    TASK SHIFTING: A NEED FOR CURRENT HEALTH CARE SYSTEM

    World is facing a crisis in human health resources due to a critical shortage of health workers. Despite of many trained health care professionals our health care system is struggling to provide optimum services to the patients. Gaps still exist with respect to patient care, which are unevenly delivered. The data collected from 228 subjects reveals that only 7.01% of the subjects have received counseling always, 59.65% think that counseling should be given and only 41.66% are satisfied with current health care system while 56.57% are not satisfied. Our study shows that there is huge burden on health care professionals due to high number of patient load which leads to the hindrance in optimum patient care and finally lead to the degradation in the quality of health care services. Now time has approached when the term task shifting should be taken seriously especially in the health care sectors. Pharmacists, an ignored profession in health care sector have crucial role for optimum patient care. Its demand of the scenario that pharmacist should be taken as a key role player between the patient and prescriber. A proper communication between the patient and prescriber can be established only if all health care professionals will start working to assist each other not to compete with each other

    An Overview of Drugs Used in COVID-19: A Pharmacotherapeutic Approach

    Coronavirus originated pandemic disease also called Corona Virus Disease 2019 (COVID-19) is spread all over the world causing severe acute respiratory syndrome (SARS) called SARS-CoV-2 poses a difficult challenge to scientists, researchers, and practitioners to discover effective drugs for prevention and treatment. By using a huge amount of clinical data obtained from many SARS-CoV2 infected people, clinicians are trying to gather accurate evidence for effective treatment and also developing a suitable vaccine system for the prevention of spread of infection for many more people. With no proven therapies which can treat and prevent SARS-CoV-2 is developed until now, there is an opportunity for new researchers in virology to make such an attempt at this crucial time. In this regard, currently, two strategies are active. The first kind of strategy is on developing completely new molecules to prevent and treat this disease, or the second strategy is on testing the effectiveness of already available antivirals and antimalarials for possible potential recovery and prevention. This is done by testing several antivirals (Remdesivir, Favipiravir, etc) and antimalarials (Chloroquine, Hydroxychloroquine, etc) for their potential therapies. Studies show that the most promising therapy is the use of antiviral Remdesivir. Remdesivir has shown the potential ability to exhibit vitro activity to control COVID-19. The drug is currently being tested by ongoing randomized trials. Until a widely accepted drug reaches the global market, different antiviral treatment strategies are used under urgent investigation. In this article, we review the latest research developments related to the systematic treatments for COVID-19 reported from various research labs of different countries. The article also provides a summary of various clinical research experience, intermediate results, and treatment guidance to combat the novel coronavirus epidemic based on pharmacotherapeutic analysis, along with insights to the attempts on vaccine development across the world in order to curb the COVID pandemic

    New drug research is considered as a very important research area and probably found more investments of resources and time during the last few centuries. Drug research also contributed to the progress of medical sciences substantially and improving characteristics of drugs in terms of various predicted properties is continued as a challenge and agenda of these researches. Though the objective of drug research is continuous improvements of existing drugs in terms of their curing ability and to discover new drugs which have substantial improved abilities to cure many existing and new diseases, the final goal of drug research is directed to identifying one drug which has the ability to cure all diseases. Such drug system which can cure any diseases instantaneously is called ideal drug or ideal medicine. Based on the exact property expectations from a drug system as a medicine to cure all diseases is still unknown and a systematic study on the goal of ideal drug discovery is yet to be established. In this paper, we have made an attempt to identify the characteristics of an ideal drug which can cure all diseases in the universe. Further, we discussed the challenges and opportunities to realize such an ideal drug which can cure all the diseases. We also analysed the possibility of using nanotechnology which is emerging as universal technology to realize the possibility of developing ideal drug in practice. The advantages, benefits, constraints, and disadvantages of using nanotechnology in drug research with the goal of improving them towards an ideal drug system are also listed. Finally, the comparison is made between conventional drugs and nanotechnology based drugs in terms of Ideal System Characteristics. This paper is on futuristic research along with new knowledge creation and analysis based new interpretation of nanotechnology in medicinal drugs

    Organizations and individuals usually have well defined objectives at any given point of time and invest their resources systematically to fullfill the objectives by setting their goal and formulating and following a strategy to chase their goal. Various strategies are used to chase the goal in both organizational and individual cases include competitive strategies or red ocean strategies, monopoly or blue ocean strategies, sustainability or green ocean strategies, survival or black ocean strategies, and mixed or white ocean strategies. The complexities of business decisions after globalization of business and technification of business processes, winning or sustaining or even surviving in current business is considered as an intensive challenge for organizations and also to individuals. In such environment called the turbulent business or social environment for organizations and individuals respectively, a new strategy for survival called “Alternative Strategy” is proposed in this paper and the concept of the strategy, its importance in the current business environment for organizational decisions are discussed. The paper also looks into the understanding of the Generic strategies, their applicability and constraints while identifying the importance of alternative strategy. Eight postulates are developed to support the concept of alternative strategy, and an alternative strategy model is suggested using lateral thinking techniques. The model consists of evaluating and comparing the alternative strategy with primal strategy using ABCD analysis framework. Such model of alternative strategy can be used at operational level, tactical level, and strategic level of any organization to realize its objectives

    There are many types of strategies to be followed by managers and leaders depending on the type of problems, objectives, circumstances, privileges, preferences, advantages, constraints and opportunities. Alternative strategy and survival strategy are two important strategies which are to be used in the strategic management of organizations depending on the constraints in availability of required resources and the intensity of problems in hand. These strategies can be effectively used in the healthcare sector to manage the acute shortage of professionally qualified healthcare workers in developing countries.Despite many trained healthcare professionals our healthcare system is struggling to provide optimum services to the patients. This shortage is further increased by a high burden of infectious diseases, emigration of trained professionals, difficult working conditions, and low motivation by country governments. Even though, the permanent solution to this problem is to create more professional doctors by increasing training courses, training facilities through modifying government policies and enhancing resources to do so, such actions and resultant solutions takea long time to realize in practice. Hence a quick optimum solution is needed for alternative survival to address the critical shortage of trained healthcare professionals. Out of many alternatives, the best alternative for healthcare executives in any hospital or primary health centre is the strategic decision of thinking and implementing analternative survival management strategy called the strategy of task-shifting. Task-shifting and sharing is amanagement strategy in which the clinical care responsibilities are transferred from more specialized professionals to less specialized health workers or to parallel area professionals through proper delegation. In this paper,we have discussed the concept of Task-shifting in the healthcare sector as an alternative survival strategy used in strategic management subject. By means of reviewing the papers published in the concept of Task-shifting in the healthcare sectors in different countries between 2008 to 2017, and studying systematic review papers published during 2012 to 2017, we have analysed this strategy and concluded that it is acceptable and working as an alternative survival strategy in many countries. We have also discussed a basic model of Task-shifting by taking all stakeholders into account, skills required for Task-shifting, possible Task-shifting methods among different professionals in healthcare management, and other alternative solutions to solve the scarcity of qualified medical professionals. The opportunities and challenges of this alternative survival strategy are also discussed. Finally, based on the analysis of various systematic reviews on Task-shiftingsome suggestions are proposed while using Task-shifting as an alternative survival strategy

    Users Perspectives on Online Pharmacy Model

    The advents in technology through continuous innovations have supported to improve the quality of the human life in the society. One of the innovation due to the invention of ICT based online/mobile high-speed internet is e-business which allowed the development of online shopping/purchase of both intangible and tangible commodities online. The major advantage and benefit of this online model is its ubiquity, convenience, and low cost to the customer. The online shopping/purchase model is now becoming popular especially among youngsters. One variation of this e-purchase model is online pharmacy model where a consumer can purchase the prescribed drug online to take some of the advantages of this model. In this paper, we have studied the customer’s attitude, views, and concern on this new channel of drug distribution using the data collected by online questionnaire from the general population. The data obtained from this survey is analysed, assessed and interpreted. Based on information collected and focused group study, the online pharmacy model is analysed using ABCD analysis framework. The various issues related to online pharmacy, affecting factors are studies under ABCD constructs using ABCD listing methodology. Based on the outcome of the study, many recommendations are suggested for developing proper controlling mechanism/regulations for drug control authorities of the country to avoid misuse of the online channel for the drug purchas

    ABCD Analysis of Task Shifting – An optimum Alternative Solution to Professional Healthcare Personnel Shortage

    Task shifting is an optimum alternate solution in the healthcare system to address the acute shortage of professionally qualified healthcare workers in developing countries. This will temporarily help in solving the big problem of providing quality and timely health services to the needy people. Despite many trained health care professionals, our health care system is struggling to provide optimum services to the patients due to many reasons including the high burden of infectious diseases, emigration of trained professionals, difficult working conditions, and low motivation by country governments. It is logically argued and in many places practically proven that out of many alternatives, the best alternative for healthcare executives in any hospital of primary health centres is the strategic decision of thinking and implementing the so called strategy of taskshifting. In this paper, we have analysed the process of task-shifting by considering its advantages, benefits, constraints, and disadvantages using an analysing technique called ABCD analysing framework. As per the ABCD framework, the various determinant issues of related to the task shifting for organizational health service performance are identified using focus group method are : (1) Organizational Issue, (2) Alternative Acceptors Issue, (3) Donor Physicians Issue, (4) Patients and Relatives Issue (5) Societal Issue, and (6) Country Issue. Each determinant issue has sub-issues called key attributes used for analyzing the advantages, benefits, constraints, and disadvantages which are the four major constructs of the framework. The factors affecting the various determinant issues of for task shifting for each key issues under four constructs are derived and 172 critical constituent elements (CCE) of these factors are listed under the ABCD constructs

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
