    Evolutionary stages of COVID - 19 infection in children - clinical - imagistic characteristics

    Abstract Introducere: Infecția cu virusul SARS-CoV-2 a generat o provocare globală, inclusiv pentru sănătatea pediatrică. Deși formele severe sunt mai rare la copii, studiile recente relevă prezența unor modificări imagistice pulmonare importante, inclusiv în formele medii sau asimptomatice. Obiectiv: Evaluarea caracteristicilor clinico-imagistice și a evoluției pulmonare post-COVID-19 la copiii cu vârsta între 0–7 ani, utilizând tomografia computerizată toracică. Material și metodă: Studiul retrospectiv a inclus 101 copii internați între 2021–2024 la Clinica de Pneumologie Pediatrică din cadrul IMSP Institutul Mamei și Copilului, care au avut forme medii sau severe de COVID-19. Au fost analizate datele clinice, gradul de severitate, vârsta, sexul și modificările imagistice prin CT. Rezultate: Leziuni imagistice pulmonare au fost depistate la 68,31% dintre copii. Sugarii au prezentat cea mai mare rată de afectare pulmonară (45,3%). Cele mai frecvente modificări CT au fost ariile de consolidare (71,5%), opacitățile de tip „sticlă mată” (17,2%) și semnele de fibroză (30,7%). La 15,09% dintre pacienți s-au observat aderențe pleurale, sugerând procese inflamatorii persistente. Concluzie: Tomografia computerizată rămâne o metodă valoroasă în monitorizarea afectării pulmonare post-COVID-19 la copii, oferind date esențiale pentru înțelegerea evoluției bolii în această categorie vulnerabilă.Sumarry Introduction: The SARS-CoV-2 infection posed a major global health challenge, including in the pediatric population. Although severe forms are rare in children, recent studies highlight the presence of significant pulmonary imaging changes, even in mild or asymptomatic cases. Objective: To assess the clinical and imaging characteristics and post-COVID19 pulmonary evolution in children aged 0–7 years, using chest computed tomography (CT). Materials and Methods: This retrospective study included 101 children hospitalized between 2021 and 2024 at the Pediatric Pulmonology Clinic of the Mother and Child Institute (IMSP), who had moderate or severe COVID-19. Clinical severity, age, sex, and chest CT changes were analyzed, with imaging performed on average 2.32 months after the acute phase. Results: Pulmonary CT abnormalities were found in 68.31% of the children. Infants had the highest rate of lung involvement (45.3%). The most common CT findings were consolidations (71.5%), ground-glass opacities (17.2%), and fibrotic changes (30.7%). Pleural or pleuro-diaphragmatic adhesions were observed in 15.09% of cases, suggesting ongoing inflammatory processes. Conclusion: Chest CT remains a valuable tool for monitoring post-COVID-19 lung involvement in children, providing critical insights into the disease’s progression in this vulnerable group

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Introducere. Infecția cu virusul SARS-CoV-2 a reprezentat o provocare globală, afectând și populația pediatrică. Deși formele severe sunt mai rare la copii, tabloul clinic este variat, incluzând forme asimptomatice, forme ușoare cu afectarea căilor respiratorii superioare și cazuri severe cu pneumonii complicate, insuficiență respiratorie și disfuncție multiorganică. Studiile recente evidențiază că vârsta mică și severitatea episodului acut influențează gradul de afectare pulmonară și riscul de sechele postinfecțioase. În acest context, analiza clinicoimagistică are un rol esențial în monitorizarea evoluției bolii. Scop. Evaluarea particularităților clinice, imagistice și a evoluției afectării pulmonare postCOVID-19 la copiii spitalizați cu forme medii și severe ale infecției SARS-CoV-2. Material și metode. Studiul retrospectiv a inclus 200 de copii internați în perioada 2021– 2024 la Clinica de pneumologie pediatrică a IMSP Institutului Mamei și Copilului, repartizați în cinci loturi conform vârstei. Vârsta medie a fost de 2,06 ± 0,28 ani. Procesarea statistică a fost efectuată prin programele Epi-Info 7.2 și Microsoft Excel. Un subgrup de 101 pacienți, cu forme medii și severe, a fost supus examinării imagistice prin tomografie computerizată toracică (CT) pentru evaluarea afectării pulmonare și a sechelelor postinfecțioase. Rezultate. Mai mult de o treime dintre copiii spitalizați au avut vârsta sub un an (38,8%). Debutul sever al bolii a fost mai frecvent la nou-născuți (58,3%) și sugari (52,4%). Manifestările dominante au fost sindromul febril (48,4%), tusea (63,5%) și semnele catarale respiratorii (50%). Semne de severitate – cianoza, dispneea și tirajul toracic s-au observat preponderent la copiii sub un an. Modificări imagistice pulmonare au fost depistate la 68,3% dintre pacienți, cu predominanța ariilor de consolidare (71,5%), a opacităților de tip „sticlă mată” (17,2%) și a semnelor de fibroză (30,7%). La 15% dintre copii au fost identificate aderențe pleurale, sugerând inflamație persistentă. Sugarii au prezentat cea mai înaltă rată de afectare pulmonară (45,3%). Evoluția post-COVID-19 a fost favorabilă la majoritatea pacienților, însă leziunile reziduale au persistat variabil, necesitând monitorizare ulterioară. Concluzii.Infecția COVID-19 determină un spectru larg de manifestări respiratorii la copii, cu predominanța tusei (63,5%), febrei (48,4%) și semnelor catarale (50%). Sugarii și nounăscuții sunt categoriile cele mai vulnerabile, dezvoltând frecvent forme severe cu insuficiență respiratorie. Examinările imagistice prin CT au evidențiat afectarea pulmonară în 2/3 dintre cazuri, în 71,5% – leziuni de consolidare, 30,7% – de fibroză și 15,1% – aderențe pleurale. Aceste date confirmă necesitatea monitorizării în dinamică a copiilor post-COVID19 și importanța tomografiei computerizate toracice ca metodă de referință în evaluarea și supravegherea afectării pulmonare reziduale

    Multisystem inflammatory syndrome in children (MIS-C) with COVID-19

    Rezumat. Sindromul inflamator multisistemic la copii (MIS-C) este o complicație post-infecție cu SARS-CoV-2, caracterizată prin afectare multisistemică, inflamație severă și risc crescut de insuficiență a organelor vitale. Manifestările clinice includ febră persistentă, afectare cardiovasculară, tulburări respiratorii, gastrointestinale, hepatice și hematologice. Diagnosticul se bazează pe criterii clinice, biologice și serologice conform ghidurilor internaționale recente. Managementul implică terapie intensivă multidisciplinară, suport respirator și cardiovascular, corecție hidroelectrolitică și tratament imunomodulator (corticosteroizi, imunoglobuline intravenoase). Cazul clinic prezentat descrie evoluția filminantă a MIS-C la un sugar de 6 luni, care în pofida terapiei intensive a dezvoltat insuficiență multiplă de organe soldată ulterior cu deces. Literatura de specialitate subliniază importanța recunoașterii rapide a simptomelor, aplicării protocoalelor terapeutice standardizate și a colaborării multidisciplinare. Supravegherea continuă a parametrilor clinici și biochimici, împreună cu terapia imunomodulatoare, rămân pilonii esențiali în reducerea mortalității.Abstract Multisystem Inflammatory Syndrome in Children (MIS-C) is a postinfectious complication of SARS-CoV-2, characterized by multisystem involvement, severe inflammation, and an increased risk of vital organ failure. Clinical manifestations include persistent fever, cardiovascular impairment, and respiratory, gastrointestinal, hepatic, and hematological disorders. Diagnosis is based on clinical, biological, and serological criteria, according to recent international guidelines. Management involves multidisciplinary intensive care, respiratory and cardiovascular support, hydro-electrolytic correction, and immunomodulatory treatment (corticosteroids, intravenous immunoglobulins). The presented clinical case describes the fulminant evolution of MIS-C in a 6- month-old infant, who, despite intensive therapy, developed multiple organ failure that ultimately resulted in death. The specialized literature emphasizes the importance of early recognition of symptoms, implementation of standardized therapeutic protocols, and multidisciplinary collaboration. Continuous monitoring of clinical and biochemical parameters, together with immunomodulatory therapy, remain essential pillars in reducing mortality

    Evolutionary bronchopulmonary manifestations in children with COVID-19 infection

    Introduction: In the assessment of bronchopulmonary complications from the COVID-19 infection, the imaging methods are of choice, and the gold standard is HRCT (high-resolution computed tomography). Aim: assessment of the volume, location and type of bronchopulmonary damage by imaging techniques (HRCT) in children at the evolutionary stages of the COVID-19 infection. Material and methods: The research is a descriptive study of 88 children, aged 0-17 years, admitted to the Pneumology Clinic, between March 2021 and December 2022. All children were examined by chest CT. Results: According to the imaging data, 60 children (68.2%:95%CI 58.54-76.51) had pulmonary pathological changes, and 28 children (31.8%:95%CI 23.49-41 ,46) a normal lung appearance was confirmed. The highest proportion according to age was infants, with CT changes in 45,3%:95%CI 35.9-52.96. The fibrotic pattern of different degrees, was found in 35% of children. Areas of lung tissue consolidation had an impressive prevalence of 71%. The "ground glass" type changes suggestive of lung damage in COVID-19 infection were present in 9 children. Solitary, as well as multiple air bubbles up to 1cm in size were in 7.54% of cases. 8 Conclusions: The COVID-19 infection is not only an acute infection, but is a complex entity with post-infection complications and long-lasting effects, especially pulmonary ones. Children are predisposed to develop lung changes post COVID-19 with a higher proportion of infants. Pulmonary fibrotic changes were attested in 35%

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
