Kuznetsova Odessa National Medical University Abstract The authors try to decide a problem of ovarian cancer resistance to platinum drugs by the way of correlation finding between platinum-resistance of tumor and presence of gene mutations in the patient. It was shown a variety of options for BRCA gene mutations in patients with ovarian cancer: BRCA 1 (185delAG) - 64,2%, BRCA 1 (5382 insC) - 55,7%, and BRCA 2 (6174delT) - 53,8%. Authors discovered a significant positive correlation between carriage of mutations in the BRCA genes 1/2 and the sensitivity of malignant ovarian tumors to chemotherapy with platinum. Mutations in these genes occurred significantly more often in patients with platinum-sensitive ovarian cancer.     QUALITY OF LIFE OF PATIENTS WITH OVARIAN CANCER OF THE THIRD STAGE USING DIFFERENTIATED TREATMENT

    The aim of this research was to assess the quality of life in patients with stage III ovarian cancer during the treatment period, depending on the applied therapeutic approaches. A comparative analysis of the results of treatment of 350 patients with adenocarcinoma of the ovaries of III-IV stage, which have undergone suboptimal or non-optimal cytomelective surgery, was conducted. To assess the quality of life and to examine the relationship between changes in physical health and quality of life, patients in both groups were asked to complete the questionnaires EORTC QLQ-C30 and SF-36. According to the questionnaire of the EORTC QLQ-C30, the indicators of physical activity, the ability to do everyday affairs, were higher in the second group of patients who received differentiated treatment. The indicator of "social functioning" was also higher in the II group of patients. When evaluating the patients' liver on the scale of the questionnaire SF-36, it was found that the positive effect of differentiated therapy is maintained for 3 years. Life quality is an important criterion for evaluating the results of antitumor therapy, and when it is continuously monitored in patients with ovarian cancer the safety of treatment for patients could be improved, including at an advanced stage</jats:p

Рыбин   Одесский национальный медицинский университет   Реферат             Автор, на основании анализа результатов исследования гомеостатистических механизмов (детоксикационная функция печени, цикл оксида азота; обмен серусодержащих аминокислот) у 121 больной раком яичника и разной чувствительностью к препаратам платины,  приходит к заключению, что реализация генетически определенной чувствительности рака яичника к препаратам платины связана с этими механизмами. Автор полагает, что активность цикла оксида азота влияет на уровень эндогенной интоксикации и устойчивость организма к внешним воздействиям. Уровень содержания SS-SH групп и их связь с белковыми или липопротеидными комплексами, очевидно, определяет уровень сульфид-платиновых комплексов, а состояние детоксикационной системы, возможно, определяет жизни таких комплексов и эффективность лечения, соответственно.               Ключевые слова: рак яичника, гомеостатические механизмы; SS-SH группы, оксид азота.         ROLE hOMEOSTATISTIC MECHANISMS IN THE FORMATION OF PERFORMANCE PLATINUM TREATMENT OF PATIENTS WITH OVARIAN CANCER     A. Rybin   Odessa National Medical University Abstract The author, based on the analysis results of the study gomeostatisticheskih mechanisms (liver detoxification function, nitric oxide cycle, exchange of sulfur-containing amino acids) in 121 patients with ovarian cancer and different sensitivities to drugs of platinum, comes to the conclusion that the implementation of a genetically determined sensitivity of ovarian cancer to the drugs of platinum associated with these mechanisms. The author believes that the activity of nitric oxide cycle affect the level of endogenous intoxication and the body's resistance to external influences. The level of SS-SH groups content and their relation to protein or lipoprotein complexes, obviously determines the level of sulphide-platinum complexes, and the state of detoxification system may determine the lifetime of such systems and the effectiveness of treatment, respectively.   Keywords: ovarian cancer, homeostatic mechanisms; SS-SH group, the nitrogen oxide

    THE EXPERIENCE AND RESULTS OF CYTOREDUCTIVE SURGERY AND HIPEC USED IN ADVANCED OVARIAN CANCER

    The research object&nbsp;was life quality of patients with ovarian cancer after complex treatment. Nowadays cytoreductive surgery and HIPEC is the mainstay of management of advanced stages ovarian cancer. Description of the problem:&nbsp;study was conducted to assess the effectiveness of combined treatment in IIIA-IIIC ovarian cancer, its impact on quality of life. Main scientific results:&nbsp;37 patients of main group (CRS + HIPEC) were compared with 25 patients of control group (surgery + systemic chemotherapy). The quality of life was assessed with Medical Outcomes Study 36-Item Short Form Health Survey&nbsp;(SF-36). Comparative analysis of quality of life 6 months after treatment completion did not show significant statistical difference. Combination of cytoreduction with HIPEC improves quality of life in patients with ovarian cancer, is tolerated better and has less systemic toxicities than systemic chemotherapy. The area of practical use of research results:&nbsp;hyperthermic intraperitoneal chemotherapy is performed for patients suffering from malignant tumors of the abdominal cavity, including metastatic colon cancer, ovarian cancer, stomach cancer, pseudomyxoma, mesothelioma, rhabdomyosarcoma, and some other types of oncology. Innovative technological product:&nbsp;combination of cytoreduction with HIPEC improves quality of life in patients with ovarian cancer. Scope of application of an innovative technological product:&nbsp;HIPEC is an innovative treatment option for advanced cancer that has spread to the abdominal cavity (carcinomatosis) and has not gone beyond it

    Epidemiology of papillomavirus infection in Ukraine

    The aim of this study was to assess the prevalence of HPV infection among women in different regions of Ukraine. Materials and methods. An analysis of data from the Quintip Recordati database, containing 195 records, was performed using frequency analysis methods. The distribution of different HPV strains among the women surveyed, who live in different regions, was determined. An analysis of the frequency of seropositive results depending on the degree of urbanisation was performed. The main ways of preventing HPV infection were identified. Results. It was found that the number of positive tests accounted for 38.5% of the total sample. When comparing the frequency of detection of high-oncogenic strains, it was found that they are more often registered in cities with larger populations. Conclusions: 1. The prevalence of HPV in the women examined is twice the European average. 2. The frequency of detection of highly oncogenic HPV strains is higher in urbanised regions. 3. Widespread implementation of HPV screening methods in Ukraine could significantly reduce the incidence of cervical cance

    When endometrioma becomes malignant: a clinical case and conclusions for oncogynecological practice

    Objective. To evaluate the clinical and morphological features of malignant transformation of ovarian endometrioma and to emphasise the importance of oncological vigilance in the management of patients with endometriosis. Materials and methods. A clinical case of early endometrioid carcinoma of the ovary against the background of endometrioma in a woman of reproductive age is presented. An analysis of current literature data and ESGO, ESHRE, and JSGO recommendations for the diagnosis and treatment of such patients was performed. Results. Morphological and radiological signs of increased oncological risk were identified, which became the basis for timely surgical intervention. Endometrioid carcinoma G1, FIGO IA was histologically verified. Fertility-preserving staging surgery was performed. No recurrence was detected during 24 months of follow-up. Conclusions. Ovarian endometrioma can be a source of early gynaecological cancer processes even in young patients. Vigilance, high-quality imaging and adequate surgical tactics are key factors for successful treatment

Rybin** *National Cancer Institute, Kyiv **Odessa National Medical University, Odessa Abstract The aim of this study was to evaluate quality of life in patients with platinumresistant ovarian cancer, depending on the pharmacoresistance and differentiated use of chemotherapy. It is shown that the output value by the subscales of the questionnaires EORTC QLQ-C30 and FACT-G in patients assigned to different clinical groups were comparable. When using a differentiated approach in the treatment of patients with ovarian cancer significantly improved performance on physical scales (PF), role (RF) and emotional functioning (EF). In addition, patients from III and IV groups had decreased intensity of nausea (NV) and general weakness (FA). With differentiated application of chemotherapeutic agents with metabolic support total score by FACT-G questionnaire was in Group III 77,8±0,9 points, and in the IV group - 77,9±0,8 points, which was significantly higher than obtained in the first and group II - 72,2±1,2 and 71,6±0,9 points.     Medical education during the COVID-19 pandemic: problems and solutions

    Modern education, including medical, is currently experiencing unprecedented difficulties caused by the growing incidence of new coronavirus infection. The difficulties are primarily due to the fact that most educational institutions in Ukraine and around the world have switched to remote operation. Thus, educational organizations, and especially teachers, face an unfamiliar situation, and many do not know how to support students in these new conditions or to adhere to the curriculum and train students for coming exams. The situation is complicated by the fact that the peak incidence coincided with the end of the school year, the most difficult period for both universities and students and clinical residents. Many of them have graduated and have taken qualification exams, procedures of primary and primary-specialized accreditation. How in the current situation to continue teaching students effectively? How to help them develop new skills? How to apply the assessment and in what form? Is it possible to conduct examinations in the format of an objective structured clinical examination (OSCE)? How will medical education change after the restrictions are strengthened?
