Management change in papillary thyroid cancer : the role of SPECT/CT lymphoscintigraphy and radioguided sentinel lymph node biopsy
No full textOrientadores: Elba Cristina Sá de Camargo Etchebehere, Carlos Takahiro ChoneTese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciências MédicasResumo: Objetivo: Avaliar a capacidade da biópsia radioguiada do linfonodo sentinela (BLSR) de detectar metástases ocultas do carcinoma papilífero de tireoide (CPT) e correlacionar a presença dessas metástases com as características clínicas e patológicas. Método: Quarenta e dois pacientes com diagnóstico de CPT e sem evidências clínicas e ultrassonográficas de comprometimento linfonodal cervical (34 mulheres, idade média de 47 anos) foram prospectivamente estudados. Os pacientes foram submetidos à linfocintilografia pré-operatória, após injeção peritumoral de fitato-99mTc (7,4 MBq) guiada por ultrassonografia. Foram adquiridas imagens da região cervical através de uma tomografia computadorizada por emissão de fóton único acoplada à tomografia computadorizada (SPECT/CT), 15 minutos após a injeção do radiotraçador. Em seguida, o paciente foi conduzido ao centro cirúrgico para realização da BLSR (após cerca de 2 horas da linfocintilografia com SPECT/CT). Todos os linfonodos sentinelas identificados foram removidos, juntamente com pelo menos um linfonodo não sentinela do mesmo nível cervical. O carcinoma papilífero de tireoide, linfonodos sentinelas e linfonodos não sentinelas foram submetidos à análise histopatológica e imuno-histoquímica. Resultados: 23 pacientes apresentavam tumores T1, 8 pacientes, tumores T2 e 6 pacientes, tumores T3. Foram identificados linfonodos sentinelas em 92% dos pacientes, dentre esses, 46% apresentaram metástases linfonodais. Em 3 pacientes (8%), o linfonodo sentinela foi falso negativo. Cinco pacientes apresentaram drenagem do linfonodo sentinela contralateral ao tumor primário, mas nenhum foi positivo para metástase. Houve uma associação significativa entre a presença de metástase linfonodal e os estadios tumorais T1, T2 e T3 (p = 0,018), bem como para os pacientes com estadios T1 e T2 apenas (p = 0,041). Os pacientes com linfonodos metastáticos foram submetidos à RIT com doses maiores de iodeto-131I. Conclusões: A linfocintilografia com SPECT/CT e BLSR é capaz de detectar metástases cervicais ocultas em pacientes com carcinoma papilífero de tireoide. Houve mudança de conduta 46% dos pacientes, e 18% das metástases ocorreram no compartimento lateralAbstract: Background: The aims of this study were to investigate the efficacy of lymph node (LN) metastasis detection with radioguided sentinel lymph node biopsy (SLNB) in papillary thyroid carcinoma (PTC) and to correlate the presence of LN metastasis with clinical and pathological features. Methods: Forty-two PTC patients with clinical exam and ultrassound (US) negative for nodal metastasis were prospectively enrolled. Patients underwent preoperative lymphoscintigraphy after a peritumoral injection of 99mTc-nanocolloid (7.4 MBq) under ultrasound guidance. Cervical images were acquired with a single-photon emission computed tomography and computed tomography scanner (SPECT/CT) 15 minutes after radiotracer injection. Patients were submitted to intraoperative sentinel lymph node (SLN) procedure using a hand-held gamma probe approximately 2 hours after lymphoscintigraphy. All SLNs identified were removed along with non-SLNs from the same compartment. PTC, SLNs and non-SLNs were submitted for histopathology and immunohistochemistry analysis. Results: 23 patients were T1, 8 were T2 and 6 were T3. SLN was identified in 92% of the patients; among these, 46% had metastatic LNs. SLN was false-negative in 3 patients. Five patients presented SLNs drainage contra-lateral to the primary tumor although none were metastatic. There was a significant association between LN metastasis and tumor size for patients T1, T2 and T3 (p = 0.018), as well for patients T1 and T2 only (p = 0.041). Patients with LN metastasis were submitted to higher radioiodine ablation doses. Conclusions: SLNB detects occult cervical LN metastasis in patients with PTC. Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
Full text link“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
Full text linkWe provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
