    Analysis of matrix metalloproteinases and their inhibitors in heart tissue of chronic Chagas disease cardiomyopathy patients

    A Cardiomiopatia Chagásica Crônica (CCC) é uma cardiopatia dilatada inflamatória caracterizada por remodelamento cardíaco, uma miocardite rica em células T e macrófagos, hipertrofia e fibrose. Essa acomete 30% dos pacientes infectados com Trypanossoma cruzi (T. cruzi). Os pacientes com CCC apresentam uma pior sobrevida e prognóstico quando comparados com pacientes portadores de cardiomiopatia de etiologia não inflamatória, como a cardiomiopatia dilatada idiopática (CDI). No processo de remodelamento cardíaco, ocorre uma reestruturação da matriz extracelular (MEC), mediada em grande parte por proteínas como as enzimas proteolíticas, metaloproteinases da matriz (MMPs) e seus inibidores específicos (TIMPs), sendo que a alteração e/ou atividade de algumas dessas enzimas em particular está relacionado com as doenças cardiovasculares. Dentro deste contexto, a hipótese deste trabalho é a de que o perfil da expressão e atividade das MMPs e de seus inibidores TIMPs na CCC será distinto do encontrado na CDI. Foi avaliado por qRT-PCR a expressão gênica e por \"Western blotting\" a expressão proteica das MMPs: -1, -2, -3, -8, -9, -12-13 e EMMPRIN e dos seguintes inibidores TIMPs: -1, -2, -3, -4 e RECK e atividade da MMP-2 e -9 por zimografia, em amostras de miocárdio (ventrículo esquerdo) de pacientes com CCC, pacientes com CDI e doadores de órgãos, obtidos durante o procedimento de transplante. Embora algumas das alterações da expressão das MMPs, TIMPs, e RECK tenham sido compartilhadas com a CDI, observamos que os pacientes com CCC apresentam o aumento da atividade da MMP-2 e MMP-9 e aumento exclusivo da proteína MMP-9. Analisamos através da razão entre a atividade da MMP-2 e MMP-9 com a expressão proteica dos inibidores TIMPs e RECK, observamos o aumento da atividade da MMP-2 e MMP-9 e a diminuição dos TIMPs: -1, -2, -3 e -4 e TIMPs -1, -2 e -3, respectivamente em pacientes com CCC, ocasionando um desequilíbrio entre sua ativação e inibição o que contribui com o remodelamento do miocárdio. Também foi avaliado a fração de colágeno intersticial por Picrosirius Red, observamos que o aumento de colágeno em pacientes com CCC e CDI, o que indica que o aumento da atividade da MMP-2 e MMP-9 está modulando a síntese, conformação e organização do colágeno, contribuindo com a fibrose em pacientes com CCC. Com objetivo de verificar se estas MMPs e seus inibidores estariam sob-regulação pós-transcricional, realizamos análise in silico e identificamos 8 microRNAs (miR): miR-21, miR-29- 5p, miR-146a-5p, miR-155-5p, miR-188-5p, miR-214-3p, miR-491-5p e miR885-5p e analisamos suas expressões por qRT-PCR, mas não observamos correlação com as MMPs e seus inibidores, sugerindo que estas proteínas não estão sob regulação pós-transcricional pelos miRs analisados. Em conjunto, nossos resultados sugerem que a expressão diferencial de MMPs e seus inibidores TIMPs estudados contribuem com a fibrose, remodelamento do miocárdio e disfunção cardíaca, observado em pacientes com CCC. Desta forma, este trabalho pode ajudar na elucidação dos mecanismos de desenvolvimento da patogênese da doença de ChagasChronic Chagas Cardiomyopathy (CCC) is an inflammatory dilated cardiomyopathy characterized by cardiac remodeling, a myocarditis rich in T cells and macrophages, hypertrophy and fibrosis. This affects 30% of patients infected with Trypanosoma cruzi (T. cruzi). Patients with CCC have a poorer prognosis and survival when compared with cardiomyopathy patients with non-inflammatory etiology, such as idiopathic dilated cardiomyopathy (IDC). In cardiac remodeling process, there is a restructuring of the extracellular matrix (ECM), largely mediated by proteins such as proteolytic enzymes, matrix metalloproteinases (MMPs) and their specific inhibitors (TIMPs). The alteration and/or activity of some these particular enzymes is associated with cardiovascular disease. Taking this into account, the hypothesis of this study is that the expression profile and activity of MMPs and their TIMPs inhibitors in the CCC will be different from those found in IDC. The gene expression was evaluated by qRT-PCR, the MMP protein expression: -1, -2, -3, -8, -9, -12-13 and EMMPRIN by \"Western blotting\" and the following TIMP inhibitors: -1 , -2, -3, -4 and RECK and MMP-2 and -9 activity by zymography, in myocardial samples (left ventricle) of CCC and IDC patients and organ donors (Control) obtained upon transplantation. Although some of these changes in the expression of MMPs, TIMPs and RECK were shared with the IDC, we observed that patients with CCC have increased MMP-2 and MMP-9 activity and exclusive increase in MMP-9 protein. Through the ratio of MMP-2 and MMP-9 activity with protein expression of TIMPs and RECK inhibitors, we found that the increase in MMP-2 and MMP-9 activity and the decrease in TIMP: -1, -2, -3, and -4 and TIMP -1, -2 and -3, respectively, in myocardial samples of CCC patients causes an imbalance in their activation and inhibition, contributing to the myocardium remodeling. The fraction of interstitial collagen as stained by Picrosirius Red, indicates increase in collagen in patients with CCC and IDC, indicating that the increase of MMP-2 and MMP-9 activity is modulating the synthesis and conformation of collagen organization, contribute to fibrosis in patients with CCC. Aiming to verify whether these MMPs and their inhibitors would be under post-transcriptional regulation, we performed in silico analysis and it was identified 8 microRNAs (miR): miR-21, miR-29-5p, miR-146a-5p, miR-155-5p , miR-188-5p, miR-214-3p, miR-491-5p and miR885-5p, but no correlation was found with the MMPs and their inhibitors, suggesting that these proteins are not under post-transcriptional regulation by miRs analyzed. Together our results suggest that differential expression of MMPs and their TIMPs inhibitors studied contribute to fibrosis, myocardial remodeling and cardiac dysfunction observed in patients with CCC.     Identification of HIV-1 protease epitopes target of CD4+ T cell responses in HIV-1 infected patients

    Introdução: Uma proporção significante de pacientes infectados por HIV-1 (pacientes HIV-1+) tratados com inibidores de protease (IPs) desenvolve mutações de resistência. Estudos recentes têm mostrado que células T CD8+ de pacientes HIV- 1+ reconhecem epitopos de Pol incluindo mutações selecionadas por drogas. Nenhum epitopo CD4+ da protease foi descrito na base de dados de Los Alamos. Objetivo: Considerando que a protease de HIV-1 é alvo de terapia antiretroviral e que essa pressão pode selecionar mutações, nós investigamos se mutações selecionadas por IPs afetariam o reconhecimento de epitopos da protease de HIV-1 por células T CD4+ em pacientes tratados com IPs. Nós investigamos o reconhecimento de três regiões da protease preditas de conter epitopos de células T CD4+ bem como mutações induzidas por IPs por células T CD4+ em pacientes HIV- 1+ tratados com IPs. Materiais e Métodos: Quarenta pacientes HIV-1+ tratados com IPs foram incluídos (30 em uso de Lopinavir/ritonavir, 9 em uso de Atazanavir/Ritonavir e 1 em uso exclusivo de Atazanavir). Para cada paciente determinou-se a seqüência endógena da protease de HIV-1, genotipagem viral e tipagem HLA classe II. Utilizamos o algoritmo TEPITOPE para selecionar peptídeos promíscuos, ligadores de múltiplas moléculas HLA-DR, codificando as três regiões da protease de HIV-1 cepa HXB2 (HXB2 4-23, 45-64, e 76-95) e 32 peptídeos adicionais contidos nas mesmas regiões incorporando as mutações induzidas por IPs mais freqüentes no Brasil. Os 35 peptídeos foram sintetizados. Respostas proliferativas de células T CD4+ e CD8+ aos peptídeos foram determinadas por ensaios de proliferação com diluição do corante CFSE. Ensaios de ligação a alelos HLA classe II foram realizados para confirmar a promiscuidade desses peptídeos e avaliar a habilidade de se ligarem a moléculas HLA presentes em cada paciente. Resultados: Todos os peptídeos foram reconhecidos por pelo menos um paciente e respostas proliferativas de células T CD4+ e CD8+ a pelo menos um peptídeo da protease de HIV-1 foram encontradas em 78% e 75% dos pacientes, respectivamente. A terceira região (Protease 76 95) foi a mais freqüentemente reconhecida. Ao compararmos as respostas de células T às seqüências da protease do HIV-1 endógeno, observamos que a maioria dos pacientes não foi capaz de reconhecer peptídeos idênticos às essas seqüências, porém reconheceram peptídeos variantes diferentes das mesmas regiões. Apenas sete pacientes responderam às seqüências endógenas. Verificamos que diversos peptídeos endógenos que não foram reconhecidos apresentaram ausência de ligação a alelos HLA portados por estes pacientes, sugerindo que mutações selecionadas por pressão imune tenham levado ao escape de apresentação de antígeno e evasão de resposta de linfócitos T CD4+. Alternativamente, isso poderia ser explicado pela presença de um vírus replicante distinto presente no plasma uma vez que somente foram obtidas seqüências provirais. Conclusão: Epitopos selvagens e mutantes da protease do HIV-1 reconhecidos por células T CD4+ foram identificados. Também verificamos que a maior parte dos pacientes não reconheceu as seqüências da protease endógena enquanto que reconheceram seqüências variantes. O reconhecimento de seqüências não-endógenas poderia ser hipoteticamente conseqüência de alvo de populações HIV-1 minoritárias; protease de HERV que contém regiões de similaridade com a protease do HIV-1; ou seqüências de HIV-1 presentes apenas em parceiros virêmicos. A falha de reconhecimento de seqüências endógenas seria mais provável devido ao escape imune, do que ao nível de apresentação ou reconhecimento por células T. Isso implica em uma conseqüência patofisiológica na evasão de respostas de células T contra a protease de HIV-1 e no fato de ser tradicionalmente considerada uma proteína pouco antigênicaIntroduction: A significant proportion of protease inhibitor (PI)-treated HIV-1 infected (HIV-1+) patients develop resistance mutations. Recent studies have shown that CD8+ T cells from HIV-1 patients can recognize antiretroviral drug-induced mutant Pol epitopes. No HIV-1 protease CD4 epitopes are described in the Los Alamos database. Aims: Given that the protease of HIV-1 is a target of antiretroviral therapy and this pressure may lead to the selection of mutations, we investigated whether PI-induced mutations affect the recognition of HIV-1 protease epitopes by CD4 + T cells in PI-treated patients. We investigated the recognition of three protease regions predicted to harbor CD4+ T cell epitopes as well as PI-induced mutations by CD4+ T cells of PI-treated HIV-1+ patients. Methods: Forty PI-treated HIV-1+ patients were included (30 undergoing Lopinavir/ritonavir, 9 undergoing Atazanavir/ritonavir and 1 undergoing exclusively Atazanavir treatment). For each patients, the endogenous HIV-1 protease sequence, viral genotype and HLA class II typing were determined. We used the TEPITOPE algorithm to select promiscuous, multiple HLA-DR-binding peptides encoding 3 regions of HIV-1 HXB2 strain protease (HXB2 4-23, 45-64, and 76-95) and 32 additional peptides contained in the same regions, but encompassing the most frequent PI-induced mutations in Brazil. The 35 peptides were thus synthesized. Proliferative responses of CD4+ and CD8+ T cells against peptides were determined by the CFSE dilution assay. HLA class II binding assays were made to confirm the promiscuity of these peptides and evaluate their ability to bind the HLA molecules carried by each patient. Results: All tested peptides were recognized by at least one patient and proliferative responses of CD4+ and CD8+ T cells against at least one HIV-1 protease peptide were found in 78% and 75% patients, respectively. The third region (Protease 76-95) was the most frequently recognized. By comparing T-cell responses to HIV-1 endogenous protease sequences, we found that most patients failed to recognize identical peptides of those sequences, but recognized different variant peptides of the same region. Only seven patients responded to endogenous sequences. We found that several endogenous peptides that failed to be recognized showed no binding to the HLA alleles carried by that given patient, suggesting that mutations selected by immune pressure have led to escape of antigen presentation, as well as direct escape of the CD4+ T cell response. Alternatively, it could have been due to the presence of a different replicating virus in the plasma-since we only obtained proviral sequences. Conclusion: Wild-type and mutant HIV-1 protease epitopes recognized by CD4+ T cells were identified. We also found that most patients failed to recognize their endogenous protease sequences, while they recognized variant sequences. The recognition of non-endogenous sequences could hypothetically be a consequence of targeting a minor HIV-1 population; HERV protease, that contains regions of similarity with HIV-1 protease; or HIV-1 sequences present only in viremic partners. The failure to recognize endogenous sequences is most likely due to immune escape, either at the level of presentation or direct T cell recognition. This may have a pathophysiological consequence on evasion of T cell responses against protease and the fact that it has been considered traditionally a poorly antigenic HIV-1 protein

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Expression profile of microRNAs in myocardium during acute infection with Trypanosoma cruzi in mice

    A doença de Chagas é uma doença crônica causada pela infecção pelo protozoário Trypanosoma cruzi (T.cruzi). A sua principal consequência clínica é o desenvolvimento da cardiomiopatia chagásica crônica (CCC), que acomete 30% dos pacientes. Não foi determinado um indicador de evolução para a CCC ou permanência na forma indeterminada assintomática da doença de Chagas. Diversos trabalhos têm mostrado alterações no perfil de expressão gênica e proteômica ocorridas na fase aguda e crônica da doença de Chagas experimental e humana. Tais alterações advêm da regulação estabelecida em diversos estágios da expressão gênica e podem ser fatores relevantes no prognóstico da doença. Neste contexto, os microRNAs (miRs), podem exercer uma importante função reguladora. Sua ação se dá pela associação a um RNA mensageiro (RNAm) alvo, inibindo sua tradução ou degradando este transcrito. Assim, a hipótese deste trabalho é a de que a infecção aguda por T. cruzi modula a expressão de miRs no miocárdio de camundongos. Foi avaliado por qRT-PCR o perfil de expressão de miRs 15, 30 e 45 dias após a infecção. O perfil de expressão de miRs resultante foi suficiente para segregar os grupos de acordo com o tempo da infecção. O número de miRs diferencialmente expressos aumentou com a progressão da infecção. Além disso, seis miRs tiveram sua expressão correlacionada à piora na parasitemia e intervalo QTc dos animais: miR-142-3p miR-142-5p, miR-145, miR-146b, miR-149 e miR-21. Análises de correlação realizadas com todos os miRs avaliados ressaltaram este mesmo grupo de miRs entre os mais significativamente correlacionados, além de outros 73 correlacionados com a parasitemia, 67 com o intervalo QTc e 16 com ambos os parâmetros simultaneamente. Nas análises in silico, TNF-alfa e ciclina-D1 foram moléculas nodais recorrentes nas redes criadas com alvos dos miRs diferencialmente expressos em todos os tempos avaliados. Na única rede criada com os miRs correlacionados às alterações na parasitemia e intervalo QTc, TNF-alfa, TGF-beta, Rac1 e Src foram as moléculas nodais. Este trabalho apresenta de maneira inédita o envolvimento dos miRs durante a infecção aguda por T. cruzi, proporcionando novas perspectivas em relação a potenciais ferramentas terapêuticas e prognósticasChagas disease is a chronic illness caused by infection with the protozoan Trypanosoma cruzi (T. cruzi). Its main clinical outcome is the development of chronic Chagas cardiomyopathy (CCC), which affects 30% of the patients. The factors that define the progression to CCC or maintenance in the asymptomatic indeterminate form of the disease are still poorly understood. Several studies have presented changes occurred in the gene and proteomic expression profiles in both acute and chronic phases of experimental and human Chagas disease. Such changes result from regulation established at different stages of gene expression and may be relevant for the disease prognosis. In this context, microRNAs (miRs) may play an important regulatory function. miRs act by association to a target messenger RNA (mRNA), inhibiting translation or degrading the transcript. Thus, our hypothesis is that acute infection by T. cruzi modulates the expression of microRNAs in the myocardium of mice. The miR expression profile was evaluated by qRT-PCR 15, 30 or 45 days after the infection. This profile was sufficient to segregate the samples according to the time of infection. The number of differentially expressed miRs was higher as the infection progressed. Moreover, six miRs had their expression correlated with worsening of parasitaemia and QTc interval: miR-142-3p miR-142- 5p, miR-145, miR-146b, miR-149 and miR-21. Secondary unbiased correlation analyses showed this cluster of miRs among the most significant and other 73 miRs correlated with parasitaemia, 67 with QTc and 16 with both parameters simultaneously. In silico target prediction analyses showed TNF-alfa and cyclin-D1 as recurrent nodal molecules of the networks created with miRs targets from all time points. The network generated with miRs correlated to changes in parasitaemia and QTc interval showed TNF-alfa, TGF-beta, Rac1 and Src as nodal molecules. This work points out for the first time the involvement of miRs in the acute infection by T. cruzi, providing new insights about potential diagnostic and prognostic tool

    Biomarkers in cerebrospinal fluid and leukocytes in Major Depressive Disorder, Mild Cognitive Impairment and Alzheimer\'s disease

    O acelerado processo de envelhecimento tem trazido não só mudanças no perfil demográfico, mas também no perfil epidemiológico da população. Entre os problemas de saúde encontrados em idosos, merecem destaque os transtornos mentais, visto que acometem cerca de um terço dessa população, sendo os transtornos depressivos e a demência os problemas de saúde mental mais prevalentes. O transtorno depressivo maior (TDM), bem como o comprometimento cognitivo leve (CCL), tem sido associado prejuízo das funções cognitivas, além de aumentar o risco de desenvolvimento de demência, principalmente doença de Alzheimer (DA). Sendo assim, é importante que haja identificação preventiva de pessoas com maior risco para o desenvolvimento de DA a fim de proporcionar um tratamento precoce. Dessa maneira este estudo objetivou comparar os marcadores biológicos envolvidos na cascata amiloide, expressos em leucócitos e líquor, nas condições clínicas TDM, CCL, DA e controles saudáveis. Para tanto, foi determinada a expressão proteica das secretases ADAM10, BACE1 e PSEN1 e do peptídeo A? em leucócitos e líquor pelos métodos de Western Blotting, ELISA e Luminex. Ao analisarmos a expressão das secretases e do peptídeo Abeta em leucócitos não observamos diferenças estatisticamente significantes entre os grupos, exceto pela expressão da proteína PSEN1, que apresentou menores níveis em DA em relação à TDM. Em relação ao líquor, observamos uma significativa diminuição do peptídeo A? no grupo DA quando comparado aos grupos CCL, TDM e também a controles saudáveis; porém não foram observadas diferenças de expressão desse peptídeo entre CCL, TDM e controles saudáveis. Esses resultados sugerem que a matriz leucocitária, apesar de expressar os componentes desta via, sugerindo dispor da maquinaria enzimática necessária para o processamento da proteína precursora do amiloide (APP), não é a ideal para estudos sobre a cascata amiloide. Além disso, os resultados obtidos em amostras de líquor, de acordo com a casuística analisada, reforçam os dados da literatura que estabelecem a redução da concentração do peptídio Abeta no líquor como um biomarcador da DA, sem termos detectado alterações nos demais grupos estudados. Nossos resultados (negativos) em relação à expressão das APP-secretases no líquor acrescentam dados a este domínio ainda controverso da literatura. Assim sendo, mais estudos devem ser realizados para que possamos compreender melhor as vias relacionadas ao desenvolvimento da DAThe accelerated aging process has brought not only changes in the demographic profile, but also in the epidemiological profile of the population. Among the health problems found in the elderly, mental disorders are worth mentioning, since they affect about a third of this population, being the depressive disorders and dementia the most prevalent mental health problems. Major depressive disorder (MDD), as well as mild cognitive impairment (MCI), have been associated with higher levels of cognitive symptoms and increased risk of developing dementia, especially Alzheimer\'s disease (AD). Therefore, it is important to identify individuals at greater risk for the development of AD in order to provide treatment at early stages of the disease process. In this way, this study aimed to compare the biological markers involved in the amyloid cascade under the clinical conditions TDM, CCL, DA and healthy controls expressed in leukocytes and CSF. We determined the protein expression of ADAM10, BACE1 and PSEN1 and A? peptides in leucocytes and CSF using the Western Blotting, ELISA and Luminex methods. When analyzing the expression of the secretases and the A? peptide in leukocytes, we did not observe statistically significant differences between the groups, except for the expression of the PSEN1 protein, which presented lower levels in AD in relation to MDD. In relation to CSF, we observed a significant reduction of the Abeta peptide in AD when compared to MCI, TDM and also to healthy controls, but no differences in expression of this peptide were observed between MCI, MDD and healthy controls. These results suggest that the leukocytes, although expressing the key components and the enzymatic machinery necessary for the proteolytic processing of the amyloid precursor protein (APP), may not represent an adequate biological matrix for studies addessing the amyloid cascade. In addition, results obtained in CSF samples, according to the analyzed series, reinforce the literature data that establish the reduction of A? peptide concentration in CSF as a biomarker of AD, without detecting alterations in the other groups studied. Our (negative) results in relation to the expression of APP-secretases in CDescriptors: MSF add data to this still controversial domain of the literature.     Proinflamatory cytokine and antagonists levels in patients with chronic severe aortic regirgitation

    Determinamos o comportamento destes mediadores em pacientes com insuficiência aórtica crônica importante ( IAo ). Materal e métodos:Analisamos 89 portadores de Insuficiência Aórtica crônica importante, média etária de 33,6±11,5 anos, 84,6% sexo masculino, 60% assintomáticos, todos de etiologia reumática . Os pacientes foram submetidos a avaliação clínica e ecocardiográfica. Os valores médios foram: diâmetro diastólico (DD ) do ventrículo esquerdo ( VE ) de 71,9±8,3mm e o diâmetro sistólico ( DS ) do VE de 50,4±9,3mm, e a fração de ejeção ( FE ) do VE de 0,64±0,11. Realizamos a dosagem de Fator de Necrose Tumoral ( TNF ), seus receptores solúveis tipo I e II ( sTNFRI e sTNFR II ) , Interleucina-6 ( IL-6 ), seu receptor ( IL-6R), interleucina 1-beta ( IL-1beta ) , seu antagonista ( IL1-RA ) e endotelina-1 ( ET-1 ). Comparamos com níveis séricos de controles saudáveis. Conjuntamenete analisamos o polimorfismo genético do gene do TNF, localizado a -308 pares de bases do sítio de iniciação. Resultados: Os níveis séricos de TNF forma significativamente maiores em pacientes com IAo do que em controles normais ( 92,65±110,24 pg/ml contra 1,67±1,21 em controles normais, p < 0,001 ). Tiveram comportamentos similares os níveis séricos de sTNFRI ( 894,75±348,87pg/ml vs 521,42±395.13pg/ml em controles, p=0,007 ) e IL-6 ( 7,17±7,78pg/ml vs 0,81±0,38pg/ml, p=0,0001 ). Observamos correlação significativa entre níveis de sTNFRII e DDVE ( r=-0,329, p=0,038 ) e DSVE ( r=-0,352, p=0,027). Não observamos relação de níveis séricos de citocinas com sintomas. As outras citocinas não guardaram relação com parâmetros de gometria e função ventricular. A presença do alelo 2 do polimorfismo genético do TNF associou-se a paciente assintomáticos com IAo. Conclusão: Demonstramos níveis elevados de citocinas proinflamatórias em pacientes com IAo em relação a controles normais. Os níveis de sTNFRII diminuem com o aumento dos diâmetros ventriculares. A presença do alelo 2 do polimorfismo -308 do TNF associa-se a pacientes assintomáticos com IAoBackground - Proinflamatory cytokines are implied in the phisiopatology of heart failure secoundary to ischaemic or idiophatic dilated cardiomiopathy, but there are few studies regarding these mediators in valular heart disease. We determined the behaivour of proinflamatory cytokines and their antagonists in patients with chronic severe aortic regurgitation ( IAo ) Methods - We analised 89 patients with IAo mean age 33.6±11.5 years, 84.6% male, 60% asymptomatic, all of rheumatic etiology. Patients were evaluated clinnicaly and by echocardiography. Mean values were : left ventricular ( LV ) diastolic diameter ( DD ) 71.9±8.3mm, LV systolic diameter ( SD ) 50.4±9.3mm and ejection fraction 0.64±0.11. We made the plasma dosages of tumor necrosis factor-alpha ( TNF ), its soluble receptors type I and II ( sTNFRI and sTNFR II ) , Interleukin 6 ( IL-6 ) and its receptor ( IL-6R ), Interleukin 1-beta ( IL-1beta ) , its antagonist ( IL1-RA ) and endothelin-1 ( ET-1 ). Plasma levels were compared to healthy controls. We also analysed the TNF gene polimorphism, located at - 308 base pairs from the initation site. Results - Plasma TNF levels were significantly increased in IAo patients in telation to normal controls (92.65±110.24 pg/ml vs 1.67±1.21 pg/ml in controls, p < 0.001 ). Similar behaivour was observed with IL-6 (7.17±7.78pg/ml in IAo patients vs 0,81±0,38pg/ml in controls , p=0.0001 ) and sTNFRI (894.75±348.87pg/ml vs 521.42±395.13pg/ml in controls, p=0,007 ). We observed and significant relation between sTNFRII levels and LVDD ( r=-0,329, p=0,038 ) e LVSD ( r=-0,352, p=0,027). Levels of cytokines were similar in asymptomatic and sympromatic patients and the other cytokines had no relation to ventricular diameters or function. Presence of the alele 2 of the -308 TNF polimorphism was associated to asymptomatic patients. Conclusion - We showed increased plasma levels of proinflamatory cytokines in patients with IAo in relation to normal controls. There was an decrease of sTNFRII levels with increase in ventricular diameters. The presence of the alele 2 of the -308 TNF polimorphism was associated to asymtomatic patient

    A pandemia de AIDS é um dos principais problemas de saúde pública no mundo e demanda o desenvolvimento de uma vacina eficaz. Uma abordagem vacinal ideal, baseada em resposta celular contra o HIV-1, deveria induzir uma resposta imune mediada tanto por células T CD4+ quanto CD8+. A diversidade genética do HIV-1 é uma grande preocupação para o desenvolvimento de uma vacina e sequências consenso têm sido utilizadas a fim de contornar a barreira imposta por essa diversidade. A escolha apropriada dos antígenos a comporem as construções vacinas também é relevante, visto que proteínas como Gag e Vif têm se mostrado bastante imunogênicas, enquanto alguns trabalhos têm demonstrado que Env possui características imunossupressoras e que respostas celulares contra esse antígeno podem ser danosas aos indivíduos vacinados. Nosso grupo demonstrou que uma vacina de DNA (HIVBr18) codificando 18 peptídeos para linfócitos T CD4+, promíscuos (capazes de se ligarem a múltiplas moléculas HLA-DR) e conservados na sequência consenso do subtipo B do HIV-1 foi capaz de induzir uma resposta celular ampla, polifuncional e de longa duração em camundongos BALB/c e transgênicos para moléculas HLA. Neste trabalho identificamos 34 peptídeos potencialmente reconhecidos por linfócitos T CD4+, promíscuos e conservados na sequência consenso dos consensos do grupo M do HIV-1. Uma vacina de DNA (HIVBr27) codificando 27 dos 34 peptídeos (exceto os 7 peptídeos de Env identificados) induziu uma resposta mais ampla e de maior magnitude que a vacina HIVBr18 em camundongos BALB/c. Além disso, a vacina HIVBr27 induziu maior frequência de linfócitos T CD4+ e CD8+ polifuncionais, capazes de proliferar e produzir as citocinas IFN-gama e TNF-alfa. Desenvolvemos também uma vacina de DNA (HIVenv7) codificando os 7 peptídeos de Env do HIV-1 identificados. A co-imunização de HIVenv7+HIVBr27 reduziu a amplitude da resposta celular contra peptídeos codificados pela vacina HIVBr27. Além disso, a co-imunização reduziu a magnitude da resposta e a frequência de linfócitos T CD4+ e CD8+ polifuncionais contra o pool de 27 peptídeos codificados por essa vacina. A vacina HIVBr27, desenhada para induzir uma resposta de linfócitos T CD4+ ampla e intensa contra peptídeos promíscuos e conservados da sequência consenso dos consensos do grupo M do HIV-1, é mais imunogênica e mais completa que a vacina HIVBr18, tendo potencial de conferir, em grande cobertura populacional, imunidade contra os diversos subtipos circulantes do vírus. O fenômeno observado na co-imunização com HIVenv7 sugere que a inclusão do envelope em imunógenos contra o HIV-1 possa ser prejudicial. Por outro lado, isto faz desse plasmídeo um alvo promissor para terapias imunológicas que visem indução de imunossupressãoThe AIDS pandemic is a worldwide major public health problem and requires the development of an effective vaccine. An ideal vaccine approach based on cellular immune responses against HIV-1 should induce an immune response mediated by both CD4+ and CD8+ T cells. HIV-1 genetic diversity is a major concern for developing a vaccine and consensus sequences have been used to circumvent the barrier posed by this diversity. The appropriate choice of antigens to compose the vaccines is also relevant, since proteins such as Gag and Vif have been shown to be immunogenic, while some studies have shown that Env has immunosuppressive characteristics and cellular responses against this antigen can be harmful to vaccinated individuals. Our group has demonstrated that a DNA vaccine (HIVBr18) encoding promiscuous multiple HLA-DR binding, conserved B-subtype HIV-1 CD4+ T cell epitopes was able to induce a broad, polyfunctional and long lasting T cell response in BALB/c and HLA transgenic mice. In this work we identified 34 promiscuous and conserved sequences within the group M HIV-1 consensus of the consensus sequence, potentially recognized by CD4+ T cells. A DNA vaccine (HIVBr27) encoding 27 of the 34 peptides (except the 7 Env identified peptides) induced a broader and higher magnitude T cell response than HIVBr18 vaccine in BALB/c mice. Moreover, the vaccine HIVBr27 induced a higher frequency of polyfunctional CD4+ and CD8+ T cells, able to proliferate and produce the cytokines IFN-gama and TNF-alfa. We also developed a DNA vaccine (HIVenv7) encoding the 7 HIV-1 Env identified peptides. Co-immunization with HIVenv7+HIVBr27 reduced the breadth of the cellular immune response against the HIVBr27 encoded peptides. Besides, co-imunization reduced the magnitude of the response and the frequency of polyfunctional CD4+ and CD8+ T cells against the pool of 27 peptides encoded by this vaccine. The HIVBr27 vaccine, designed to induce a broad and intense CD4+ T cell response against promiscuous and conserved peptides within the group M HIV-1 consensus of the consensus sequence, is more immunogenic and more complete than the vaccine HIVBr18, having the potential to provide, with wide population coverage, immunity against various circulating subtypes of the virus. The phenomenon observed in the co-immunization with HIVenv7 suggests that the inclusion of the envelope in immunogens against HIV-1 may be harmful. On the other hand, these results suggest that HIVenv7 is a promising target for immune therapies aimed at inducing immunosuppressio

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
