    Treatment of patients with acute renal failure during Marmara earthquake

    Crush syndrome or traumatic rhabdomyolysis is a serious and epidemic clinical case that develops among casualties of earthquakes or other catastrophic events. Hypocalcaemia is one of the main symptoms that accompanies crush syndrome. It is essential that immediate IV volume replacement treatment be initiated for such patients, which is known to positively affect prognosis and help prevent the development of acute renal failure. Following the earthquake of 17th August 1999 in the north-west of Turkey, 333 casualties were hospitalised in the Medical Faculty of Uludag University. 60 (18%) of these patients [34 males (56.6%) and 26 females (43.4%)] received haemodialysis treatment. The average number of HD sessions received by the patients was 48. Intermittent HD applied to 44 patients (73.3%) was the preferred method of treatment. Fluid overload and azotemia were the leading indications (76.6%) for dialysis. The lower extremities were mainly injured in these patients [33 (55%)]. Therefore, subclavian catheterisation was preferred in those patients with trauma at this site. 594 (67.1%) HD sessions were applied with limited heparin due to the risk of bleeding. Hypotension in 94 patients (61%) and thrombosis in 60 (45.7%) were the main complications. Of the complications not due to HD, infection in 23 patients (38.3%) was the most significant. 21 patients (35%) were lost during the treatment. It was thanks to the sacrifices and co-operation among national and international medical teams that the mortality rate was low

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used

    Bu çalışmada, hipertansif hemodiyaliz olgularında anjiiyotensin reseptör blokeri losartanın ambulatuar kan basıncı, insülin direnci ve lipid profili üzerine etkisini araştırmayı amaçladık. 28 stabil hemodiyaliz olgusu çalışmaya dahil edildi. Olgular 2 gruba ayrıldılar. 1. gruptaki 17 olguya 16 hafta boyunca 50-100 mg/gün losartan tedavisi uygulandı (Losartan grubu). Kalan 11 olguya ise herhangi bir antihipertansif ilaç verilmedi (Kontrol grubu). Tüm olgularda hemen tedavi öncesi ve 16 hafta sonra, hafta başı prediyaliz 24 saatlik ambulatuar kan basıncı takibi (AKBT) yapıldı. Ayrıca tedaviden 2 hafta ve 1 hafta önce, hemen tedavi öncesi ve tedavi süresince 2 hafta aralıklarla diyaliz öncesi ofis sistolik (SKB) ve diyastolik (DKB) kan basıncı ölçümleri, nabız dakika sayıları, diyaliz öncesi ve sonrası vücut ağırlıkları kaydedildi. Hemen tedavi öncesi ve sonrası, 12 saat açlık sonrası serum glikoz, insülin, üre, kreatinin, ürik asid, sodyum, potasyum, klor, kalsiyum, fosfor, total protein, albumin, AST, ALT, total kolesterol, trigliserid, HDL-kolesterol, apoprotein A1 ve B, lipoprotein (a), serum demir ve total demir bağlama kapasitesi, paratiroid hormon (PTH) düzey ölçümleri ve tam kan sayımı yapıldı. İnsülin direnci ve beta hücre fonksiyonu HOMA yöntemi kullanılarak hesaplandı. Diyaliz etkinliği üre kinetik model ile değerlendirildi. İstatistiksel analiz; grup içi karşılaştırmalarda Wilcoxon işaret testi, gruplar arası karşılaştırmalarda ise Mann Whitney U-testi veya eşleştirilmemiş serilerde student's t-testi ve oranların karşılaştırılmasında ise Fisher tam test ile yapıldı. 25 olgu çalışmayı tamamladı. Losartan grubunda diyaliz öncesi kan basınçları tedavinin 2. haftasından itibaren anlamlı azaldı. Çalışma süresince olguların ağırlıkları anlamlı değişmedi. Losartan grubunda 24 saatlik, gün boyu ve gece boyu SKB ve DKB değerleri azaldı. Gün boyu SKB. dışındaki azalmalar istatistiksel olarak anlamlıydı. Ama sadece losartan ve kontrol grubundaki gece boyu ortalama SKB ve DKB'ndaki yüzde değişiklikler arasında anlamlı bir fark vardı (sırasıyla, -%6 ± 9'a karşılık %1 ± 4 ve -%8 ± 12'ye karşılık %1 ± 7, p>0.05). Tedavi sonrası losartan grubunda AKBT ile SKB'nı 140 ile 160 mmHg arasında okuma oranı (%20), kontrol grubundan (%70) daha düşüktü. Losartan grubunda AKBT ile SKB'nı > 160 mmHg okuma oranı (%33.3), ofis ölçümüne göre (%0) daha yüksekti. Losartan grubundaki iki olgunun diurnal varyasyonu düzeldi. Losartan grubunda gece boyu SKB'nı > 140 mmHg ve 24 saatlik ve gece boyu DKB'nı > 90 mmHg okuma yüzdeleri anlamlı azaldı. Kontrol grubunda ise 24 saatlik, gün boyu ve gece boyu DKB'nı ] 90 mmHg okuma yüzdeleri anlamlı arttı. Her iki grubun serum açlık insulin, glikoz ve PTH düzeyleri ve URR, Kt/V, HOMA insülin direnci ve ß hücre fonksiyonu değerleri dikkate alındığında, tedavi öncesi değerler ile karşılaştırıldıklarında anlamlı değişmediler. Lipid parametreleri değerlendirildiğinde sadece kontrol grubunda total kolesterol, trigliserid ve LDL-kolesterol düzeyleri anlamlı arttı. Ayrıca, her iki grupta da çalışma sonrası böbrek fonksiyonlarında, AST, ALT düzeylerinde ve hematalojik parametrelerde anlamlı bir değişiklik olmadı. Losartan tedavisi olgular tarafından iyi tolere edildi. Sonuç olarak, hipertansif hemodiyaliz olgularında losartan tedavisinin glikoz ve lipid metabolizmaları gibi metabolik parametreleri etkilemeden kan basıncını ve kan basıncı yükünü azalttığı, fakat hipertansiyonun hedef organ komplikasyonları yönünden önemli olan gece boyu kan basıncı kontrolü dışında, losartartanın etkinliğinin kontrol grubuna göre ustun olmadığı kanaatine vardık.In this study, we aimed to investigate the effect of losartan on ambulatory blood pressure, insulin resistance and lipid profile in hypertensive hemodialysis cases. 28 stable outpatients were included in the study. The cases were divided into two groups. While 17 cases in group I were receiving 50 to 100 mg/day losartan for 16 weeks (Losartan Group), 11 cases did not receive any medication (Control group). In all cases, predialysis ambulatory blood pressure monitoring (ABPM) was performed in the first day of the week just before and after the treatment. In addition, predialysis office systolic (SBP) and diastolic (DBP) blood pressures, heart rates, pre- and post-dialysis body weights were recorded two and one week before the treatment, just before the treatment, and during the treatment with two week-intervals. After 12 hours fasting, serum glucose, insulin, urea, creatinine, uric acid, natrium, potassium, chloride, calcium, phosphorus, total protein, albumin, AST, ALT, total cholesterol, triglyceride, HDL-cholesterol, apoprotein A1 and B, lipoprotein (a), serum iron and total iron binding capacity, parathyroid hormone (PTH) level measurements and whole blood count were performed before and after treatment. Insulin resistance and ß cell reserves were calculated by homeostasis model assessment method. Dialysis adequacy was assessed by ureakinetic modelling. Statistical analysis was done with Wilcoxon signed rank test in intragroup comparisons, with Mann Whitney U-test and Student's t-test in unpaired series in intergroup comparisons, and with Fisher exact test in the comparison of rates. 25 cases completed the study. In the losartan group, blood pressures were significantly decreased starting from the second week of the treatment. Losartan treatment significantly decreased systolic and diastolic blood pressures in hemodialysis cases. The body weights of the cases did not change significantly throughout the study. In the losartan group, the 24-hour, daytime and nighttime SPB and DBP values were reduced. The reductions were statistically significant except daytime SBP. But, there was only a significant difference between the percentage changes in nighttime SBP and DBP of losartan and control groups (-%6 ± 9 vs %1 ± 4 and -%8 ± 12 vs %1 ± 7, p>0.05, respectively). After the treatment, the SBP reading rate between 140 to 160 mmHg (20%) in the losartan group was lower than the rate of the control group (70%). The SBP reading rate > 160 mmHg (33.3%) in the losartan group with ABPM was higher than the rate of the office measurement (0%). The diurnal variations of two cases in the losartan group were improved. The percentages of nighttime SBP reading > 140 mmHg, and 24-hour and nighttime DBP readings > 90 mmHg in the losartan group were significantly decreased. The percentages of 24-hour, nighttime and nighttime DBP readings > 90 mmHg in the control group were significantly increased. When serum fasting glucose and insulin, PTH levels, and URR, KW, HOMA insulin resistance and ß-cell function values of two groups were considered, they did not change significantly compared with the pretreatment values. When lipid parameters were evaluated, only total cholesterol, triglyceride and LDL-cholesterol levels in the control group elevated significantly. In addition, the renal functions, AST and ALT levels, and haematological parameters of both groups did not significantly change after the study. All cases tolerated losartan therapy well. As a result, we concluded that losartan treatment lowered blood pressure and blood pressure load in hypertensive hemodialysis cases without affecting metabolic parameters, like glucose and lipid metabolisms, but it was not superior to control group except the night time blood pressure control which was important in preventing the target organ complications of hypertension
