Epileptic seizure with Todd's paresis in a dental context

Crizele epileptice acute au un debut neaşteptat în contextul unor consultaţii non-neurologice, complicând conduita terapeutică. Identificarea rapidă a crizelor epileptice şi iniţierea tratamentului specific în cadrul Unităţii de Primiri Urgente (UPU) au impact în prevenirea complicaţiilor neurologice severe. Scop. Prezentarea unui caz clinic complex în care crizele epileptice au fost declanşate în mod acut în timpul unei consultaţii oro-maxilo-faciale, evidenţiind importanţa intervenţiei în zona roşie a UPU. Material şi metode. Pacientă în vârstă de 64 de ani s-a prezentat la UPU cu durere şi edem în regiunea perimandibulară dreaptă. În timpul efectuării unei radiografii panoramice, a dezvoltat crize epileptice focale motorii în hemicorpul stâng, progresând ulterior la crize tonico-clonice bilaterale. Pacienta a fost transferată de urgenţă în zona roşie. Rezultate. Pacientă de 64 de ani s-a prezentat cu durere în regiunea perimandibulară dreaptă în UPU. La examen radiologic a dezvoltat crize epileptice focale motorii în hemicorpul stâng, ulterior crize tonico-clonice bilaterale. Pacienta a fost transferată în zona roşie. S-a administrat 30 mg Diazepam i.v. consecutiv. La EEG -descărcări epileptiforme paroxistice, iar CT cerebral fără modificări acute. Monitorizarea a prezentat episoade de hipotensiune şi fibrilaţie atrială tahisis-tolică tranzitorie. Post-critic, s-a observat un deficit motor tranzitoriu în hemicorpul stâng, pareză Todd. A fost internată în secţia de chirurgie oro-maxilo-facială. Concluzii. Crizele epileptice pot apărea inopinat în afara contextului neurologic şi necesită intervenţie rapidă, interdisciplinară. Acest caz evidenţiază rolul esenţial al evaluării neurologice imediate şi al monitorizării avansate în zona roşie a UPU, în cadrul consultaţiilor de rutină din alte specialităţi.Introduction. Acute epileptic seizures can occur unexpectedly during non-neurological consultations, significantly complicating the therapeutic approach. Rapid recognition of seizure activity and timely initiation of specific treatment in the Emergency Department (ED) are crucial for preventing serious complications. Objective. To present a complex clinical case in which epileptic seizures were acutely triggered during an oro-maxillofacial consultation, highlighting the importance of intervention in the red zone of the ED. Material and methods. A 64-year-old female presented to the ED with pain and swelling in the right perimandibular region. During panoramic radiographic evaluation, the patient developed focal motor seizures affecting the left he-mibody, which progressed to bilateral tonic-clonic seizures. She was urgently transferred to the red zone, where continuous monitoring. Results. A 64-year-old patient presented with pain in the right perimandibular region in the ED. On radiological examination, she developed focal motor epileptic seizures in the left hemibody, subsequently bilateral tonic-clonic seizures. The patient was transferred to the red zone. Diazepam 30 mg i.v. was administered consecutively. On EEG - paroxysmal epileptiform discharges, and cerebral CT without acute changes. Monitoring showed episodes of hypotension and transient tachysystolic atrial fibrillation. Post-critically, a transient motor deficit was observed, Todd paresis. She was admitted to the oral-maxillofacial surgery department. Conclusion. Epileptic seizures arise unexpectedly outside neurological contexts and require urgent, interdisciplinary intervention. This case underscores the essential role of immediate neurological assessment and monitoring in the red zone of the ED, during routine consultations in other medical specialties
Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

Get PDF“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
Get PDFWe provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
