    Seismic Response of a Strategic Building in Catania (Italy) by 2-D FEM Analysis

    AbstractIn the last 900 years, the east coast of Sicily has been struck by several disastrous earthquakes. The earthquakes occurred in 1169, 1542, 1693, 1818, 1908 and more recently in 1990, with estimated magnitudes, ranging from less than 5.0 to greater than 7.0, testify the high level of hazard in this area. Many towns were destroyed during the above-mentioned earthquakes. The high level of seismicity that affects the area, together with the considerable high density of inhabitants living in its urban area, contribute to classify it as one of the area having the highest seismic risk in Italy. It is also not negligible the high potential damage to which its historic-architectural patrimony could undergo even at moderate magnitudes. Many researches show that the site seismic response is strongly influenced by stratigraphic and topographic features. Site response can reduce or amplify the earthquake induced ground motion. In this study seismic response of strategic building located in Catania has been done, and main results are presented

    Male patients, especially the young, are at a higher risk of recurrent venous thromboembolism (RVTE) than females. Recent scientific reports show the use of D-dimer does not help predict RVTE risk in males. In the present report, we reviewed the data obtained in the DULCIS study (main report published in Blood 2014), focusing on D-dimer results recorded in non-elderly patients of both genders included in the study, and their relationship with RVTE events occurring during follow-up. Using specifically designed cutoff values for positive/negative interpretation, serial D-dimer measurements (performed during warfarin treatment and up to 3 months after discontinuation of anticoagulation) in 475 patients (males 57.3%) aged ≤ 65 years were obtained. D-dimer resulted positive in 46.3% and 30.5% of males and females, respectively (p = 0.001). Following management procedure, anticoagulation was stopped in 53.7% of males and 69.5% of females, who had persistently negative D-dimer results. The rate of subsequent recurrent events was 1.7% (95% CI 0.5-4.5%) and 0.4% (95% CI 0-2.5%) patient-years in males and females, respectively, with upper limits of confidence intervals always below the level of risk considered acceptable by international scientific societies for stopping anticoagulation (< 5%). In conclusion, using sensitive quantitative assays with specifically designed cutoff values and serial measurements during and after discontinuation of anticoagulation, D-dimer testing is useful to predict the risk of RVTE and is of help in deciding the duration of anticoagulation in both male and female adult patients aged up to 65 years

    Singling out the place where rules apply: materials from the discussion on Panini’s description of substitution

    This paper aims at taking Panini’s substitution descriptive method out of its supposed limited domain of allomorphy (Kiparsky 2009: 86), to show that it is rather part of a broader conceptualization of linguistic units in spatial terms, i.e. as “places” where “things” i.e. linguistic phenomena described by rules, happen (following the lead indicated by Kahrs (1998: 184)). In particular, while discussing on the thirteen dosavarttikas (vts. 18–30) on A. 1.1.56, by mainly relying on the textual dimension of the As..ta ̄dhya ̄y ̄ı and on the usus scribendi of the author, we advanced that Pa ̄n.ini’s linguistic units are not endowed with a mandatory fixed status, a misunderstanding which instead prevented ancient and modern interpreters (e.g. Joshi and Rood- bergen 1985, 1990) from solving these supposed shortcomings in the mechanism of substitution. On the contrary, he might have envisioned the different units as scalar categories, whose prototypes were verbal bases and affixes on the one hand and the single meaningless phoneme on the other. Consistently, Panini’s explicit classification of rules again is not abstract and functional but based upon the explicit mention of a relevant linguistic “place” and the way in which it is, each time, described: Panini’s strategy encompasses both devices to shift the status of a linguistic unit from phone to morph and vice versa

    Recurrent Thrombotic Events after Discontinuation of Vitamin K Antagonist Treatment for Splanchnic Vein Thrombosis: A Multicenter Retrospective Cohort Study

    It is generally recommended that patients with splanchnic vein thrombosis (SVT) should receive a minimum of 3 months of anticoagulant treatment. However, little information is available on the long-term risk of recurrent thrombotic events. The aim of this study was to evaluate the risk of venous and arterial thrombosis after discontinuation of vitamin K antagonist (VKA) in SVT patients. Retrospective information from a cohort of SVT patients treated with VKA and followed by 37 Italian Anticoagulation Clinics, up to June 2013, was collected. Only patients who discontinued VKA and did not receive any other anticoagulant drug were enrolled in this study. Thrombotic events during follow-up were centrally adjudicated. Ninety patients were included: 33 unprovoked SVT, 27 SVT secondary to transient risk factors, and 30 with permanent risk factors. During a median follow-up of 1.6 years, 6 venous and 1 arterial thrombosis were documented, for an incidence of 3.3/100 patient-years (pt-y). The recurrence rate was highest in the first year after VKA discontinuation (8.2/100'pt-y) and in patients with permanent risk factors (10.2/100'pt-y). Liver cirrhosis significantly increased the risk of recurrence. In conclusion, the rate of recurrent vascular complications after SVT is not negligible, at least in some patient subgroups

    Direct speech, subjectivity and speaker positioning in London English and Paris French

    This paper examines functional similarities and differences in the use of pragmatic features – in particular quotatives and general extenders – on the right and left periphery of direct quotations. This comparative study, based on the analysis of a contemporary corpus of London English and Paris French (MLE – MPF) , finds that the form and frequency of these particles tend to vary not only with respect to social factors such as speakers’ age and gender, but also with respect to the different pragmatic functions they come to perform in different interactional settings. The contemporary data is analysed both qualitatively and quantitatively to show how different variants position the speaker in relation to: i) the content of the quote, ii) the interlocutors, iii) the presumed author of the quote.     Neste trabalho busca-se explicitar como o discurso de promoção de autonomia se instaura e se reproduz entre os diversos atores do campo da saúde mental, especialmente os trabalhadores da saúde mental, que mais o proferem como forma de embasar sua atuação. Para tal tarefa, explora-se o modo como a noção de autonomia é sustentada institucionalmente e como ela atinge de forma incisiva os usuários de serviços de saúde mental, sobre os quais recaem as ações e as práticas institucionais. Ao longo do percurso da pesquisa, considera-se que a psicanálise, que pensa o homem como sujeito do grupo e do vínculo, consiste no mais adequado aporte teórico para discutir os processos que envolvem a temática, por implicar as dimensões da subjetividade, do social, do grupal e do institucional. Avalia-se ser fundamental compreender como a autonomia do sujeito é condicionada desde sua constituição, que ocorre na relação com o outro e o mundo, definindo sua realidade intrapsíquica e intersubjetiva. A partir da obra de Kaës e do conceito de alianças inconscientes, discute-se como o sujeito se constitui, sendo a relação entre mundo interno e externo indissociável. As alianças exercem a função de intermediário. Assim, para compreender como o sujeito se insere no mundo, verifica-se o imperativo dos contratos e pactos, mais especialmente o contrato narcísico e o pacto denegativo, por um processo de economia psíquica que visa garantir sua sobrevivência e a convivência possível para cada sujeito. É por meio dessas modalidades de alianças inconscientes, que o sujeito busca a autopreservação e a perpetuação do grupo, da instituição e da espécie. Com a referência da autora Piera Aulagnier, empreende-se uma imersão nos processos de constituição do sujeito, que se define como tal, condicionado ao vínculo com a mãe, representante primeira do mundo, que lhe apresenta suas possibilidades de representação sobre ele. Assim, as possibilidades de subjetivação, de construção de identidade e de agir do sujeito são limitadas e delimitadas pelo que a realidade, anterior a ele, lhe concede. O intuito do estudo aqui apresentado consiste na promoção de uma discussão que possa trazer possibilidades de análise e de ruptura dos discursos instituídos e alienadamente reproduzidos. Por meio da elaboração dos processos psíquicos envolvidos na trama institucional, é possível se pensar na superação de tal quadro por cada sujeito que se insere nessa realidade. Considerando que a busca pelo saber, pelo novo, é inerente a todo sujeito, discute-se o processo de criação que consiste na tensão entre o estabelecido e seu diferente, e que pode surgir, ao se estabelecer a dialética entre ambos, superando o estado dilemático que impõe uma paralisação nessa relação. Assim, a problemática da autonomia desloca-se para a de criaçãoIn the present work, we aim to explicit how the discourse of autonomy promotion is established and reproduced by several actors in the mental health field, especially the workers, who utter it most of the time as a way to support their actions. In order to do so, we have analysed how the notion of autonomy is sustained by institutions and how it sharply reaches the users of mental health services, upon whom institutional actions and practices fall. Throughout the research, we have considered that the type of psychoanalysis that interpret an individual as a part of a group consists in the most appropriate theoretical framework to discuss the processes that encompass the theme, as it considers the dimensions of subjectivity, society, groups and institutions. We think it is fundamental to understand how the autonomy of the subject has been shaped since his constitution, which occurs in the relationship with the other and the world, thus defining his intrapsychic and intersubjective reality. Based on René Kaës\'s work and the concept of unconscious alliances, we have discussed the way by which the subject is built, considering the relationship between external and internal worlds inseparable. The alliances play the role of intermediary. In order to understand how the subject enters the world, we stress the importance of the imperative of contracts and pacts, especially the narcissist contract and the denegativo pact, by means of a process of psychic economy that is aimed at guaranteeing each subject possible ways of survival and interactions. By means of these types of unconscious alliances, the subject seeks self-preservation and the perpetuation of the group, the institution and the species. Based on Piera Aulagnier, we have immersed ourselves in the processes of the constitution of the subject, which is determined by the relationship with the mother, the first representative of the world, who presents him his possibilities of representation of the world. So, his possibilities of subjectivation, identity building and action are limited and shaped by all the aspects that the reality, which comes before the subject, gives to him. The aim of this work consists in stimulating a discussion that may bring possibilities of analysis and ruptures as well of the discourses that are established and reproduced in an alienated way. By elaborating the psychic processes that are involved in the institutional framework, it is possible to identify the possibility that each subject who participates in this reality has to overcome this context. Considering that the search for knowledge and innovation is inherent in every subject, we have discussed the process of creation that consists in a tension between the established situation and different scenarios, which may arise when there is a dialectical tie between them, thus overcoming the dilematic state that imposes a stoppage on this relationship.     l contributo, costituisce il terzo capitolo del volume ed è strutturato in due paragrafi in cui si torna a riflettere sull’attribuzione del progetto architettonico dell'interessantissima Collegiata del San Giovanni Battista Morbegno, edificata tra il 1680 ed il 1714, oggi quasi completamente restaurata dalla Fondazione Isabel & Balz Baechi. Necessariamente in questo contesto si riprendono gli studi già svolti da Simona Benedetti, pubblicati nel 2019, in cui si è individuato il progettista dell'opera valtellinese nella persona di Gerolamo Quadrio, architetto del Duomo di Milano dal 1658 al 1679, anno della sua morte. Qui si sviluppano ulteriori analisi riferite all’attività del progettista milanese, che ha concepito la splendida spazialità architettonica tra le ultimissime opere della sua carriera. Vengono proposte nell’ultimo paragrafo puntuali analisi comparative tra le sue opere, individuando tra queste le più significative per l’attribuzione del progetto della Collegiata di San Giovanni Battista a Morbegno.The research represents the third chapter of the volume specifically dedicated to the history of the church of St. John the Baptist in Morbegno. Built between 1680 and 1714, this celebrated ‘Collegiata’ had been recently restored by the Isabel & Balz Baechi Foundation, which also supported a historical in-depth analysis. The paper consists of two paragraphs, in which we return to reflect on the attribution of the architectural design. Starting from the discoveries carried out by the Author in 2019 and already published, in which the designer of the Valtellina church was identified with Gerolamo Quadrio, chief architect of Milan’s Cathedral from 1658 to 1679, this new detailed work discusses the whole activity of the professional, who seemed to have conceived the architectural spatiality of St. John the Baptist at the very end of his career. By comparing his projects, it emerges the strictly existing relationship between his ‘modus operandi’ and the Morbegno church, confirming his participation in the invention and the following construction

    Por meio do material clínico derivado do processo analítico de uma paciente histérica, esta dissertação tem o objetivo de desvendar como se dá a problemática identificatória relativa ao feminino que se faz presente em um caso específico de histeria feminina. A compreensão e a análise do material clínico, que será utilizado na construção desta pesquisa, encontram seus fundamentos teórico-clínicos na teoria freudiana e nas contribuições de Piera Aulagnier, autora contemporânea que segue as orientações de Sigmund Freud, ao lado de influências de Jacques Lacan. Para viabilizar esta pesquisa, direcionei meu trabalho clínico ao atendimento psicanalítico de seis pacientes com organização histérica e, no decorrer do processo de análise destas pacientes, foram surgindo questões que guardavam uma certa semelhança entre si e eram comuns nos diferentes percursos analíticos de cada uma das pacientes. Estas questões, que se apresentaram como saliências no discurso de cada paciente analisada, são relativas à feminilidade. A predominância da problemática identificatória relativa ao feminino, nos diversos casos analisados, exigiu que meus estudos teóricos seguissem um caminho que se iniciou com a teoria freudiana sobre a constituição do psiquismo feminino e a psicopatologia da histeria feminina. Assim, considerando a metapsicologia freudiana e a compreensão da psicopatologia por meio do conceito de conflito identificatório, proposto por Aulagnier, esta pesquisa apresenta o material clínico de um caso específico de histeria feminina e mostra que o sofrimento desta paciente refere-se às questões relativas à feminilidade, estando enraizado nos conflitos presentes em sua história libidinal e identificatóriaThe aim of this dissertation is to disclose how the identifying problematic related to the feminine present in a specific case of female hysteria occurs; through the clinical material obtained from the analytical process of a hysteric patient. The comprehension and analysis of the clinical material, which will be used to build this research, find their theoretical-clinical base in the Freudian theory and in the contributions of Piera Aulagnier, a contemporary author that follows Sigmund Freud s orientations and is also influenced by Jacques Lacan. To make this investigation viable, I have directed my clinical examination to the psychoanalytical treatment of six patients with hysteric organization. And during the time of the analytical process of these patients, some questions were raised which shared a certain similarity among themselves and were common in the different analytical courses of each one of the patients. These questions, which presented themselves as significant in the discourse of each analyzed patient, are related to femininity. The predominance of the identifying problematic related to the feminine, in the various analyzed cases, demanded my theoretical studies to follow a path that was initiated by the Freudian theory on the female psychism constitution and the psychopathology of female hysteria. And so, considering the Freudian metapsychology and the understanding of psychopathology through the concept of identifying conflict proposed by Aulagnier, this research presents the clinical material of a specific case of female hysteria and shows that the suffering of this patient refers to questions related to femininity, and is rooted in the conflicts present in her libidinal and identifying histor

    Periprostatic Perineurioma, Description of a Case With Unusual Localization

    AbstractPerineurioma is a rare entity, it is a benign peripheral nerve sheath tumor entirely composed of perineurial cells. A 62-year-old male patient was admitted to our hospital, suffering from scrotal and pelvic pain combined with a severe and continuous pain in his right thigh. A transrectal ultrasound revealed a periprostatic oval lesion of about 5 cm in maximum diameter. A sovrapubic laparotomy was performed with a complete tumor excision. The morphological and immunohistochemical data were most consistent with the diagnosis of perineurioma

    Predictive value of the Diverticular Inflammation and Complication Assessment (DICA) endoscopic classification on the outcome of diverticular disease of the colon: An international study

    Background: Diverticular Inflammation and Complication Assessment (DICA) endoscopic classification has been recently developed for patients suffering from diverticulosis and diverticular disease. Aims: We assessed retrospectively the predictive value of DICA in patients for whom endoscopic data and clinical follow-up were available. Methods: For each patient, we recorded: age, severity of DICA, presence of abdominal pain, C-reactive protein and faecal calprotectin test (if available) at the time of diagnosis; months of follow-up; therapy taken during the follow-up to maintain remission (if any); occurrence/recurrence of diverticulitis; need of surgery. Results: We enrolled 1651 patients (793 M, 858 F, mean age 66.6 ± 11.1 years): 939 (56.9%) patients were classified as DICA 1, 501 (30.3%) patients as DICA 2 and 211 (12.8%) patients as DICA 3. The median follow-up was 24 (9–38) months. Acute diverticulitis (AD) occurred/recurred in 263 (15.9%) patients; surgery was necessary in 57 (21.7%) cases. DICA was the only factor significantly associated to the occurrence/recurrence of diverticulitis and surgery either at univariate (χ2= 405.029; p < 0.0001) or multivariate analysis (hazard ratio = 4.319, 95% confidence interval (CI) 3.639–5.126; p < 0.0001). Only in DICA 2 patients was therapy effective for prevention of AD occurrence/recurrence with a hazard ratio (95% CI) of 0.598 (0.391–0.914) (p = 0.006, log rank test). Mesalazine-based therapies reduced the risk of AD occurrence/recurrence and needs of surgery with a hazard ratio (95% CI) of 0.2103 (0.122–0.364) and 0.459 (0.258–0.818), respectively. Conclusions: DICA classification is a valid parameter to predict the risk of diverticulitis occurrence/recurrence in patients suffering from diverticular disease of the colon
