Democratic evaluation of clinical clerkship in a medical school. A case study
We report a case study of democratic evaluation of a clinical clerkship for medical students. We used a mixed model combining interviews and questionnaires to students, teachers and nurses involved in 9 weekly clerkship rotations in 7 hospital wards. The main outcome variable in quantitative analysis was the score of perceived usefulness for professional development of 42 Observable Practice Activities (OPAs) that each student should perform during clerkship. Scores were higher when the OPA was performed more consistently, was performed in small groups, and when the teaching and the evaluation methods were perceived as valid. The overall satisfaction for the program of teacher was high and did not correlate with students’ perceptions. The burden for patients was perceived as slightly higher in nurses, and did not correlate with teachers’ perceptions. When returned to students, teachers and nurses, these data could contribute to improve the program

Temas contemporâneos na formação docente a distância: uma introdução à teoria do caos

Programa de Pós-Graduação em Educação Científica e TecnológicaNeste trabalho realizamos um estudo de caso envolvendo a elaboração e aplicação de um mini-curso na modalidade a distância sobre o tema Caos em Sistemas Dinâmicos para a formação de professores, baseada na educação problematizadora de Paulo Freire. O processo de elaboração do mini-curso foi inspirado no conceito de investigação temática, de Freire, e o método de execução foi o dos três momentos pedagógicos, de Delizoicov e Angotti. Utilizamos o ambiente virtual Moodle como mediação tecnológica de educação a distância. Concluímos que a inserção de temas contemporâneos na formação de professores mediada por tecnologia pode ter caráter problematizador. Para tanto, depende da colaboração entre especialistas da área específica e pedagógica na escolha do conteúdo científico, da participação dos professores ou futuros professores na adequação do material didático e de um ambiente virtual para possibilitar a interação entre os participantes, flexibilizar o processo de avaliação, o espaço e o tempo de aprendizagem. In this work, we completed a case study by elaborating and applying a mini-course in distance education mode about Chaos in Dynamic Systems for teacher training, based on the problem-posing education, by Paulo Freire. The mini-course elaboration process was inspired on thematic investigation concept, by Freire, and the execution method was that of three pedagogical moments, by Delizoicov and Angotti. We employed Moodle virtual environment as technological mediation for distance education. We concluded that inserting contemporaneous topics into teacher training by means of technology could have problem-posing characteristics. For this, it depends on the collaboration between specialists from pedagogical and specific area when choosing scientific contents, as well as on the teachers or future teachers' participation in adapting the didactic material and on having a virtual environment for enabling the interaction between participants and adjust the evaluation process, the space and the learning time
Investigando a telefonia celular: ensinando-aprendendo com a interatividade em uma abordagem temática no ensino de física

Analisa-se como se dá o "pensar complexo" nesta atividade educacional. No hipertexto são desvelados os aspectos técnicos do princípio de funcionamento e fabricação da telefonia celular, assim como algumas relações entre Ciência, Tecnologia e Sociedade. Aponta-se para a inércia que há em promover rupturas de uma concepção bancária de educação em favor de uma concepção dialógico-problematizadora, isto é, uma concepção consciente de educação
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Persistent nonbilious vomiting in a child: Possible duodenal webbing
An association between malrotation and congenital duodenal webbing is rare. We present our experience with four patients at two centers, and a review of published reports. There are currently 94 reported cases of duodenal pathology associated with malrotation. However, only 15 of the 94 cases (15.9%) include patients with malrotation and a duodenal web. We suggest that nonbilious vomiting in a child must prompt the surgeon to consider duodenal pathology even in the presence of malrotation

Mitomycin C modulation of corneal wound healing after photorefractive keratectomy in highly myopic eyes.

Abstract
PURPOSE: To evaluate the role of topical mitomycin C in corneal wound healing (CWH) after photorefractive keratectomy (PRK) in highly myopic eyes.
DESIGN: Prospective, double-masked, randomized clinical trial.
PARTICIPANTS: Seventy-two eyes of 36 patients affected by high (>7 diopters) myopia.
METHODS: In each patient, one eye was randomly assigned to PRK with intraoperative topical 0.02% mitomycin C application, and the fellow eye was treated with a placebo. Postoperatively, mitomycin C-treated eyes received artificial tears (3 times daily, tapered in 3 months), whereas the fellow eye was treated with fluorometholone sodium 2% and artificial tears (3 times daily, tapered in 3 months).
MAIN OUTCOME MEASURES: Uncorrected visual acuity (UCVA) and best-corrected visual acuity (BCVA), contrast sensitivity, manifest refraction, and biomicroscopy. Contrast sensitivity was determined using the Pelli-Robson chart. Corneal confocal microscopy documented CWH.
RESULTS: Mean follow-up was 18 months (range, 12-36). No side effects or toxic effects were documented. At 12-month follow-up examination, UCVAs (logarithm of the minimum angle of resolution) were 0.4+/-0.48 and 0.5+/-0.53 (P = .03) in mitomycin C-treated eyes and corticosteroid-treated eyes, respectively. At 1 year, corneal haze developed in 20% of corticosteroid-treated eyes, versus 0% of mitomycin C-treated eyes. At 12, 24, and 36 months, corneal confocal microscopy showed activated keratocytes and extracellular matrix significantly more evident in untreated eyes (Ps = 0.004, 0.024, and 0.046, respectively).
CONCLUSION: Topical intraoperative application of 0.02% mitomycin C can reduce haze formation in highly myopic eyes undergoing PRK.

Robot-assisted Heller myotomy for achalasia
Achalasia is a rare neuromuscular esophageal disorder in children. There are many surgical options to treatment including botulinum toxin (Botox) injections, oral pharmacologic therapies with nitrates and calcium channel blockers, pneumatic dilation (PD), and surgical myotomy (open surgery, endoscopy, laparoscopy and recently robotic approach). In pediatric age, usually, Heller's myotomy is the main choice. Laparoscopic approach is known and standardized. Few robotic have been published. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
