    Endocrine system exerts relevant effects on multiple organs and tissues. Pleiotropic and redundant functions of circulating hormones are due to the various receptors expressed on multiple target cells. Moreover, receptors of circulating hormones share common transducing elements with receptors of many other molecules. Consequently, endocrine system participate to an integrated network of mediators that communicate and coordinate responsive cells to achive effective functions in an appropriate fashion. The presence of such a complex interplay contribute to unravel previously unappreciated functions of circulating hormones and the mechanisms of coordination and integration of several pathways. An example of this interplay is the complex interaction between endocrine and immune system. Cytokines and growth factors, in fact, after their binding to cell-surface receptos, activate common intracytoplasmatic signaling molecules. In the recent years the description of complex phenotypes, in which immunodeficiency and growth failure were associated at a different extent, greatly contributed to define how several signaling molecules play a role in both systems. As well as immunodeficiency, autoimmune diseases represent a unique model to study interactions between endocrine and immune systems. Cardiovascular system also represents a biological system higly sensitive to the effects of circulating hormones as suggested by the presence of several cardiovascular and metabolic alterations found in many endocrine diseases. In fact, patient with growth hormone deficiency as well as thyroid dysfunction, may present an increased morbidity due to cardiovascular events and an increased incidence of atherosclerosis. Aim of this project was to evaluate physiological and pathological implications of these complex molecular interactions on paediatric endocrinopathies starting from complex phenotype involving endocrine and other biological systems. To this aim, we started from the study of 3 model of complex diseases: • The model of autoimmune diseases • The model of Growth Hormone (GH) Deficiency (GHD) • The model of Congenital Hypothyroidism (CH) Moreover, during the study of patients affected with Growth Hormone and thyroid disorders we also identified new genetic causes of GH deficiency and hyperthyroidism, highlithing new insights in the genetics of endocrine diseases, as described in the last chapter of the thesis

    Cardiac Output Evaluation on Septic Shock Patients: Comparison between Calibrated and Uncalibrated Devices during Vasopressor Therapy

    There are no reliable, non-invasive methods to accurately measure cardiac output (CO) in septic patients. MostCare (Vytech Health (TM), Vygon, Padova, Italy), is a beat-to-beat, self calibrated method for CO measurement based on continuous analysis of reflected arterial pressure waveforms. We enrolled 40 patients that were suffering from septic shock and requiring norepinephrine infusion to target blood pressure in order to to evaluate the level of agreement between a calibrated transpulmonary thermodilution device (PiCCO System, Pulsion Medical Systems, Feldkirchen, Germany) and the MostCare system in detecting and tracking changes in CO measurements related to norepinephrine reduction in septic shock patients,. PiCCO was connected to a 5 Fr femoral artery catheter and to a central venous catheter. System calibration was performed with 15 mL of cold saline injection over about 3 s. The MostCare device was connected to the artery catheter to analyze the arterial waveform. Before reducing norepinephrine infusion, the PiCCO system was calibrated, the MostCare waveform was optimized, and the values of the complete hemodynamic profile were recorded (T1). Norepinephrine infusion was then reduced by 0.03 mcg/Kg/min. After 30 min, a new calibration of PiCCO system and a new record on both monitors were performed (T2). Static measurements agreements were assessed using the Bland-Altman test, while trending ability was investigated using polar plot analysis. If volume expansion occurred, then related data were separately analyzed. At T1 mean the CO was 5.38 (SD 0.60) L/min, the mean difference was 0.176 L/min, the limits of agreement (LoA) was +1.39 and -1.04 L/min, and the percentage error (PE) was 22.6%; at T2 the mean CO was 5.44 (SD 0.73) L/min, the mean difference was 0.053 L/min, the LoA was +1.51 and -1.40, and the PE was 27%. After considering the volume expansion between T1 and T2, the mean CO at T1 was 5.39 L/min (SD 0.47), the LoA was +1.09 and -0.78 L/min, and the percentage error (PE) was 17%; at T2 the mean CO was 5.35 L/min (SD 0.81), the LoA was +1.73 and -1.52 L/min, and the PE was 30%. The polar plot diagram seems to confirm the trending ability of MostCare system versus the reference method. In septic patients, when the arterial waveform is accurate, MostCare and PiCCO transpulmonary thermodilution exhibit good agreement even after the reduction of norepinephrine and changes in vascular tone or volume expansion. MostCare could be a rapid to set, reliable, and useful tool to monitor hemodynamic variations in septic patients in emergency contexts where thermodilution methods or other advanced systems are not easily available

    The data analysis package is composed of three distinct scripts designed to be run in Matlab with png image files. ContactAngle.m measures the contact angle of a liquid bridge and the spreading distance on the substrate for a series of images during liquid bridge thinning. Startpoint_Symmetry finds the minimum radius for a series of liquid bridge thinning images, identifies the start and end point of thinning, and gives information about the overall symmetry of the trials. EC_binning identifies the fitting region, fits and reports relaxation time for elastocapillary thinning fluids. For all three scripts, an example dataset is provided to demonstrate script functionality.This collection of scripts is a package for analyzing extensional rheology data of liquid bridges. While this was created with Dripping-onto-Substrate datasets, this data analysis package may be useful for other liquid bridge thinning applications such as CaBER.     Retrospective matched case–control comparison of Totally Robotic Sleeve Gastrectomy (RSG) during the learning curve with Laparoscopic Sleeve Gastrectomy (LSG): why is operative time different?

    : Robotic approach is slowly rising in metabolic surgery, and laparoscopy is still considered the gold standard for Sleeve Gastrectomy. Aim of our study was to assess and compare outcomes of RSG through a matched comparison with LSG. Retrospective search of prospectively maintained database of our surgical department was carried out find all consecutive patients who underwent RSG from April 2023 to August 2024. Each subject who underwent RSG was matched one-to-one with a patient treated with LSG in the same period. Operative time (docking + console time for the robotic procedures), length of stay, need for rescue drugs, and perioperative complications were recorded calculated and compared. A total number of 50 patients (25 RSG and 25 LSG) were included in the present analysis. Operative time in the LSG group was significantly shorter than in the RSG group (57.8 ± 12.3 VS 80.6 ± 16.6 min, p < 0.01), but it was comparable to console time (57.8 ± 12.3 VS 56.9 ± 19.6, p = 0.85). Mean docking time was 23.7 ± 11 min. Length of stay, readmissions, conversion to laparoscopy/open surgery, early complications, and rescue drugs administration were comparable between the two groups. Age, sex, and BMI were not good predictors neither of laparoscopic nor robotic operative time. RSG during the learning curve proved as safe as LSG, but it was associated with longer operative time due to the duration of the docking step. Operation length may become comparable once the learning curve plateau is reached. Age, BMI, and sex are not good criteria of choice between the two approaches

    The bridge: suggestions about the meaning of a pictorial motif

    Developing research begun at the Warburg Institute in 1983, this paper reflects on the construction of meaning in a work of art, through the analysis of the bridge’s function in painting. It tries to reply to some objections the author received there from Gombrich, about the chance of finding a stable content in the configuration of the bridge. Hence, the study reconsiders the concept of ‘motif’ applied to this structure. In a semiotic perspective a motif is partially independent as regards to a single textual organization, because it has a mobile and migrant feature. However, it is also partially flexible as it depends upon the same organization. The inquiry shows that bridge’s internal structure corresponds to the category of a ‘junction’, with two opposite items, ‘conjunction’ and ‘disjunction’. The development of this theoretical object can be carried out also by figures that are not ‘bridges’, in the natural sense of the word. Furthermore, its meaning does not depend upon the number of examples we can find but only upon their relevance for constructing a ‘grammar of cases’. Differently from the traditional iconographical approach, but also from panofskian iconology, the analysis moves not only towards the simple or complex content of a figure but also towards its description
