Recent advances and unmet needs in the treatment of drug-resistant tuberculosis
IntroductionDrug-resistant tuberculosis (DR-TB) still represents a critical global health threat despite recent therapeutic advances. «Le chevalier errant», els Saluzzo del Montferrat i l’autoria del «Curial»
RESUM: Es vol cridar l’atenció de la crítica literària catalana sobre Le chevalier errant (Torí-París, 1394-1405) del marqués de Saluzzo (l’aventura al·legoritzada, autobiogràfica i dantesca, d’un cavaller caigut i redimit, a través de tres llibres: d’Amor, de Fortuna i de Coneixença, respectivament), obra conservada al Montferrat des del 1435. Pogué servir de precedent i motiu d’inspiració per a l’autor de Curial e Güelfa (Nàpols-Milà?, ca. 1445-1448) a l’hora de concebre la novel·la cavalleresca catalana com un relat on el protagonista travessa per tres llibres també: d’Amor, de Cavalleria i de Virtut. A més de coincidències estructurals, argumentals i onomàstiques, és possible que Enyego d’Àvalos –a qui atribuïm l’autoria del Curial– haguera tingut accés a la biblioteca Saluzzo de Casale Monferrato, pels vincles que mantingué amb bibliòfils montferratesos mentre residí a la cort de Milà (1435-1447), on curava dels interessos del rei d’Aragó i Nàpols, Alfons el Magnànim.MOTS CLAU: Curial e Güelfa, Enyego d’Àvalos, Montferrat, Le chevalier errant, marqués de Saluzzo. ABSTRACT: We would like to encourage the Catalan literary criticism to pay attention to Le chevalier errant (Turin-Paris, 1394-1405) by the marquesss of Saluzzo (the allegorised, autobiographical and Dantesque adventure of a fallen and redeemed knight all over three books: about Love, Fortune and Knowledge, respectively), a work kept in Monferrato since 1435. It could serve as a precedent and inspiration motive for Curial e Güelfa’s author (Naples-Milan?, ca. 1445-1448) when he conceived this Catalan chivalric romance as a story where the protagonist goes also through three books: about Love, Chivalry and Virtue. Apart from structural, plot and onomastic coincidences, it is possible that Enyego d’Àvalos –to whom we attribute the Curial’s authorship– could have access to Saluzzo de Casale Monferrato’s library, according to the bonds he kept with bibliophiles from Montferrat during his stay in the court in Milan (1435-1447), where he looked after the interests of the king of Aragon and Naples, Alfonso the Magnanimous. L’attività architettonica più interessante di Filippo Buonocore si svolge a Napoli nel periodo della produzione artistica che va dal 1726 alla fine degli anni quaranta. Coetaneo di Ferdinando Sanfelice e di Domenico Antonio Vaccaro, in questi anni egli ricopre importanti incarichi sia di committenza religiosa, che nobiliare. Il saggio esamina in particolare gli interventi effettuati per il duca Agostino Saluzzo nella chiesa di San Giorgio dei Genovesi e nel Palazzo Corigliano in piazza San Domenico Maggiore, in cui emerge la piena adesione al Rococò napoletano. Geotrichum capitatum Invasive Infection Early After Liver Transplant
Geotrichum capitatum is a rare fungal pathogen that has infrequently affected immunocompromised patients with onco-hematologic diseases. Geotrichum capitatum invasive infection has been associated with poor prognosis, with a mortality rate ranging from 50% to 90%. Here, we report the first case of Geotrichum capitatum invasive fungal infection in a liver transplant recipient from an unrelated deceased donor, who was effectively treated with amphotericin B and voriconazole. Uomini illustri a Siviglia e a Saluzzo: modelli umanistici e rinascimentali nella Casa di Pilatos e Casa Cavassa
In the first mid of 16th century the Casa de Pilatos in Seville and the Casa Cavassa in Saluzzo, in north Italy, reveal a similar artistic and cultural patronage. In both cases, the commitments of the two buildings, don Fadrique Enriquez de Ribera, Marquis of Tarifa, and Francesco Cavassa, main functionary of the Marquisate of Saluzzo, promote a whole decoration of their palaces clearly inspired by models drawns from Humanistic culture and Italian Renaissance art, more precisely the sculpure decoration of the Certosa of Pavia in Lombardy. In particular, in both the palaces one of the most important painted decoration is devoted to the humanistic theme of the Uomini illustri (Wase or Famous Men), following a very similar iconography. The renaissance style showed by the two palaces is dramatically in contrast, on one hand, with that of Seville still dominated by the Arabic culture and, on the other hand, that of Saluzzo at that time caracterized by the Gothic art.
As Vicente Lleò Cañal has demonstrated, the Marquis of Tarifa committed the plastic and painted decoration of his palace after coming back of a journey in the Holy Land, made between 1518 and 1520, during which he visited many cities and was fascinated above all by the Certosa of Pavia and Genoa. Therefore he committed to Genoan sculptores the graves for his parents whereas the painted decoration of the Galleria of his palace was realized by Spanish painters but the subjct, the Uomini illustri – Cicerone, Creso, Tito Livio, Orazio, Cornelio Nepote e Quinto Curzio are still visible – is based on a topos of the Italian Humanistic culture. The composition of these figures reminds closely that of the Uomini illustri in Casa Cavassa, with each figure seating in a nich accompanied by an inscription drawn by his own work, which is a very unique solution (in Italy, where this subject was common, usually the inscription was by an author different from the personage represented). Also the plastic decoration committed by Francesco Cavassa was made by an artist, Matteo Sanmicheli, who reveal a style derived from the Certosa of Pavia. Both the palaces, therefore, demonstrate the intention of the patrons to provide the city with an example of cultural and artistic renewer inspired to Classics, throw models uptodate with the Italian Renaissance art.Un interesante testimonio del paralelismo de la floritura de la cultura humanística y renacentista italiana y española, lo constituyen la Casa Cavassa de Saluzzo, en el Piamonte suroccidental, y la Casa de Pilatos de Sevilla. En ambos casos hay dos promotores, don Fadrique Enríquez de Ribera, marqués de Tarifa, y Francesco Cavassa, vicario general del marquesado de Saluzzo, quien patrocinó la compleja decoración de su palacio, claramente inspirada en los modelos de la cultura humanística y del arte renacentista italiano, concretamente la decoración escultórica de la Cartuja de Pavía, en Lombardía. También en los dos recintos se destaca la decoración pictórica, dedicada al tema humanístico de los Uomini illustri, de acuerdo a una iconografía muy similar. El estilo renaciente reflejado en los dos palacios contrasta con lo que en Sevilla es la tradición mudéjar, aún imperante, y con el gótico aún perviviente en Saluzzo.
Como Vicente Lleó Cañal ha demostrado, el marqués de Tarifa encargó la decoración escultórica de su palacio a la vuelta del viaje a Tierra Santa, realizado entre 1518 y 1520, durante el cual tuvo la oportunidad de conocer la ciudad de Génova y la Cartuja de Pavía. Por lo que encargó a escultores genoveses los sepulcros de sus antepasados, en tanto que la decoración pictórica de la galería de su palacio fue realizada por pintores españoles, aunque el tema, los Uomini illustri (se pueden ver Cicerone, Creso, Tito Livio, Orazio, Cornelio Nepote y Quinto Curzio) está basado en el topos de la cultura humanística italiana. La composición de estas figuras está próxima a los Uomini illustri de la Casa Cavassa, con cada personaje sentado dentro de un nicho y con una inscripción, constituyendo un unicum iconográfico, aunque la decoración plástica encargada por Francesco Cavassa fue realizada por un artista, Matteo Sanmicheli, que muestra un estilo derivado de la Cartuja de Pavia. Ambos palacios, en los que sus respectivos comitentes tratan de ofrecer a las ciudades en que se encuentran un ejemplo de renovación cultural y artístico, inspirado en el clasicismo, a través de modelos actualizados del arte renacentista italiano.La Casa Cavassa di Saluzzo, nel Piemonte sud-occidentale, e la Casa de Pilatos di Siviglia sono un'interessante testimonianza del parallelismo tra la fioritura della cultura umanistica e rinascimentale italiana e spagnola. In entrambi i casi sono due i promotori, don Fadrique Enriquez de Ribera, marchese di Tarifa, e Francesco Cavassa, vicario generale del marchesato di Saluzzo, che sponsorizzarono la complessa decorazione del suo palazzo, chiaramente ispirata ai modelli della cultura umanistica e dell'arte rinascimentale italiana, in particolare alla decorazione scultorea della Certosa di Pavia in Lombardia. Anche nei due recinti è presente la decorazione pittorica, dedicata al tema umanistico degli Uomini illustri, secondo un'iconografia molto simile. Lo stile rinascimentale che si riflette nei due palazzi contrasta con quella che è la tradizione mudéjar ancora prevalente a Siviglia e con il gotico che ancora sopravvive a Saluzzo.

Entrambi i palazzi, in cui i rispettivi committenti cercavano di offrire alle città in cui si trovavano un esempio di rinnovamento culturale e artistico, ispirato al classicismo, attraverso modelli aggiornati dell'arte rinascimentale italiana.
SARS-CoV-2 Antibody Rapid Tests: Valuable Epidemiological Tools in Challenging Settings
During the last year, mass screening campaigns have been carried out to identify immunological response to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and establish a possible seroprevalence. The obtained results gained new importance with the beginning of the SARS-CoV-2 vaccination campaign, as the lack of doses has persuaded several countries to introduce different policies for individuals who had a history of COVID-19. Lateral flow assays (LFAs) may represent an affordable tool to support population screening in low-middle-income countries, where diagnostic tests are lacking and epidemiology is still widely unknown. However, LFAs have demonstrated a wide range of performance, and the question of which one could be more valuable in these settings still remains. We evaluated the performance of 11 LFAs in detecting SARS-CoV-2 infection, analyzing samples collected from 350 subjects. In addition, samples from 57 health care workers collected at 21 to 24 days after the first dose of the Pfizer-BioNTech vaccine were also evaluated. LFAs demonstrated a wide range of specificity (92.31% to 100%) and sensitivity (50% to 100%). The analysis of postvaccination samples was used to describe the most suitable tests to detect IgG response against S protein receptor binding domain (RBD). Tuberculosis (TB) therapy was identified as a potential factor affecting the specificity of LFAs. This analysis identified which LFAs represent a valuable tool not only for the detection of prior SARS-CoV-2 infection but also for the detection of IgG elicited in response to vaccination. These results demonstrated that different LFAs may have different applications and the possible risks of their use in high-TB-burden settings.IMPORTANCE Our study provides a fresh perspective on the possible employment of SARS-CoV-2 LFA antibody tests. We developed an in-depth, large-scale analysis comparing LFA performance to enzyme-linked immunosorbent assay (ELISA) and electro-chemiluminescence immunoassay (ECLIA) and evaluating their sensitivity and specificity in identifying COVID-19 patients at different time points from symptom onset. Moreover, for the first time, we analyzed samples of patients undergoing treatment for endemic poverty-related diseases, especially tuberculosis, and we evaluated the impact of this therapy on test specificity in order to assess possible performance in TB high-burden countries
How We Treat Drug-Susceptible Pulmonary Tuberculosis: A Practical Guide for Clinicians
Tuberculosis (TB) remains one of the leading causes of morbidity and mortality worldwide and pulmonary TB (PTB) is the main variant responsible for fueling transmission of the infection. Effective treatment of drug-susceptible (DS) TB is crucial to avoid the emergence of Mycobacterium tuberculosis-resistant strains. In this narrative review, through a fictional suggestive case of DS PTB, we guide the reader in a step-by-step commentary to provide an updated review of current evidence in the management of TB, from diagnosis to post-treatment follow-up. World Health Organization and Centre for Diseases Control (CDC) guidelines for TB, as well as the updated literature, were used to support this manuscript
