    The First Sentences of the French Author Mobasan's Stories.

    This study reviews the importance of the first sentences in the first and second parts of (stories and novels) for the French author Mobasan. Why did we choose these parts? Because we could collect the first sentences in 31 stories divided into two parts, the first part includes 17 stories and the second one includes 14 stories. There are various topics in the two parts as Mobasan reviews scenes of everyday life, for example: the concept of death, lying, greed, tragedy and so on. Mobsan depends on the descriptive style in reviewing these scenes, and usually the first sentences of Mobasan's stories have great effect on the characters and morals of his stories

    تسعى هذه الدراسة إلى تسليط الضوء على رواية (يا مريم) للروائي العراقي(سنان أنطون)، والكشف عن تجلّيات العنف والطائفية التي أظهرتها الرواية، خاصة وأن العراق عانى وما زال يعاني من الطائفية، التي ظهرت في أقبح صورها، منذ ما يزيد عن العقد من الزمن، وعلى وجه الخصوص ما نال الأقليات ــ ومنها المكوِّن المسيحي ــ من تلك الطائفية. وتحاول الدراسة الكشف عن أسلوب السرد والتشكيل في تناول هذا الموضوع، والتقنيات الفنية التي استخدمها المؤلف في إيصال مفهومه في عدة محاور، هي: أولًاــ العنوان. ثانيًاـ البُنى الحدثية للرواية، وتجلّيات العنف فيها: ـــ ملخص الرواية. ـــ الشخوص الروائية: بين جيلين، صور من عراق الأمس، وعراق اليوم، والفيصل بينهما الأحداث التي حصلت بعد الاحتلال الأميركي في (2003م). ثالثًاـــ التمثُّلات الرمزية.This study aims to shed light on the novel "Ya Maryam" by the Iraqi novelist Sinan Antoon, and to discuss the manifestations of violence and Sectarianism that the novel portrays. Noting that Iraq has faced and is still facing Sectarianism in its most gruesome forms for over a decade, which in turn particularly affects the minorities - including Christians - in that sect. This study aims to discuss the narrative style and plot in tackling these themes, and the techniques the author used to deliver his message on several axes, namely: First - the title.  Second - the sequence of events, and manifestations of violence: - Summary of the novel. - The characters: spanning over two generations, pictures of yesterday's Iraq and today's Iraq, and the gap between both caused by the American occupation in 2003.  Third - Symbolism

    Impact of the follicular fluid Coenzyme Q10 level in women undergoing intracytoplasmic sperm injection (ICSI) on the pregnancy rate

    Background: The most crucial problem with in vitro fertilization (IVF) cycles is still oocyte quality. The women age and the condition of their ovarian reserve are the primary determinants of oocyte quality. Objectives: to assess the effects of intracytoplasmic sperm injection (ICSI) on the result of pregnancies and the coenzyme Q10 (CoQ10) value in follicular fluid (FF) in the women who had the procedure. Patients and methods: this cohort investigation was conducted on 81 infertile patients (age between 20-42 years, both normal or poor responders’ patients and patients with unexplained infertility) who underwent ICSI cycles. Results: patients were divided into two groups: the pregnant group (n= 32) and the non-pregnant group (n= 49).There was a statistically insignificant difference in antral follicle count (AFC), number of retrieved oocytes, number of embryos, number of metaphase II (MII) oocytes, and maturation index between pregnant and non-pregnant females. CoQ10 level in FF was substantially greater in pregnant than non-pregnant females. Conclusion: FF CoQ10 levels were positively correlated with eventual embryo quality and rates of conception. Our findings might be in favour of CoQ10 supplementation in women undergoing IVF for enhancement of the ovum and embryo quality

    Tumor expression of Nectin-4 as a prognostic marker in breast cancer

    Background: Identification of new molecular tumor associated biomarkers is the most important current challenge in cancer research. Nectin-4 is one of the Nectin glycoproteins, which are cell adhesion molecules have been involved in tumor biology. Objectives: The objective was to evaluate Nectin-4 expression by immunohistochemistry (IHC) as a prognostic tumor marker in breast cancer (BC). Patients and Methods: This study was carried out on 100 female patients with BC. Their ages ranged between 29 and 67 years, with a mean of 41.3 years. Fifty other age-matched patients, subjected to reduction mammoplasty, served as controls. Data collected prospectively included patient demographics and tumor characteristics, including histopathological type and grade, IHC for Nectin-4 expression, estrogen receptors (ER), progesterone receptors (PR), and human epidermal growth factor receptor 2 (HER2-Neu). Patients were regularly followed-up for 2 years, recording loco-regional recurrence, distant metastasis, and mortality. Results: Nectin-4 expression by IHC was detected in 62% of BC patients, but in none of the tumor-free controls (P = 0.0001). Nectin-4 expression showed a statistically significant positive correlation with higher tumor grade (P = 0.003) and axillary lymph node involvement (P = 0.0001), but not with increasing tumor size (P = 0.273). It had a significant inverse correlation with ER and PR, and a significant positive correlation with HER2-Neu (P = 0.0001). Furthermore, there was a significant correlation between Nectin-4 expression and the development of distant metastases (P = 0.014), local recurrence (P = 0.046), and mortality (P = 0.049). Conclusions: Nectin-4 is a highly recommended biomarker for predicting progression and prognosis of BC. [Arch Clin Exp Surg 2015; 4(4.000): 178-184

    Effect of Designing and Implementing Nursing Guidelines on Nurses' Performance in Caring Women with Ectopic Pregnancy

    Context: Ectopic pregnancy is a condition presenting a significant health problem for women of the childbearing period. Ectopic pregnancy remains one of the primary causes of maternal mortality. Aim: The study aimed to evaluate the effect of nursing guidelines for improving maternity nurses' performance regarding ectopic pregnancy. Methods: A quasi-experimental design was used to achieve the aim of the study. The study was conducted at Obstetrics and Gynecology Department at Benha University Hospital. A convenient sample of all nurses (90 nurses) working in Obstetrics and Gynecology departments at Benha university hospital. Tools of data collection included a structured self-administered questionnaire to assess nurses' knowledge regarding ectopic pregnancy, an observational checklist to evaluate nursing practice regarding ectopic pregnancy before and after implementing the nursing guidelines. Results: 78.9% of the studied nurses had a total incorrect knowledge preprogram, which improved to 88.9% of them and had correct knowledge post-program. The study also shows that 47.2% of the studied nurses had unsatisfactory total practice score preprogram, which improved to 54.4% of the studied nurses who had high satisfactory total practice post-program. There was a highly statistically significant difference before and after nursing guidelines of the studied nurses’ knowledge and practice regarding ectopic pregnancy (P ≤ 0.001). Conclusion: The study concluded that nursing guidelines positively affect the nurses' performance in terms of their knowledge and practice regarding ectopic pregnancy. The study recommended that simple guidelines regarding ectopic pregnancy nursing care should be distributed to nurses in the emergency obstetrics department to standardize and optimize nursing care provided to women with ectopic pregnancies. Continuous refreshment courses and follows up programs for nurses regarding ectopic pregnancy

    Background: Epithelial ovarian cancer is one of the leading causes of death in gynecological malignancies and the seventh most common cancer in the world among women. Aim and objectives: The main aim of this study was to study the feasibility and ability of laparoscopy in predicting optimum respectability in cases of ovarian cancer. Conclusion: Laparoscopy has an effective feasibility and ability in predicting optimum respectability in cases of ovarian cancer. As laparoscopy before starting treatment was a reliable additional diagnostic tool in predicting result of PDS, this can prevent laparotomies. This will optimize treatment for the individual patient

    The therapeutic mechanisms of propolis against CCl4-mediated liver injury by mediating apoptosis of activated hepatic stellate cells and improving the hepatic architecture through PI3K/AKT/mTOR, TGF-?/Smad2, Bcl2/BAX/P53 and iNOS signaling pathways

    Background/Aims: Propolis is one of the most promising natural products, exhibiting not only therapeutic but also prophylactic actions. Propolis has several biological and pharmacological properties, including hepatoprotective activities. The present study aimed to investigate the underlying molecular mechanisms of propolis against CCl4-mediated liver fibrosis. Methods: Three groups of male BALB/c mice (n=15/group) were used: group 1 comprised control mice; groups 2 and 3 were injected with CCl4 for the induction of liver fibrosis. Group 3 was then orally supplemented with propolis (100 mg/kg body weight) for four weeks. Different techniques were used to monitor the antifibrotic effects of propolis, including histopathological investigations using H&E, Masson's trichrome and Sirius red staining; Western blotting; flow cytometry; and ELISA. Results: We found that the induction of liver fibrosis by CCl4 was associated with a significant increase in hepatic collagen and ?-smooth muscle actin (?-SMA) expression. Moreover, CCl4-treated mice also exhibited histopathological alterations in the liver architecture. Additionally, the liver of CCl4-treated mice exhibited a marked increase in proinflammatory signals, such as increased expression of HSP70 and increased levels of proinflammatory cytokines and ROS. Mechanistically, the liver of CCl4-treated mice exhibited a significant increase in the phosphorylation of AKT and mTOR; upregulation of the expression of BAX and cytochrome C; downregulation of the expression of Bcl2; a significant elevation in the levels of TGF-? followed by increased phosphorylation of SMAD2; and a marked increase in the expression of P53 and iNOS. Interestingly, oral supplementation of CCl4-treated mice with propolis significantly abolished hepatic collagen deposition, abrogated inflammatory signals and oxidative stress, restored CCl4-mediated alterations in the signaling cascades, and hence repaired the hepatic architecture nearly to the normal architecture observed in the control mice.     First ever case report of co-occurrence of hobnail variant of papillary thyroid carcinoma and intrathyroid parathyroid adenoma in the same thyroid lobe

    Introduction: The hobnail variant of papillary thyroid cancer (PTC) is rare. Intrathyroid parathyroid adenoma (ITPA) is also rare. Co-ocurrence of PTC and ITPA in the same thyroid lobe is extremely rare. Likewise, primary hyperparathyroidism with such non-medullary thyroid carcinoma is rare. The specific molecular profile of hobnail PTC (HPTC) is different from the classic, poorly differentiated and anaplastic variants and may contribute to its aggressive behavior. HPTC's genetic profile remains unclear. Presentation of case: A 61-year-old woman presented to our endocrine clinic with generalized aches, bone pain, polyuria, and right neck swelling of a few months’ duration. Laboratory findings revealed hypercalcemia and hyperparathyroidism. Ultrasound of the neck showed 4.6 cm complex nodule within the right thyroid lobe. Sestamibi scan suggested parathyroid adenoma in the right thyroid lobe. Fine-needle aspiration (FNA) revealed atypical follicular lesion of undetermined significance. She underwent right lobectomy, which normalized the intraoperative intact parathyroid hormone levels. Final pathology with immunohistochemical stains demonstrated HPTC and IPTA (2 cm each). Next-generation sequencing investigated the mutation spectrum of HPTC and detected BRAFV600E mutation. Conclusions: A parathyroid adenoma should not exclude the diagnosis of thyroid carcinoma. Thyroid evaluation is needed for patients with primary hyperparathyroidism to prevent missing concurrent thyroid cancers. Cytomorphologic features to distinguish thyroid from parathyroid cells on FNA cytology must be considered. Immunohistochemical stains are important. BRAFV600E is the most common mutation in HPTC. This is possibly the first reported case of HPTC and ITPA co-occurring within the same thyroid lobe. Studies that define other molecular abnormalities may be useful as therapeutic targets. © 2020 The Author(s

    Induction of liver fibrosis by CCl4 mediates pathological alterations in the spleen and lymph nodes: The potential therapeutic role of propolis

    In an animal models, carbon tetrachloride (CCl4) is a carcinogenic agent that causes liver fibrosis. The current study aims to investigate whether induction in liver-fibrosis by CCl4 in the mouse model could promote the initiation of fibrosis in lymph node and spleen due to sustained increase of inflammatory signals and also aimed to clarify the protective therapeutic effects of propolis. The male mice (BALB/c) were categorized into three experimental sets and each group involved 15 mice. Control group falls into first group; group-II and group-III were injected with CCl4 to induce liver-fibrosis and oral supplementation with propolis was provided in group-III for 4-weeks. A major improvement with hepatic collagen and alpha-smooth muscle actin (alpha-SMA) production was aligned with the activation of liver fibrosis from CCl4. Mice treated with CCl4 exhibited collagen deposition towards liver sections, pathological alterations in spleen and lymph node architectures, and a significantly increase the circulation of both T&B cells in secondary lymphoid organs. Mechanically, the secondary lymphoid organs treated with CCl4 in mice exposed a positive growth in alpha-SMA and collagen expression, increased in proinflammatory cytokine levels and a significant increase in TGF-beta, NO and ROS levels. A manifest intensification in the expression of Nrf2, COX-2, and eNOS and upregulation of ASK1 and P38 phosphorylation. Interestingly, addition of propolis-treated CCl4 mice, substantially suppressed deposition of liver collagen, repealed inflammatory signals and resorted CCl4-mediated alterations in signaling cascades, thereby repairing the architectures of the secondary lymphoid organs. Our findings revealed benefits of propolis against fibrotic complications and enhancing secondary lymphoid organ architecture. (C) 2020 The Author(s).
