    L’infestation palpébrale et faciale au Demodex folliculorum

    Le Demodex folliculorum [D. folliculorum] est l’ectoparasite le plus fréquent dans les follicules des cils et de la peau. Son rôle précis dans la microflore n’est pas encore déterminé, mais il semble agir comme un commensal. Par contre, lorsque sa quantité dépasse un certain seuil, il devient pathogénique. Son infestation est associée à une blépharite antérieure et/ou postérieure, une sécheresse oculaire et plusieurs types d’affectation du visage tels que l’acné rosacée. Comme il peut se retrouver autant dans les paupières que la peau du visage, cela suggère qu’une infestation palpébrale pourrait être associée à une plus grande densité faciale dudit parasite. Pour vérifier cette hypothèse, des participants ayant diverses sévérités de blépharite antérieure à D. folliculorum ont été recrutés. Une épilation de cils infestés a été effectuée, permettant un décompte des parasites. D’un autre côté, l’infestation faciale a été évaluée par une biopsie superficielle standardisée de la peau du front, prélevant ainsi quelques couches de l’épiderme superficiel et le contenu des follicules pileux. L’analyse des résultats révèle que la présence d’une blépharite à D. folliculorum est associée à une densité faciale du parasite au-delà du seuil de normalité. De plus, le degré d’érythème facial augmente avec la sévérité de l’infestation des paupières et du visage. Certains signes et symptômes oculaires sont aussi affectés par la présence de la blépharite. Par exemple, on retrouve une réduction de la hauteur du ménisque lacrymal et une augmentation des yeux larmoyants lors d’une infestation légère à modérée, de même qu’une augmentation de la sensation de picotement à la bordure des paupières en présence d’une infestation sévère. En conclusion, comme les infestations faciale et palpébrale sont concomitantes, autant les professionnels de la vue que les dermatologistes doivent être impliqués dans le diagnostic et la prise en charge de cette condition. In addition, the degree of facial erythema increases with the severity of palpebral and facial infestations. Some ocular signs and symptoms are also affected by the presence of Demodex blepharitis. For example, a mild to moderate infestation is associated with a tear meniscus height reduction as well as more watery eyes, and a severe infestation with increased itching along the lids. In conclusion, since facial and palpebral infestations are concomitant, both eye care professionals and dermatologists should be involved in the diagnosis and management of this common parasite. In addition, a multidisciplinary approach would ultimately benefit the affected individuals

    Objectif. La sécheresse oculaire est connue pour altérer la qualité de la vision, augmenter les distorsions optiques et les aberrations du front d’onde d’ordre supérieur. Les traitements de la sécheresse oculaire et leur effet sur les aberrations ont déjà été étudiés, mais aucune étude n’a évalué l’impact de l’usage séquentiel des compresses chaudes (WC) combinées à des massages digitaux des paupières afin d’exprimer (MGX) les glandes de Meibomius (GM). Cette étude s’intéresse à l’influence du traitement WC/MGX sur les aberrations du front d’onde sur une période de 5 semaines, en posant l’hypothèse que ce traitement améliorera la qualité du film lacrymal et réduira les aberrations. Méthodes. Une étude prospective randomisée a évalué les aberrations (total, ordre supérieur (HO), coma, trèfle) après l’usage séquentiel (³ 4X/semaine) d’un masque chauffant (BruderÔ) et MGX lors d’une visite hebdomadaire pendant 5 semaines chez des participants avec une sécheresse évaporative légère à modérée. À chaque visite, l’acuité visuelle (AV), symptômes (questionnaire OSDI), la stabilité des larmes (NIBUT), l’épaisseur de la couche lipidique (LLT), l’expression du meibum (MGE) étaient mesurés avant et après WC/MGX. Les aberrations de l’oeil (OPD scan-III) étaient mesurées avant et à 1, 5, 15, 30, 45 et 60 minutes. Les données ont été traitées avec analyse de variance mixte. Résultats. Les participants (n = 56 ; 31H ; 25F, âge 26,3 ± 4,5 ans) étaient démographiquement comparables dans les groupes (p > 0,05). Aucune différence significative n’est notée entre les groupes lors de l’évaluation des aberrations avant l’application du traitement pour chacune des visites (p total = 0,552 ; HO p = 0,468 ; coma p = 0,575 ; trèfle p = 0,425). À chaque visite, les aberrations augmentent généralement entre 1 et 5 minutes après l'application (p 0.05). For each visit, there were no significant differences between groups when assessing for aberrations prior to treatment (total p = 0.552; HO p = 0.468; coma p = 0.575; trefoil p = 0.425). Within each visit, aberrations typically increased between 1 and 5 minutes immediately following the eye mask application (p<0.001), with the effect beginning to subside from minute 15 onward. After 5 weeks, an improvement in NIBUT and MG expression (MGE) was observed for the test group (p < 0.001), with no difference in acuity (p = 0.546), LLT (p = 0.474) or OSDI (p = 0.078). Conclusion. In the test group, high-order aberrations remained unchanged after 5 weeks of WC/MGX compared to control. Any transient increase in aberrations dissipates in under 15 minutes. Sequential use of an eyelid warming mask did improve tear film stability and MG expression without impacting vision, further supporting WC/MGX recommendations for the management of evaporative dry eyes

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
