SARCOPENIA IN THE ELDERLY: PATHOGENESIS, DIAGNOSIS AND TREATMENT
‘Sarcopenia’ involves a progressive age-related loss of muscle mass and associated muscle weakness that renders frail elders susceptible to serious injury from sudden falls and fractures and losing their functional independence This disease has a complex multifactorial pathogenesis, which involves not only age-related changes in neuromuscular function, muscle protein turnover, and hormone levels and sensitivity, but also a chronic pro-inflammatory state, oxidative stress, and behavioral factors – in particular, nutritional status and degree of physical activity. In the previous definition by the European Working Group on Sarcopenia in Older People (EWGSOP) in 2010, the diagnosis of sarcopenia requires the presence of both low muscle mass and low muscle function. Since the 2010 definition is difficult to be translated to clinical practice, the EWGSOP uses low muscle strength as the primary parameter of sarcopenia in the 2018 definition; sarcopenia is probable when low muscle strength is detected. A sarcopenia diagnosis is confirmed by the presence of low muscle quantity or quality. When low muscle strength, low muscle quantity/quality and low physical performance are all detected, sarcopenia is considered severe. According to the pathophysiological factors involved in the pathogenesis of sarcopenia, different treatment strategies against sarcopenia are resistance exercise training, increase essential amino acids intake, vitamin D supplementation for those with vitamin D deficiency, polyunsaturated fatty acids (PUFAs) supplementation, testosterone supplementation, angiotensin-converting enzyme inhibitor administration
This study addresses the concept of the soul according to Edith Stein as a foundation
for understanding her notion of human person. We will show through three studies
published by the author at distinctly different moments: Zum Problem der Einfühlung
(1916), Der Aufbau der menschlichen Person (1932) and Kreuzeswissenschaft
(1942)], a parallel between the way she delves into the phenomenological analysis of
the soul, very much influenced by the phenomenological method taught by Edmund
Husserl, and her own existential experience in her intimate relation with God. We
found that in her 1916 thesis, On the problem of Empathy, Edith Stein had already
elaborated a structural concept of soul which, while enhanced and amplified,
remained largely unaltered throughout her subsequent works. Thus, the
phenomenological analysis of the concept of soul manifested in these three works,
allows us to observe a profound coherence and continuity of thought throughout an
significant period of her life, as well as her unswerving adherence to Edmund
Husserl’s phenomenological method
Occupational History as a Predictor of Cognitive Ability in the Elderly
Cognitive ability is important for the elderly to be able to live independently. Many factors influence this ability in elderly, one of them being occupational history. The purpose of this study was to analyze occupational history as a cause of decreased cognitive ability. An observational analytic study with a crosssectional approach. 87 subjects who met the inclusion criteria were recruited from Nursing Homes in Surabaya. 71.3% of the subjects have a history of physical-dominated occupation and 28.7% have intelectual dominated occupation. There were no differences regarding duration of work and perception of workload in both groups. The average cognitive score is 18.57 (SD 5.939). The ANOVA test between occupational history and cognitive abilities shows p=0.001, R Square=0.172, and estimated standard of error of 5.501. This study concluded that occupational history can be used as a predictor of cognitive ability in elderly
Culture, contexts and connections: A conversation with Dennis Klass about his life and work as a bereavement scholar
This article is a conversation between Edith Maria Steffen and Dennis Klass, one of the originators of the continuing bonds model in bereavement. Steffen and Klass recently co-edited a new anthology on continuing bonds that follows the developments in the continuing bonds model of grief over the last two decades. Klass got into the field of bereavement studies initially through working with Elisabeth Kübler-Ross and continued by engaging in ethnographic research with a self-help group of bereaved parents. Trying to make sense of the parents’ experiences, he tested different theories against the data from the bereaved parents, which led him to discard conventional theories and instead apply what he saw in Japanese ancestor rituals to the practices of the bereaved parents. Klass discusses how the idea of continuing bonds was born, how the 1996 publication came about, how it was received, and how he took this work further. He looks back on his life and work and looks ahead towards work that still needs to be done. The conversation then turns to Klass’ idea that consolation is a neglected area in bereavement and ends with Klass giving advice to novice bereavement scholars, which could be summarised in his motto, follow the data.© 2018, Informa UK Limited, trading as Taylor & Francis Group.
The present dissertation analyzes the intellectual trajectory of the philosopher is phenomenologist Edith Stein (1891-1942). The research focuses especially on the role of the empirical phenomenon (Einfühlung), not only as thematic and its dissertation object "On the Problem of Empathy" in 1916, but rather as a condition of possibility for the elaboration of present historical and philosophical knowledge in all his works. To pursue this objective we chose to follow the guidance of Ângela Alles Bello, "in order to understand the Steiniano philosophical itinerary, it is necessary to begin the analysis from its first work to the last. 3
Scholars in Edith Stein's work often explain that her philosophical production is divided into three periods, which help us understand the coherence and continuity between the author's biography and her intellectual production. The first phase can be characterized as the phenomenological period, which extends from his doctoral thesis in Gottingen in 1916 until his conversion to Catholicism in 1922; the second phase begins in 1922 and goes until its passage through the convent of Carmel in Cologne, where the focus of the study was the relationship between the human person and society through the pedagogical-anthropological character; and the third phase that began in 1938 to 1942, this phase is known by eminently mystical writings with the focus of the dialogue between the philosophy of St. Thomas Aquinas and Husserlian phenomenology.
4
Through the concept of experiences (Erlebins) we will analyze works of the three phases of Steiniano thought based on the relationship between the life mapped by the letters - present in the complete works - and two biographies written by the author being the works History of a Jewish family and Yellow Stars.
Among the writings of the phenomenological phase are mainly his PhD thesis - On the Problem of Empathy, which he wrote under the guidance of Edmund Husserl (defended in 1916), as well as an essay written in 1925, A Survey on the State. Regarding this phase, I intend to focus on the problem of experiences (Erlebins). I will analyze his doctoral thesis where he already delineates some important themes that will lead to his later reflections, as well as his position in the phenomenology proposed by Husserl especially after his transfer to Freiburg.
Among the works that make up the second period, we highlight the Intellect and the Intellectuals, written in 1930, as well as the Structure of the Human Person, both belong to the pedagogical-anthropological period, are lectures in which Edith Stein tests her phenomenological method to understand her own time.
In these works we will try to explain the hypothesis that Edith Stein presents a harmonization between the application of the phenomenological method and the metaphysical questions, having as a means of this dialogue the historical processes that would make it possible to unveil the essential structure common to all human beings and simultaneously, discover the ultimate essence that guarantees uniqueness
