    Research priorities about stoma-related quality of life from the perspective of people with a stoma: A pilot survey

    Background: There is a recognized need to include patients in setting research priorities. Research priorities identified by people with a stoma are rarely elicited.  Objectives: To improve the quality of life of people with a stoma through use of evidence-based practice based on research priorities set by patients.  Design and Methods: Online pilot survey publicized in 2016 via United Kingdom stoma charities. People ranked nine stoma-related quality-of- life topics in order of research priority.  Participants: People 16 years of age and over who currently have or have had a stoma for treatment for any medical condition.  Analysis: Distributions of the priority scores for each of the nine research topics were examined. Group differences were explored using either the Mann–Whitney U-test or the Kruskal–Wallis test depending on the number of groups.  Results: In total, 225 people completed the survey. The most important research priority was pouch leak problems and stoma bag/appliance problems followed by hernia risk. There were statistically significant differences in ranking research priorities between males and females, age, underlying disease that led to a stoma, stoma type and length of time with a stoma.  Conclusion: People with a stoma are willing to engage in and set research priorities. The results should contribute towards future research about setting the research agenda for the study of stoma-related concerns that impact quality of life

    The article concerns the eighteen-year-old sculptor Wiktor Jan Brzeziński from Żyrardów in Mazowieckie voivodeship. Wiktor has been a sculptor since he was 11 years old and has a raw talent. His sculptures depict what he can observe in the street – residents of block of flats, people standing by the shops, subculture of tracksuit wearers, sportsmen as well as pensioners, old women and regional groups such as Highlanders, Kashubians or people of Łowicz. Most of his sculptures were made to be further “brought to life” in animations which are made in his amateur film studio – his own bedroom. The project @ekipa_z_pod_nocnego is being prepared now and it can be seen on YT and TikTok. The discovery of Wiktor’s art show a new dimension of amateur art.W ramach artykułu zaprezentowano sylwetkę osiemnastoletniego rzeźbiarza Wiktora Jana Brzezińskiego, który pochodzi z Żyrardowa w woj. mazowieckim. Wiktor rzeźbi od 11 roku życia i jest obdarzony talentem samorodnym. W swojej rzeźbie przedstawia to, co współcześnie obserwuje na ulicy – mieszkańców blokowisk, ludzi spod sklepów, dresiarzy, sportowców, ale też emerytów, babki, czy grupy regionalne, jak Górale, Kaszubi i np. Łowiczanie. Większość swoich rzeźb wykonał po to, aby je „ożywić” w animacjach, które nagrywa w zaaranżowanym w swoim pokoju amatorskim studiu filmowym. Tam powstaje projekt @ekipa_z_pod_nocnego, który można zobaczyć na kanałach YT i TikTok. W etiudach tych Wiktor prezentuje swoje rzeźby, które odgrywają konkretne scenki, mają imiona i możliwość wypowiedzi. Wiktor wszystkim podkłada głos odpowiednio go modelując w zależności od postaci.     In Poland, most of the existing information on transgender has been heavily influenced by the pathologizing, medicalizing discourses of the 1980s and early 1990s, and deeply rooted in the essentialist per­ception of gender. In contrast, under the influence of queer theory and social constructionism, Polish social studies re-discovered the theory of transgender in the late 2000s. Combining these two competing viewpoints and discourses has shaped and determined that which currently constitutes transgender studies as they are gradually emerging in Poland. The article aims to explore these alternative approaches, including the discourse prevalent in the Polish medical community at present, the accompanying gatekeeping practices that it conse­quently employs (even though WHO no longer categorizes transgender as a disorder), and how it is perceived in the field of social sciences. Next, this article will present an analysis of the broader social perceptions of trans individuals in Poland. The authors will conclude with a number of varying perspectives from transgender persons. Based on these particular analyses, the article will argue that the existing Polish gatekeeping system not only makes transgender people dependent on diagnostic outcomes, but also promotes a specific brand of experience policing among trans communities, in which people are often labeled as being disordered.Większość polskiej wiedzy na temat transpłciowości powstała pod wpływem patologizujących i medykalizujących dyskursów lat osiemdziesiątych i dziewięćdziesiątych ubiegłego wieku, mając swoje źródło w esencjalnym rozumieniu płci. Dla porównania polskie nauki społeczne – zainspirowane teorią queer i konstrukcjonizmem społecznym – „odkryły” transpłciowść pod koniec pierwszej dekady XXI wieku. Połączenie tych dwóch konkurujących ze sobą perspektyw i dyskursów ukształtowało i ustaliło to, co dziś składa się na stopniowo wyłaniające się polskie studia nad transpłciowością. Celem niniejszego artykułu jest przyjrzenie się tym różnym podejściom. Przedstawiony jest dyskurs obecny w polskim środowisku medycznym, wraz towarzyszącymi mu praktykami gatekeepingu (istniejacymi pomimo zaprzestania klasyfikacji transpłciowości jako zaburzenia przez Światową Organizację Zdrowia). Zaprezentowane jest też stanowisko rozpowszechnione w obrębie nauk społecznych. Ponadto w artykule znajduje się analiza szerszej percepcji społecznej osób transpłciowych w Polsce. Na zakończenie zaprezentowane się różnorodne perspektywy samych osób transpłciowych. W oparciu o te analizy autorka sugeruje, że istniejące obecnie w Polsce procedury gatekeepingu nie tylko uzależniają osoby transpłciowe od wyniku procesu diagnostycznego, lecz także promują swoistą formę kontroli doświadczeń wewnątrz transpłciowych społeczności, które często nadają osobom miano zaburzonych

A better understanding of these behaviours following stoma formation surgery and their experiences and attitudes towards receiving lifestyle advice, could help identify specific gaps and inform interventions going forward. The aim of this study was to describe changes in current lifestyle following stoma formation and to explore concerns, desire for lifestyle information, advice and support among people who have or have had a stoma. Methods: A sample of adults who currently had or in the past had a stoma for treatment for any medical condition was recruited online through relevant charities and companies, and invited to complete a cross-sectional, online survey. Consenting participants (n = 425) provided demographic information and completed brief, validated questionnaires about their lifestyle, alongside questions around their concerns regarding permanent stoma and experiences of lifestyle information and advice. Responses were summarised using descriptive statistics, and associations between reported concerns about stoma and changes in health behaviours were explored. Results: Most respondents (93%) still had a stoma at the time of completing the survey. The majority (80%) had not consumed at least 5 portions of fruit and vegetables on the previous day and 20% reported they had not participated in at least 30 min of physical activity on any day in the previous week. Most respondents were non-smokers (84%) and did not exceed recommendations for alcohol intake (60%). Most (56%) felt their PA had decreased following stoma formation. Frequencies of concerns about a permanent stoma were high, and appeared to be associated with reported decreases in PA. Of those reporting nausea, 40% felt their diet had worsened since having their stoma. A large proportion of respondents had not received PA (42%) or dietary (30%) advice, and of these > 90% would have liked guidance. Conclusions: Few respondents to this survey were eating the recommended amount of fruit and vegetables, and most reported a decrease in their PA following stoma surgery. Lifestyle advice would be welcomed by this population, which professionals should take into account when addressing stoma- related concerns.Peer reviewe

    Bakalářská práce se zabývá kvalitou života pacientů se střevní stomií. Cílem práce bylo analyzovat a posoudit subjektivní hodnocení úrovně kvality života se střevní stomií a do jaké míry stomie ovlivňuje kvalitu života. Práce je rozdělena na teoretickou a praktickou část. Teoretická část je rozdělena na tři kapitoly, které jsou zaměřeny na kvalitu života, střevní stomie a život pacienta se střevní stomií. Praktická část je zpracována formou kvantitativního dotazníkové šetření pomocí generického dotazníku WHOQOL-BREF, který byl doplněn o položky vlastní konstrukce, zaměřující se na vliv proměnných, dobu založení stomie a typ stomie, a specifického dotazníku Stoma-Qol. Výzkumného šetření se zúčastnilo 142 respondentů se střevní stomií. Z výzkumu vyplynulo, že kvalita života tázaných respondentů je na nízké úrovni ve dvou oblastech. Výstup výzkumu práce poukazuje na řešení jejich problematiky.The bachelor thesis deals with the quality of life of patients with intestinal stoma. The aim of the thesis was to analyze and assess the subjective assessment of the level of quality of life with intestinal stoma and to what extent the stoma affects the quality of life. The theoretical part is divided into theoretical and practical part. the theoretical part is divided into three chapters which focus on quality of life, intestinal stoma, and life of the patient with intestinal stoma. The practical part is developed by a quantitative questionnaire survey method using the generic WHOQOL-BREF questionnaire, which was supplemented with items of its own design focusing on the influence of variables, time of stoma establishment and type of stoma, and the specific Stoma-Qol questionnaire. A total of 142 respondents with intestinal stoma participated in the survey. The research showed that the quality of life of the respondents was low in two areas.     Stoma reversal after emergency stoma formation – the importance of timing. A multi-centre retrospective cohort study

    Background: Restoration of intestinal continuity is a key consideration for patients having a stoma created under emergency conditions. There is contrasting evidence about the outcomes of stoma reversal for these patients. This research aims to describe the post-operative outcomes of stoma reversal after emergency formation, and whether these are affected by the timing of reversal. Methods: A retrospective review of a prospectively maintained emergency laparotomy (EmLap) database for 4 hospitals was performed between 2018 and 2021. Adult patients undergoing emergency stoma formation were identified and followed up until 2024. Those undergoing stoma reversal surgery were included in the final analysis. A Cox proportional-hazards model was created to identify factors associated with increased time to reversal. Results: 1775 patients had an EmLap, with 505 (28.5%) having a stoma created. Of those patients with a stoma, 97 patients (19.2%) died within one year post-operatively. 146 (28.9%) of the emergency stoma patients underwent stoma reversal, with median time to reversal of 16.9 months. Median post-operative length of stay was 7 days, and 52.1% of patients sustained complications within 30 days post-operatively. Patients reversed within 18 months of stoma formation had fewer significant complications (7.9% v 35.1%, p < 0.001), a shorter length of stay (6 days v 7 days, p < 0.001), and reduced post-operative ileus rates (21.3% v 64.9%, p < 0.001) than those reversed after this period. Receiving adjuvant therapy for malignancy (adjusted Hazard ratio 0.36, 0.17–0.78, p = 0.001) and being male (adjusted Hazard ratio 0.69, 0.49–0.97, p = 0.032) were significantly associated with increased time to reversal. Conclusion: Emergency stoma formation is commonly performed during EmLap, but the majority of emergency stomas are never reversed. The complication profile for reversing these stomas is significant, but early reversal is associated with better post-operative outcomes. Standards of care for emergency stoma patients would be welcome in order to improve outcomes for this cohort

    Wiktor Grygorenko was born on March 6, 1927. He passed his final high school exam in Katowice in 1949 and after a military training he began his studies at the Military School of Topographers. Then he studied geodesy at the Military Engineering Academy in Moscow. Having returned to Warsaw in 1959, he worked in the Military Cartographic Works and later in the Topographic Board of the General Staff of the Polish Army. Simultaneously, he took a lecturer post at the Military Technical Academy in Warsaw where he elaborated student textbooks on maps editing, cartographic reproduction, and mathematical cartography. His comprehensive manual Elaboration of general geographical maps was published in 1970. He was a member of the editorial staff at the popular World Atlas, published in 1962-1968 and also known under the name of Pergamon World Atlas. A new and innovative projection devised by Wiktor Grygorenko was used in the World Atlas, nowadays known as the projection of the Polish Army Topographical Service. In 1971 he obtained the degree of a Doctor of Technical Sciences in cartography at the Warsaw University of Technology. The subject of his thesis were theoretical fundamentals of the automatic cartographic data processing. Wiktor Grygorenko was one of the Polish pioneers of computer usage in cartography and was the author of many valuable papers on the subject. He began to work at University of Warsaw in 1972, In 1978 he completed his habilitation procedures on the basis of his dissertation entitled Quantitative Parameters of the Cartographic Content Composition in which he formulated the idea of the quantitative model of a map, thus drawing a new direction of theoretical and practical works in cartographic design and evaluation. Many papers on cartographic generalisation with usage of computer methods were created under his guidance. In 1980, three years after the unexpected death of professor Lech Ratajski, Wiktor Grygorenko became the Head of the Chair of Cartography at the Faculty of Geography and Regional Studies. In 1981 he became the Vice-dean for student affairs and served two terms. His first term fell in the difficult period of the martial law when he defended students on many occasions, gaining their gratitude and respect. In autumn 1982 he also became the Head of the Department of Cartography at the Maria Curie-Skłodowska University in Lublin, which he administered until 1992. Making a good use of his broad didactic experience, he published several absorbing articles on educating cartographers with the geographic training. He actively participated in conferences, conventions, and cartographic trainings, animating discussions with his inspiring speeches. In summer 1982, during the 11th conference of the International Cartographic Association in Warsaw he presented the paper Cybernetic model of cartographic communication that was in certain degree a continuation and an explication of professor Lech Ratajski's idea of communication model. The main field of professor Grygorenko's (he obtained the professorship in 1992) scientific interest were theoretical issues related to the automation of the maps editing and usage processes. His interest in the history of cartography resulted in the publication of several interesting papers on the subject and participation in meetings of cartography historians. He served many social functions both at the University of Warsaw and among the geodesists and cartographers. Professor Wiktor Grygorenko died on February 25, 2008
