    Tek delik laparoskopik kolesistektomi ile konvansiyonel laparoskopik kolesistektominin karşılaştırılması

Araştırma ProjesiAraştırma Projesi elektronik ortamda bulunmaktadır.Giriş: Son yıllarda minimal invaziv cerrahi uygulamaları içinde tek insizyondan laparoskopik cerrahi tekniği güncel olarak uygulanmaktadır. Tek insizyon tekniği, ameliyat sonrası daha az ağrı, daha az komplikasyon ve daha iyi kozmetik sonuç vaat etmektedir. Ancak bazı çalışmalarda bu yöntemin avantajlarının yanında ciddi dezavantajlarının olduğu da gösterilmiştir. Bu çalışma ile kliniğimizde yapılacak tek insizyon laparoskopik kolesistektomi olguları ile standart laparoskopik kolesistektomi olguları karşılaştırılarak tek insizyon laparoskopik kolesistektomi operasyonunun standart laparoskopik kolesistektomi operasyonuna göre avantajları ve dezavantajları araştırılacaktır. Yöntem: Her iki guruptan benzer özellikte 50'şer hasta olmak üzere toplamda 100 hasta çalışmaya dahil edildi. Yaş, cinsiyet, uygulanan ameliyat, ameliyat süreleri, kan kaybı, açığa dönme oranları, ağrı skoru, komplikasyon oranı ve hastanede kalış süreleri iki grup arasında karşılaştırıldı. Bulgular: Her iki grup arasında postoperatif 6. ve 12. saat ağrı skoru, ek analjezi gereksinimi, kan kaybı, postoperatif hastanede kalış süresi arasında anlamlı fark yoktu. Operasyon süresinin tek insizyon tekniğinde daha uzun olduğu görüldü ve bu grup kozmetik açıdan daha tatmin edici bulundu. Sadece multi-port laparoskopik kolesistektomi grubunda olan bir hastada açık tekniğe geçildi. 2 perop kanama, 1 erken postop yüzeyel cerrahi alan enfeksiyonu komplikasyonu gelişti. Tüm komplikasyonlar multi-port laparoskopik kolesistektomi grubunda izlendi. Sonuç: Tek insizyon laparoskopik kolesistektomi daha uzun operasyon süresi ve cerrah açısından teknik zorluğu olmasına rağmen seçili hasta grubunda güvenle uygulanabilecek bir prosedürdür.     Effects of botulinum toxin and factors on weight loss in patients with gastric balloon and without gastric balloon

    Introduction: In this research, it was aimed to evaluate effects of botulinum toxin (BoNT) and factors on weight loss in patients with gastric balloon (GB) and without GB. Materials and Methods: A total of 629 patient files attempted to our clinic between December 2020 to De- cember 2022 were subjected to the study divided by two groups as patients with (GB, n=512) and without GB (NGB, n=117). Results: Male rate in GB group was significantly higher (p0.05). Weight difference was significantly correlated with group (r=−0.212; p<0.01), gender (r=0.161; p<0.01), BMI (r=0.305; p<0.01), BMR (r=0.268; p<0.01), height (r=0.151; p<0.01), weight (r=0.333; p<0.01), and medicine usage (r=−0.072; 0.05). BMI difference was signif- icantly correlated with group (r=−0.209; p<0.01), BMI (r=0.308; p<0.01), BMR (r=0.165; p<0.01), and weight (r=0.250; p<0.01). GB (B=2.410; p<0.01), BMI (B=0.344; p<0.01), and BMR (B=0.004; p<0.01) had significant contribution on weight difference. Conclusion: BoNT is more effective in patients with GB than patients without GB

    Meckel’s Diverticulum Perforation Due to Strangulated Inguinal Hernia (Littre’s Hernia): A Rare Case

    Meckel's diverticulum is remnant of omphalomesenteric ductus and it is the most frequent congenital malformation of the gastrointestinal tract. Rarely, any hernia sac contains it and named as a Littre hernia. A 59-year-old male patient with Meckel's diverticulum perforation due to strangulated inguinal hernia, was successfully treated with diverticullectomy using linear GIA stapler and performing graft-free hernia repair. Littre’s hernia may also be present in abdominal wall hernias requiring urgent surgical intervention due to incarceration. For this reason, it is important to have information about treatment management of this clinical entity

    Comparison of open surgery and transoral approach in Zenker’s diverticulum

    Introduction: In the surgical treatment of Zenker’s diverticulum, endoscopic methods applied under general anesthesia have been used frequently in recent years. Endoscopic methods have gained popularity due to the shorter operative time, shorter hospital stay and transition to oral feeding, and low complication rates in selected patients. In this article, in the surgical treatment of Zenker’s diverticulum; the open surgical ap- proach and the transoral endoscopic surgical technique were compared. Materials and Methods: The patients in the study were divided into two groups those who underwent open surgery and those who underwent stapled diverticulectomy. Demographic data (age and gender), pre-opera- tive symptoms (vomiting, dysphagia, and regurgitation), presence of post-operative complications, duration of hospitalization, diverticulum size, and long-term recurrence status of all patients were included in the study. Results: A total of 12 patients were included in the study. Staple diverticulectomy was performed in six of 12 patients by open surgical technique and in the other six patients by endoscopic technique. The mean ages of the patients who underwent endoscopic and open surgical modalities were 69 (61–84) and 68 (53–81), re- spectively. Of the patients who underwent open surgery, five were male and one was female. Of the patients who underwent endoscopic stapler diverticulectomy, four were male and two were female. Of the patients in the group who underwent open surgery, one had preoperative vomiting, three had dysphagia, and two had regurgitation. Conclusion: We think that transoral diverticulotomy is an effective surgical option that can be safely per- formed in selected patients and can provide successful results at least in the early period

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
