Successful outcomes in arthroplasty surgery have significantly contributed to more hip replacement procedures being performed annually. This has also increased the incidence of femoral periprosthetic fractures, leading to more revision hip replacement procedures being performed. Methods This is a retrospective cohort study carried out at a district general hospital in the United Kingdom. Theatre records were reviewed from 2018 to 2022 to identify patients with a femoral periprosthetic fracture. Following the data collection, the patients were split into two groups. The first group analysed the patient outcomes through the type of surgical intervention they had, and the second group analysed the patients according to the timing of surgical intervention. Results There were 88 patients included in the study, out of which 49 had revision surgery and 39 had fixation for the femoral periprosthetic fracture. No statistically significant difference was observed in 30-day mortality and one-year mortality for the patients having revision or fixation surgery. Similarly, the results were not found to be significant in 30-day mortality and one-year mortality for the patients having surgery within 36 hours or after 36 hours of diagnosis. Conclusion Overall, the findings of this study are in keeping with the literature. Input from the specialist arthroplasty team is often required for the management of femoral periprosthetic fractures. There is no significant impact on mortality with a delay in surgical intervention for femoral periprosthetic fractures, unlike hip fractures. Medical optimisation and careful planning lead to better patient outcomes for this group of patients

From Clicks to Conflicts: The Impact of Social Media Narratives on Political Polarization

    No full text
Background: Social media has turned out to be one of the most influential dynamics in modifying the political discourse and general opinion in Pakistan. Despite increasing the accessibility of political news and political participation, such platforms have also increased biased narratives, misinformation, and the choice of selective exposure, hence promoting political polarization. Objective: The current qualitative research study focuses on (1) mechanisms of how people get political information on social media platforms, (2) the role of biased narratives in the shaping of political attitudes, and (3) the impact of digital political polarization on social relations. Methodology: The study relies on the Agenda-Setting Theory, Framing Theory, and Selective Exposure Theory to explain these findings through interviews carried out on sixteen semi-structured participants who engage in active or passive consumption of political content on Facebook, TikTok, WhatsApp, and YouTube. Findings: Findings show that social media are an agenda-setter because they give priority to some political topics in user feeds. Interviewees observed that such content is frequently biased with implications of such framing strategies that enhance the reiteration of partisan views. There was also selective exposure because most participants only listened to the content that reaffirmed their prior political ideologies, thus forming echo chambers that further polarized them. Others considered social media space as a friendly area of connection and some people said that the internet made online political disagreements add to the breakdown of connections offline. It seemed that awareness of misinformation was widespread, and interviewees often checked the news with their peers, family, or even international media. Conclusion: All of these findings together undermine the assumption that social media is a two-edged sword that, on the one hand, allows people to become relatively easily accessible to political discussion, and, on the other, fuels divisiveness and mistrust. The theoretical perspectives that Agenda-Setting Theory, Framing Theory, and Selective Exposure Theory provide are sufficient to explain the process of political polarization in Pakistan's Digital space. Increased media literacy, ethically responsible journalism, and political content regulation, thus, become instrumental efforts to address the issue of polarization and lead toward healthier democratic practices. This research paper attempts to evaluate the consumer rights and their violation by electronic media through advertisement. This is misguiding the consumer to buy harmful and unnecessary products. This research also aims at interpolating and ascertaining the onus of responsibility of a responsible media with regards to advertising. In the recent past the principle of "Caveat Emptor" which meant "Buyers beware" governed the relationship between a seller and buyer. In the era of open markets the buyer and seller came face to face, seller exhibited his goods, and buyer thoroughly examined them and then purchased them. The assumption would be consumer would use all his care and skill while entering into a transaction. The buyer to examine the goods beforehand and most of the transactions are now concluded by correspondence. Discussion on consumer's rights and their violation by media through advertisements is suggestive that consumers today enjoy a good amount of legal protection. But, even today, a large number of us remain exploited

It has been observed that these injuries can be present in both young and elderly patients. Surgical intervention with open reduction and internal fixation is often performed for unstable ankle fractures. Methods This study was performed at a district general hospital in the United Kingdom and is a retrospective cohort study. The electronic database was reviewed from January 2021 to December 2024 to identify and review the notes for the patients to be included in the study. After identifying the patients, they were divided into two groups. The patients who had surgical intervention in less than 10 days were allocated to the early surgery group, and the patients who had surgery at day 10 or after day 10 of the injury were allocated to the delayed surgery group.  Results Overall, 96 patients were included in this study, where 29 (30.21%) had early surgical intervention and 67 (69.79%) had delayed surgical intervention. The results were not statistically significant for patient outcomes between the two groups. A correlation was observed between the age of the patients and the length of hospital stay. Conclusion At present, there is no definitive consensus with regards to the optimal timing for surgical intervention of ankle fractures. The results of this study highlight that there was no significant difference for post-operative complications between the two groups, which is consistent with the literature. A study with a larger sample size is required to analyse the long-term patient outcomes
