An unusual presentation of acute myocardial infarction in physiotherapy direct access: findings from a case report
Abstract Background Shoulder pain (SP) may originate from both musculoskeletal and visceral conditions. Physiotherapists (PT) may encounter patients with life-threatening pathologies that mimic musculoskeletal pain such as Acute Myocardial Infarction (AMI). A trained PT should be able to distinguish between signs and symptoms of musculoskeletal or visceral origin aimed at performing proper medical referral. Case presentation A 46-y-old male with acute SP lasting from a week was diagnosed with right painful musculoskeletal shoulder syndrome, in two successive examinations by the emergency department physicians. However, after having experienced a shift of the pain on the left side, the patient presented to a PT. The PT recognized the signs and symptoms of visceral pain and referred him to the general practitioner, which identified a cardiac disease. The final diagnosis was acute myocardial infarction. Conclusion This case report highlights the importance of a thorough patient screening examination, especially for patients treated in an outpatient setting, which allow distinguishing between signs and symptoms of musculoskeletal from visceral diseases
The Effectiveness and Optimal Dose of Resistance Training in Patients With Subacute and Persistent Low Back-Related Leg Pain: A Systematic Review
A subgroup of patients with low back pain (LBP) suffers from low back -related leg pain (LBLP), which can be classified as radicular pain, or somatic referred pain without nerve root involvement. LBLP is considered an obstacle to recovery and a strong negative prognostic factor for medium- and long-term disability. In this review, we aimed to investigate the effectiveness and optimal dose of resistance training (RT) in patients with subacute or persistent LBLP to provide clinical recommendations for practice. This systematic review was conducted by adhering to the Preferred Reporting Items for Systematic Reviews and MetaAnalyses (PRISMA) guidelines and the recommendations of the Cochrane Collaboration. We conducted a literature search on PubMed, PEDro, Cochrane Library, Scopus, and Web of Science databases. Only randomized controlled trials (RCTs) involving patients >= 18 years of age were included. The risk of bias in the included studies was assessed using "the Cochrane Collaboration's tool for assessing risk of bias" (RoB) and the inter -rater agreement for full -text selection was evaluated using Cohen's Kappa (K). The search elicited a total of 4.537 records, and two RCTs involving a total of 196 participants were identified through a selection process based on title, abstract, and full -text assessment. Both studies had a low to moderate risk of bias. The inter -examiner concordance index for the selection of full text was excellent (K=1). RT seems to be an effective and safe intervention for patients with LBLP, but its long-term effectiveness, superiority over other types of exercise -based therapies, and optimal dosage still constitute a gray area in the literature
Headache, Loss of Smell, and Visual Disturbances: Symptoms of SARS-CoV-2 Infection? A Case Report
Objective The purpose of this case report is to describe the main components of the history and physical examination that led to idiopathic intracranial hypertension differential diagnosis, which initially was investigated as COVID-19. Methods (Case Description) A 28-year-old woman complaining of constant headache and loss of smell and taste was suspected as SARS-CoV-2 infection by her general practitioner. She underwent 3 molecular swab tests, all negative, then decided to seek her physical therapist for relieving headache. Results The full cranial nerve examination revealed impaired olfactory (CNI), abducens (CN VI), and facial (CN VII) nerves, leading the physical therapist to refer the patient to a neurosurgeon for a suspected central nervous system involvement. The neurosurgeon prescribed a detailed MRI and an ophthalmologic examination, which allowed for the final diagnosis of idiopathic intracranial hypertension. Conclusion An urgent lumbo-peritoneal shunting surgery resolved the patient's symptoms and saved her sight. Despite the ongoing COVID-19 pandemic, health care professionals must pay attention to properly investigating patients' signs and symptoms using comprehensive clinical reasoning, considering the screening for referral to specialist medical attention. Impact A thorough physical examination is required for every patient even if patients' signs and symptoms are in line with apparent common and widespread pathologies. Cranial nerve evaluation is an essential component of the physical therapist assessment and decision-making process. The ongoing pandemic highlighted the fundamental assistance of physical therapists toward physicians in the screening and management of musculoskeletal diseases
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
L'accuratezza diagnostica dei test clinici per la sindrome dello stretto toracico: una revisione sistematica della letteratura
Obiettivo: indagare l’accuratezza diagnostica dei principali test provocativi per la diagnosi di sindrome dello stretto toracico (TOS) neurogena.
Metodi: tra Aprile e Settembre 2022 sono state condotte ricerche su 5 banche dati/motori di ricerca elettronici, utilizzando stringhe di ricerca costituite da parole chiave e termini MeSh (Medical Subject Headings). Sono stati inclusi tutti gli studi che indagassero l’accuratezza diagnostica dei test clinici. La valutazione metodologica degli studi è avvenuta attraverso lo strumento di critical appraisal CASP Checklist per gli studi diagnostici. È stata svolta una sintesi qualitativa degli studi considerati eleggibili.
Risultati: sono stati identificati in totale 7784 studi, ma solo 12 sono stati inclusi nella revisione. Gli index test indagati erano diversi nei vari studi, così come i reference standard con cui venivano comparati (quando presenti). La qualità metodologica della maggior parte degli studi si è rivelata bassa. L’assenza di reporting standard crea un’eterogeneità tale non permettere la comparazione e la sintesi dei risultati con strumenti statistici. Gli studi sull’accuratezza diagnostica delle manovre provocative sono tra loro contrastanti in termini di risultati e non è possibile trarre conclusioni sicure. Nemmeno gli esami di imaging sembrano così utili per la diagnosi di TOS, ma devono essere utilizzati solo per escludere altre condizioni che possono mimare quadri di TOS neurogenico.
Conclusione: la letteratura disponibile non sembra supportare l’utilizzo dei test clinici per la diagnosi di TOS. Al fine di migliorare la ricerca futura, dovrebbero essere introdotti criteri diagnostici univocamente accettati per aiutare nella diagnosi, oltre a ben definite linee guida per il reporting, al fine di creare omogeneità tra gli studi, come suggerito dal comitato della Società di Chirurgia Vascolare
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Efficacia della Pain Neuroscience Education in pazienti con dolore cronico da sensibilizzazione centrale: una revisione sistematica della letteratura.
Background: Il dolore muscoloscheletrico (MSK) cronico influisce in termini di disabilità, qualità di vita e costi di gestione a livello mondiale; è conseguenza di più fattori (biologici, psicologici e sociali), infatti per gestirlo è fondamentale un approccio multidisciplinare (farmacologico, riabilitativo e psico-educativo). In questo scenario, la PNE (Pain Neuroscience Education) è una tecnica educativa mirata a “riconcettualizzare” il dolore, spiegandone i meccanismi di base ai pazienti, è molto usata per quelli con dolore cronico da sensibilizzazione centrale.
Obiettivi: valutare l’efficacia della PNE su dolore, disabilità, fattori psicosociali; in pazienti con dolore MSK cronico da sensibilizzazione centrale - attraverso una revisione sistematica di RCTs.
Materiali e metodi: La ricerca è stata condotta su tre banche dati: PubMed, PEDro, CINAHL. Sono stati inclusi RCTs che avessero adottato criteri di inclusione specifici per la sensibilizzazione centrale o che avessero individuato condizioni cliniche, strumenti o fattori psicosociali che la letteratura ha dimostrato essere correlati a dominanza da sensibilizzazione centrale (CSI, TSK, FABQ, PCQ, PSEQ, PCS). Qualità metodologica e rischio di bias sono stati valutati con Rob1 e PEDro scale.
Risultati: sono stati inclusi quindici RCTs, la maggior parte di questi presentava un basso rischio di bias, eccetto il performance bias.. I risultati sono stati analizzati solo qualitativamente, in base ai criteri diagnostici dei pazienti inclusi (fibromialgia, lombalgia cronica, sindrome da fatica cronica, dolore spinale cronico, lombalgia e/o fibromialgia) rapportati agli outcome d’interesse (dolore, disabilità, fattori psicosociali).
Conclusioni: La PNE sembra efficace nel migliorare gli outcome indagati, soprattutto se combinata ad altri interventi, in pazienti con fibromialgia e lombalgia cronica; mentre nel breve termine sembra efficace anche sola, in pazienti con dolore spinale cronico o sindrome da fatica cronica
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
