    Sutureless aortic valve replacement for aortic incompetence.

    OBJECTIVE: Sutureless prostheses for surgical aortic valve replacement (AVR) are usually used in degenerative calcified aortic stenosis. Less is known on the application of sutureless prostheses for pure aortic incompetence. METHODS: Between 2011 and 2014, 442 patients were operated on with the Perceval aortic sutureless valve implant. We identified 11 patients (10 female, mean age 70.5) who underwent sutureless AVR for pure aortic incompetence (off-label use). Three patients had a left ventricle ejection fraction of 30% or less. Mean logistic EuroSCORE was 15.2 (range 2.2-45.2). In five patients associated mitral procedures (three [60%] repair and two [40%] replacement) were performed. Four procedures were performed through a minimally invasive approach (three right minithoracotomies and one partial sternotomy). RESULTS: Mean cardiopulmonary bypass time was 130.2 min and aortic cross clamp time was 82.2 min. Mean implanted prosthesis size was 24.5 ± 1.3 (median 25) mm (insignificant correlation with preoperative aortic valve annulus measurement by transthoracic echocardiography: 21.6 ± 1.5 [median 21] mm, Pearson's r = 0.373, p = 0.259). One patient died on 24th day after AVR associated with aortic arch replacement and hypothermic circulatory arrest (10 years after correction for type A aortic dissection). No residual para- or intravalvular leakage was present on discharge and 12-month follow-up. No migration of the prosthesis occurred. CONCLUSION: Sutureless AVR is an option in selected patients with aortic incompetence. Preoperative aortic annulus measurement by echocardiography has poor predictive value for estimation of prosthetic valve size

    Adolescents Who Self Harm: A Comparison of Those Who Go to Hospital and Those Who Do Not

    Background: Deliberate self harm (DSH) by adolescents is a major problem at both hospital and community levels although little is known about the factors associated with hospital presentation. Method: Using a community survey a comparison was undertaken of adolescents in 41 schools who reported DSH and presented to hospital with those who reported DSH but did not attend hospital. Results: While hospital presentation following DSH was associated with several factors, multivariate analysis indicated that the most important were method of DSH (overdose, methods other than self-cutting and multiple methods) and help-seeking before the act. Conclusions: The few differences between adolescents who present to hospital following DSH and those who do not, other than in method of DSH and prior help-seeking, highlight the need for primary preventive initiatives. © 2009 The Author. The accepted version of the publication may differ from the final published version. Available online at https://doi.org/10.1093/llc/fqab103The primary purpose of this paper is author verification of the Nahj Al-Balagha, a book attributed to Imam Ali and over which Sunni and Shi’i Muslims are proposing different theories. Given the morphologically complex nature of Arabic, we test whether morphological segmentation, applied to the book and works by the two authors suspected by Sunnis to have authored the texts, can be used for author verification of the Nahj Al-Balagha. Our findings indicate that morphological segmentation may lead to slightly better results than whole words, and that regardless of the feature sets, the three sub-corpora cluster into three distinct groups using Principal Component Analysis, Hierarchical Clustering, Multi-dimensional Scaling and Bootstrap Consensus Trees. Supervised classification methods such as Naive Bayes, Support Vector Machines, k Nearest Neighbours, Random Forests, AdaBoost, Bagging and Decision Trees confirm the same results, which is a clear indication that (a) the book is internally consistent and can thus be attributed to a single person, and (b) it was not authored by either of the suspected authors

    The Structure and Fluid Flow Analysis for a New DSH Dynamic - Static Combined Mixer

    A new DSH type dynamic-static combined mixer, which combines the advantages of dynamic mixing agitator and SH type static mixer, with the structure of alternative connection of the turning impeller, liquid shunt unit and double-barreled helical stationary blade, is original designed by the author. The allotype impeller is driven by the two-phase flow and meanwhile the two-phase flow is sheared by the allotype impeller. The double helix eddy flow is formed out of two-phase flow by the stationary blade due to the shearing effect of allotype impeller which results in the improvement of mixture efficiency. The most characteristic of this design is: the mixture is driven by the dynamic force of liquid itself, therefore the energy is saved and the mixture efficiency is enhanced. Followed by the illustration of the structure, the preliminary analysis of the flow field is carried out, which provides some proofs for the selection and optimization of the static-dynamic combined high efficient mixer structural parameters.</jats:p

    Davidsonian semantic theory and cognitive science of religion

    This article investigates the extent to which the cognitive science of religion (CSR) and Donald Davidson’s semantic holism (DSH) harmonize. We first characterize CSR, philosophical semantics (and more specifically DSH). We then note a prima facie tension between CSR and DSH’s view of First-Person Authority (that we know what is meant when we speak in a way that we do not when others speak). If CSR is correct that the causes of religious belief are located in cognitive processes in the mind/brain, then religious insiders might have no idea what they are talking about: only the scholar of CSR would have a chance of knowing what they ‘really’ mean. The article argues that the resolution to this problem is to take seriously DSH’s rejection of semantic bifurcation, specifically rejecting the idea that religious and non-religious language can be sharply distinguished. We conclude by supporting the following claims: (i) common cognitive neural/psychological processes are explanatorily relevant in proposed meaning-theories for any discourse, and (ii) those processes need semantic supplementation with reference to external and naturalistic factors (biological, cultural, environmental etc.)

    Who wrote Wuthering Heights?

    Emily Brontë published Wuthering Heights in 1847 under the pseudonym Ellis Bell. It was not until the later second edition, published after Emily’s death, that she was credited as the novel’s author. Those Victorian attitudes towards women which compelled Brontë to publish as Bell have not been wholly eradicated, with her legitimacy as the sole author being called into question by male commentators at several junctures since. Their claim is that Emily’s brother Branwell is the real author of Wuthering Heights. Using stylometry, a computer-assisted technique which measures the likely author of a text, this brief experiment demonstrates that it is highly unlikely that Branwell Brontë contributed to the writing of Wuthering Heights, and that Emily, as generally considered, is the novel’s sole author. Furthermore, considering a number of limitations with the corpus being tested, this study provides a good and necessary example of stylometry in practice, and how such an experiment should be conducted in less-than-ideal circumstances

    Wood biomass functions for Acacia abyssinica trees and shrubs and implications for provision of ecosystem services in a community managed exclosure in Tigray, Ethiopia

    In the Ethiopian highlands, remarkable recovery of vegetation has been achieved using exclosures, protecting vegetation against livestock browsing and firewood harvesting. But these emerging forest resources require tools for sustainable use, implying knowledge on biomass stocks and growth. In this study we developed biomass functions estimating total, stem and branch biomass from diameter at stump height (DSH) and tree height (H) for an 11-year old exclosure in Tigray, Ethiopia. In a systematic grid of 55 plots, DSH and H of all trees and shrubs were recorded. 40 Acacia abyssinica trees were selected for destructive sampling. Allometric relationships using a natural logelog model were established between aboveground biomass, DSH and H. Models with only DSH were found best with R2 between 0.95 and 0.98. The functions were 10 fold cross-validated and R2 _cv ranged from 0.94 to 0.97, indicating good model performance. The models were found well in range with those of other seasonal forests in East Africa. Total aboveground biomass was estimated 25.4 ton ha1 with an annual production of 2.3 ton ha 1, allowing sustainable wood fuel use for 4 persons ha1.
