Thrombophilic screening in young patients (< 40 years) with idiopathic ischemic
stroke: a controlled study.

INTRODUCTION: Despite extensive clinical and laboratory investigations, the
etiology of ischemic stroke remains unknown in approximately one third of
patients.
MATERIALS AND METHODS: Thirty-four consecutive patients less than 40 years old
(Males 13, Females 21, mean age 26.6 years, range 2-39) with documented ischemic
stroke underwent, one year after the acute event, laboratory evaluation of
antithrombin, protein C, free and total protein S, activated protein C
resistance, fibrinogen, factor VII:C, homocysteine levels and antiphospholipid
antibodies (APA). Moreover, prevalence of F5 R506Q, F2 G2021A and homozygosis for
thermolabile variant C677T of the methylenetetrahydrofolate reductase (MTHFR)
were also evaluated and compared to the results obtained in 120 normal controls.
RESULTS: Antithrombin and protein C levels resulted normal in all cases. One
patient (2.9%) showed free protein S deficiency and 3 patients (8.8%) had
activated protein C resistance. Homocysteine levels above 15 μmol/L were found in
one patient (2.9%). APA were found in 21 patients (61.7%) and in only 2 out of
120 (1.66%) controls (OR=95.31; 95% C.I.: 18.22-667.81). The multivariate
analysis selected that the presence of APA was significantly associated with an
increased risk of stroke (OR=156.60; 95% C.I.: 25.99-943.47) in this cohort of
patients. The combination between APA and cardiovascular risk factors determined
a risk of 29-fold (OR=29.31; 95% CI: 3.28-261.69).
DISCUSSION: Our data suggest that the presence of APA is associated with an
increased risk of idiopathic ischemic stroke in young patients. Furthermore, also
the combination of APA and cardiovascular risk factors is significantly
associated with development of idiopathic ischemic stroke.

Convexity and Semiconvexity along Vector Fields
Given a family of vector fields we introduce a notion of convexity and of semiconvexity of a function along the trajectories of the fields and give infinitesimal characterizations in terms of inequalities in viscosity sense for the matrix of second derivatives with respect to the fields. We also prove that such functions are Lipschitz continuous with respect to the Carnot-Carathéodory distance associated to the family of fields and have a bounded gradient in the directions of the fields. This extends to Carnot-Carathéodory metric spaces several results for the Heisenberg group and Carnot groups obtained by a number of authors
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
A multi-adaptive framework for the crop choice in paludicultural cropping systems
The conventional cultivation of drained peatland causes peat oxidation, soil subsidence, nutrient loss, increasing greenhouse gas emissions and biodiversity reduction. Paludiculture has been identified as an alternative management strategy consisting in the cultivation of biomass on wet and rewetted peatlands. This strategy can save these habitats and restore the ecosystem services provided by the peatlands both on the local and global scale. This paper illustrates the most important features to optimise the crop choice phase which is the crucial point for the success of paludiculture systems. A multi-adaptive framework was proposed. It was based on four points that should be checked to identify suitable crops for paludicultural cropping system: biological traits, biomass production, attitude to cultivation and biomass quality. The main agronomic implications were explored with the help of some results from a plurennial open-field experimentation carried out in a paludicultural system set up in the Massaciuccoli Lake Basin (Tuscany, Italy) and a complete example of the method application was provided. The tested crops were Arundo donax L., Miscanthus(x)giganteus Greef et Deuter, Phragmites australis L., Populus(x)canadensis Moench. and Salix alba L. The results showed a different level of suitability ascribable to the different plant species proving that the proposed framework can discriminate the behaviour of tested crops. Phragmites australis L. was the most suitable crop whereas Populusxcanadensis Moench and Miscanthusxgiganteus Greef et Deuter (in the case of biogas conversion) occupied the last positions in the ranking
The F-MACHOP sequential combination chemotherapy regimen in advanced diffuse aggressive lymphomas: long-term results.
The present study reports the long-term results of the F-MACHOP sequential combination chemotherapy regimen in 113 adult patients with advanced stages of diffuse large-cell or undifferentiated lymphoma who were consecutively treated with this regimen from January 1980 to December 1986 at a single institution. The CR rate was 78% and 17 relapses were observed, so that 80% of CR patients are expected to be free of relapse at 5 and 10 years after a median follow-up off-therapy of 5.7 years (range 3.2-10.1). Projected survival and event-free survival at 8 years are 68% and 62%, respectively. Prognostic factor analysis indicates that the CR rate was influenced by systemic symptoms and number of extranodal sites, disease-free survival by tumor burden and number of courses to achievement of CR, event-free survival and survival by tumor mass diameter, systemic symptoms and number of extranodal sites. Toxicity was mainly hematological, and 3 patients (3%) died while severely neutropenic. The rapidity of response appears to have prognostic value, particularly in patients presenting with high tumor burden, in whom achievement of a CR within 3 courses indicates a group of patients with a very high chance of cure with this regimen
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
