    The need for Evidence Based Medicine (EBM) paradigm in medical practice is becoming greater. Consequently the determinants of the adoption of evidence-based guidelines are of increasing interest. The aim of the paper is to verify the existence of a “network effect” on the awareness and frequency of use of evidence-based guidelines among paediatricians. We focus on the causal relation between a paediatrician's centrality in the network of paediatricians and guidelines adoption. Interactions and knowledge exchanges may affect the frequency of use of evidence-based guidelines. This effect may have important implications for local health unit managers, for professional associations, and for the final customers themselves: families and children

    In this paper a population ecology approach is adopted to measure direct competition among hospitals starting from the concept of niche overlap. Organizational niches specify how competitive processes vary as functions of environmental dependencies. Overlapping resource requirements represent common dependencies from the same resources spaces represented by geographical areas where admitted patients live for health care organizations. We employ longitudinal models for relational data collected on 35 hospitals in the Abruzzo Region. The results reported describe the evolution of the competitive network at the regional level and assess the impact of provider attributes on the evolution of competitive interdependencies among hospitals

    Co-opetition e performance organizzativa: un'analisi empirica nel settore sanitario

    The present paper explores the impact that co-opetition - simultaneous cooperation and competition - has on organizational performance, proposing that performance of organizations is contingent upon the embeddedness of firms in a network of inter-organizational relationships. To explore the effects on performance resulting from both collaborative network ties and competitive relationships between organizations, we used panel data collected between 2003-2007 from 31 hospitals in Abruzzo, one of the most populated regions of the central Italy. We employed secondary data of hospital activities regarding both clinical and administrative aspects. For each year, we examined the intensity of inter-hospital competition and the unique position each provider has within a larger network of relationships with other hospitals. As well, other idiosyncratic organizational characteristics were examined. Our results show that hospital performance is both negatively related to the degree of competition an organization faces, and positively related to the degree with which hospitals establish collaborative relationships. We also found that the negative impact on organizational performance due to competitive aggressiveness was lessened when hospitals were more likely to create cooperative network ties

    ‘When the Time Comes’: Exploring Temporal Differences in the Adoption of Medical Innovation

    The effects of institutional, organizational, and professional factors on the adoption of medical innovations are widely recognized, but scant empirical evidence for how they jointly affect adoption behaviors over time is available. This study aimed to shed new light on the impact of these factors on the timing of organizations’ decisions to adopt an innovative technology. We collected data on temporal patterns of diffusion of a robotic surgical system among Italian hospital organizations. We adopted a dyadic approach to examine the time elapsed between paired hospitals’ adoption decisions. Negative binomial regression analysis was then performed to test our research hypotheses. The results indicate that professional factors, as well as regional governance and technological policies, significantly predicted the temporal lag in adoption decisions. Our study provides empirical support for the strong effects of professional and institutional characteristics on the temporal aspects of medical innovation diffusion

    Professional Networks and The Alignment of Individual Perceptions About Medical Innovation

    BACKGROUND:: In recent decades, the role of technology in health care organizations has become increasingly relevant because it enhances health care outcomes and the achievement of clinical goals. Extant research demonstrates that the effectiveness of a medical innovation depends largely on health care professionals’ perceptions of its usefulness and impact on their activities and practices. We also know that interaction among social actors contributes to the shaping of their judgments and opinions regarding innovation. PURPOSE:: This study investigated the role of professionals’ social networks and social capital in the formation of similar individual perceptions about a highly innovative robotic surgical system. METHODOLOGY:: We collected data from a sample of 50 professionals, including both physicians and nurses, working in three hospital wards belonging to an Italian hospital organization. Using a survey, we gathered data on professionals’ demographic characteristics, the adoption and impact of the new technology, and social networks. We tested our hypotheses using a dyadic perspective and logistic regression quadratic assignment procedures. FINDINGS:: Our findings document that professionals’ perceptions regarding technological change were more likely to be similar when they were connected and exhibited similarity in some social capital characteristics and adoption behavior. PRACTICE IMPLICATIONS:: These results have important implications for health care executives and administrators, as well as for health professionals characterized by high degrees of autonomy and for which organizational change can be affected by professional or organizational resistance

    Knowledge development and advice networks in professional organizations

    In this paper, social network analysis techniques and regression models are used to explain the impact of the level of knowledge development on ego-network redundancy in a community of hospital physicians. Our findings document that the level of knowledge development and the extent to which knowledge is homogeneously distributed amongst collaborating physicians are related to the redundancy of their advice networks, albeit with opposite effects. Our results highlight also that the impact of these relationships on network redundancy is moderated based on whether partnering individuals belong to different professional groups. Our results provide valuable input for the management of knowledge networks within professional organizations
