    Optimal selection of high‐risk patients with stage II colon cancer is crucial to ensure clinical benefit of adjuvant chemotherapy. Here, we investigated the prognostic value of genomic intratumor heterogeneity and aneuploidy for disease recurrence. We combined targeted sequencing, SNP arrays, fluorescence in situ hybridization, and immunohistochemistry on a retrospective cohort of 84 untreated stage II colon cancer patients. We assessed the clonality of copy‐number alterations (CNAs) and mutations, CD8(+) lymphocyte infiltration, and their association with time to recurrence. Prognostic factors were included in machine learning analysis to evaluate their ability to predict individual relapse risk. Tumors from recurrent patients displayed a greater proportion of CNAs compared with non‐recurrent (mean 31.3% versus 23%, respectively; p = 0.014). Furthermore, patients with elevated tumor CNA load exhibited a higher risk of recurrence compared with those with low levels [p = 0.038; hazard ratio (HR) 2.46], which was confirmed in an independent cohort (p = 0.004; HR 3.82). Candidate chromosome‐specific aberrations frequently observed in recurrent cases included gain of the chromosome arm 13q (p = 0.02; HR 2.67) and loss of heterozygosity at 17q22–q24.3 (p = 0.05; HR 2.69). CNA load positively correlated with intratumor heterogeneity (R = 0.52; p < 0.0001). Consistently, incremental subclonal CNAs were associated with an elevated risk of relapse (p = 0.028; HR 2.20), which we did not observe for subclonal single‐nucleotide variants and small insertions and deletions. The clinico‐genomic model rated an area under the curve of 0.83, achieving a 10% incremental gain compared with clinicopathological markers (p = 0.047). In conclusion, tumor aneuploidy and copy‐number intratumor heterogeneity were predictive of poor outcome and improved discriminative performance in early‐stage colon cancer. © 2022 The Authors.     El cáncer de colon es la tercera causa prevalente tanto en hombres como mujeres a nivel mundial generando altas tasas de morbilidad y mortalidad, lo cual no es diferente en nuestro país. Estudios recientes siguen ratificando el papel pronóstico y predictivo de la localización del tumor primario en cáncer de colon. Se realiza con este proyecto de investigación un estudio con enfoque cuantitativo implementando un diseño observacional analítico de cohorte retrospectivo nuestra realidad para así evaluar el comportamiento de estos factores sobre la respuesta al tratamiento y su desenlace en pacientes colombianos con cáncer de colon.Colon cancer is the third most prevalent cause in both men and women worldwide, generating high morbidity and mortality rates, which is no different in our country. Recent studies continue ratifying the prognostic and predictive role of the location of the primary tumor in colon cancer.     O trabalho procura reconstituir a produção de Carlos Drummond de Andrade da década de 20 a partir da análise de três eixos interligados: a pesquisa estética em diálogo cerrado com o Modernismo, a investigação da subjetividade e o cadente debate sobre o nacionalismo da época. Trata-se, assim, de propor um percurso mais ou menos contínuo que permita interrogar como esses aspectos foram sendo progressivamente conquistados e articulados pelo poeta mineiro ao longo dos anos 20 e como culminaram em seu primeiro livro, Alguma poesia, de 1930. A hipótese desenvolvida é de que a trajetória resultante da conquista desses aspectos e da publicação do seu livro de estreia representou um momento importante em que ele assimilou influências, experimentou procedimentos e decantou temas. O trabalho visa a sugerir, desse modo, a formação poética do Autor, detendo-se no contexto em que estava inserido e a partir do qual construiu sua obra. A fim de discutir a especificidade desta, o argumento se organiza no sentido compará-la aos desdobramentos do Modernismo do mesmo período.The work seeks to reconstitute the production of Carlos Drummond de Andrade of the decade of 20 from the analysis of three interconnected axes: the aesthetic research in closed dialogue with the Modernism, the investigation of the subjectivity and the cadent debate on the nationalism of the time. It is a question of proposing a more or less continuous course that allows us to question how these aspects were progressively conquered and articulated by the poet throughout the 1920s and culminated in his first book, Alguma poesia. The hypothesis developed is that the trajectory resulting from the conquest of these aspects and the publication of his debut book represented an important moment in which he assimilated influences, tried procedures and themes. The work aims to suggest the poetic formation of the Author, stopping in the context in which he was inserted and from which he constructed his work. In order to discuss its specificity, the argument is organized to compare it with the developments of Modernism of the same period

    Sojeitos da terra: representations of Brazilian rural man in Carlos Alberto Soffredini\'s dramaturgy

    Esta tese investiga a representação do caipira na dramaturgia de Carlos Alberto Soffredini. Para isso, analiso quatro peças teatrais que tratam dessa temática: Na carrêra do divino, de 1979, A estrambótica aventura da música caipira, de 1990, Auto de Natal caipira, de 1992, e A madrasta, de 1995. A fim de entender como se construíram essas representações em sua obra, são examinados diversos elementos da cultura popular mobilizados pelo autor, a construção de personagens em cada peça, as escolhas entre recursos dramáticos ou épicos e as variadas fontes de pesquisa utilizadas pelo dramaturgo para a composição de seus textos, bem como a maneira como elas foram incorporadas. O trabalho também procura investigar as transformações ocorridas nessa construção do caipira dentro da produção do dramaturgo, entre o final dos anos 1970 e a década de 1990, e as mudanças em relação aos referentes anteriores nas letras e nas artes brasileiras do início do século XX (sobretudo a figura paradigmática de Jeca Tatu, criada por Monteiro Lobato). Sustento que há variações importantes no trabalho feito por Soffredini envolvendo a figura do caipira, que não é uma construção homogênea, como pretendo mostrar, o que nos leva a perceber, como um traço importante da sua dramaturgia, a consciência das contradições envolvidas no debate sobre a cultura popular.This thesis investigates the representations of Brazilian rural man in Carlos Alberto Soffredini\'s dramaturgy. To do so, we analyze four plays that deal with such theme: Na Carrêra do Divino, from 1979, A estrambótica aventura da música caipira, from 1990, Auto de Natal Caipira, from 1992, and A Madrasta, from 1995. In order to understand how these representations were built into the author\'s work, we will analyze his use of different aspects of popular culture. We will also pay attention to: the construction of the characters in each play; the choices made between dramatic and epic resources and the variable research sources used by the author for the composition of his texts, as well as the way in which these sources were incorporated in his plays. The dissertation also seeks to investigate the transformations which occurred within Soffredini\'s work, between the late 70\'s and the 90\'s, and the changes in relation to the representation of the rural man in Brazilian\'s early twentieth century literature and culture (in particular, with reference to the paradigmatic figure of Jeca Tatu, by Monteiro Lobato). I argue and intend to show that there are important variations in Soffredini\'s work involving rural man\'s figure, which is not a homogeneous construction. This leads us to perceive, as an important aspect of his dramaturgy, a critical consciousness about the contradictions involving the debate surrounding popular culture

    Assessing the extent of colon lengthening due to splenic flexure mobilization techniques: a cadaver study

    CONTEXT: Failure of a colorectal anastomosis represents a life-threatening complication of colorectal surgery. Splenic flexure mobilization may contribute to reduce the occurrence of anastomotic complications due to technical flaws. There are no published reports measuring the impact of splenic flexure mobilization on the length of mobilized colon viable to construct a safe colorectal anastomosis. OBJECTIVE: The aim of the present study was to determine the effect of two techniques for splenic flexure mobilization on colon lengthening during open left-sided colon surgery using a cadaver model. DESIGN: Anatomical dissections for left colectomy and colorectal anastomosis at the sacral promontory level were conducted in 20 fresh cadavers by the same team of four surgeons. The effect of partial and full splenic flexure mobilization on the extent of mobilized left colon segment was determined. SETTING: University of Sao Paulo Medical School, Sao Paulo, SP, Brazil. Tertiary medical institution and university hospital. PARTICIPANTS: A team of four surgeons operated on 20 fresh cadavers. RESULTS: The length of resected left colon enabling a tension-free colorectal anastomosis at the level of sacral promontory achieved without mobilizing the splenic flexure was 46.3 (35-81) cm. After partial mobilization of the splenic flexure, an additionally mobilized colon segment measuring 10.7 (2-30) cm was obtained. After full mobilization of the distal transverse colon, a mean 28.3 (10-65) cm segment was achieved. CONCLUSION: Splenic flexure mobilization techniques are associated to effective left colon lengthening for colorectal anastomosis. This result may contribute to decision-making during rectal surgery and low colorectal and coloanal anastomosis

    La muerte de Ivan Illitch, de León Tolstoi: elementos para considerar las múltiples dimensiones de la gestión del cuidado

    The author uses the short story The Death of Ivan Ilyich, by Leo Tolstoy, to explore the theme of the multiple dimensions of healthcare management (professional, organizational and systemic) by pointing out the omnipresence and complementariness of these dimensions. Although recognizing that there is no hierarchy between these three dimensions, the discussion focuses on the professional dimension, within which professionals meet users, and highlights how this has been the favored territory for management strategies governed by the search for increasing rationality, predictability and control over current health practices, either in the public or in the private healthcare sector. The author points out the risk that qualification and humanization healthcare programs might be contributing towards instrumentalization and excessive formalization of the meeting point between workers and users, which adds further difficulty to truly caring encounters like the one narrated by Tolstoy in his short story, or even makes them impossible.El autor utiliza el cuento La muerte de Ivan Illitch, de León Tolstoi, para explorar el tema de las múltiples dimensiones de la gestión del cuidado en salud (profesional, de organización y sistémica) apuntando hacia la inherencia y complementación que guardan entre si. Aunque reconociendo que no hay una jerarquía entre esas tres dimensiones, enfoca la discusión en la dimensión profesional - donde se da el encuentro profesional - usuario - destacando como ha sido el territorio preferente para las estrategias de administración pautadas por la búsqueda de una creciente racionalidad, previsión y control de las prácticas de salud actuales, ya en el sector público, ya en el sector de la salud suplementaria.     Peritoneal fluid changes in horses subjected to small colon distension

    Intestinal devitalization in cases of small colon obstruction may be difficult to detect based only in clinical signs. The purpose was to serially evaluate blood and peritoneal fluid of horses subjected to small colon distension. Seventeen adult horses were allotted in three groups. In the small colon-distended group (DG, n=7) a surgically-implanted latex balloon was inflated to promote intraluminal small colon distension. In the shamoperated group (SG, n=5), the balloon was implanted but not inflated, and no surgery was done in the control group (CG, n=5). Blood and peritoneal fluid were sampled before and after (6 samples with a 30-minute interval) intestinal obstruction for cytological and biochemical analyses. No significant changes in clinical signs occurred within groups or across time during the experimental period. There were no statistical differences among SG and SG groups in hematologic and blood chemistry variables. Although total protein concentration and lactate dehydrogenase (LDH) activity in peritoneal fluid remained most of the time within reference values during the experimental period in all groups, increases from baseline values were detected in SG and DG groups. Such increases occurred earlier, progressively and with greater magnitude in the DG when compared with the SG (P<0.05). Increases from baselines values were also observed in total nucleated cells and neutrophils counts in the DG (P<0.05). In conclusion, distension of the equine small colon induced progressive subtle increases in total protein and LDH concentrations in the peritoneal fluid during the first hours. Serial evaluation of these variables in peritoneal fluid may be useful for early detection of intestinal devitalization in clinical cases of equine small colon obstruction

    Em 1909, no palco do Teatro de Arte de Moscou, o afamado diretor russo Konstantin Stanislávski realiza a montagem da peça de Ivan Turguêniev Um mês no campo. Além de ser uma brilhante interpretação de obra clássica da literatura russa de um autor pouco difundido nesse país, o espetáculo marca o início da elaboração dos primeiros elementos do sistema Stanislávski, que definiria novas possibilidades para a arte do ator, momento decisivo na sua trajetória teatral. Esta dissertação apresenta o processo de montagem do espetáculo, reflete os primeiros germes do sistema Stanislávski e analisa as abordagens interpretativas do encenador na poética dramática da peça Um mês no campo.In 1909, on the stage of Moscow Art Theatre, the acclaimed Russian director Konstantin Stanislavsky staged Ivan Turgenev s play, A Month in the Country. This performance was a decisive moment in his theatrical trajectory. Besides being a brilliant interpretation of a classic work of the Russian literature written by an author not widely known in this country, it was the starting point of development of the first elements of Stanislavskys \"system\", which would define new possibilities for the art of the actor. This dissertation presents the creation process of the spectacle, reflects on the first seeds of Stanislavskys \"system\" and analyses the directors interpretive approaches to the poetic drama of A Month in the Country

    PROGNOSTIC FACTORS FOR LEFT COLECTOMY FOR COLON CANCER: A TEN YEARS EXPERIENCE OF A SINGLE UNIVERSITY INSTITUTION

    ABSTRACT Background: Colorectal cancer is the third most common cancer in the world. In Brazil, it is the leading cause of cancer in the gastrointestinal tract. Aim: To evaluate the preoperative, perioperative, and postoperative risk factors for recurrence and overall survival of patients with left colon cancer operated during a ten-year period. Methods: Patients with left colon cancer surgically treated underwent clinical preoperative workout and cancer staging. The following factors were studied: gender, age, tumor location, T stage, lymph node yield, N stage, M stage, histological type, and tumor differentiation. It was analyzed the influence in five-year overall survival. Results: A total of 173 patients underwent left colectomy for colon cancer. There was a slight predominance of male gender with 50.9%. The mean age was 60.8 years old. Fifteen (8.7%) tumors were located at splenic flexure, 126 (72.8%) at sigmoid colon, and 32 (18.5%) at descending colon. The median length of hospital stay was seven days. Mean survival was 47.5 months. At 60 months seven patients (4%) lost follow-up, 38 patients (21.9%) deceased and 135 patients (78%) were alive. Overall survival time was 48 months. Conclusion: Advanced stages (T3-T4, N+ and M+) were the only factors associated with poor long term survival in left colon cancer.</jats:p
