    Myocardial infarction due to paradoxical embolism in a patient with large atrial septal defect

    We present the case of a patient who presented with acute inferior myocardial infarction and embolic occlusion of the distal left anterior descending and proximal right coronary artery. A large atrial septal defect (ASD) was seen on transesophageal echocardiography and the ASD was closed during the same session as coronary angiography and percutaneous coronary intervention. The presence of embolic or thrombotic occlusions of coronary arteries should prompt interventional cardiologists to look for a patent foramen ovale or ASD and perform percutaneous closure right away

    Der Einfluss von eHealth Literacy auf gesundheitsförderliches Verhalten bei chronisch kranken Patienten

    Hintergrund. Bisherige Untersuchungen ergaben, dass chronisch kranke Patienten mehrheitlich von einer geringeren eHealth Literacy betroffen sind, was sich negativ auf die Fähigkeit auswirkt, von der Nutzung digitaler Gesundheitsanwendungen und deren positiven Auswirkungen auf das Gesundheitsverhalten zu profitieren. Die vorliegende Arbeit untersucht den Einfluss der eHealth Literacy auf gesundheitsförderliches Verhalten unter Berücksichtigung des subjektiven Gesundheitszustands und der Nutzung digitaler Gesundheitsanwendungen bei chronisch kranken Patienten. Methoden. Diese Querschnittstudie wurde mit 312 Patienten mit chronischen Krankheiten ab 18 Jahren in der Schweiz durchgeführt. Die Datenerhebung erfolgte mithilfe der eHealth Literacy Scale und des Health-Promoting Lifestyle Profile II. Die Probanden wurden über Online-Medien sowie in einigen Arztpraxen rekrutiert. Vergleiche zwischen den Charakteristiken der Befragten mit eHEALS und HPLP-II wurden anhand des t-Tests gezogen. Die Datenanalyse erfolgte mittels Pearson-Korrelation und multipler linearer Regression, um den Zusammenhang zwischen eHealth Literacy, gesundheitsförderlichem Verhalten und individuellen Faktoren zu prüfen. Ergebnisse. Die Korrelationsanalysen ergaben einen signifikanten Einfluss der eHealth Literacy auf gesundheitsförderliche Verhaltensweisen bei chronisch kranken Patienten. Darüber hinaus waren sowohl eHealth Literacy als auch gesundheitsförderliche Verhaltensweisen signifikant mit dem subjektiven Gesundheitszustand und der Mediennutzung korreliert. Zudem wiesen soziodemographische Faktoren signifikante Unterschiede in Bezug auf eHealth Literacy und gesundheitsförderliches Verhalten auf. Schlussfolgerungen. Die Teilnehmer wiesen einen signifikant positiven Zusammenhang zwischen der eHealth Literacy und gesundheitsförderliche Verhaltensweisen auf. Strategien und Interventionen zur Förderung von Gesundheitsverhaltensweisen bei chronisch Kranken sollten sich auf die Verbesserung der Kompetenzen im Bereich der eHealth Literacy und Mediennutzung fokussieren. Darüber hinaus sollten Patienten mit chronischen Krankheiten bei der Nutzung digitaler Gesundheitsanwendungen ermutigt und unterstützt werden, um gesundheitsfördernde Verhaltensweisen weiterzuentwickeln

    Ultrathin-strut, biodegradable-polymer, sirolimus-eluting stents versus thin-strut, durable-polymer, everolimus-eluting stents for percutaneous coronary revascularisation: 5-year outcomes of the BIOSCIENCE randomised trial

    BACKGROUND: Drug-eluting stents combining an ultrathin cobalt-chromium stent platform with a biodegradable polymer eluting sirolimus have been shown to be non-inferior or superior to thin-strut, durable-polymer, everolimus-eluting stents in terms of 1 year safety and efficacy outcomes. METHODS: In the randomised, single-blind, multicentre, non-inferiority BIOSCIENCE trial, we compared biodegradable-polymer sirolimus-eluting stents with durable-polymer everolimus-eluting stents in patients with chronic stable coronary artery disease or acute coronary syndromes. Here, we assess the final 5-year clinical outcomes of BIOSCIENCE with regards to the primary clinical outcome of target lesion failure, which was a composite of cardiac death, target vessel myocardial infarction, and clinically indicated target lesion revascularisation. The primary analysis was done by intention to treat. The BIOSCIENCE trial is registered with ClinicalTrials.gov, number NCT01443104. FINDINGS: 2008 (95%) of 2119 patients recruited between March 1, 2012, and May 31, 2013, completed 5 years of follow-up. Target lesion failure occurred in 198 patients (cumulative incidence 20·2%) treated with biodegradable-polymer sirolimus-eluting stents and in 189 patients (18·8%) treated with durable-polymer everolimus-eluting stents (rate ratio [RR] 1·07, 95% CI 0·88-1·31; p=0·487). All-cause mortality was significantly higher in patients treated with biodegradable-polymer sirolimus-eluting stents than in those treated with durable-polymer everolimus-eluting stents (14·1% vs 10·3%; RR 1·36, 95% CI 1·06-1·75; p=0·017), driven by a difference in non-cardiovascular deaths. We observed no difference between groups in cumulative incidence of definite stent thrombosis at 5 years (1·6% in both groups; 1·02, 0·51-2·05; p=0·950). INTERPRETATION: 5-year risk of target lesion failure among all-comer patients undergoing percutaneous coronary intervention is similar after implantation of ultrathin-strut, biodegradable-polymer, sirolimus-eluting stents or thin-strut, durable-polymer, everolimus-eluting stents. Higher incidences of all-cause and non-cardiovascular mortality in patients treated with biodegradable-polymer stents eluting sirolimus than in those treated with durable-polymer stents eluting everolimus warrant careful observation in ongoing clinical trials

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
