    Das Bundesinstitut für Arzneimittel und Medizinprodukte (BfArM) ist unter anderem verantwortlich für die Zulassung von wirksamen und unbedenklichen Arzneimitteln (AM) sowie für die Überwachung von Arzneimittelrisiken (Pharmakovigilanz). Der Markt für AM wird zunehmend europäisiert, und nach der letzten EU-Erweiterung betätigen sich insgesamt 47 nationale Zulassungsbehörden sowie die European Medicines Agency (EMEA) in London auf dem Gebiet der Zulassung und Risikoabwehr. Die Zusammenarbeit zwischen den Zulassungsbehörden ist zwar gut, aber zeitaufwendig und damit letztlich unökonomisch. Das BfArM muss deshalb nach umfassender Reorganisation in die Spitzengruppe der europäischen Zulassungsinstitute aufrücken, auch um für die pharmazeutische und die Medizinprodukteindustrie in Deutschland als positiver Standortfaktor zu wirken. Die politisch gewollte, aber jetzt gescheiterte Umwandlung des BfArM in eine Deutsche Agentur für Arzneimittel und Medizinprodukte (DAMA), eine Anstalt des Öffentlichen Rechts, wäre wegen der damit verbundenen erweiterten operativen Gestaltungsräume ein weiterer Schritt zu einem anzustrebenden „Center of Excellence“ gewesen.     Seit Mai 2020 können Hersteller einen Antrag zur Aufnahme einer digitalen Gesundheitsanwendung (DiGA) in das Verzeichnis nach § 139e Fünftes Buch Sozialgesetzbuch (SGB V) beim für das Bewertungsverfahren zuständigen Bundesinstitut für Arzneimittel und Medizinprodukte (BfArM) stellen. Diesem neuen Antragsverfahren liegen spezifische Anforderungen und Bewertungsparameter zugrunde, zu denen sich herstellerseitig eine Vielzahl an Verfahrens- und wissenschaftlichen Fragestellungen an das BfArM ergibt. Um diesem Bedarf Rechnung zu tragen, hat das BfArM seine etablierten Informations- und Beratungsangebote gezielt für diese neuen Fragestellungen erweitert. Welche dies bezogen auf DiGA im Einzelnen sind, wo entsprechende Informationen und Unterlagen zu finden sind und was diese jeweils auszeichnet, wird in diesem Beitrag dargelegt. Des Weiteren wird mit Blick auf die zwischen Mai 2020 und April 2021 durchgeführten Beratungsgespräche analysiert, welchen Einfluss diese auf das Ergebnis im jeweiligen Antragsverfahren zur Aufnahme in das DiGA-Verzeichnis haben. Dabei bestätigt sich, dass die frühzeitige Erörterung bewertungsrelevanter Parameter und die Berücksichtigung der Beratungsempfehlungen mit einem positiven Bewertungsergebnis korrelieren: Zu den im Verzeichnis gelisteten DiGA hatten 80 % der Hersteller zuvor eine Beratung durch das BfArM in Anspruch genommen. Die Quote zurückgezogener bzw. abgelehnter Anträge war dagegen deutlich höher, wenn Hersteller zuvor keine Beratung in Anspruch genommen hatten, gegenüber Herstellern, die im Vorfeld zentrale Aspekte mit dem BfArM diskutiert hatten (63 % vs. 35 %). Insgesamt profitieren alle Seiten von dem frühzeitigen, intensiven Austausch – letztendlich insbesondere die Patientinnen und Patienten durch einen unverzögerten Einzug von DiGA in die Regelversorgung aufgrund höherer Antragsqualität.Since May 2020, manufacturers have been able to submit an application to the Federal Institute for Drugs and Medical Devices (BfArM) for the inclusion of a digital health application (DiGA) in the directory pursuant to section 139e of the German Social Code Book V. This new application procedure is based on specific requirements and assessment parameters for which a large number of procedural and scientific questions arise on the side of the manufacturers. In order to meet this need, the BfArM has specifically expanded its established information and advisory services for these new questions. This article describes in detail what these are in relation to DiGA, where the corresponding information and documents can be found and what distinguishes them in each case. Furthermore, with a view to the counselling interviews conducted between May 2020 and April 2021, their influence on the outcome of the respective application procedure for inclusion in the DiGA directory is analysed. This confirms that the early discussion of parameters relevant to the evaluation and the consideration of the advisory recommendations correlate with a positive evaluation result. For the DiGA listed in the directory, 80% of the manufacturers had previously received advice from the BfArM. The rate of withdrawn or rejected applications, on the other hand, was significantly higher if manufacturers had not previously sought advice from the BfArM in advance compared to manufacturers who discussed key aspects (63% vs. 35%). Overall, all sides benefit from the early, intensive exchange – in the end especially the patients through introduction of DiGA into standard healthcare due to higher application quality without delay

    Die Digitalisierung stellt auch im Gesundheitsbereich einen eindeutigen Megatrend dar, der neben der Wegbereitung durch geänderte rechtliche Rahmenbedingungen auch durch die Coronapandemie eine enorme Beschleunigung erfahren hat. Mit Blick in die Zukunft wird dieser Trend durch immer kürzere Entwicklungszyklen und technologischen Fortschritt zu einer zunehmenden Verschmelzung einzelner digitaler und nicht digitaler Produkte, darunter Arzneimittel und digitale Medizinprodukte, zu einem digitalen Ökosystem beitragen. Die Digitalisierung wird nicht nur die Patientensouveränität stärken, sondern unter anderem auch eine patientenzentrierte, stärker auf den individuellen Patienten ausgerichtete Medizin ermöglichen; digitale (patientenberichtete) Endpunkte und „dezentrale“ Studienkonzepte werden die Möglichkeiten klinischer Studien erweitern. Künstliche Intelligenz wird Diagnosestellungen verbessern und beschleunigen und zum besseren Verständnis von Krankheitsbildern beitragen. Um auch zukünftig Innovationen zu ermöglichen, aufkommende Trends im Fokus zu haben und vor allem die Patientensicherheit weiter zu verbessern, trägt das Bundesinstitut für Arzneimittel und Medizinprodukte (BfArM) an vielen Stellen dazu bei, die mit der Digitalisierung verbundenen Chancen in Möglichkeiten zu verwandeln – ohne dabei die Risiken aus dem Blick zu verlieren: Dazu gehören der Ausbau des Forschungsdatenzentrums, Aufgaben zu Interoperabilität und Klassifikationen sowie Ansätze zur datengestützten Signalerkennung aus Risikoinformationen. Wie darüber hinaus Forschungsprojekte unter Nutzung von künstlicher Intelligenz, (inter)nationale Kooperationen, die Einbeziehung von Real World Data in Nutzen-Risiko-Bewertungen, die Bewertung digitaler Gesundheits- und Pflegeanwendungen neben weiteren Aktivitäten zur „digital readiness“ in Deutschland und Europa beitragen, dazu im Folgenden ein Überblick.Digitalization is a clear megatrend of our time, also in the health sector, which is currently experiencing enormous acceleration due to the COVID-19 pandemic in addition to paving the way due to changes in the legal framework. Looking to the future, this trend will contribute to further digitalization and the merging of individual digital products, including medicinal products and medical devices, into a digital ecosystem. This will be supported by ever-shorter development cycles and technological progress. Digitization will not only strengthen patient sovereignty, but also enable more patient-centered medicine; artificial intelligence will improve and accelerate diagnoses and will contribute to a better understanding of disease patterns and underlying mechanisms or causes. In order to continue to enable innovations in the future, to focus on emerging trends, and, above all, to further improve patient safety, the BfArM is contributing in many places to transforming the opportunities associated with digitalization into possibilities – without losing sight of the risks. The following is an overview of how, for example, the expansion of the Research Data Center, activities addressing interoperability, research projects using artificial intelligence, (inter-)national cooperation, the utilization and inclusion of “Real World Data” in our benefit/risk assessments, and the evaluation of digital health and digital care applications among other activities of the BfArM contribute to “digital readiness” in Germany and Europe

    Mit dem Digitale-Versorgung-Gesetz (DVG) hat der Gesetzgeber digitalen Gesundheitsanwendungen, kurz DiGA, einen Einzug in die Regelversorgung nach dem Fünften Buch Sozialgesetzbuch (SGB V) ermöglicht. Voraussetzung für die „App auf Rezept“ ist die Listung im DiGA-Verzeichnis nach positiv durchlaufenem Bewertungsverfahren beim Bundesinstitut für Arzneimittel und Medizinprodukte (BfArM). Dafür ist von den Herstellern neben umfangreichen Qualitäts- und Sicherheitsparametern auch ein positiver Versorgungseffekt nachzuweisen. Mit dem DiGA-Verzeichnis bietet das BfArM seit Oktober 2020 adressatengerechte umfassende Transparenz zu den DiGA und deren Eigenschaften. Der Artikel erläutert den Weg zur „App auf Rezept“ von den Unterstützungs- und Beratungsangeboten durch das BfArM über den Ablauf des Bewertungsverfahrens (DiGA-Fast-Track) und die Bewertungskriterien bis hin zu den Inhalten des Verzeichnisses. Es zeigt sich, dass das Interesse an dem Fast-Track-Verfahren groß ist. Der Nachweis der positiven Versorgungseffekte, also eines tatsächlichen Mehrwerts für die Patienten, ist ausbalanciert und mit überwiegend randomisierten kontrollierten Studiennachweisen auf einem angemessenen Niveau. Dass das Verfahren auch mit Herausforderungen für die Antragsteller verbunden sein kann, z. B. Mängel adäquat in dem gesetzlich vorgegebenen Bewertungszeitraum zu adressieren, zeigen die Zahlen zurückgenommener Anträge im Verhältnis zu den gelisteten DiGA an. Das BfArM steht zu diesem neuen Verfahren in engem Austausch mit allen Beteiligten. Welche Überlegungen und Potenziale sich daraus für die Weiterentwicklung aus Sicht des BfArM ergeben, zeigt der Ausblick.With the Digital Health Care Act (DVG), the legislator has made it possible for digital health applications (DiGA) to be included in standard care and reimbursement in accordance with the Fifth Book of the German Social Code (SGB V). The prerequisite for the “app on prescription” is a listing in the DiGA directory after a positive evaluation procedure at the Federal Institute for Drugs and Medical Devices (BfArM). In addition to comprehensive quality and safety parameters, the manufacturers must also prove a positive healthcare effect. With the DiGA directory, the BfArM has been offering addressee-oriented comprehensive transparency on DiGA and their properties since October 2020. The article explains the path to the app on prescription from the support and advisory services offered by the BfArM to the evaluation procedure (DiGA Fast Track) and the evaluation criteria to the contents of the directory. It can be seen that there is great interest in the Fast Track procedure. The evidence of positive healthcare effects, i.e. an actual added value for patients, is balanced and at an appropriate level with predominantly randomised controlled trial evidence. The fact that the procedure can also be associated with challenges for the applicants, e.g. to adequately address deficiencies in the legally prescribed assessment period, is indicated by the numbers of withdrawn applications in relation to the listed DiGA. The BfArM is in close exchange with all parties involved regarding this new procedure. The conclusion of this article shows which considerations and potentials result from this for further development from the point of view of the BfArM

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    The purpose of writing this Field Work Practice (PKL) report is to design the user interface (UI) and user experience (UX) of the BFarm application using user-centered design (UCD) techniques. UCD is a design approach that focuses on the needs, preferences, and involvement of users at every stage of application development. In this study, we analyzed user needs through interviews and questionnaires and created personas and usage scenarios. An initial prototype was developed and tested with end-users to obtain relevant feedback. The results show that the implementation of UCD enhances user satisfaction and facilitates interaction with the BFarm application. This application aims to provide more effective and efficient solutions for managing agricultural activities by focusing on user experience

    This report describes the Field Work Internship (PKL) conducted at PT. Lima Benua Koneksindo, specifically in the software department. The internship lasted two months and focused on the development of a mobile application called BFarm. This application was designed to improve efficiency in the land reservation and soil content checking process. Through the BFarm application, landowners can request land checks independently or through investor funding. The application design process involved various stages, from system requirements analysis and UML diagram creation, use case diagrams, architectural design, class diagrams, and user interface implementation. The project resulted in a functional and user-friendly mobile application, which is expected to provide significant benefits to landowners, companies, and investors. This experience provided valuable insights into mobile application development and enhanced understanding of work processes in the software industry
