This study employs assumptions from different theoretical and empirical works on economic growth. The most important are the neoclassical concept of conditional convergence (the effects past conditions have on future growth), Robert Barro's endogenous growth assumptions about the importance of education and institutions, and panel data analysis methodology inspired by Prochniak and Kahouli & Kadhraoui

Biological therapy: a Risk Factor for Developing Tuberculosis - a Case Report

Introduction: Crohn`s disease is an autoimmune chronic inflammatory disease that most commonly affects the terminal ileum and the proximal colon. It causes local tissue damage that considerably affects the mucosa and leads to the development of fissures and ulcerations. The contemporary strategy of treatment includes corticosteroids and immunosuppressants, as well as biological therapy with infliximab and adalimumab.Materials and Methods: We present you the case of B.T. - a 21-year-old male diagnosed with Crohn`s disease 3 years ago. He had been subjected to biological treatment for 26 months with adalimumab prior to admittance to St. Marina University Hospital, Varna with clinical manifestation of acute respiratory infection. Physical examination and an imaging test (X-ray) showed no evidence of lobar infiltrate on the left. The persistent manifestation of the disease with fever, no evident recovery despite adequate antibacterial therapy, a history of positive past T-SPOT TB test and undergoing treatment with immunosuppressants required the implementation of additional tests: chest CT, FBS, Mantoux test, microbiology and laboratory testing, in order to determine the diagnosis. Results: Direct bacterioscopy of bronchoalveolar lavage was positive for acid-fast bacteria. Based on the presented data and the clinical course of the disease, the final diagnosis was infiltrative pneumonic form of pulmonary tuberculosis. Tuberculostatic treatment was started in standard doses. Microbiological cultures were positive twice, the second time during tuberculostatic therapy, which required a longer period of treatment. Conclusion: Biological therapy is a risk factor for developing tuberculosis. That requires accurate judgment and exclusion of underlying latent tuberculosis infection before starting the treatment. Monitoring and control tests are highly important for early diagnosis and successful treatment should a specific infection develop

    Reginistas also known as “Reginas”

The Reginista movement is a New Religious Movement that follow Antonio Velazco Piña's best-selling novel "Regina. Dos de Octubre no se olvida" as the basis for their believe system and organization. The Movement was officially formed when, after publishing the novel, Antonio Velasco Piña appointed seven women to start the “First Circle of Regina”. The seven women, gathered in Aldea de los Reyes to commemorate Reginas sacrifice on October 2. Each of them, made the promise to recruit seven new women (total 49 new women) next year to make their circle bigger. In the fictional novel, Regina, the main character has a unique life. She lives in several countries (Tibet, China, Mexico) where she is trained by different spiritual teachers. In the book, this messianic character, who through her journey discovers her mission to “awaken” the energy of Mexico, which is believed to have been disrupted after the Spanish Conquest. Regina decided to go back to Mexico to find out how she can complete her spiritual mission. On her return, she encounters Mexico’s social unrest. It is in this part of the narrative, where fiction and history are entangled. The student movement of 1968 is where the author positions Regina’s final mission: not just to awaken Mexico’s energy, but to awaken the female energy of the country. In her meditations, Regina founds that the only way to achieve her goal is to commit self-sacrifice. With a group of followers and Indigenous teachers, she marches on October second, knowing she, and her followers would be assassinated. “Regina. Dos de octubre no se olvida” has been a controversial novel in Mexico. On one hand, it created the religious movement of the “Reginistas” or “reginas”, but on the other, it re-narrates a historical event in Mexican history to de-politicize the event in order to “spiritualized” it. Another controversial event is that Antonio Velasco Piña named her character Regina Teuscher, after one of students that died and whose photo appeared in the national press after the Tlatelolco Massacre. The family members from Regina Teusche confronted Antonio Velasco Piña in a book presentation. However, followers of the novel defend Velasco Piña’s narrative as real and do not separate fiction from reality. The "reginistas" (community members of the movement) gather at least once a year on October second to commemorate the self-sacrifice of Regina, as based on the fictional novel. This annual ritual takes place between Aldea de Los Reyes (2 hours away from Mexico City) and Tlatelolco, the neighborhood in Mexico City where the 1968 student massacre occurred. In Aldea de Los Reyes, the ritual is a night vigil with rituals, songs, and dances (the “reginistas” have created their own dance that is believed to be a combination of Conchero and Tibetan dances). Food is shared at dawn, and a committed then is sent to Tlatelolco to lay down the main offering made during the night with flowers. Since Tlatelolco is protected by the INAH (Mexican Institute of Anthropology and History), “reginistas” request a permit in advance to enter the area, but when not granted they leave the offering outside of the site. In addition to the annual ritual, “reginistas” also participate in “sacred walks” around Mexico City that are meant to “awaken” Mexico’s energy. These walks are organized by older reminisces and the dates for the walks and the map with a guided walk is usually shared through their personal emails or social media. There are two general routes around the city, one that corresponds to the masculine energy and one that corresponds to the femenine energy. For “reginistas”, they main objective is to finish the tasks that Regina herself was not able to complete. Because of the nature of the book, this religious movement is syncretic, mixing belief systems, practices, and material culture from Indigenous Mesoamerica, Hinduism, Buddhism, and Theosophy. The “Regina” movement is a good example of the Mexican New Age, where all these religions collude to form one movement with a nationalistic ideology. Most of the women that joined the “reginista” movement were practitioners of other “non-tradition” (Catholicism is the most practiced religion in Mexico) religious affiliations. Some of them were part of the Conchero Movement (an Indigenous Dance that mixes Catholic belief and Indigenous ritual), The Gran Fraternidad Universal -Universal Great Brotherhood- (which follows New Age belief and incorporated yoga practice), and Theosophy. Although most of the women (and Men) in the “reginista” movement grew up Catholic, there any connection to Catholicism is through the syncretic images used (like the Virgin of Guadalupe). Since the 2020 covid Pandemic, some of the oldest members have passed away, including the writer of “Regina. Dos de Octubre no se olvida”. This is a small movement that is always surprising historians and anthropologists. With Velasco Piña’s passing, some of the main leadership got disappeared, and although it seemed this may be the indication of the movement dyeing, a Mexican production announced in 2022 they are working on Regina, the movie.Non UBCUnreviewedFacult

The aim of the study is to evaluate the relationship between the season of birth and the birth weight and length of Bulgarian newborns. The weight and length data of 6517 (6098 full-term and 419 preterm) live births in 2000–2001 were collected from the birth registry of II Hospital of Obstetrics and Gynaecology “Sheynovo”, Sofia, Bulgaria. Statistical analyses were done using the SPSS 16 software for Windows: descriptive statistics; the t-test (p<0.05); One-Way ANOVA, (Tukey, HSD-test, p<0.05) and the Pearson’s correlation. The sunshine dura-tion data for 2000 and 2001 were collected at the Sofia Meteorological Station. The mean weight of Bulgarian neonates born in 2000-2001 was 3389.8 g in boys and 3261.8 g in girls. The average newborn’s length was 51.0 cm and 50.3 cm in boys and girls, respectively. In all seasons, significant gender differences were observed with a priority for boys (p≤0.001). The winter period was identified with a peak in birth length for both sexes, and spring and summer were the seasons with the lowest values for boys and girls, respectively. A significant positive correlation between birth length and the daily amount of sunshine during the prenatal period was found (p<0.001).Seasonal fluctuations influenced weight and length in Bulgarian neonates. The results obtained in this study can be useful in prenatal diagnostics, neonatal care, and health prevention of pregnant women and neonates

No abstrcatThe author attempts to describe the town of Yanitsa (Turkish Yenidje-Vardar) near Thessaloniki, as it was during the end of 19th century. He gets his information from the Turkish land registers and the land-tax ones, kept at the Historical Archives of Macedonia, in Thessaloniki. After a brief description of the registers, he presents the results of his research, and gives the names of the Turkish and Greek districts, the names of the roads and the numbers of houses and shops in every district. He comes to the conclusion that the town had 1.483 Moslem houses divided in ten districts, and 925Christian houses divided in four districts. A map of the old town is provided, where the names of roads and districts as well as the sites of several Turkish monuments, preserved until today, have been added by the author.

    No full text
Introduction: Spinal tuberculosis (Pott`s Disease) is a secondary form of tuberculosis, which occurs in less than 1% of all infected patients with Mycobacterium tuberculosis. The clinical presentation consist of osteomyelitis and arthritis, resulting in local destruction of the vertebral bodies, mainly in the thoracic region of the spine. Common complications are related to neurological impairment, kyphosis and gibbus formation. Methods and materials: We present a 4-year-old boy with normal physical and neurological development and no history of frequent infections. The child received Bacillus Calmette - Guérin (BCG) vaccine in the neonatal period but the local reaction and the induced immunity were not evaluated subsequently. In October 2017 the patient was admitted to St. Marina University Hospital with clinical evidence of persistent pneumonia and lower extremity paraparesis with muscular hypertonus, as well as hyperreflexia with expanded reflex receptive fields, clonus of the lower limbs and bilaterally positive Babinski sign. At the same time a tender, painful deformity in the interscapular region was present at hospitalization. Results: Diagnostic images revealed the presence of mediastinal and supraclavicular lymphadenopathy, significant paravertebral liquefaction in the thoracic region, destruction of vertebral bodies with spinal cord compression and formation of a gibbus. The Mantoux test was positive, the T-SPOT.TB test - negative, because of suppression of functional T-lymphocytes activity. Conclusion: Pott`s disease is a rare condition, but remains one of the most dangerous forms of secondary tuberculosis, due to the neurological complications. Regular vaccination and strict evaluation of the induced immunity are the most important part of the preventive strategy

Diagnosis of Fanconi anemia - a clinical practice challenge

    No full text
Introduction: Fanconi anaemia is a rare genetic disorder with autosomal recessive or X-linked recessive inheritance. This condition is characterized by multiple gene mutations leading to errors in DNA replication and manifests with progressive pancytopenia, bone marrow failure, multiple congenital malformations and predisposition to malignancies.Materials and methods: The patient is a 4-year-old male, born preterm at 36 weeks with a weight of 2220g and a length of 43cm. At birth multiple skeletal anomalies were found -right radial aplasia, bilateral absence of metacarpal and phalangeal bones of the first row, as well as radial deviation of the right metacarpal bones and improper alignment of the left metacarpal bones. Facial dysmorphism, atrial septal defect and single hypopigmented macules were also observed. At the age of 3 the patient was diagnosed with hypopituitarism, due to clinical and laboratory evidences of hypogonadism, secondary hypothyroidism and growth hormone deficiency he was treated with L-Thyroxine and growth hormone for a year. Results: During a routine clinical examination a hemorrhagic rash of the lower limbs was found which required the implementation of a haemogram, biochemistry test and bone marrow biopsy with flow cytometry. The results revealed signs of moderate to severe anemia, and Stage III - IV thrombocytopenia, as well as a hypocellular bone marrow without cytomorphological and flowcytometrical signs of clonal proliferation. These deviations from the normal hematological values combined with all existing physical abnormalities and short stature led to the consideration of Fanconi anemia in the differential diagnosis. Diepoxybutane (DEB) analysis was performed, proving chromosome breakage and deletions in 3rd, 7th, 12th and 13th chromosomes. Conclusion: Fanconi Anemia is a rare hereditary disorder with a poor prognosis. Well-timed diagnostics allows early bone marrow transplantation, which can improve complications and reduces the risk of cancer development

A rare case and diagnostic process of `gray zone` lymphoma

    No full text
Introduction: Gray Zone Lymphoma (GZL) is a rare subtype of lymphoid neoplasm with aggressive development and difficult diagnostics. The term `gray zone lymphoma` has been used to denote a new pathomorphological category of unclassified B - cell lymphoma with intermediate features between diffuse large B-cell lymphoma (DLBCL) and classical Hodgkin`s lymphoma (CHL). It typically occurs in young male and presents as a solitary mediastinal mass, although often its clinical manifestation is not specific. GZL requires a specific treatment approach, because of the fundamental differences in the development of CHL and traditionally more aggressive B - cell lymphoma.Materials and methods: A clinical case of 50-years-old female, complaining of generalized pruritus is presented. Conducted tests reveal anaemia, increased cholesterol, liver enzymes at the upper limit of the reference values and slightly elevated. A few months later the patient is admitted to a rheumatology clinic with constitutional symptoms and a small-joint inflammation, where conducted computed tomography of the chest revealed a solitary mediastinal mass, which also affects the right lung. Results: Diagnostic searching necessitates performing of fibrobronchoscopy with lavage and fine needle aspiration, followed by core biopsy. Microbiological testing proved no specific inflammation and cytological examination reveals evidence of Rosai - Dorfman disease. Histological conclusion from the biopsy describes inflammatory myofibroblastic tumor. After upper right lobectomy, pathomorphological examination of the resected material shows classical Hodgkin`s lymphoma of nodular sclerosis subtype (CHL-NS) with primary lung affection, although immunohistochemical analysis and flow cytometry prove expression of tumor markers, normally produced by DLBCL.Conclusion: These results lead to a new pathological revision, which determined the final diagnosis - Gray Zone Lymphoma. Currently the patient is undergoing treatment with 8 cycles of standard chemotherapy for diffuse large B-cell lymphoma
