Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
C-kit receptor expression in the development and progression of malignant melanoma
Der c-kit-Rezeptor spielt einerseits eine wichtige Rolle bei der Entwicklung und Differenzierung einer ganzen Reihe von Zellen, unter anderem von Melanozyten, und ist an Signalkaskaden beteiligt, die zum Zellüberleben beitragen, andererseits ist er aber auch bei diversen Tumoren als Protoonkogen bekannt. Darüberhinaus reguliert c-kit jedoch auch Signaltransduktionswege, die bei Melanomzellen in Anwesenheit seines Liganden SCF zur Apoptose führen. Über seine Rolle bei der Entstehung sowie bei der Progression des malignen Melanoms liegen bislang noch ungenügende Daten vor und so zeigt sich letztlich ein recht uneinheitliches Bild über seine Funktion.
In bislang verfügbaren Arbeiten über c-kit wurde der Schwerpunkt größtenteils auf einzelne Funktionen von c-kit gelegt oder es wurde nur eine kleine Anzahl an unterschiedlichen Tumorzellinien untersucht.
Das Ziel der vorliegenden Arbeit war die Erstellung eines umfassenden Expressionsprofils von c-kit. Die c-kit-Expression wurde einerseits durch die Untersuchung einer Vielfalt an Zellinien, die von Melanozyten über Nävi, RGP- und VGP-Melanome hinweg bis hin zu Melanommetastasen und einigen anderen Tumorzellinien sowie Nativgeweben reichte, analysiert. Andererseits wurde die Expressionsuntersuchung der Zellinien auf verschiedenen Expressionsstufen des Rezeptors, das heißt auf RNA-, DNA- und Proteinebene, durchgeführt. Hierzu wurden die Methoden des Northern Blottings, der PCR, des Western Blottings, der FACS-Analyse sowie der Immunhistologie angewandt.
Vor dem Hintergrund der bislang verfügbaren Daten sollte anhand dieser Zellinien- und Methodenvielfalt untersucht werden, ob ein Zusammenhang zwischen der c-kit-Expression und der Progression des malignen Melanoms hergestellt werden kann. Von zentralem Interesse war hier die Frage, ob und zu welchem Zeitpunkt der Melanomprogression die Expression von c-kit verloren geht.
In der vorliegenden Arbeit konnte durch die zellbiologische Methode der FACS-Analyse in 82,84% der Melanozyten eine c-kit-Rezeptorexpression nachgewiesen werden, während die Nävuszellen, alle Melanom- sowie alle Tumorzellen negativ für c-kit waren.
Mit Hilfe der proteinchemischen Methode des Western Blots zeigten sich neben den Melanozyten zusätzlich Nävuszellen positiv für den Rezeptor, während die Melanom- sowie die anderen Tumorzellinien c-kit-negativ waren. Diese Ergebnisse deuten darauf hin, daß die Expression des c-kit-Proteins schon zu einem sehr frühen Zeitpunkt der Melanomprogression nicht mehr stattfindet.
In den immunhistologischen Untersuchungen an mehreren Nävi, superfiziell spreitenden Melanomen, einem Lentigo Maligna Melanom sowie einigen Hautmetastasen konnten die Ergebnisse des Western Blots bestätigt werden. Darüberhinaus zeigt sich anhand der Färbung eines superfiziell spreitenden Melanoms, daß die c-kit-Rezeptorexpression negativ mit dem Invasionspotential von Melanomzellen korreliert. Während hier eine deutliche Färbung der Tumorzellen an der Junktionszone auf eine c-kit-Expression hinweist, ist mit zunehmender Invasionstiefe des Melanoms jedoch eine abnehmende Anfärbung der Tumorzellen zu erkennen, in der tiefen Dermis geht die c-kit-Expression sogar gänzlich verloren. Dies könnte ein Hinweis darauf sein, daß durch den Expressionsverlust von c-kit die Invasionsfähigkeit der Melanomzellen ansteigt. Das Lentigo Maligna Melanom sowie die Färbung einer Melanomhautmetastase waren negativ für c-kit. Mit Hilfe der Methode der PCR konnte ein Teilfragment von c-kit einmalig erfolgreich amplifiziert werden, alle weiteren PCR-Reaktionen zur Amplifikation von c-kit waren negativ. Ebenso waren im Northern Blot alle Melanom- sowie Tumorzellen negativ für c-kit.
Insgesamt zeichnete sich über alle in der vorliegenden Arbeit angewandten Methoden hinweg die Tendenz ab, daß die Expression des c-kit-Rezeptors mit fortschreitender Progression des malignen Melanoms herunterreguliert wird.
Die Ergebnisse in der vorliegenden Arbeit lieferten Hinweise, daß dieser Expressionsverlust stattfindet, wenn die Tumorzellen ihr Invasionspotential in die Dermis erlangen.
Zusammenfassend läßt sich feststellen, daß eine negative Korrelation zwischen der Progression des malignen Melanoms und der c-kit-Rezeptor-Expression gefunden werden konnte.On the one hand, c-kit receptor plays an important role in the development and differentiation of several cells, amongst others of melanocytes, and is involved in signaling pathways contributing to cell survival, on the other hand c-kit is also known as proto oncogene in several tumours. Furthermore, c-kit regulates pathways leading to apoptosis in melanoma cells if c-kit ligand SCF is present.
Up to now, as far as malignant melanoma is concerned only inconsistent data exists about the role of c-kit in development and progression of melanoma.
Previous studies either focused on particular functions of c-kit or investigated only a small number of different tumour cell lines.
The aim of the present study was to create an extensive expression analysis of c-kit. For this purpose, c-kit expression was analysed on a variety of cell lines ranging from melanocytes to nevi, radial growth phase (RGP) and vertical growth phase (VGP) melanomas to melanoma metastases and finally numerous different tumour cell lines as well as native tissues. The expression analysis was carried out on different levels of c-kit receptor expression i.e. on RNA, DNA and protein level by northern blotting, PCR, western blotting, FACS analysis and immunohistochemistry.
The main question was to find out if c-kit expression is downregulated during melanoma progression and if so, at which point.
In this study, FACS analysis showed c-kit receptor expression in 82,84% of melanocytes whereas nevus cells, all melanoma cell lines and all other tumour cell lines were negative for c-kit.
Western blot analysis showed c-kit receptor positivity for melanocytes and an additional c-kit receptor positivity in nevus cells whereas all melanoma and other tumour cell lines were c-kit negative.
These results indicate that c-kit protein expression is lost at a very early point in melanoma progression.
Immunohistochemistry on several nevi, superficially spreading melanomas, one lentigo maligna melanoma as well as several skin metastases confirmed the results obtained by western blot analysis. Furthermore, staining of a superficially spreading melanoma showed negative correlation between c-kit receptor expression and invasion potential of melanoma cells: while there was an obvious c-kit staining of tumour cells in the junction zone, staining diminished with increasing melanoma invasion level and was completely absent in the deep dermis. This could indicate that invasion potential of melanoma cells increases with loss of c-kit expression. Accordingly, lentigo maligna melanoma and skin metastases were all negative for c-kit. In PCR, except for the successful amplification of one fragment of c-kit, all amplifications of c-kit were negative. Northern blotting results were also c-kit negative for all tested melanoma and tumour cell lines.
All in all, the present study shows a tendancy of downregulation of c-kit receptor expression during progression of malignant melanoma in all applied methods.
Moreover, the results of this study indicate that this loss of expression occurs when tumour cells gain invasion potential into the dermis.

Response of Psoriasis to Interleukin-10 is Associated with Suppression of Cutaneous Type 1 Inflammation, Downregulation of the Epidermal Interleukin-8/CXCR2 Pathway and Normalization of Keratinocyte Maturation
Psoriasis is a chronic inflammatory skin disease in which epidermal hyperplasia results from the release of cytokines by infiltrating type 1 T cells. Up- regulation of endogenous interleukin-10 controls type 1 skin responses in animal models; however, interleukin-10 production is low in psoriatic lesions. Consistent with an important role of interleukin-10 in psoriasis, we and colleagues have recently demonstrated clinical efficacy of subcutaneous administration of recombinant interleukin-10 to affected patients. Here, we studied the effects of interleukin-10 on disease-related inflammatory pathways. Patients were treated with recombinant interleukin-10 over 6 wk in an open-label phase II clinical trial. Tissue was obtained before and after therapy and examined by histology/immunohistochemistry, in situ hybridization, and quantitative real-time reverse transcription–polymerase chain reaction. Ten of 14 patients showed a marked reduction of the clinical disease activity. The clinical response was associated with a significant decrease of cutaneous T cell infiltration and the lesional expression of type 1 cytokines interferon-γ and tumor necrosis factor-α. Interleukin-10 inhibited the epidermal interleukin-8 pathway by downregulating the expression of interleukin-8, its receptor CXCR2, and its inducer interleukin-17, and partially reversed the aberrant keratinocyte maturation defining psoriatic epidermal pathology. Remarkably, there was evidence that genetic factors are involved in the response to interleukin-10 as individual variations in the downregulation of tumor necrosis factor-α were related to the presence of polymorphisms in the tumor necrosis factor-α promoter. These data suggest that excessive production of type 1 cytokines in human skin disease can be counter-regulated by the administration of recombinant interleukin-10. Genotypic analysis may help to identify patients that will preferentially respond to interleukin-10 therapy
Comparison of two different instruments measuring the severity of atopic dermatitis and the health-related quality of life of patients with atopic dermatitis
Die atopische Dermatitis ist eine chronisch-rezidivierende Hauterkrankung mit hoher Prävalenz, die die Lebensqualität der Betroffenen sehr einschränken kann. Zur besseren Betreuung der Patienten wurde an der Universitätshautklinik Tübingen eine Spezialsprechstunde für atopische Dermatitis eingerichtet. Im Rahmen dieser Sprechstunde werden der Schweregrad der Erkrankung mit dem Scoring atopic dermatitis (SCORAD) Index beurteilt und die Lebensqualität des Patienten mit Hilfe des Dermatology Life Quality Index /Children Dermatology Life Quality Index (DLQI/ CDLQI) dokumentiert.
Das Hauptanliegen der vorliegenden Arbeit bestand darin, zu prüfen, ob sich der vergleichsweise zeitaufwendige SCORAD Index durch den DLQI zur Verlaufsbeurteilung und Behandlung der atopischen Dermatitis ersetzen läßt. Weiterhin wurde während des Untersuchungszeitraumes in der Sprechstunde erprobt, ob die Anwendung einer kortikosteroidhaltigen Creme auf Lecithin-Basis mit einer besseren Penetration durch das Stratum corneum im Vergleich zu klassischen Grundlagen therapeutische Vorteile bringen kann. Dieser Fragestellung sollte unter Auswertung der Verlaufsparameter SCORAD und DLQI ebenfalls nachgegangen werden.
Anhand der Entwicklung beider Verlaufsparameter zeigte sich, dass die klassische Therapie mit topischen Kortikosteroiden und pflegenden Externa den Hautzustand und die Lebensqualität des Patienten verbessern kann. Die Verbesserung von Hautzustand und Lebensqualität wurde in etwa gleichem Ausmaß sowohl bei Verwendung eines topischen Kortikosteroids auf Lecithinbasis als auch bei Anwendung eines Kortikosteroids in klassischen Grundlagen beobachtet. Trotz einer signifikanten Assoziation von SCORAD und DLQI fanden sich doch deutliche Unterschiede im Verlauf beider Parameter und ein Ersatz der Verlaufsbeurteilung mittels des SCORAD durch den DLQI erscheint auf Grund der vorliegenden Untersuchungsergebnisse nicht möglich. Daher sollten beide Parameter weiterhin in der Verlaufsbeurteilung der atopischen Dermatitis parallel verwendet werden.Atopic dermatitis is a chronic, relapsing skin disorder with increasing prevalence that severly confines the patient's quality of life. In order to assess novel regimens and allow for close supervision of patients the Dermatology department of Tübingen University established a special outpatient clinic for atopic dermatitis.
Health-related quality of life was assessed by the patient using the Dermatology Life Quality Index/Children Dermatology Life Quality Index (DLQI/ CDLQI). The severity of disease was recorded by the Scoring atopic dermatitis (SCORAD) Index assesd by the dermatologist.
The aim of this investigation was to examine if the time-consuming SCORAD Index can be replaced by the DLQI in order to assess the course of disease during treatment. Furthermore a topical lecithin-based corticosteroid cream was tested against classical bases. The lecithin-based cream was expected to have a higher penetration rate than the classical cream bases.
Regarding the course of both parameters, the DLQI and SCORAD Index, the results reveal that the classical therapy with topical corticosteroids and emollients improves the quality of life and decreases the severity of the atopic dermatitis. There was no difference between the two cream bases concerning the clinical outcome and quality of life.
However, despite a significant correlation between SCORAD Index and DLQI there were marked differences between the courses of both parameters. Therefore we resume, that as long as the relationship between the SCORAD Index and the DLQI is not yet clear both parameters should be used to assess the course of the atopic dermatitis
Luminometric measurement of protein S100 at malignant melanoma
Die Prognose des malignen Melanoms wird erheblich von einer Fernmetastasierung verschlechtert. Ein frühzeitiges Therapieren dieser Metastasierung verbessert die Über-lebenswahrscheinlichkeit und Langzeitprognose. Das Ziel für den Einsatz des Protein S100 als Tumormarker ist daher eine Metastasierung möglichst zeitnah zu erkennen. Für die Protein-S100-Bestimmung aus dem Serum wurde der luminometrische Elecsys®S100-Immunoassay entwickelt. Innerhalb der vorliegenden Arbeit wurde dieses Testverfahren für den Einsatz in der klinischen Routine untersucht. Für dessen Aufgabe in der Melanomnachsorge sind eine gute Reproduzierbarkeit und eine gute Sensitivität und Spezifität entscheidend. Ein weiteres Ziel dieser Arbeit war die Evaluierung eines geeigneten cut-off-Wertes für das Elecsys®-Testverfahren. Es wurde geprüft welcher cut-off-Wert und welche damit verbundene Kombination aus Sen-sitivität und Spezifität für die Verwendung des Testes in der Nachsorge von Melanom-patienten sinnvoll erscheint. Als Vergleichsverfahren diente der Sangtec®100LIA-mat®-Immunoassays, der sich bereits im klinischen Einsatz befindet.
Das in vorliegender Arbeit verwendete Elecsys®S100-System arbeitet nach dem Sand-wich-Prinzip bei welchem 2 Antikörper und ein Mikropartikel an das S100-Protein binden und anschließend das Anlegen von Spannung zu einer messbaren chemoluminis-zenten Emission führt. Es wurden 944 Proben von 449 Patienten aus der Melanom-nachsorge der Universitäts-Hautklinik Tübingen in die Untersuchung eingeschlossen.
Betrachtet man die Ergebnisse beider Testverfahren, liegen die Messungen des Sangtec®100 um 30% höher als die des Elecsys®S100. Auffallend ist, dass 24,2% der Sangtec®100-Messungen das Ergebnis 0,00μg/l S100 ergaben, gegenüber nur 0,2% beim Elecsys®S100. Da auch beim Gesunden von niedrigen physiologischen S100-Konzentrationen im Serum ausgegangen wird, besitzt der Elecsys®S100 in diesem niedrigen Wertebereich ein besseres Differenzierungsvermögen, dessen Bedeutung jedoch bisher noch unklar ist.
Der Elecsys®S100-Assay zeigte auch bei einem Chargenwechsel der Testsubstanzen eine gute Reproduzierbarkeit, erkennbar an den Steigungen der Regressionsgeraden und den entsprechenden Korrelationskoeffizienten.
Bei der Festlegung eines cut-off-Wertes wurde auf eine hohe Spezifität geachtet, um falsch positive Protein-S100-Messungen zu minimieren. Dafür bietet sich ein cut-off-Wert von 0,1µg/l an, der eine Spezifität von 94% mit einer Sensitivität von 41% kombiniert.
Der luminometrische Elecsys®S100-Immunoassay zeichnet sich bei einem empfohlen-en cut-off-Wert von 0,1µg/l durch gute Reproduzierbarkeit, Spezifität und Sensitivität aus. Beim Elecsys®S100 handelt es sich somit um ein gutes Testverfahren zur Messung des Tumormarkers Protein S100 in der Nachsorge von Patienten mit malignem Melanom.Prognosis of malignant melanoma is worsened considerably by distant metastasis. Early treatment of this metastasis improves the likelihood of survival and long term prognosis. Therefore the aim of using protein S100 as tumor marker is the early detection of metastasis. For the protein S100 measurement in serum the luminometric Elecsys®S100-immunoassay was developed. In the underlying study the use of this test in the clinical routine was detemined. For its assignement regarding the follow-up of melanomas a good reproducibility, sensitifity and specificity are of vital importance. Another aim of the present study was to evaluate a suitable cut-off-value for the Elecsys®-assay. It was verified which cut-off-value and which coherent combination of sensitifity und specificity seems to be useful in the follow-up-examination of melanoma patients. The Sangtec®100-LIAmat-immunoassay, which is already in clinical use, served as reference test.
The Elecsys®S100-system used in this study works according to the sandwich-principle, in where 2 antibodies and 1 micro particle link up with the S100-protein and applying voltage then leads to a measurable chemoluminiscent emission. 944 samples of 449 patients of to the melanoma follow-up of the department of dermatology, Universitäts-Hautklinik Tübingen, were included.
Considering the results of both testing procedures, the measurements of the Sangtec®100 are 30% higher than those of the Elecsys®S100. It was conspicuous that 24,2% of Sangtec®100-measurements showed 0,00µg/l S100 as a result, whereas only 0,2% of the Elecsys®S100 showed that result. Proceed from the assumption that a healthy person physiologically contains low concentration of protein S100, the Elecsys®S100 is able to distinguish in the lower values range more precisely. The implication of which is not clear so far.
The Elecsys®S100-assay also showed a good reproducibility by changing of the test kit, recognisable due to the gradient of the regression lines as well as the corresponding correlation coefficient.
When determining the cut-off-value we laid emphasis on high specificity, to minimise falsely postitive test results. Therefore a cut-off-value of 0,1µg/l seems suitable, which combinates a specificity of 94% with a sensitifity of 41%.
At the recommended cut-off-value of 0,1µg/l the luminometric Elecsys®S100-immunoassay distinguishes itsself by good reproducibility, specificity and sensitifity. Therefore the Elecsys®S100 is a good testing procedure for measuring the protein S100 in follow-up of patients with malignant melanoma
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
