Ketogenic diet for the treatment of catastrophic epileptic encephalopathies in childhood.
The ketogenic diet for the treatment of refractory epileptic encephalopathies has been suggested as an early treatment option in very young children. The aim of the present study was to assess the efficacy and tolerability of the ketogenic diet in children younger than 5 years, all affected by different types of catastrophic childhood encephalopathies. The study group is composed of 38 children (22 males and 16 females), aged between 3 months and 5 years, affected by symptomatic partial epilepsy (6) and cryptogenic-symptomatic epileptic encephalopathies (32). Psychomotor delay-mental retardation was present in all of the patients: mild to moderate (9), severe (7), and profound (22). Cerebral palsy was present in 74% of the cases. Children were started on a 4:1 ketogenic diet as ketocal formula alone or supporting about the 80% of the daily caloric amount. Children poorly complying with ketocal milk were shifted to a classic 4:1 ketogenic diet. The average time (months +/- S.D.) on the diet was 10.3 +/- 7.4. All the children initiating the diet remained on it at 1 month and 35 of them (92%) at 3 months, 28 (73.7%) remained on it at 6 months, and 20 (52.7%) at 1 year. At 12-month follow-up, 11 children (28.9%) had a greater than 50% reduction of seizures and the other 9 (23.7%) were seizure-free. Adverse side effects were recorded in 25 of 38 patients (65.8%), including drowsiness, constipation, weight loss, vomiting, gastroesophageal reflux, fever, and hyperlipidemia. This report confirms that severe epileptic encephalopathies are much suitable for the ketogenic diet

Expression of appetite or the motivational drive toward an energy source is a highly regulated phenomenon in vertebrates. In humans, energy intake is not influenced solely by nutritional needs but also by emotional, social and cultural factors; however, over the long-term the cumulative match between energy expenditure and energy intake is tuned with high precision (Edholm, 1977). This phenomenon reflects an active regulatory process, termed energy homeostasis, that promotes stability in the amount of body energy stored in the form of fat, and it is considered essential for rigidly guarding the body weight around a set point. Abnormalities in the onset, periodicity, duration and magnitude of eating episodes generally underlie augmented appetite (Stunkard, 1996; Bray et al., 1990). Increased appetite, whether temporary, as seen clinically in transient bingeing, or permanent, invariably culminates in an increased rate of body weight gain and obesity (Kalra, 1997; Wadden, 1993; Perri et al., 1993; Bray, 1996). On the other hand, anorexia due to psychobiological causes (Stewart-Agras et al., 1984; Crisp, 1984) or in response to acute and chronic infections, inflammation and trauma is followed by severe loss of body weight (Moldawer et al., 1988; Plata-Salaman et al., 1991; Grunfeld et al., 1992)
The Aresys FF-SAR Service for Cryosat-2 at ESA GPOD
The Fully-Focused SAR (FF-SAR) processing, introduced in Egido and Smith (2016) allows obtaining a maximum resolution of 0.5 m in the along-track direction. It provides significant benefits for inland water altimetry investigations allowing the successful investigation of very small rivers and canals (Kleinherenbrink, 2020) that are typically harder to be analysed by using unfocused Delay-Doppler SAR (DD-SAR) data (about 300 m resolution in the along-track direction).In its development, two major limitations were associated with the FF-SAR processing: 1) the presence of evenly spaced high sidelobes in the Point Target Response (PTR) due to the closed-loop burst mode implemented in Sentinel-3 & Cryosat-2 altimeter payloads, used for initial FF-SAR investigations, and 2) the heavy computational burden with respect to the unfocused DD-SAR processing. The first limitation can be overcome by designing the radar system differently adopting an open-loop transmission scheme as, for instance, the one implemented in the altimeter payload of the Sentinel-6 Michael Freilich mission, launched on 21 November 2020. The second limitation has been addressed in research works following Egido and Smith (2016) indicating that an improvement in terms of computational burden can be achieved by adopting algorithms in the frequency domain (Guccione et al., 2018).Being the role of FF-SAR for future inland water altimetry well understood, along with the possibility to see it implemented with reduced drawbacks during the Sentinel-6 Michael Freilich mission, a collaboration has started between the ESA GPOD Team, already hosting the successful SARvatore services portfolio for unfocused SAR & SARin altimetry, and Aresys. Aresys has developed a generic FF-SAR prototype processor, that is able to process data acquisition from different instruments and exploiting the frequency-domain Omega-K algorithm (Guccione et al., 2018 & Scagliola et al., 2018). The Aresys's FFSAR prototype processor for CryoSat-2 allows users to process, on line and on demand, low-level CryoSat FBR products in SAR mode up to FF-SAR Level-1 products with self-customized options. Additionally a wide set of processing parameters is configurable, allowing as an example to select the along-track resolution or to obtain FFSAR multilooked waveforms at the desired posting rate. The collaboration led to the creation of a new service for the processing of CryoSat Baseline D data in FF-SAR mode. Users will be able to select the following options: 1) range oversampling factor, 2) bandwidth factor (responsible for the along-track resolution value) and 3) multilook posting rate (1Hz-500Hz). Geophysical corrections and L2 estimates from both a threshold peak retracker and an ALES-like subwaveform retracker are part of the output package. In preliminary open ocean analyses, very good results on SSH noise have been obtained by the ALES-like subwaveform retracker.In this presentation, the Aresys FF-SAR prototype processor is described and the outcome of some preliminary validation activities, performed by a group of altimetry researchers, is reported. The service is scheduled to open to all GPOD/SARvatore users in the first semester of 2021. Future evolutions should include the extension of the service to Sentinel-3 and Sentinel-6 data.Astrodynamics & Space Mission

Conversion of hydroxycinnamic acids by Furfurilactobacillus milii in sorghum fermentations: Impact on profile of phenolic compounds in sorghum and on ecological fitness of Ff. milii
The conversion of phenolic compounds by lactobacilli in food fermentations contributes to food quality. The metabolism of phenolics by lactobacilli has been elucidated in the past years but information on the contribution of specific enzymes in food fermentations remains scarce. This study aimed to address this gap by disruption of genes coding for the hydroxycimmanic acid reductase Par1, the hydroxycinnamic acid decarboxylase Pad, the hydrocinnamic esterase EstR, and strains with disruption of all three genes in Furfurilactobacillus milii FUA3583. The conversion of phenolics by Ff. milii and its isogenic mutants in sorghum fermentations was studied by LC-UV and LC-UV-MS/MS analyses. Ff. milii FUA3583 converted hydroxycinnamic acids predominantly with Par1. Vinylphenols were detected only in mutants lacking par1. A phenotype for the estR defective mutant was not identified. The formation of pyrano-3-deoxyanthocyanidins was observed only after fermentation with strains expressing Pad. Specifically, formation of these compounds was low with Ff. milii FUA3583, substantially increased in the Par1 mutant and abolished in all mutants with disrupted pad. Competition experiments with Ff. milii FUA3583 and its isogenic mutants demonstrated that expression of one of the two metabolic pathways for hydroxycinnamic acids increases the ecological fitness of the strain. Disruption of EstR in a Δpar1Δpar2Δpad background improved ecological fitness, indirectly demonstrating a phenotype of the esterase in Ff. milii. The documentation of the functionality of genes coding for conversion of hydroxycinnamic acids may support the selection of starter cultures for improved quality of fermented cereal products
Determinants of response to inhaled extrafine triple therapy in asthma: analyses of TRIMARAN and TRIGGER
Background: A number of single-inhaler triple therapies are being developed for asthma, including the extrafine formulation of beclometasone dipropionate (BDP), formoterol fumarate (FF), and glycopyrronium (G). Given asthma is a heterogenous disease, we investigated whether the clinical response to the addition of the long-acting muscarinic antagonist component within inhaled triple therapy was impacted by a range of clinical characteristics. Methods: These were pre-specified and post-hoc sub-group analyses of TRIMARAN and TRIGGER, which were double-blind, 52-week studies comparing medium-strength (100/6/10 μg; TRIMARAN) and high-strength (200/6/10 μg; TRIGGER) BDP/FF/G with the respective BDP/FF strengths in adults with uncontrolled asthma and a history of ≥ 1 exacerbation. Co-primary endpoints were pre-dose forced expiratory volume in 1 s (FEV1) at Week 26 and the rate of moderate-to-severe exacerbations over 52 weeks. Key secondary endpoints: peak FEV1 at Week 26 and average morning peak expiratory flow over the first 26 weeks in each study, and severe exacerbation rate over 52 weeks (pooled data). Results: Baseline clinical characteristics (pre-specified analyses) had no consistent effect on the lung function improvements with BDP/FF/G. For the exacerbation endpoints, sub-groups with higher reversibility gained greatest relative benefit from BDP/FF/G versus BDP/FF. In post-hoc analyses with patients sub-grouped by screening blood eosinophil values, in TRIMARAN the greatest relative effect of BDP/FF/G versus BDP/FF on the lung function endpoints was in the ≤ 300 cells/μL group; in TRIGGER, eosinophil levels did not markedly influence the relative efficacy of BDP/FF/G versus BDP/FF. Eosinophil levels did not influence relative efficacy on moderate-to-severe or severe exacerbations. Conclusion: Overall, the relative efficacy of extrafine BDP/FF/G versus BDP/FF was not influenced by a range of clinical characteristics. However, some patient sub-groups gained additional benefit from BDP/FF/G for certain endpoints. In particular, for exacerbations the relative efficacy of BDP/FF/G was greater in more reversible patients. According to the sign of g between b and λ, we obtain results about existence, uniqueness and boundedness of solutions to this boundary value problem, that we denote by . In this way, we pursue and complete the study done in 2008
