An Econometric model for the evolution of the Romanian Interbank Bid Rate (ROBID) in the context of the international financial crisis
The paper presents the econometric modeling of overnight inter-banking interest rates (ROBID) in our country, the analyzed period is between 1999-2010. The international financial crises had a great impact on the level of inter-banking interest rates after 2007 and it reflects the new level of risk for the Romanian system banking. The econometric model used in modeling the interest rates is an autoregressive moving average (ARMA) model, the ARMA model is typically applied to time series data; the paper propose several ARMA models, applies econometric tests and based on them the analyzed series (the inter-banking interest rates) forecast will be made.ROBID, ARIMA model, financial crisis, forecast.
The ratio of circulating endothelin-1 to endothelin-3 associated with TIMI risk and dynamic TIMI risk score in ST elevation acute myocardial infarction
In ST segment elevation acute myocardial infarction (STEMI), the endothelin (ET) system imbalance, reflected by the circulating ET-1:ET-3 ratio has not been investigated. This study’s primary objective was to measure the circulating ET-1:ET-3 ratio and correlate it with the risk stratification for 1 year mortality of STEMI based on TIMI score. On admission, the TIMI risk score and at discharge, the dynamic TIMI risk score were calculated in 68 consecutive subjects with STEMI. Subjects with high TIMI risk score were associated with higher mean ET-1 level and ET-1:ET-3 ratio. The ET-1:ET-3 ratio more accurately predicted the high on admission TIMI risk score than the ET-1 level. Subjects with high dynamic TIMI risk score were associated with higher mean ET-1 level and ET-1:ET-3 ratio. The ET-1:ET-3 ratio more accurately predicted the high at discharge dynamic TIMI risk score than ET-1 level. From multivariable analysis, the ET-1:ET-3 ratio was not independently associated with high on admission TIMI risk score but independently predicted high at discharge dynamic TIMI risk score (odds ratio = 9.186, p = 0.018). In conclusion, combining the ET-1 and ET-3 levels into the ET-1:ET-3 ratio provided a prognostic value by independently predicting the increased risk to 1 year mortality as indicated by at discharge dynamic TIMI risk score in patients with STEMI.The presentation of the authors' names and (or) special characters in the title of the pdf file of the accepted manuscript may differ slightly from what is displayed on the item page. Újdonságok a dapagliflozinról: a DECLARE-TIMI 58 vizsgálat alcsoport-elemzései [Novel data about dapagliflozin: subgroup analyses from DECLARE-TIMI 58 randomised trial]
Az utóbbi években a nemzetközi ajánlásokban az életmód-terápia kiegészítéseként a metformin a 2-es típusú cukorbetegek első vonalbeli gyógyszeres kezelésének preferált készítményeként szerepel. A metforminkezelés kiegészítésére alkalmazott SGLT-2-gátlók a vesék proximális tubulusaiban gátolják a glukóz visszaszívódását, a következményes glucosuria a HbA1c , az éhomi és a postprandialis vércukorértéket egyaránt csökkenti, mindezt az endogén inzulintermeléstől függetlenül. Az SGLT-2-gátló dapagliflozinnal elvégzett, 2018-ban publikált, randomizált, kontrollált kardiovaszkuláris kimenetelt vizsgáló DECLARE-TIMI 58 tanulmány igazolta a dapagliflozin addigi antidiabetikus kezelést kiegészítő alkalmazásának – placebóval szemben – kardiovaszkuláris biztonságosságát, teljesült a non-inferioritás kritériuma. Egyúttal a dapagliflozin szignifikánsan csökkentette a szívelégtelenség miatti kórházi felvétel szükségességét, valamint jelentősen csökkentette az előre meghatározott összetett vesevégpontokat. A jelen közleményben a DECLARE vizsgálat négy különböző alcsoport-analízisének eredményeit tekinti át a szerző, nevezetesen a dapagliflozinnak a posztinfarktusos betegekre, az előzményükben szívelégtelenségben szenvedő betegekre, a diabeteses vesebetegség progressziójára, valamint a perifériás érbetegséggel társult, illetve attól mentes cukorbetegekre gyakorolt hatásait. | In recent years, according to international guidelines, metformin is the preferred initial glucose-lowering medication for most people with type 2 diabetes. SGLT-2 inhibitors reduce glucose reabsorption in the renal proximal tubules and increases renal glucose excretion via the urine leading to reduced HbA1c , fasting and postprandial plasma glucose levels, independently from endogenous insulin secretion. DECLARE-TIMI 58 study, published in 2018 was a cardiovascular outcome trial. This study demonstrated that dapagliflozin, when added to the standard of care treatment, was non-inferior to placebo with respect primary safety endpoint of major adverse cardiovascular events. However, dapagliflozin significantly reduced rate of hospitalization for heart failure. Furthermore, dapagliflozin administration markedly reduced prespecified composite renal outcome. In this review the author summarizes the data on four subgroup analyses of DECLARE-TIMI 58 study, namely patients with previous myocardial infarction, effect of dapagliflozin on heart failure depending on history of heart failure at baseline, and development and progression of diabetic kidney disease, and in patients with and without peripheral artery disease

Obesity and effects of dapagliflozin on cardiovascular and renal outcomes in patients with type 2 diabetes mellitus in the DECLARE-TIMI 58 trial

For permissions, please email: [email protected] : We investigated the associations between obesity, cardiorenal events, and benefits of dapagliflozin in patients with type 2 diabetes mellitus (T2DM). METHODS AND RESULTS : DECLARE-TIMI 58 randomized patients with T2DM and either atherosclerotic cardiovascular (CV) disease or multiple risk factors to dapagliflozin vs. placebo. Patients were stratified by body mass index (BMI, kg/m2): normal (18.5 to <25), overweight (25 to <30), moderately obese (30 to <35), severely obese (35 to <40), and very-severely obese (≥40). Outcomes analysed were CV death, hospitalization for heart failure (HHF), renal-specific composite outcome, and atrial fibrillation or flutter (AF/AFL). Of 17 134 patients, 9.0% had a normal BMI, 31.5% were overweight, 32.4% were moderately, 17.2% severely, and 9.8% were very-severely obese. Higher BMI was associated with a higher adjusted risk of HHF and AF/AFL (hazard ratio 1.30 and 1.28, respectively, per 5 kg/m2; P < 0.001 for all). Dapagliflozin reduced body weight by similar relative amounts consistently across BMI categories (percent difference: -1.9 to -2.4%). Although relative risk reductions in CV and renal-specific composite outcomes with dapagliflozin did not significantly differ across the range of BMI (P for interaction ≥0.20 for all outcomes), obese patients (BMI ≥ 30 kg/m2) tended to derive greater absolute risk reduction in HHF and AF/AFL (P for interaction 0.02 and 0.09, respectively) than non-obese patients. CONCLUSIONS : In DECLARE-TIMI 58, patients with T2DM and higher BMI were more likely to have HHF and AF/AFL. Whereas relative risk reductions in CV and renal outcomes with dapagliflozin were generally consistent across the range of BMI, absolute risk reduction in obesity-related outcomes including HHF and AF/AFL tended to be larger in obese patients with T2DM. Plasma apelin-12 levels may predict in-hospital major adverse cardiac events in ST-elevation myocardial infarction and the relationship between apelin-12 and the neutrophil/lymphocyte ratio in patients undergoing primary coronary intervention
PubMedID: 27770057Objective: We aimed to investigate the compliance of plasma apelin-12 levels to show angiographic properties and hospital MACE in patients with ST-elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI). Material and Methods: The association of apelin-12 levels with the N/L ratio on admission was assessed in 170 consecutive patients with primary STEMI undergoing primary PCI. All patient SYNTAX scores and thrombolysis in myocardial infarction (TIMI) flow grades were also assessed. Patients were divided into two groups according to their TIMI flow grade. Patients with a TIMI 0-2 flow and TIMI 3 flow with grade 0/1 myocardial blush grade (MBG) score were defined as the no-reflow group and patients with TIMI grade 3 flow with ?2 MBG were considered as the normal flow group. Results: Baseline apelin-12 levels were significantly lower in the no-reflow group than in the normal flow group (3.3±1.81 vs 6.2±1.74, p<0.001). In-hospital events, including death, myocardial infarction (MI) and re-infarction were significantly higher in patients in the no-reflow group than normal flow group (23% vs 7%, p<0.001). Apelin-12 level was negative correlated with the N/L ratio (r= -0.352, p<0.001), Hs-Crp (r=-0.272, p=0.01) and SYNTAX score (r= -0.246, p=0.029). In the multivariate regression analysis, apelin-12, presence of no-reflow and the SYNTAX score were independent predictors of in-hospital MACE (odds ratio [OR] 1.41, 95% confidence interval (CI) [1.27 to 1.67], p=0.001 for apelin-12, OR 1.085, [0.981 to 1.203], p<0.001 for no-reflow and OR 0.201, 95% CI [0.05 to 0.47], p= 0.004 for SYNTAX score). Conclusion: We have shown that lower apelin-12 level on admission is associated with higher SYNTAX scores and no-reflow phenomenon and may be used as a prognostic marker for hospital MACE in patients with STEMI. © The Author(s) 2016

Voucher s pecimens (MLP-He- 6803) were deposited in the Helminthological Collection of the Museo de La Plata (HCMLP), La Plata, Argentina. Redescription. Based on 20 specimens. Body robust, 611 (480–754; n = 20) long; greatest width 196 (137–277) usually at middle of body. Two pairs of cephalic lobes, 1 lateral and 1 anterior pair divided into 3 lobules. Five pairs of head organs and cephalic glands present. Four eyespots, frequently dissociated, accessory granules often present in cephalic region and anterior to pharynx. Pharynx ovate, muscular 43 (33–48) in greatest width; esophagus short to moderately long. Haptor sub-hexagonal, 85 (71–89) long, 113 (100–132) wide. Ventral anchor 40 (31–44) long, base 26 (20–28) wide; with elongate superficial root, short deep root, straight shaft and curved distal point. Dorsal anchor 36 (27–42) long; base 22 (16–23) wide; with well-developed roots, straight shaft and curved distal point. Ventral bar 36 (30–46) long, with medial suture with expanded ends. Dorsal bar widely Ushaped, 40 (37–49) long, with slightly enlarged ends. Vitellaria moderately dense throughout trunk, except in regions of reproductive organs; oviduct, ootype and uterus not observed.Vaginal aperture strongly sclerotized and sinistral; vagina a long and tortuous sclerotized tube, directed posteriorly and convoluted before connecting with seminal receptacle. Seminal receptacle, medial and anterior to germarium. Eggs not observed. Remarks. A detailed morphological examination of the specimens newly collected from O. jenynsii from both, the type locality and Nahuel Rucá Lake, and their comparison with the descriptions provided by Suriano (1981; 1997) showed that all are members of the same taxon by having identical morphology of MCO, accessory piece, hooks and internal organs. It is noteworthy that no type material is available for this species, which was originally described as Androspira chascomusensis by Suriano (1981) and later transferred to Palombitrema chascomusense by the same author (Suriano 1997). However, the morphology of the MCO and hooks does not agree with the diagnosis of Palombitrema Price & Bussing, 1968 to which the species was assigned by Suriano (1997). Palombitrema is characterized by having the MCO directly articulated to the accessory piece, pairs 6 and 7 of hooks different in shape than hooks 1–5, being the pair 7 also considerably larger, and dorsal anchors with little developed roots and larger than ventral anchors. (Price & Bussing 1968; Kritsky & Leiby 1972; Kritsky & Thatcher 1974; Mendoza Franco et al. 1999; 2003; 2009).In contrast, in our specimens, the MCO is articulated by a ligament to accessory piece, hooks are of similar shape and length and anchors have developed roots. These characteristics that clearly differentiate Palombitrema from Characithecium, were not considered in the resurrection of Palombitrema by Suriano (1997), indicating that the inclusion of specimens from C. voga and O. jenynsi in this genus was erroneous. Consequently, this species is transferred to Characithecium as C. chascomusensis n. comb. Characithecium chascomusensis n. comb. can be readily differentiated from C. costaricensis, the only member of the genus, by the morphology of the accessory piece, the number of coils in the MCO (3–4 in C. chascomusensis n. comb. vs ½ – 1 in C. costaricensis), the position of vaginal aperture (lateral in C. chascomusensis vs midventral in C. costaricensis) and the absence of a posterior ventral projection in the ventral bar as present in C. costaricensis. Low-cost thin film patch antennas and antenna arrays with various background wall materials for indoor wireless communications

Single antenna and four-element antenna arrays were characterized, which were attached to a subminiature version A connector via an innovative solderless, 3D-printed ‘plug-and-play-type’ tightener. Furthermore, indoor wireless communication and Internet of Things scenarios with commonly used wall materials including gypsum and plywood boards, on which patch antennas and antenna arrays can be attached, were also presented. In order to validate the concept, design and fabrication iterations in parallel with numerical and experimental investigations were executed. To elaborate, single antenna and antenna array configurations without and with wall materials were characterized to see their functionality at 2.4 GHz resonance frequency and beyond 300 MHz bandwidth, respectively. The results demonstrate that the investigated configurations fulfill short-range radio transmission and can be utilized, e.g., for indoor backscattering-type communications and wireless sensing applications, as an affordable and versatile alternative to their conventional counterparts. Being attached to their corresponding background materials, single-antenna specimens were measured to have return losses beyond 18 dB and peak gains around 1 dBi, while higher peak gains above 6 dBi were detected for antenna arrays. Moreover, the antenna arrays can enable multiple-input and multiple-output communication. The proposed arrays had diversity performance in terms of return losses higher than 15 dB, isolation of more than 20 dB, envelope correlation coefficient 9.95 dB, mean effective gain < − 3 dB, power ratio factor < 0.5 dB, and channel capacity loss < 0.4 bits/s/Hz.Peer reviewe

Abstract Vlhorf claims that any thought process takes place within the boundaries of a language, that each language fleshes out a worldview and that the world views structured by languages or, at least, the language families 'differ greatly. Whorf shrouds his view on world views in a metaphysical mystery which makes up the gist of his epistemic paradigm of cultural relativism. The paper sets out to discuss some of the tenets and conclusions of Whorfs views, unveiling their traces into the contemporary linguistics and philosophy and expounds Whorf's staunch relativistic creed, reviews the arguments against his position and appraises the arguments for a soft version relativistic version of the theory. We call aftermath the array of positions taken up by those who prove and disprove linguistic relativism alike. The author of the paper sets forth his own soft version of linguistic relativism called perspectivism, which permeates the entire linguistic field and is tightly linked up with cognition and cognitive paradigm approaches
Low-dose anticoagulation with rivaroxaban in the long-term prophylaxis of cardiovascular complications in patients with acute coronary syndrome
The ongoing search of optimal antithrombotic drug maintenance of patients with acute coronary syndrome [ACS] after performing intracoronary interventions is an obvious trend of modern cardiology. In the publication, the opportunities of new oral anticoagulants in long-term prophylaxis of cardiovascular complications after ACS are shown from the standpoint of evidence-based cardiology. The results of randomized study ATLAS ACS 2-TIMI 51, which evaluated the effectiveness and safety of low-dose anticoagulation with rivaroxaban for patients with ACS in addition to dual antiplatelet therapy, are studied in details. The author makes the conclusion that with suitable selection of “candidates" the low-dose anticoagulation therapy with rivaroxaban can represent a new strategy in treating patients with recent ACS.Продолжающийся поиск оптимального лекарственного антитромботического сопровождения больных острым коронарным синдромом [ОКС] после выполнения интракоронарных вмешательств - очевидный тренд современной кардиологии. В публикации с позиций доказательной кардиологии освещаются возможности новых оральных антикоагулянтов в долгосрочной профилактике сердечно-сосудистых осложнений после перенесенного ОКС. Детально рассматриваются результаты рандомизированного исследования ATLAS ACS 2-TIMI 51, оценивавшего эффективность и безопасность низкодозовой антикоагуляции ривароксабаном у больных ОКС в дополнение к двойной антитромбоцитарной терапии. Автором делается заключение, что при условии адекватного отбора «кандидатов» низкодозовая антикоагулянтная терапия ривароксабаном может представлять новую стратегию лечения больных с недавно перенесенным ОКС
