    I detta Meddelande nr 55 från Veterinärhistoriska museet har författaren - professor emeritus Lars-Erik Appelgren - gjort ett urval av den korrespondens som Peter Hernquist hade med sina mentorer Carl von Linné och Abraham Bäck under sin vistelse i Frankrike, varvid ”Lärda nyheter” varit en ledstjärna för urvalet. Speciellt har breven till Bäck försetts med författarens personliga kommentarer men även kompletterats med faktaupplysningar från andra källor än breven om berörda nyheter. För att underlätta läsningen har dessa kommentarer omgetts med enkelkonturerad och utvikningarna med dubbelkonturerad ram. Lars-Erik Appelgren har inte bara genom sitt veterinärhistoriska författarskap utan också genom att ställa sina fackliga kunskaper och sin eminenta estetiska läggning till förfogande gjort Veterinärhistoriska museet ovärderliga tjänster. Det är med stor glädje jag noterar att region Uppsala nyligen visat sin uppskattning genom att tilldela Lars-Erik sitt Medicinhistoriska stipendium med motiveringen att de vill ”lyfta fram ett viktigt men ofta förbisett område inom medicinhistorien: veterinärmedicinen”.     Fredagen den 22:a Maj 2015 samlades medarbetare från forskningsprojektet Re:heritage, Göteborgs Universitet och Göteborgs Stadsmuseum på parkeringsplatsen/torget på Magasinsgatan för att tillsammans genomföra en stadssittning. Medverkande under dagen var Karl Arvidsson, Carina Sjöholm, Daniel Gillberg, Marie Nyberg och Ida Backström från Göteborgs Stadsmuseum, Anna Bohlin och Staffan Appelgren projektledare för Re:heritage, Helene Brembeck och Niklas Hansson, medarbetare från CFK och Merja Liimatainen, forskningsassistent inom Re:heritage samt författare av detta working paper.     Health technology assessments on human papillomavirus vaccinations in Europe: A survey from VENICE network

    Background: VENICE II is a project funded in 2008 by the European Centre for Disease Prevention and Control to collect information on the national vaccination programmes, to increase their knowledge and to know the impact of new vaccines introduced in member states (MS). In 2006-2007, two vaccines against human papillomavirus (HPV) were authorized in Europe. Methods: An online survey was carried out to investigate the decision-making process undertaken regarding the potential introduction of the HPV vaccinations into MS national immunization programmes as well as to investigate the modalities of implementation of the vaccination programmes. There were specific questions about health technology assessment and reports of the countries that had carried them out were reviewed. Results: In 21 of the 29 MS, the national advisory body recommended to introduce HPV vaccination in their national immunization schedule and in 18 countries introduced it. Only 6 countries have realized a health technology assessments (HTA) report, each one with different methodology, but in all of them both vaccines show positive evaluations. Conclusion: From the available HTA, HPV vaccination is cost-effective under the assumption of a lifelong protection. Screening programme for cervical cancer and HPV vaccination programme should be always complementary. Organizational aspects need to be taken into account to improve the vaccination. HPV vaccination should target girls before the debut of their sexual life. Instead HPV vaccination of boys has not been demonstrated as bringing significant epidemiological benefits and has not been shown as being cost-effective

    A survey of models for determining optimal audit strategies

    The problem studied in this survey is how to optimize the allocation of audit resources over an auditee population with respect to available population statistics. The auditees are assumed to optimize their expected utility based on information about the audit strategy. This survey is limited to models where the auditee can vary the fraud amount along a continuous scale. If the auditor is not able or willing to announce the audit strategy, a Nash equilibrium can be derived in which the auditor and auditee correctly anticipate each others strategies. If the auditor announces the audit strategy in advance, the problem is formulated as a sequential game with perfect information which is solved as an optimization problem. Early models in the literature resulted in unrealistically high degrees of fraud. Later models have incorporated a split into one group of inherently honest auditees and another group of potentially dishonest auditees. The fraction of inherently honest auditees is exogenous. In this paper, the four combinations of non-announcing/pre-announcing the strategy and all potentially dishonest/some inherently honest auditees are studied. For the case of pre-announcing the strategy with some inherently honest auditees, two new solution methods are presented. Two main conclusions are as follows. First, models with some inherently honest auditees have greater external validity. Second, when a pre-announced strategy is feasible, as it often is with tax and benefit audits, the pre-announced strategy is preferred by the auditor over the non-announced strategy.     The Contribution of Innate Immunity to the Pathogenesis of ANCA-associated Vasculitis

    Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) constitute a group of vasculitides characterized by neutrophil-rich necrotizing inflammation of small vessels and the presence of ANCA in the circulation. Dying neutrophils surrounding the walls of small vessels are a histological hallmark of AAV. Traditionally it has been assumed that these neutrophils die by necrosis, but neutrophil extracellular traps (NETs) have recently been visualized at the sites of vasculitic lesions. NETs were first described to be involved in capture and elimination of pathogens but dysregulated production and/or clearance of NETs are thought to contribute to vessel inflammation in AAV; directly by damaging endothelial cells and indirectly by acting as a link between the innate and adaptive immune system through the generation of pathogenic PR3-ANCA and MPO-ANCA that can activate neutrophils. ANCA can, however, be found in all individuals and are therefore suggested to belong to the repertoire of natural antibodies produced by innate-like B cells, implying that not all ANCA are pathogenic.  In paper I, we found neutrophils in patients to be more prone to undergo NETosis/necrosis spontaneously compared with neutrophils in healthy controls (HC), as well as that active patients possessed elevated levels of NETs in the circulation. Our results also suggest that ANCA during remission could contribute to the clearance of NETs as we observed an inverse relation between ANCA and NETs. In paper II, we observed neutrophils in patients to be more easily activated upon ANCA stimulation as they produced more ROS than neutrophils in HC. In paper III, we showed for the first time that cells of adaptive immunity (B and T cells) in addition to cells of innate immunity can release ET-like structures, in this case consisting of mitochondrial (mt) DNA. mtDNA can act as a damage-associated pattern molecule (DAMP) and promote inflammation, and increased levels of mtDNA has been observed in AAV. Our finding broadens our perspective of the possible roles of T and B cells in immunological responses, and should be further investigated in AAV. In paper IV, we observed reduced frequencies of MZ-like B cells, considered to be innate-like B cells that produce natural antibodies, and of the proposed regulatory B (Breg) cell populations CD24highCD27+ and CD25+CD27+ B cells in patients, particularly in those with active disease. We also observed the phenotypes of these different Breg cell populations to be different from the corresponding cells in HC. We hypothesize that the increased activation potential by neutrophils in AAV to produce ROS and undergo NETosis/necrosis contribute to the excessive inflammation as well as an increased antigen load of PR3 and MPO, and that this in combination with dysregulation of innate-like B cells and Breg cells could lead to break of tolerance to these antigens and production of pathogenic autoantibodies. ANCA can in turn activate neutrophils to release NETs, suggesting a vicious circle in disease development

    “Hur ska vi nå ut med denna information?” - En kvalitativ undersökning om intern kommunikation

    I dagens samhälle har kommunikationskanalerna på företag i flertalet fall blivit för många, vilket har bidragit till svårigheter för medarbetare att orientera sig och veta var relevant information finns att hämta. Denna studie syftar därför till att undersöka de anställdas åsikter kring användandet av kommunikationskanalerna i en informationsintensiv organisation inom banksektorn. För att undersöka detta syfte har en kvalitativ undersökning genomförts på en organisation inom denna bransch. Semistrukturerade intervjuer genomfördes med åtta deltagare då vårt metodval grundar sig på att få en djupare insyn i organisationen och de anställdas synpunkter. Resultatet visar att de anställda har skilda uppfattningar kring den interna kommunikationen och dess användning, vilket kan innebära svårigheter för återkoppling och vid organisationsförändringar. Trots att kommunikationskanalerna inbjuder till viss återkoppling används inte dessa, istället löser de anställda eventuella oklarheter sinsemellan då detta anses vara mer tidseffektivt. Respondenternas skilda syn på informationsspridningen via kommunikationskanalerna visar på en viss brist i kommunikationen. Däremot upplever de anställda sig vara välinformerade, men då synen kring hur informationen når fram skiljer sig åt bland de anställda kan det upplevda budskapet variera beroende på kommunikationsväg
