Introduction Melatonin could be useful in critically ill thanks to its antioxidant, immunomodulating, and hypnagogue effects. When administered orally, its bioavailability is limited by a marked first pass hepatic metabolism. Incorporation in SLN, targeted to lymph after enteral administration may create a sustained release delivery system. This could maintain a prolonged blood concentration, and allow cellular internalization independent from MT1/MT2 membrane receptors
N-(substituted-anilinoethyl)amides: design, synthesis, and pharmacological characterization of a new class of melatonin receptor ligands
A novel series of melatonin receptor ligands, characterized by a N-(substituted-anilinoethyl)amido scaffold, along with preliminary structure–activity relationships (SARs), is presented. MT1 and MT2 receptor binding affinity and intrinsic activity have been modulated by the introduction of different substituents on the aniline nitrogen, on the benzene ring, and on the amide side chain. Modulation of intrinsic activity and MT2 selectivity of the newly synthesized compounds has been achieved by applying SAR models previously developed,
providing compounds with different binding and intrinsic activity profiles. Compound 3d, with a bulky ß-naphthyl group, behaves as an MT2-selective antagonist with sub-nM affinity. Size reduction of the substituent enhances intrinsic activity, as in the nonselective N-methyl-anilino agonist 3i. The phenyl derivative 3g is an MT2-selective partial agonist, with MT2 binding affinity higher than melatonin, showing promising

The benefit of synthetic versus biological patch augmentation in the repair of posterosuperior massive rotator cuff tears : a 3-year follow-up study
Background: Rotator cuff repair typically results in a satisfactory, although variable, clinical outcome. However, anatomic failure of the repaired tendon often occurs. Hypothesis: Patch augmentation can improve the results of open rotator cuff repair by supporting the healing process, protecting the suture, and reducing friction in the subacromial space. Study Design: Cohort study; Level of evidence, 3. Methods: A total of 152 patients with a posterosuperior massive rotator cuff tear were treated by open repair only (control group; n = 51; mean age, 67.06 ± 4.42 years), open repair together with collagen patch augmentation (collagen group; n = 49; mean age, 66.53 ± 5.17 years), or open repair together with polypropylene patch augmentation (polypropylene group; n = 52; mean age, 66.17 ± 5.44 years) and were retrospectively studied. Patients were evaluated preoperatively and after 36 months with a visual analog scale (VAS) and the University of California, Los Angeles (UCLA) shoulder rating scale and by measuring elevation of the scapular plane and strength with a dynamometer. The VAS and UCLA scores were also obtained 2 months postoperatively. Tendon integrity was assessed after 1 year by ultrasound. Patients were homogeneous as per the preoperative assessment. Results: After 2 months, results (mean ± standard deviation) for the control, collagen, and polypropylene groups, respectively, were as follows: VAS scores were 6.96 ± 1.11, 6.46 ± 1.02, and 4.92 ± 0.90, while UCLA scores were 11.29 ± 1.46, 11.40 ± 1.51, and 19.15 ± 1.99. After 36 months, the mean scores for the respective groups were 3.66 ± 1.05, 4.06 ± 1.02, and 3.28 ± 1.10 for the VAS and 14.88 ± 1.98, 14.69 ± 1.99, and 24.61 ± 3.22 for the UCLA scale. In addition, after 36 months, elevation on the scapular plane was 140.68° ± 9.84°, 140.61° ± 12.48°, and 174.71° ± 8.18°, and abduction strength was 8.73 ± 0.54 kg, 9.03 ± 0.60 kg, and 13.79 ± 0.64 kg for the control, collagen, and polypropylene groups, respectively. The retear rate after 12 months was 41% (21/51) for the control group, 51% (25/49) for the collagen group, and 17% (9/52) for the polypropylene group. In particular, the reduced 12-month retear rate and the increased UCLA scores, abduction strength, and elevation at 3-year follow-up were statistically significant for patients treated with a polypropylene patch compared with those treated with repair only or with a collagen patch. Conclusion: Polypropylene patch augmentation of rotator cuff repair was demonstrated to significantly improve the 36-month outcome in terms of function, strength, and retear rate. © 2014 The Author(s)

Una presencia virgiliana en el libro de Alexandre

En este articulo el autor muestra, a través de una confrontación textual directa, que el episodio de Nicánor y Símaco en el anónimo medieval español Libro de Alexandre es una reescritura del episodio de Niso y Euríalo 'del canto IX de la Eneida virgiliana con algunas reminiscencias del episodio del canto V de dicha epopeya en el que intervienen los mismos personajes. Después de una caracterización general del mitema a través de versiones producidas por poetas y escritores de distintas épocas, analiza los elementos básicos del mismo y las equivalencias empleadas por el autor español en el traslado de espacios, tiempos, circunstancias sociales y personales, recursos literarios y expresivos a partir del texto virgiliano.Abstract: In this paper the author shows, by means of a direct textual comparison, that the Nicanor and Simacus episode in the medieval anonyme spanish Libro de Alexandre is a re-writing of Nisus and Euryalus episode in book IX of virgilian Eneid, with sorne reminiscences from that episode of book V in this epic poem in which the same characters take parto After a general description of the myth along the versions produced by poets and writers in different times, he analyses the basic constitutive elements of that myth and the equivalences used by the spanish author in transfering spaces, times, social and personal circumstances, literary and expressive resources from the virgilian texto
Filología y didáctica : Las reflexiones metalingüísticas de Juan Escoto Eriúgena
Son objetivos del presente trabajo lograr una caracterización de las operaciones metalingüísticas empleadas por Escoto en su tratado De divisione naturae, su frecuencia y su justificación como recurso del discurso teológico, y determinar la intención didáctica del autor que se manifiesta precisamente por el manejo de tales operaciones. El método de trabajo se basa en el relevamiento de los numerosos segmentos autonímicos que aparecen en los cinco libros de la obra y su clasificación de acuerdo con sus caracteres formales y sus necesidades discursivas. Ellos están dirigidos, en su gran mayoría, a determinar equivalencias semánticas de términos y expresiones griegas con otras latinas, y unos pocos a usos particulares y polisémicos de términos latinos.The objectives of this paper are to fit a characterization of metalinguistic operations employed by Scottus in his treatrise De divisione naturae, its frequency and its justification as a resort of theological speech, and to definite the didactic intention of the author that is precisely expressed by the handling of such operations. The work’s method is based on the survey of numerous autonomic quotes that appear along the five books of the treatise, and their classification that is based on their formal characters and discursive needs. They tend principally to definite semantic equivalences between greek words, phrases, and latin ones, in a few cases to particular and polysemic latin terms.Facultad de Humanidades y Ciencias de la Educació
