    Analysis of Prescribing Patterns of Drugs Used in Osteoarthritis in Tertiary Care Center

    Background: Osteoarthritis (O.A.) is a common degenerative lesion of the articular cartilage that corresponds to hypertrophy deterioration of bone structure is osteoarthritis.&nbsp; The treatment is mainly focused on the management of pain. Methods: &nbsp;An Observational study was conducted among 163 patients visiting Orthopedics Out Patient Department&nbsp;&nbsp; at GMC Hospital. The duration of study was for 6 months from Jan 2024-Jun 2024. The analysis of collected data was done by SPSS version Software. Descriptive Statistics tools like frequency, Percentage were used to express the findings. Results: We found that female was (68.7%) in comparison to male (31.3%). Oral route (79%) was most commonly preferred route of drugs administration than topical route (21%). Non-Steroidal Anti-Inflammatory Drugs (NSAIDS) (41.78%), Vitamins and minerals (22.5%), Antiulcers (19.5%), Symptomatic Slow-acting Drugs in Osteoarthritis (SYSODA) (6.82%), Muscle Relaxants (5.02%) and Others (4.07%) were Prescribed groups of drugs. Diclofenac (19.52%) was most commonly prescribed Non-Steroidal Anti-Inflammatory Drugs followed by Aceclofenac (11.37%). Average no. of drugs prescribed was 5.12, % of drugs Prescribed from National List of Essential Medicine (NLEM) was 79.88%, % of drugs prescribed by generic name was 0% and Average dispensing time was 225.13 sec. Conclusion: Osteoarthritis (O.A) was mostly common in female than male. Lumbar was most commonly affected site of Osteoarthritis. Non-Steroidal Anti-Inflammatory Drugs was most commonly prescribed groups of drugs.     Objectives: This study aimed to develop and evaluate the patient information leaflets (PILs) for type 2 diabetes mellitus (T2DM) patients. Methods: The PILs were prepared in English by referring to primary, secondary, and tertiary resources. Readability of the PIL was assessed using the Flesch reading ease (FRE) test, Flesch–Kincaid grade level (FKGL), and user readability methods. User opinion on PILs content and design was obtained from T2DM patients (n=40). The Baker Able Leaflet Design (BALD) method was used to assess the layout and design characteristics of the PILs. Results: The FRE and FKGL readability scores were 62.9 and 6.6, respectively. The BALD criteria scores for English and Nepali PILs were 29 and 25, respectively. The overall user testing readability mean scores were significantly improved from 47.5 to 97.25. 90% of T2DM patients rated the PILs as good design and content. Most of them were housewife female patients (mean age 57.725) with a primary level of education and belonged to the upper-lower class of socioeconomic status. Conclusion: The developed PILs met the standard criteria for easy reading and comprehension. The majority of them had a good opinion on the content and design of the PILs

    Examining which clinicians provide admission hospital care in a high mortality setting and their adherence to guidelines: An observational study in 13 hospitals

    Background: We explored who actually provides most admission care in hospitals offering supervised experiential training to graduating clinicians in a high mortality setting where practices deviate from guideline recommendations. Methods: We used a large observational data set from 13 Kenyan county hospitals from November 2015 through November 2018 where patients were linked to admitting clinicians. We explored guideline adherence after creating a cumulative correctness of Paediatric Admission Quality of Care (cPAQC) score on a 5-point scale (0–4) in which points represent correct, sequential progress in providing care perfectly adherent to guidelines comprising admission assessment, diagnosis and treatment. At the point where guideline adherence declined the most we dichotomised the cPAQC score and used multilevel logistic regression models to explore whether clinician and patient-level factors influence adherence. Results: There were 1489 clinicians who could be linked to 53 003 patients over a period of 3 years. Patients were rarely admitted by fully qualified clinicians and predominantly by preregistration medical officer interns (MOI, 46%) and diploma level clinical officer interns (COI, 41%) with a median of 28 MOI (range 11–68) and 52 COI (range 5–160) offering care per study hospital. The cPAQC scores suggest that perfect guideline adherence is found in ≤12% of children with malaria, pneumonia or diarrhoea with dehydration. MOIs were more adherent to guidelines than COI (adjusted OR 1.19 (95% CI 1.07 to 1.34)) but multimorbidity was significantly associated with lower guideline adherence. Conclusion: Over 85% of admissions to hospitals in high mortality settings that offer experiential training in Kenya are conducted by preregistration clinicians. Clinical assessment is good but classifying severity of illness in accordance with guideline recommendations is a challenge. Adherence by MOI with 6 years’ training is better than COI with 3 years’ training, performance does not seem to improve during their 3 months of paediatric rotations

    In 1989 Oxfam approached the town council of N'Djamena, the capital of Chad, Africa, and set up a joint project to encourage community participation in improving sanitary conditions in Ambassatna. The author highlights the main problems and lessons to be learned from the project, three years into it, including issues around neglecting the role of women and their involvement in local committee structures. The nature of voluntary work in an urban environment is discussed in relation to the impact of the project, which is felt by the community to be a success but is a limited success by Oxfam's criteria

    The purpose of this thesis was to find the online shopping experience and factors driving the young consumers towards online shopping in Kathmandu, Nepal, through which, exclusive scenario of the online shopping status among the young consumer was derived, which helped the existing online companies to know their strengths and weaknesses. In addition, the influence of foreign companies over the domestic companies was also surveyed. The research also attempted to aspire young entrepreneur including the author himself to kick start their entrepreneurial journey in the field of ecommerce in Kathmandu, Nepal. The empirical part of the thesis was conducted via quantitative research method i.e., online survey. The data collection was implemented via survey questionnaires. Website of Free online survey was used to collect the data and the obtained data was derived in Microsoft Excel format for the easy interpretation of the data. The research revealed that the existing online shoppers in Kathmandu are price sen-sitive. Customers are expecting the product to be precisely as descripted in the web-site along with smooth customer and after sale service. Opinions from friends and families and the company’s rating directly influence their buying behavior. Flexible payment systems are also highly expected by the customers. Also, people are in-clined towards domestic companies rather than foreign companies when it comes to buying a product from online companies. Besides, the major opportunities to start online entrepreneurial journey with possible future trends were also identified to invest and thrive in the field of ecommerce in Nepal
