fNIRS-Neurofeedback-Training bei adulten ADHS-Patienten: Eine Untersuchung der Auswirkungen auf Aufmerksamkeit und Impulsinhibition
Die Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung, kurz ADHS, ist eine Erkrankung, die sich vor allem mit verminderter Aufmerksamkeit, gesteigerter Impulsivität und Hyperaktivität präsentiert. Aufmerksamkeitsleistungen und auch die Fähigkeit zur Impulsinhibition gehören zu den sogenannten Exekutivfunktionen. Es gibt bereits einige Studien, die zeigen, dass Patienten mit ADHS Auffälligkeiten in diesen Exekutivfunktionen aufweisen. Ebenso gibt es Studien, die zeigen, dass ADHS-Patienten von Neurofeedback-Training profitieren – bisher allerdings vor allem zu Kindern mit ADHS, Studien zu erwachsenen ADHS-Patienten sind rar. Ebenso untersuchen diese Studien bisher fast ausschließlich Neurofeedback mittels Elektroenzephalografie (EEG), Studien zu Neurofeedback mittels funktioneller Nah-Infrarot Spektroskopie (fNIRS) sind selten. So war das Ziel dieser Arbeit, die Wirkung von fNIRS-Neurofeedback bei erwachsenen ADHS-Patienten zu untersuchen. Als zentrale Frage wurde die Wirkung von fNIRS-Neurofeedback auf Exekutivfunktionen wie Impulsinhibition und Aufmerksamkeitsleistung untersucht. Das in dieser Arbeit durchgeführte fNIRS-Neurofeedback-Training zielte auf Areale des dorsolateralen präfrontalen Kortex (DLPFC) beider Hemisphären, welchen die Exekutivfunktionen zugeschrieben werden. Zum Neurofeedback wurde die Kortexaktivität im DLPFC anhand von Konzentrationsveränderungen der Oxygenierung des Hämoglobins mittels fNIRS gemessen und dem Patienten als visuelles Feedback wiedergegeben. Zehn erwachsene Patienten mit ADHS führten jeweils 30 solcher Trainingssitzungen durch. Die vor Trainingsbeginn (t0), nach 15 (t1) sowie nach 30 Trainingssitzungen (t1) erhobenen Daten zur ADHS-Symptomatik und der Kortexaktivierung im DLPFC wurden über die Zeit verglichen: Die ADHS-Symptomatik wurde einerseits anhand eines Selbstbeurteilungsfragebogens (ADHS-SB), andererseits anhand von Verhaltensdaten (Fehlern und Reaktionszeiten) während der Durchführung eines Tests zur Messung der exekutiven Aufmerksamkeit (Go/NoGo-Paradigma) untersucht. Unter Durchführung dieses Tests wurde zugleich die Kortexaktivierung im dorsolateralen präfrontalen Kortex analysiert. Im Ergebnis zeigte sich insbesondere nach den ersten 15 Trainingssitzungen eine signifikante Verbesserung der Gesamtsymptomatik sowie der Symptombereiche Impulsivität und Unaufmerksamkeit, die mit dem Selbstbeurteilungsfragebogen ADHS-SB gemessen wurde. Weder für die Verhaltensdaten (Fehler, Reaktionszeiten im Go/NoGo-Paradigma), noch für die funktionellen Daten (Kortexaktivierung) ergaben sich durch das Training signifikante Veränderungen. Bei den Verhaltensdaten zeigte sich keine Verbesserung von Impulsinhibition und Aufmerksamkeitsleistung, bei den funktionellen Daten waren die Ergebnisse inkonsistent und widersprachen größtenteils sogar den Erwartungen. Trotz dieser uneinheitlichen, teilweise unerwarteten Ergebnisse lässt sich zusammenfassend sagen, dass die fNIRS ein vergleichsweise kostengünstiges, nicht-invasives Verfahren ist, das sich zur Anwendung mit Neurofeedback eignet. Unsere Arbeit zeigt zumindest hinsichtlich der Symptomselbstbeurteilung, besonders bei den ADHS-Hauptsymptomen Impulsivität und Unaufmerksamkeit, eine Verbesserung durch das Training. Im Vergleich zu EEG-Neurofeedback zeigte das fNIRS-Neurofeedback hier sogar eine besonders rasche Symptomreduktion nach nur 15 Trainingssitzungen. Hinsichtlich der Verhaltensdaten sowie der funktionellen Daten zeigte unsere Arbeit zwar keine signifikanten Veränderungen, möglicherweise wird jedoch die fortgeführte Studie mit größeren Patientenzahlen sowie Kontrollgruppen eindeutigere Ergebnisse bringen
Open Doors By Fair Means Ein Kooperationsprojekt zur Implementierung eines Türöffnungskonzepts auf psychiatrischen Akutstationen
Ziel der vorliegenden Studie war die Implementierung einer offenen Türkonzeption an zwei Standorten mit insgesamt vier Akutstationen zu evaluieren. Hierzu wurden quantitative Maße zu Türöffnungszeiten, die Anzahl unfreiwilliger Behandlungstage mit offenen Türen sowie die Häufigkeit aggressiver Zwischenfälle, Selbstverletzungen, unerlaubter Entweichungen von Station, Suizidversuche und Zwangsmaßnahmen erhoben. Darüber hinaus wurden Fokusgruppen, jeweils zu Beginn und zum Ende der Interventionsphase auf allen teilnehmenden Akutstationen durchgeführt. Inhalt der Fokusgruppen waren Aspekte von Kommunikation, Akzeptanz, Stigmatisierung, Atmosphäre, Autonomie und Sicherheitsempfinden, die in nach Personal und Patienten getrennten Gruppen mit Hilfe eines semistrukturierten Interviewleitfadens erfragt und inhaltsanalytisch ausgewertet wurden. Es konnte gezeigt werden, dass es möglich ist, psychiatrische Akutstationen mit offenen Türen zu führen, ohne dabei gesteigerte Risiken einzugehen. Alle teilnehmenden Stationen konnten mit Beginn des Projekts häufiger offen geführt werden, insbesondere die Interventionsstationen.
Als häufigste Begründungen für den Türschluss wurden mangelnde Absprachefähigkeit, Suizidalität und Desorientiertheit genannt. Bei Patienten mit einer Diagnose aus dem schizophrenen Formenkreis ist die Wahrscheinlichkeit am höchsten, mit der Diagnose einer affektiven Störung hingegen ist die Wahrscheinlichkeit am geringsten, auslösend für einen Türschluss zu sein.
Im Rahmen der Fokusgruppen zeigte sich, dass der Intervention vorangehende Befürchtungen wie ein geringeres Sicherheitsempfinden sich auch subjektiv nicht bewahrheitet. Stigmatisierung schien für die Patienten in Hinblick auf den Türstatus keine konkrete Rolle zu spielen, eher hinsichtlich des Ansehens der Psychiatrie im Allgemeinen. Sowohl Patienten als auch Personal wünschen sich nicht eine Durchsetzung der Türöffnung zulasten einzelner Patienten, die alternativ fixiert oder isoliert werden müssten. Nichts desto trotz fühlten sich viele Patienten einer geschlossenen Station in ihrer Freiheit eingeschränkt und entmündigt, da ein Verlassen der Station zunächst erfragt werden muss. Sowohl bei Patienten, als auch Personal bestand die Befürchtung, dass die fortlaufende Beaufsichtigung der offenen Tür Kapazitäten von anderen Aufgaben abziehen würde. Es zeigte sich jedoch auch, dass die Tatsache die Tür nicht ständig öffnen und wieder schließen zu müssen für das Personal entlastende Aspekte mit sich brachte. Insgesamt wurde deutlich, dass sich im Zuge der Türöffnung auch Einstellungen insbesondere auf Seiten des Personals änderten, wenngleich das Meinungsbild über alle Stimmen hinweg ein heterogenes war
The DLPFC is centrally involved in resolving Stroop conflicts, suppressing distracting sensory input within the auditory and visual system
Introduction Cognitive control is a prerequisite for successful, goal-oriented behavior. The dorsolateral prefrontal cortex (DLPFC) is assumed to be a key player in applying cognitive control; however, the neural mechanisms by which this process is accomplished are still unclear. Methods To further address this question, an audiovisual Stroop task was used, comprising simultaneously presented pictures and spoken names of actors and politicians. Depending on the task block, participants had to indicate whether they saw the face or heard the name of a politician or an actor (visual vs. auditory blocks). In congruent trials, both stimuli (visual and auditory) belonged to the same response category (actor or politician); in incongruent trials, they belonged to different categories. During this task, activity in sensory target regions was measured via functional near-infrared spectroscopy (fNIRS) and electroencephalography (EEG), respectively. Specifically, fNIRS was used to monitor activity levels within the auditory cortex, while the EEG-based event-related potential of the N170 was considered as a marker of FFA (fusiform face area) involvement. Additionally, we assessed the effects of inhibitory theta-burst stimulation—a specific protocol based on repetitive transcranial magnetic stimulation (rTMS)—over the right DLPFC. Non-invasive brain stimulation is one of the few means to draw causal conclusions in human neuroscience. In this case, rTMS was used to temporarily inhibit the right DLPFC as a presumed key player in solving Stroop conflicts in one of two measurement sessions; then, effects were examined on behavioral measures as well as neurophysiological signals reflecting task-related activity in the frontal lobes and sensory cortices. Results The results indicate a central role of the DLPFC in the implementation of cognitive control in terms of a suppression of distracting sensory input in both the auditory cortex and visual system (FFA) in high-conflict situations. Behavioral data confirm a reduced Stroop effect following previous incongruent trials (“Gratton effect”) that was only accomplished with an intact DLPFC (i.e., following placebo stimulation). Discussion Because non-invasive brain stimulation is uniquely suited to causally test neuroscientific hypotheses in humans, these data give important insights into some of the mechanisms by which the DLPFC establishes conflict resolution across different sensory modalities
Dysfunctional learning processes and their neuronal, behavioral and (epi)genetic correlates in patients with alcohol dependence compared to controls
Ziel der aufgeführten Studien soll ein besseres Verständnis für die Entstehung und Aufrechterhaltung von abhängigem Verhalten sein. Wie kommt es dazu, dass ein ursprünglich als angenehm erlebtes, sporadisch gezeigtes Verhalten, wie z. B. der Konsum von einem Glas Wein, in der Folge zu einem für den Einzelnen schädlichen bis zerstörerischen Verhalten wird, das oft nicht mehr dem erlebten und formulierten Willen (z. B. zur Abstinenz) entspricht? Obwohl damit zahlreiche negative Konsequenzen wie der Verlust von sozialen und beruflichen Beziehungen, körperliche Erkrankungen und eine reduzierte Lebensqualität verbunden sind, wird das schädigende Verhalten aufrechterhalten. Und obwohl zahlreiche Variablen mit süchtigem Verhalten in Verbindung gebracht werden konnten, sind die kausalen Mechanismen nur unzureichend verstanden. In den oben dargelegten Forschungsarbeiten wurden drei Aspekte von süchtigem Verhalten untersucht: 1) Veränderungen im PIT-Effekt und deren neuronale Korrelate, 2) (epi)genetische Korrelate eines anhaltenden Alkoholkonsums und deren methodische Besonderheiten und 3) die Rolle von chronischem Stresserleben auf belohnungsabhängiges Lernen in Abhängigkeit von kognitiven Funktionen. Die Bedeutung dieser Arbeiten im Kontext eines besseren Verständnisses für die Entstehung und Aufrechterhaltung der Alkoholabhängigkeit wird diskutiert
Social cognition in autism: Face tuning
Faces convey valuable information for social cognition, effective interpersonal interaction, and non-verbal communication. Face perception is believed to be atypical in autism, but the origin of this deficit is controversial. Dominant featural face encoding is suggested to be responsible for face tuning scarcity. Here we used a recently developed Face-n-Food paradigm for studying face tuning in individuals with autistic spectrum disorders (ASD). The key benefit of these images is that single components do not explicitly trigger face processing. In a spontaneous recognition task, adolescents with autism and typically developing matched controls were presented with a set of Face-n-Food images in different degree resembling a face (slightly bordering on the Giuseppe Arcimboldo style). The set of images was shown in a predetermined order from the least to most resembling a face. Thresholds for recognition of the Face-n-Food images as a face in ASD individuals were substantially higher than in typically developing controls: they did not report seeing a face on the images, which controls easily recognized as a face, and gave overall fewer face responses. This outcome not only lends support to atypical face tuning, but provides novel insights into the origin of face encoding deficits in autism
Medication Adherence in a Cross-Diagnostic Sample of Patients From the Affective-to-Psychotic Spectrum: Results From the PsyCourse Study
Introduction According to the World Health Organization, medication adherence is defined as the extent to which a person's behavior corresponds with an agreed recommendation from a healthcare provider. Approximately 50% of patients do not take their medication as prescribed, and non-adherence can contribute to the progress of a disease. For patients suffering from mental diseases non-adherence plays an important role. Various factors have been proposed as contributing to non-adherence, however the literature remains heterogeneous dependent on the analyzed patient subgroups. This study comprehensively evaluates the association of sociodemographic, clinical, personality and quality of life related factors with medication adherence by analyzing data from the PsyCourse study. The PsyCourse study is a large and cross-diagnostic cohort of psychiatric patients from the affective-to-psychotic spectrum. Methods The study sample comprised 1,062 patients from the PsyCourse study with various psychiatric diagnoses (mean [SD] age, 42.82 [12.98] years; 47.4% female). Data were analyzed to identify specific factors associated with medication adherence, and adherence was measured by a self-rating questionnaire. Odds ratios (OR) were estimated by a logistic regression for binary outcomes. Missing data were imputed using multiple imputation. Results The following factors showed the strongest association with medication adherence: never having used illicit drugs (OR, 0.71), number of prescribed antipsychotics (OR, 1.40), the personality trait conscientiousness (OR, 1.26), and the environmental domain of quality of life (OR, 1.09). Conclusion In a large and cross-diagnostic sample, we could show that a higher level of conscientiousness, a higher number of antipsychotic medication, a better quality of life within the environmental domain, and the absence of substance abuse contribute to a better medication adherence independent of the underlying disorder
Hirnstimulationsverfahren zur Behandlung schizophrener Psychosen
Despite many different available pharmacological and psychosocial treatment options, an optimal control of symptoms is only partly possible for most schizophrenia patients. Especially, persistent auditory hallucinations, negative symptoms and cognitive impairment are difficult to treat symptoms. Several non-invasive brain stimulation techniques are increasingly being considered as new therapeutic add on options for the management of schizophrenia, targeting these symptom domains. The technique which has been available for the longest time and that is best established in clinical care is electroconvulsive therapy (ECT). New stimulation techniques, such as repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) allow a more pathophysiological-based approach. This review article introduces various non-invasive brain stimulation techniques and discusses recent treatment studies on schizophrenia. In total, the novel brain stimulation techniques discussed here can be considered relevant add on therapeutic approaches for schizophrenia. In this context, the best evidence is available for the application of rTMS for the treatment of negative symptoms and persistent auditory hallucinations; however, negative studies have also been published for both indications. Studies using other non-invasive brain stimulation techniques showed promising results but further research is needed to establish the clinical efficacy. Based on a growing pathophysiological knowledge, non-invasive brain stimulation techniques provide new treatment perspectives for patients with schizophrenia
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
