Disrupted nighttime sleep and sleep instability in narcolepsy
STUDY OBJECTIVES: This review aimed to summarize current knowledge about disrupted nighttime sleep (DNS) and sleep instability in narcolepsy, including self-reported and objective assessments, potential causes of sleep instability, health consequences and functional burden, and management. METHODS: One hundred two peer-reviewed publications from a PubMed search were included. RESULTS: DNS is a key symptom of narcolepsy but has received less attention than excessive daytime sleepiness and cataplexy. There has been a lack of clarity regarding the definition of DNS, as many sleep-related symptoms and conditions disrupt sleep quality in narcolepsy (eg, hallucinations, sleep paralysis, rapid eye movement sleep behavior disorder, nightmares, restless legs syndrome/periodic leg movements, nocturnal eating, sleep apnea, depression, anxiety). In addition, the intrinsic sleep instability of narcolepsy results in frequent spontaneous wakings and sleep stage transitions, contributing to DNS. Sleep instability likely emerges in the setting of orexin insufficiency/deficiency, but its exact pathophysiology remains unknown. DNS impairs quality of life among people with narcolepsy, and more research is needed to determine its contributions to cardiovascular risk. Multimodal treatment is appropriate for DNS management, including behavioral therapies, counseling on sleep hygiene, and/or medication. There is strong evidence showing improvement in self-reported sleep quality and objective sleep stability measures with sodium oxybate, but rigorous clinical trials with other pharmacotherapies are needed. Treatment may be complicated by comorbidities, concomitant medications, and mood disorders. CONCLUSIONS: DNS is a common symptom of narcolepsy deserving consideration in clinical care and future research.
Abstract : During the centuries, the notebooks are undergoing a process of naturalization. However, what we see is that even naturalized, the use of this material is not natural for children who daily demonstrate their difficulties when handling this material. This naturalization process together the use of these materials in the classroom of the day today is what motivates us to think and discuss about the meanings attributed by the children at their school notebooks. Thus, this research aims at investigating the uses and meanings attributed by the children in the first year of primary school to their school notebooks. Thus, this study of a qualitative and ethnographic based on the following instruments: participant observation of teaching and learning, informal meetings with students and with the teacher, intervention, analysis of documents (notebooks), semi-structured interviews and diary research. The research was conducted in two stages: the first is the literature review conducted in order to map the studies already undertaken on the theme and contextualize historically problematic issue and the second step is the empirical research carried out in two groups of first year of a municipal school in Florianópolis in which there was a class in the year 2012.2 and another class in the year 2013.1. Were conducted semi-structured interviews with the children and with the teacher every class regent. Data were analyzed using the content analysis, with reference to the ideas of Bardin (2011). The data analyzed show us how the meanings attributed to this material by teachers differ from the meanings attributed to this material by children. These indicators show how the use of school notebook is still a topic that needs reflection, discussion and planning, so that in fact this material meets the educational purposes

The Lovers
McEvilly's diaristic, 28-"chapter" account of Abramovic's and Ulay's respective performance-walks toward each other along the Great Wall of China forms the backbone for this catalogue-cum-travelogue; he discusses the history of the artists and his relationship to them, the spiritual and aesthetic aspects of walking, and the adventures and misadventures of preparing for and carrying out the trip. He draws from both European authors and Chinese poets to enhance his perspective on the subject. Mignot contextualizes the piece within the artists' career and gives an account of the exhibition; two sections are dedicated to photos and writings of each artist inspired by this experience.
