Primary Care Activity Level 2022 - Preliminary Report by the UMass Chan Analytics Group and MassHealth

    No full text
UMass Chan Medical School has worked with the Massachusetts Executive Office of Health and Human Services (EOHHS) since 2014 to develop risk models for MassHealth to account for how social determinants of health (SDH) and medical complexity jointly predict total health care cost.i The Primary Care Activity Level (PCAL) framework was originally developed by Ash and Ellis in 2012 to calculate risk-adjusted bundled payments for comprehensive primary care in a commercially insured population.ii It was designed to recognize patient differences in the expected cost of all services that primary care practitioners (PCPs) should be providing. Payments based on PCAL are higher for medically and/or socially complex patients than for less complex ones. In 2023 MassHealth is implementing a Primary Care Capitation program for Accountable Care Organization (ACO) primary care (PC) practices, which extends value-based payment to the provider level. Complex patients need more PC services to manage their needs than less complex patients do, yet such differences in patient complexity are not adequately reflected in fee-for-service reimbursements for primary care procedures. Here we present the development of a PCAL model that can be used to adjust primary care capitation payments based on the health status of each member; the model has been fit to MassHealth-specific data and in consideration of the state’s policy goals

Study of student satisfaction for "sandwiches" delivery approach / Eric Chan

    No full text
The summer trimester system provides students with greater flexibility to plan their study around work or lifestyle commitments, create options for students to commence a degree sooner or at a more convenient time of the year and enable students to fast track their study. There is increasing number of students enrolled in summer trimester. However, it was found that student engagement during summer is less than when those same subjects are delivered during other trimesters. This research investigates using the "Sandwiches" delivery approach to improve the learning relationship with students. This innovative approach includes the first three weeks intensive on-campus delivery with a range of lectures and tutorials. This is followed by six weeks of on-line discussions, quizzes and self-assessment activities to strengthen the students' knowledge and reinforce learning. The last week of summer trimester is revision to confirm theory prior to an examination. Positive responses from student reflected that this approach can be used for other subjects not only in summer trimester but also applicable in other trimesters. In fact, in order to improve course delivery, higher education providers always collect feedback and comment from students and previous research studies have used various methodologies. This paper demonstrates how to use survey plus case study to analyses student satisfactio

To use or not to use: ERIC database for medical education research

    No full text
Introduction Bibliographic databases are essential research tools. In medicine, key databases are MEDLINE/PubMed, Embase, and Cochrane Central (MEC). In education, the Education Resource Information Center (ERIC) is a major database. Medical education, situated between medicine and education, has no dedicated database of its own. Many medical education researchers use MEC, some use ERIC and some do not. Methods We performed a descriptive analysis using search strategies to retrieve medical education references from MEC and ERIC. ERIC references which were duplicates with MEC references were removed. Unique ERIC references were tallied. Results Between 1977 and 2022, MEC has 359,354 unique references relevant to medical education. ERIC provided 3925 unique references for the same period, all of which would be missed by searching only MEC. The mean unique ERIC medical education references per year for all 46 years is 85 (SD = ±29), or 119 (SD = ±15) for the last 10 years from 2013 to 2022. Conclusion ERIC consistently offered a small yet significant number of unique references relevant to medical education for decades. We recommend the use of ERIC for medical education research when comprehensive literature searches are required, such as in systematic reviews, scoping reviews, evidence synthesis, or guideline development
