Soft tissue composition in upper leg lipodystrophy: application of dual energy x-ray absorptiometry

    No full text
Liposuction is a safe procedure for removal of subcutaneous fat in patients with upper leg lipodystrophy. We present a young woman with lipodystrophy of upper leg who underwent ultrasound-assisted three-dimensional suction lipectomy. The aim of the study was to determine the amount of fat to be removed before liposuction and to evaluate soft tissue composition before and after liposuction. At the initial meeting and four months later, body composition parameters were examined by dual X-ray absorptiometry (DXA), which produced a precise assessment of regional fat distribution. The baseline DXA measurements revealed excess of fat tissue for the lateral regions of the right and left legs, and for the area from the waist to hip. After liposuction of established amount of tissue, the same measurements revealed a decrease of fat tissue in all three regions, whereas no significant differences were observed for lean mass. We concluded that the suction lipectomy produced decrease in the fat mass of legs without musculature damage. The DXA technique can be useful in predicting fat removal before suction lipectomy and in estimating changes in soft tissue composition after surgical treatment

A randomized trial comparing ReCell (R) system of epidermal cells delivery versus classic skin grafts for the treatment of deep partial thickness burns

    No full text
Background: Our purpose was to directly compare results obtained with the ReCell system and the classic skin grafting for epidermal replacement in deep partial thickness burns. Materials and methods: We recruited all patients with deep partial thickness burns admitted at the Burn Centre of S. Eugenio Hospital in Rome over 2 years. Enrollment was conducted with a controlled strategy-sampling chart-that allowed homogeneous groups (ReCell and skin grafting) for age, gender, type of burns and total burn surface area (TBSA). We evaluated as primary endpoints of the study the (i) time for complete epithelization (both treated area and biopsy site) and (ii) aesthetic and functional quality of the epithelization. (color, joint contractures). Secondary endpoints were the assessment of infections, inflammations or any adverse effects of the ReCell procedure, particular medications assumed, postoperative pain. Results: Eighty-two patients were analyzed in two homogeneous groups. All of them received adequate epidermal replacement, but skin grafting was faster than ReCell (p < 0.05). On the contrary, ReCell biopsy areas and postoperative pain were smaller than classic grafting (p < 0.05). The aesthetic and functional outcomes were similar between procedures. Conclusions: ReCell is a feasible, simple and safe technique. It gives similar results to skin grafting but, harvesting minor areas, can open possible future applications in the management of large-burns patients. (C) 2007 Elsevier Ltd and ISBI.

Associations between Mental Health and Diabetes: Findings from the South Carolina Medicaid Managed Care Program in 2006-2008

    No full text
Objectives: To assess differences in services associated with mental health status and prescriptions among Medicaid patients diagnosed with diabetes mellitus. Design: Secondary data analyses of South Carolina (SC) Medicaid enrollees. Participants: SC Medicaid enrollees with a diagnosis of diabetes mellitus (N5555) continuously enrolled in either managed care (MC) or fee for service (FFS) programs between 2006 and 2008. Main Outcome Measures: Health Plan Employer Data and Information Set (HEDIS)- based diabetes management service outcomes including: 1) whether the recipient received a nephrology exam; 2) the number of eye exams received; 3) the number of low-density lipoprotein cholesterol services received; and 4) the number of Hemoglobin A1c blood tests conducted. Outcomes were fitted to regression models adjusting for sex, race, health program provider type (MC or FFS), rurality, poverty indexes, clinical risk group status, whether there was a female head of household, and indicators for classes of prescription pharmaceuticals (antipsychotics, antidepressants, and anticonvulsants). Results: There are significant differences in the incidence of diabetes management service-use between enrollees in management plans and between recipients of classes of pharmaceuticals and mental health status. Enrollees in FFS have fewer claims associated with diabetes management services compared to counterparts in MC. Conclusions: Our early findings demonstrate the importance of efforts to collect HEDIS measures data and their potential as a resource for assessing quality of care. More importantly, this study illustrates the association between mental health status and associated pharmaceutical prescriptions. (Ethn Dis. 2010;20: 239–243

Invasive and non-invasive congeneric Centaurea (Asteraceae) show contrasting patterns of herbivory by snails

    No full text
Background and aims - Once introduced into new regions, exotic species often experience shifts in resource allocation in response to the different environmental conditions found in the introduction range. Plants naturally respond to specialist herbivores with quantitative defences, by producing large amounts of toxic and non-toxic compounds that typically difficult digestion (e.g. tannins, cellulose), and to generalist herbivores with qualitative defences, like specialized noxious chemicals (e.g. alkaloids). The Shifting Defence Hypothesis (SDH) poses that invasive plants decrease the production of defences against specialist herbivores in their introduction range, where specialist herbivores are usually absent, while boosting the production of defences against generalist herbivores. Methods - We empirically assessed the response of a generalist herbivore, the common garden snail (Helix aspersa), to feeding with leaves of the annual herb Centaurea sulphurea, native to Europe and naturalized in North America; and the congeneric species C. solstitialis, which is also native to Europe and invasive in the Americas. Key results - Snails fed with leaves from Spanish native populations the non-invasive C. sulphurea grew significantly less compared to snails fed with leaves from non-native California. For snails fed with the invasive C. solstitialis significant differences were also found among regions, but the response was more complex, depending on population, with snails fed with Turkish and Australian plants presenting higher growth rates than the rest of the regions. Conclusions - Overall, our results stressed the importance of colonization history in shaping adaptive responses, and the stochasticity associated with colonization events of two closely related species, with contrasting invasive success and responses to herbivoryPortuguese Fundacao para a Ciencia e a Tecnologia - FCT PCT/BIA-PLA/3389/2012 FCT IF-00066-2013 European Commission FP7-PEOPLE-MC-CIG-321909 European Union via QREN European Union via COMPETE European Union via FEDE
