    IL TRATTAMENTO DEL DOLORE POST-OPERATORIO IN INTERVENTI DI CHIRURGIA ORTOPEDICA AMBULATORIALE: DIFFERENZE NEI RISULTATI TRA 3 DIVERSI FARMACI

    Gli interventi chirurgici di neurolisi esterna del nervo mediano per Sindrome del Tunnel Carpale di tenolisi dell’apparato flessore per la tenosinovite stenosante dell’apparato flessore e di tenolisi dei tendini del primo compartimento degli estensori per Tenosinovite di De Quervain sono tra le procedure chirurgiche più frequentemente eseguite nelle Unità Operative di Chirurgia della Mano. Esse vengono sempre più frequentemente eseguite inj regime di chirurgia ambulatoriale ed in anestesia locale. Esse possono essere causa di sintomatologia dolorosa riferita da lieve a moderata, che viene solitamente trattata ricorrendo a farmaci come il Paracetamolo, i FANS od associazioni di Paracetamolo ed oppioidi. Il nostro lavoro è volto a verificare se esistano differenze, tra alcuni dei farmaci più frequentemente utilizzati a questo scopo, nell’efficacia complessiva nel trattamento della sintomatologia dolorosa che può seguire questo tipo di procedure chirurgiche. Sono stati complessivamente arruolati 350 pazienti, sottoposti ad uno dei suddetti interventi chirurgici presso la nostra Unità Operativa (U.O.C. di Ortopedia e Chirurgia della Mano, Complesso Integrato Columbus, Roma) tra il gennaio e l’agosto del 2011. Ai pazienti sono stati sottoposti 2 questionari per la valutazione della sintomatologia dolorosa mediante la scala NRS, uno da compilare prima dell’intervento ed un secondo da compilare a domicilio e recare in visione in occasione del primo controllo clinico, programmato 14 giorni dopo la procedura chirurgica. Sono stati arruolati soggetti di età superiore ai 18 anni, che non avessero riferito in anamnesi episodi di reazioni avverse od allergie ai farmaci utilizzati. Non sono stati arruolati pazienti con allergie. Non sono stati arruolati pazienti che non avessero prestato il consenso informato allo studio. I farmaci utilizzati sono stati: Kolibri (compresse rivestite 37,5 325 mg), Tachidol (cpr o bustine 500+30 mg) Orudis (cp 50 mg), prescritti al bisogno max. 1 ogni 8 h. Complessivamente sono state eseguite 350 procedure: 204 per Sindrome del Tunnel Carpale(STC) 122 per Tenosinovite Stenosante Flessori (DS), 24 per Tenosinovite di De Quervain. Kolibri è stato utilizzato in 202 casi, Orudis 33, Tachidol in 110 casi. 5 pazienti avevano assunto Efferalgan. Il dolore pre intervento, secondo la scala NRS, vedeva un valore medio di 5,64 nella STC, 5,53 nel DS, 4,75 nella Tenosinovite di De Quervain, nella valutazione post-intervento era 1 (DS), 2,1 (STC) ed 1,45 (De Quervain), con una diminuzione del 74% (De Quervain) 76% (DS) E 67% (STC). Analizzando complessivamente i dati, emerge una sostanziale efficacia dei tre farmaci in esame, con una prevalenza dell’Orudis nei casi di STC (-83% NRS score), del Kolibri nella Tenosinovite di De Quervain (-83% NRS score) e del Tachidol nel DS (-79% NRS score). Ciò indica che il trattamento del dolore post-operatorio in tali interventi, apparentemente semplici e simili tra di loro, richieda delle terapie, qualora possibile, diverse tra loro per raggiungere i migliori risultati in termini di soddisfazione del paziente.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    The Italian Version of the Michigan Hand Outcomes Questionnaire (MHQ): Translation, Cross-Cultural Adaptation and Validation

    Background: Patient-reported outcome measures are largely used in clinical practice and scientific community. Michigan hand questionnaire (MHQ) is widely recognized as a standardized reliable instrument for the assessment of any kind of hand disorders.Aim of the study: translation, cross-cultural adaptation and validation of the Italian version of MHQ.Methods: The study was composed by two phases. Phase 1 consisted in translation and cross-cultural adaptation of MHQ, from original language version (English) into Italian, according to the standard procedure of translation and back translation. The final Italian version of MHQ was tested on 136 Italian patients with hand disorders (Phase 2), in addition to Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire, Visual analogue scale (VAS) for pain assessment and grip strength, to psychometric properties of the new version, 55 patients repeated the questionnaire after 7 days for test-retest to assess the reproducibility of the questionnaire. All data were subsequently analyzed (descriptive statistics, multitrait analysis, reliability and construct validity assessment).Results: Phase 1 was performed without major problems, thus the final Italian version was approved for Phase 2. The questionnaire was clear and easily understood (missing data 0-2.9%). Multitrait analysis brought very good results for each outcome measure. High reliability (Cronbach's alpha: 0.91-0.99) and very good reproducibility (Intraclass correlation coefficients: 0.83-0.98) were revealed. High to moderate correlation was found between MHQ and DASH, grip strength and VAS.Conclusions: The Italian version of MHQ has demonstrated to be reliable and valid

    Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
