Espacios Moduli de Conexiones Planas en G-fibrados Principales
Los espacios moduli de conexiones planas surgen en diferentes áreas. Por ejemplo enfísica, estos espacios aparecen al estudiar las ecuaciones de Y ang-Mills, que son una generalización de las ecuaciones de Maxwell del electromagnetismo, en una variedad suave de dimensión 2. Las conexiones en un G-fibrado principal P ! M nos indican una manera explícita de hacer transporte paralelo sobre P. Es así como actualmente muchos tópicos de investigación en teoría cuántica de campos están relacionados con dichos espacios. En este trabajo estamos interesados en estudiar estos espacios moduli desde un punto de vista meramente topológico, como se describe a continuación: Dado un G-fibrado principal _ : P ! M, una conexión en P es un subfibrado H de TP tal que para todo p 2 P, TpP = Hp _ker(d_p) y Hpg = dRg(Hp) y a esta conexión le podemos asociar de manera única una 1-forma _ en P con valores en g, el álgebra de Lie de G. Ahora, la 2-forma F_ := d_ + 1 2 [_; _] es llamada la curvatura de _, y decimos que H es una conexión plana si su curvatura es trivial, es decir, si F_ = 0. Por otra parte, si _1 : P ! M y _2 : Q ! M son G-fibrados principales sobreM, con conexiones H1 y H2 respectivamente, decimos que estos fibrados son gauge equivalentes si existe una función suave f : P ! Q que es G-equivariante tal que _2 _ f = _1 y dfp(H1 p ) = H2 f(p) para todo p 2 P. Lo anterior nos permite definir el Espacio Moduli de Conexiones Planas en G- fibrados Principales que será nuestro principal objeto de estudio en esta tesis de la siguiente manera. De manera informal el espacio moduli de conexiones planas en G-fibrados principales sobre una variedad M, es un espacio que parametriza todas las clases gauge de conexiones planas en fibrados sobre M. De manera formal, dado un grupo de Lie G y una variedad suave y conexa M, definimos el espacio moduli de conexiones planas en G-fibrados principales sobre M, denotado por M(M;G) donde P es un G-fibrado principal sobre M, H es una conexión plana en P y _ es la relación de equivalencia gauge de fibrados principales. El primer objetivo principal de esta tesis es identificar el espacio moduli M(M;G) con el espacio de representaciones Rep(_1(M);G) = Hom(_1(M);G)=Gconj, donde G actúa por conjugación. Este trabajo está dividido de la soguiente manera. En el primer capítulo exploramos algunas definiciones básicas como lo son los fibrados principales, las conexiones y la curvatura. En el segundo capítulo identificamos el espacio moduli M(M;G) con el espacio de representaciones Rep(_1(M);G). En el tercer capítulo exploramos los espacios de representaciones y consideramos algunos ejemplos. En particular determinamos el espacio moduli M(M;G) para todas las variedades compactas con grupo fundamental abeliano y para G = U(n) y G = Sp(n). Distinct associations of DSM-5 Somatic Symptom Disorder, the Diagnostic Criteria for Psychosomatic Research-Revised (DCPR-R) and symptom severity in patients with irritable bowel syndrome
Objective: The clinical management of high symptom severity is a challenging task with patients with functional somatic disorders. We investigated the extent to which DCPR-revised (DCPR-R) syndromes and the DSM-5 category of Somatic Symptom Disorder (SSD) were able to predict symptom severity in 203 consecutive tertiary care patients with irritable bowel syndrome (IBS). Method: Semistructured interview were used for assessing DCPR-R and validated scales for SSD (combining PHQ-12 and WI-7), severity of symptoms (IBS-SSS), psychological distress (HADS), and psychosocial functioning (SF-12). Results: Compared to moderate severity (IBS-SSS = 175–300), patients in the high range of severity (IBS-SSS > 300) had significantly more DCPR-R syndromes (particularly alexithymia and persistent somatization), higher psychological distress, and poorer psychosocial functioning, but showed no difference for SSD. DCPR-R, particularly alexithymia and persistent somatization, significantly and independently predicted IBS severity by explaining 18.5% of the IBS-SSS variance with large effect size (d = 1.18), after controlling for covariables. Conversely, SSD was not able to significantly predict IBS severity. Conclusions: This study highlights the need of an integrative approach in the medical setting. Psychosomatic factors play a relevant role in the individual perception of symptom severity and should be carefully evaluated for clinical management of functional syndromes

OBJECTIVES: Radiofrequency thermal ablation (RF) and percutaneous ethanol
injection (PEI) have been employed in the treatment of small hepatocellular
carcinoma (HCC) as curative treatments. The aim of the study was to review the
available evidence comparing RF to PEI for small HCC.
METHODS: Search strategy: Cochrane, MEDLINE, CANCERLIT, and ENBASE databases were
used. Selection criteria: randomized clinical trials evaluating RF vs. PEI. Data
were extracted from each randomized controlled trial (RCT). Primary outcomes were
overall survival and local recurrence. Meta-analysis software was used and risk
differences (RDs) and their 95% confidence intervals and Q-test for heterogeneity
were calculated.
RESULTS: Five RCTs were identified including 701 patients. The overall survival
was significantly higher in patients treated with RF than in those treated with
PEI (RD 0.116, 95% CI 0.173/0.060; heterogeneity not present). Local recurrence
rate is significantly higher in patients treated with PEI than in those treated
with RF. In the RF group the 1, 2, and 3 years cancer-free survival rates were
significantly better than in the PEI-treated patients (respectively: RD 0.098-95%
CI 0.006/0.189; heterogeneity P=0.57; RD 0.187, 95% CI 0.082/0.293; heterogeneity
P=0.98; RD 0.210, 95% CI 0.095/0.325; heterogeneity P = 0.78). A small number of
adverse events were reported in the two treatments.
CONCLUSIONS: RF ablation is superior to PEI in the treatment of small HCC with
respect to overall survival, 1, 2, and 3 years survival rates, 1, 2, and 3
cancer-free survival rates, and tumor response. RF shows a significantly smaller
risk of local recurrence
