    ONYCHOMYCOSIS FINGER AND TOE NAIL BY CRYPTOCOCCUS LAURENTII.T VERRUCOSSUM AND CANDIDA SP

    Onychomycosis is the most common nail disease and contributes to 50% of all cause of onychodystropy. Onychomycosis cases due to Cryptoccus laurentii and Trychophyton spp. are quite rare. The most common cause of onychomycosis reported in Indonesia is Candida sp. Risk factors for onychomycosis include moisture, occlusion, nail trauma, decreased immunity, slow nail growth, wide nail surface, and genetic factors. Old age, cancer, psoriasis, dermatophyte infection at other sites is also a risk factor. A 54 year-old woman, with brittle nails and cracked palms since 6 months before came to outpatient clinic. She is a farmer and work in the rice fields without gloves daily; she also rarely wash her hands after work. Dermatologic examination of all right and left finger nail is revealed yellow, white, and some blackish discoloration, subungual hyperkeratotic, onycholysis, and onychodystrophy. In the right and left palm, revealed multiple erythematous and yellow-white hyperkeratotic plaques, with fissures and rough scales. From nail clippings, a 20% KOH examination revealed long branching hyphae, periodic Acid-Schiff (PAS) staining revealed spores, and fungal culture revealed growth of Criptococcus laurentii and Trichophyton spp. colonies. We gave itraconazole pulse dose: 2 x 200 mg tablet orally per day for a week every month. Other treatment was using 20% urea cream. Follow up at 8 weeks after therapy is showed clinical and mycologic improvement. The incidence of onychomycosis due to Cryptoccus laurentii and Trychophyton spp. is still rare. Thepredisposing factor of infection in these case is suspected due to old age and her habitual contact with the paddy fields without gloves and rarely wash her hand after work

    Background: Vulvovaginalis Candidiasis (VVC) is a vulva and vaginal infection of Candida, which one of predisposition factors is systemic antibiotics. Humoral immunity does not play a role on VVC, thus VVC in HIV/AIDS patients is as much as VVC in immunocompetent patient. Purpose: Identify the profile of Candida sp. of VVC in HIV/AIDS patients who got systemic antibiotics. Methods: Descriptional study to identify the pattern of Candida sp. of VVC in HIV/AIDS patient who got systemic antibiotic with cultures of Saboroud Dextrose Agar, Cornmeal Tween80 with Carbohydrat Test and Candida CHROM agar. Results: Broad-spectrum antibiotics were given to HIV/AIDS patient with VVC at Dr. Soetomo general hospital Surabaya two times higher than narrow-spectrum antibiotics (66,7% vs. 33,3%), they were ciprofloxacin (42,8%), ceftazidime (19,1%) and ceftriaxone (4,8%); the narrow-spectrum antibiotics were cotrimoxazole (19,0%) and rifampicin (14,3%). Conclusion: C.albicans (57,1%) and C. glabrata (9,6%) were found in VVC of HIV/AIDS patients that have been given broad-spectrum antibiotics, while C. albicans (28,6%) & C. glabrata (4,7%) also found in VVC of HIV/AIDS patients that have been given narrow-spectrum antibiotics.     Evaluation of Myelin Sheath Marker Krox-20 for Detection of Early Disability in Leprosy

    Early diagnosis of nerve damage in leprosy is quite important for preventing the deformities. With an aim to find a good marker we have investigated Krox-20 as early peripheral nerve damage indicator in leprosy. Ambulatory patients of Kediri Leprosy Hospital, Malang, Indonesia have been studied. Degree of disability was measured based on WHO's criteria. Immunohistochemistry was.used to study the expression of Krox-20 on Schwann cells on nerve twigs in skin biopsies from leprosy cases. Receiver Operating Characteristic (ROC) curve was used to determine cut off value. Of all 79 leprosy patients studied, 36 patiens had degree of disability 0, and 43 patients had degree of disability 1. Analysis of ROC curve shown that cut off value for Krox- 20, was 8 and the cases in whom degree of disability was 0 (zero) had significantly higher mean values than those with degree of disability 1 (mean: 12.56 vs 4.24 (p<0.05)).     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
