Management of acute postoperative pain in urology: a condition that guarantees the safety and effectiveness of the treatment

o componentă importantă a îngrijirii pacienților. Pentru a
reduce disconfortul, a încuraja mobilizarea precoce, a reduce complicațiile și a îmbunătăți recuperarea generală,
controlul eficient a durerii este obligator. Scopul lucrării.
Aprecierea gradului de durere și eficacitatea tratamentului
analgezic postoperator în diferite intervenții chirurgicale
urologice. Material și metode Studiul a inclus 1200 pacienți spitalizați în Clinica de Urologie și Nefrologie Chirurgicală a SCR „Timofei Moșneaga” Evaluarea durerii s-a realizat folosind scala vizual-analogă în ziua internării și în zilele
0-4 postoperator. Scorurile medii ale intensității durerii au
fost calculate și pacienții au fost clasificați astfel: fără durere
(scor 0-1), durere ușoară (2-4), moderată (4-7) sau severă
(7-10). Rezultate. În studiu, 31% dintre pacienți au raportat durere moderată sau severă în ziua 0, 30% în ziua 1, și
19%, 16% și 14% în zilele 2, 3 și 4. Durerea moderată sau
severă a fost mai frecventă la pacienții operați deschis în zilele 0-1 postoperator (55-91%) comparativ cu cei operați
prin tehnici endourologice (43-67%). Medicația opioidă
este crucială pentru durerea postoperatorie moderată până
la severă postoperatorie, însă trebuie monitorizată din cauza efectelor secundare și riscului de dependență. Analgezicele neopioide, cum ar fi antiinflamatoarele nesteroidiene
(AINS), combinația AINS + Tramadol și Paracetamolul, sunt
larg utilizate ca parte a unui regim multimodal, având un rol
important în gestionarea durerii. Concluzii. Pentru a crea
o strategie amănunțită și unică de management al durerii
pentru fiecare pacient, este esențială o coordonare strânsă
între echipa chirurgicală, anesteziologi și specialiști în managementul durerii.Background. Postoperative pain management is an important component of patient care. Effective pain control
is essential to reduce discomfort, encourage early mobilization, reduce complications, and improve overall recovery.
Objective of the study. To assess the degree of pain and
the effectiveness of postoperative pain treatment in various
urological surgeries. Material and methods. The study included 1200 patients hospitalized in the Clinic of Urology
and Surgical Nephrology of the Timofei Moșneaga Republican Clinical Hospital Pain assessment was conducted using
the visual analogue scale on the day of admission and on
postoperative days 0-4. The average pain intensity scores
were calculated, and patients were classified as follows:
no pain (score 0-1), mild pain (2-4), moderate (4-7), or severe (7-10). Results. In the study, 31% of patients reported
moderate or severe pain on day 0, 30% on day 1, and 19%,
16%, and 14% on days 2, 3, and 4. Moderate or severe pain
was more frequent in patients undergoing open surgery on
postoperative days 0-1 (55-91%) compared to those operated using endourological techniques (43-67%). Opioid
medication is crucial for moderate to severe postoperative
pain, but it must be monitored due to side effects and the
risk of dependency. Non-opioid analgesics, such as nonsteroidal anti-inflammatory drugs (NSAIDs), NSAIDs + Tramadol combinations, and Paracetamol, are widely used as part
of a multimodal regimen, playing an important role in pain
management. Conclusion. Close coordination between the
surgical team, anesthesiologists, and pain management
specialists is essential to create a comprehensive and individualized pain management strategy for each patient
Minimally invasive treatment of solitary renal cyst
Introduction. The solitary renal cyst is an oval or round formation of different sizes, it can be single or
multiple, most often unilaterally delimited by a thin membrane, which may contain blood or serous fluid.
Aim of study. Study and interpretation of data obtained from the analysis of patients diagnosed with renal
solitary cyst in the Clinic of Urology and Surgical Nephrology of the Republican Clinical Hospital "Timofei
Moşneaga", during the years 2018-2021 and treated by laparoscopic method.
Methods and materials. This research is a descriptive observational study, which analysed data on the
diagnosis and treatment of patients with solitary renal cyst, hospitalised in the Clinic of Urology and
Surgical Nephrology of the Republican Clinical Hospital "Timofei Moşneaga", during 2018 -2021. The
study group consisted of 66 patients. The following indicators were analysed: distribution of cases by age
groups, gender, living environment, location, size and classification of cysts.
Results. Analyzing the distribution of cases by sex, we found that CRS has a higher incidence among men
(59%) than women (41%). During the years included in the 2018-2021 study, 36 cases were registered in
rural areas (55%), and in urban areas 30 cases (45%). Analysing the data, we noticed an increase in the
incidence of CRS with age. The age of the patients in the study group ranged from 18 to> 65 years. After
the anatomical location, the cysts were distributed as follows: upper pole 38 patients (51%), lower pole 20
patients (30%), medial renal 8 patients (12%). Regarding the size of the CRS, after interpreting the data we
found that of the total number of patients, most had a cyst of 6-8 cm - 32 patients (49%), 9-11 cm - 18
patients (27%),> 11 cm - 10 patients (15%), 3-5 cm - 6 patients (9%). Analysing the data on the clinical
signs present in patients with CRS, we found that the most common symptom is low back pain (62%),
followed by hypertension (14%), anatomical defect (11%), hematuria (7%), and asymptomatic (6%).
According to the distribution of cases according to the international classification of Bosnia, we established:
52 patients - Bosniak I, 14 patients - Bosniak II, and Bosniak III - no patients.
Conclusion. There are currently several treatment methods used in solitary renal cyst, but the minimally
invasive laparoscopic method is currently the choice

sau rotundă de diferite dimensiuni, poate fi unică sau multiplă, cel mai adesea delimitată unilateral de o membrană
subțire care poate conține sânge sau lichid seros. Scopul
lucrării. Studiul și interpretarea datelor obținute în urma
analizei pacienților diagnosticați cu CR în Clinica de Urologie și Nefrologie Chirurgicală a Spitalului Clinic Republican
„Timofei Moșneaga”, în perioada 2020-2023 și tratați prin
metoda laparoscopică. Material și metode. Cercetarea dată
este un studiu observațional descriptiv, privind diagnosticarea și tratamentul pacienților cu CR, desfășurat în Clinica de Urologie și Nefrologie Chirurgicală a Spitalului Clinic
Republican „Timofei Moșneaga”, în perioada 2020 -2023.
Grupul de studiu a inclus 66 de pacienți. S-a analizat distribuția cazurilor pe grupe de vârstă, sex, mediu de viață,
localizare, dimensiune și clasificare a chisturilor. Rezultate. Analiza distribuției cazurilor CR a arătat o incidență mai
mare la bărbați (59%) decât la femei (41%). În mediul rural
au fost înregistrate 36 de cazuri (55%), iar în mediul urban
30 de cazuri (45%). Incidența CR a crescut odată cu vârsta, pacienții având între 18 și peste 65 de ani. Distribuția
anatomică a chisturilor a fost: polul superior (51%), polul
inferior (30%), renal medial (12%). Mărimea chisturilor:
6-8 cm (49%), 9-11 cm (27%), >11 cm (15%), 3-5 cm (9%).
Semnele clinice identificate au fost lombalgia (62%), hipertensiunea arterială (14%), defect anatomic (11%), hematurie (7%), și cazuri asimptomatice (6%). Clasificarea Bosniak
a arătat 52 pacienți cu Bosniak I, 14 cu Bosniak II și 0 cu
Bosniak III. Concluzii. În prezent există mai multe metode
de tratament utilizate în chistul renal, dar metoda laparoscopică minim invazivă rămâne a fi metoda de elecție.Background. Renal cyst (RC) is an oval or round formation of varying sizes, which can be single or multiple, most

Cyst sizes: 6-8 cm (49%), 9-11 cm (27%), >11 cm (15%),
3-5 cm (9%). Clinical signs identified were lower back pain
(62%), hypertension (14%), anatomical defect (11%), hematuria (7%), and asymptomatic cases (6%). The Bosniak
classification showed 52 patients with Bosniak I, 14 with
Bosniak II, and 0 with Bosniak III. Conclusion. Currently,
there are several treatment methods used for renal cysts,
but the minimally invasive laparoscopic method remains
the method of choice
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
