna per violazione dell’art. 7 CEDU sull’ordinamento interno. Più nello specifico, l’articolo intende soffermarsi sulla posizione delle persone giuridiche titolari di diritti reali sui beni oggetto di confisca urbanistica e sulle possibili opzioni a disposizione dello stato italiano per garantire la loro partecipazione al pro- cedimento penale, così da conformarsi all’art. 7 CEDU
Fiscal Policy, the Real Exchange Rate and Traded Goods
We estimate the effects of government spending shocks on the CPI real exchange rate, the trade balance and their co-movements with GDP and private consumption. We decompose the variations of the CPI real exchange rate into variations of the traded goods real exchange rate and the relative price of traded to non-traded goods. We reach three main conclusions: a rise in government spending induces a "depreciation" of the CPI real exchange rate and a trade balance deficit; private consumption rises in response to a government spending shock and therefore co-moves positively with the real exchange rate; both components of the CPI real exchange depreciate. Laparoscopic modified double stapling technique with transanal resection for low anterior resection of rectal cancer
BACKGROUND AND AIM: Anterior resection of the rectum with a total mesorectal excision is the standard surgical technique for the treatment of rectal cancer. Laparoscopic low anterior resection (LALAR) is an alternative to open surgical approach and was validated in diverse randomized control trials to be as safe and oncologically effective. That said, confronting a low rectal tumor in an obese patient with a narrow pelvis can be technically challenging even for the most expert surgeon. METHODS: We propose a modified double stapling technique with transanal eversion and staple resection of the rectal stump. RESULTS: We applied the above technique in 3 patients with a dubious distal resection margin due to patient/tumor characteristics. The mean length of operation was 272 minutes and a R0 resection with a mean number of 16 nodes could be obtained in all the patients. No recurrence occurred during a follow-up of 28 months. CONCLUSIONS: We conclude that this technique is a feasible, safe and valid adjunct to the double staple technique whenever intraabdominal application of the linear staple is difficult or unsafe. Splenic hamartoma associated with abdominal discomfort and pain. Case report
Hamartomas are benign splenic neoplasms asymptomatic in most of the cases. Symptoms, when present, may either be related to the growth of the mass with abdominal discomfort and pain or be related to a hypersplenism syndrome. Certain preoperative diagnosis cannot be made with current diagnostic imaging. Splenectomy is therefore indicated in order to obtain histological diagnosis, rule out malignancy or achieve regression of symptoms. We report the case of a 39-year-old woman referred for a splenic hamartoma causing pain located on the upper abdominal quadrant. She underwent splenectomy through a left subcostal access followed by complete resolution of symptoms. Resection of splenic masses is indicated to complete diagnosis, achieve cure and, when present, relieve symptoms
Simultaneous resection of an adenocarcinoma of the cardia and a synchronous adenocarcinoma of the sigmoid. Report of a case
Introduction: Adenocarcinoma of the cardia synchronous with other intraabdominal neoplasms is very rare. We
report the case of a Siewert type II adenocarcinoma of the cardia synchronous with an adenocarcinoma of the
sigmoid both treated simultaneously by transjatal oesophago-gastrectomy and anterior resection of the sigmoid.
Case report: A 62 year-old male was admitted for a progressing dysphagia and weight loss. Oesophago-gastric
fibroscopy detected an adenocarcinoma of the cardia extending to the distal 2 cm of the esophagus (Siewert
typeII). A CT-scan of the chest and abdomen confirmed the cancer of the cardia and also decealed a synchronous
tumor of the sigmoid. Both neoplasms were resected through a xipho-pubic laparotomy, with an ileostomy
completing the procedure. Postoperative course was uneventful and ileostomy was closed four weeks later. The
patient was subsequently addressed to oncological for adjuvant treatment.
Discussion: This report supports the indication of aggressive, simultaneous treatment of an adenocarcinoma of
the cardia associated with a synchronous abdominal neoplasm, provided that both are resectable through the
same surgical access, as anticipated at a preoperative, through diagnostic work-up.
Conclusion: Simultaneous resection of synchronous adenocarcinoma of the cardia and the sigmoid is feasible and
avoids possible progression of the untreated neoplasm during the interval between two separate resections,
provided that a curative resection can be obtained for both diseases
State ownership - a residual role?
The author reviews the state of thinking on the governance role of state ownership. He argues that a gradual transfer of operational control and financial claims over state assets remains the most desirable goal, but it needs to be paced to avoid regulatory capture, and the capture of the privatization process itself. In addition, the speed of transfer should be timed on the progress in developing a strong regulatory governance system, to which certain residual rights of intervention must be vested. In many countries institutional weakness limits regulatory capacity and reliability, yet the author's conclusion is that in such environments, maintaining state control undermines the very emergence of institutional capacity, and so the balance should tip toward progressively less direct state control. After all, what are"institutions"if not governance mechanisms with some degree of autonomy from both political and private interests? The gradual creation of institutions partially autonomous from political power must become central to the development of an optimal mode of regulatory governance. Del "Ars grammaticae" al "Ars epistolaris" : el "De componendis epistolis" de Niccolò Perotti
El propósito de este trabajo es poner de relieve el papel\ud
primordial que algunas artes grammaticae, como los Rudimenta\ud
grammatices de Niccolo Perotti, representaron en el desarrollo y evolución\ud
del arte epistolar en los albores del Renacimiento. Numerosas\ud
ediciones del De componendis epistolis, última sección del ars de\ud
Perotti, permanecen inéditas en las bibliotecas, siendo objeto de lecturas\ud
parciales que han minusvalorado su significación en la historia de\ud
la epistolografia. La carta-modelo que cierra el tratado es el verdadero\ud
eje nuclear de cuantas enseñanzas gramaticales inserta en su tercer\ud
libro el preceptista italiano. El análisis de esta epístola conclusiva nos\ud
permite advertir en su justa medida el espíritu humanista que alienta la\ud
concepción esencialmente globalizadora y pedagógica que el autor\ud
tenía del género epistolar.The object of this paper is to show the fundamental role that\ud
some artes grammaticae, such as, Rudimenta grammatices de Niccolo\ud
Perotti, had in the unfolding of the epistolary art at the beginnings of the\ud
Renaissance. However, still many copies on Perotti's last section of his\ud
ars, De componendis epistolis, continue to hide unpublished in libraries,\ud
being the object of neglet and partial readings that understimate the role\ud
ot the work in the history of epistolography. The model letter which ends\ud
the thirde book of this treatise is the true centre of all his grammatical\ud
teaching. A closed analysis of this epistle allows us to fully appreciate the\ud
humanistic spint that permeates the global and pedagogial conception the\ud
Italian author had on the epistolary genre
