    La congiura è un tema trasversale nell'opera del Machiavelli. All'analisi del discorso 'pedagogico' sulle congiure del cap. 6 del III libro dei Discorsi, l'autrice ha accostato quella delle notazioni pratiche delle Legazioni e Commissarie e poi quella dei racconti di congiure delle Istorie fiorentine : ha rilevato così continuità e differenze di lessico, linguaggio e punti di vista. Alla luce del confronto, da un lato, con altri scritti di contemporanei, dall'altro con le fonti cui il Machiavelli attinge, ha cercato di esaminare lo sviluppo della sua riflessione sulle forme e le prospettive della lotta politica del suo tempo. Ne ha ravvisato un esito significativo per un verso nella sua critica dei topoi retorici del repubblicanesimo antitirannico ; per un altro nella contrapposizione tra la congiura propria del mondo comunale, larga e 'tumultuaria' e quella 'ristretta' del '400-'500, destinata a fallire perché ormai priva del supporto popolare. In ciò si manifesta la presa di coscienza, da parte del Machiavelli, delle nuove condizioni in cui si svolge la lotta politica nel Cinquecento italiano.Fasano Guarini Elena.     Cell-Free Fetal DNA for the Prediction of Pre-Eclampsia at the First and Second Trimesters: A Systematic Review and Meta-Analysis

    Objective: A systematic review and pooled analysis was carried out to estimate whether the increase in the quantity of cell-free fetal DNA (cffDNA) before the onset of pre-eclampsia (PE) can predict the disease using real-time polymerase chain reaction (PCR). Method: A comprehensive literature search of the PubMed, Scopus, and Web of Knowledge databases was conducted to identify relevant studies that included evaluated cffDNA levels in pregnant women before the clinical onset of PE. A simulation model was generated to calculate the detection rate (DR) of cffDNA for PE, and a random variable was generated using the same number of cases and same statistical measurements of central tendency and dispersion as those reported in the papers considered for the analysis. Simulation of the receiver operating characteristic (ROC) curves was also carried out. Results: Four studies (82 cases and 1315 controls) evaluated cffDNA in early-onset PE, with DRs of 18 and 68.8% at 11â13 and 17â28 weeks, respectively, at a false positive rate of 10%. Nine studies (including two considered for early-onset PE) encompassing 376 cases and 1270 controls were available for the evaluation of âany PEâ. At 11â14 weeks no significant DR was found, while at 15â28 weeks the DR was 37%. Conclusion: CffDNA quantification is a marker for predicting the development of both early-onset PE and âany PEâ; however, it can probably only be used from the beginning of the second trimester, otherwise its predictive value is burdened with a DR that is too low or not significant. Due to the heterogeneity and difficulty in interpreting the published data, no conclusion regarding the statistical and clinical relevance, especially for screening âany PEâ, can be made at present

    Amnioinfusion: from termination of pregnancy to therapy

    AMNIOINFUSION: FROM TERMINATION OF PREGNANCY TO THERAPY Amnioinfusion or infusion of saline into the amniotic cavity was first used as a technique for late termination of pregnancy in the 1960s (Jonas and Slate. Obstet Gynecol.1966;27:494-6). By the mid 1970s, transabdominal amnioinfusion of hypertonic (20%) saline was the most widely used method of pregnancy termination after 14 weeks in the US. This technique was associated with around 10% morbidity including hemorrhage requiring transfusion, retained tissue requiring surgical removal, infection, coagulopathy, and hypernatremia (Ballard and Ballard. Am J Obstet Gynecol.1972;114:575-81). There were also risks of intravenous, intraperitoneal, or intramyometrial injection of the saline and thus amnioinfusion for abortion was abandoned in the 1980s in favour of prostaglandins (PGE2) which were more efficient and with fewer side effects. Miyazaki and Taylor were the first to report on the use of saline amnioinfusion in 42 patients having repetitive variable or prolonged decelerations that did not respond to conventional therapy, such as maternal position changes and oxygen administration (Miyazaki and Taylor. Am J Obstet Gynecol.1983;146:670‐8). The technique used normal saline or ringers lactate infused transcervically through a catheter into the uterine cavity, or transabdominally using a 'spinal' needle when membranes are intact. More recently, transcervical amnioinfusion was proposed for women presenting with thick meconium staining of the amniotic fluid to reduce the risk of perinatal death and meconium aspiration syndrome. Although the first results seemed encouraging (Hofmeyr et al.BJOG.1998;105:304-8), the 2006 UK National Institute for Health and Care Excellence (NICE) concluded that there was insufficient evidence to support the practice (www.nice.org.uk › guidance › ipg192) and it has rarely been used for this indication. More recent data from India have suggested that 1 transcervical amnioinfusion in labour for meconium-stained amniotic fluid can be performed safely in a setup with limited neonatal care facilities, to decrease the incidence of caesarean deliveries and improve neonatal outcome (Choudhary and Bano. Arch Gynecol Obstet.2010;282:17-22). Amnioinfusion was also used prophylactically in various conditions commonly associated with oligohydramnios to limit the risk of cord compression or oligohydramnios-related pulmonary hypoplasia (Figure) (Fisk et al. BJOG.1992;99:464-8) but was not found to improve neonatal outcomes. Two large trials have assessed whether infusion of fluid into the amniotic cavity could improve pregnancy outcome after second-trimester premature rupture of the membranes (AMIPROM trial Roberts et al.UOG 2014;43:490-9;PPROMEXIL-III trial van Teeffelen et al. BMC Pregnancy Childbirth. 2014; Apr 4;14:128). The first trial suggested an improvement in long-term healthy survival after amnioinfusion but neither of these studies showed a significant better outcome after amnioinfusion. In this issue, De Ruigh et al. (BJOG 2021) report on the long-term outcome of the PPROMEXIL-III trial. Overall, of the 14 survivors (all born at a median gestational age of 24 weeks), 71% had no neurodevelopmental delay. The small sample size of this study does not allow to draw any definitive conclusions and survival without developmental delay nor respiratory problems did not differ between the treatment arm and controls with no amnioinfusion. From termination of pregnancy to therapy, amnioinfusion has still to find its place in modern obstetrics

    Dressing Design è un termine coniato dal gruppo Radical fiorentino Archizoom Associati per circoscrivere l’ambito di progettazione di oggetti per l’abbigliamento: gli autori stessi hanno affermato provocatoriamente che “vestirsi è facile perché l’eleganza è morta”, rivelando sin dall’inizio un rapporto ambiguo con la moda. Alla moda tradizionale e alle sue rigide convenzioni gli Archizoom contrappongono un uso libero e asessuato dell’abito che diventa uno strumento di comunicazione nelle mani dell’utente. Queste sperimentazioni nascono in concomitanza alle proposte di organizzazione dello spazio abitativo attuate dall’architettura Radicale tra la metà degli anni sessanta e la prima metà del decennio successivo, e sono legate ai cambiamenti che nello stesso periodo interessano la moda e l’industria dell’abbigliamento. Questa ricerca ha messo in luce l’iter progettuale e le peculiarità delle differenti sperimentazioni sull’abito: dalla fase germinale individuata all’indomani della mostra Superarchitettura e caratterizzata da consonanze con la Body Art, fino ad arrivare alla prima indagine sull’abito inteso quale elementare forma di habitat umano (Nearest Habitat System) da cui scaturisce la collaborazione con Fiorucci, passando per il progetto Vestirsi è facile presentato alla XV Triennale di Milano. Ne emerge la natura vitale, complessa e talvolta contraddittoria del Dressign Design, nato come riflessione teorica contro la moda e le sue ossessioni e trasformatosi in una proposta progettuale che abbandona le provocazioni e cerca occasioni produttive concrete
