    Presepsin Sebagai Parameter Diagnosis Sepsis Pada SIRS Anak

    Latar Belakang. Sepsis merupakan salah satu penyebab utama mortalitas dan morbiditas pada anak di negara industri dan negara berkembang. Keadaan ini sering dijumpai pada unit perawatan intensif anak. Meskipun menunjukkan perbaikan yang signifikan dalam beberapa dekade terakhir, sepsis masih merupakan penyebab utama kematian pada anak. Diagnosis dini sepsis sangat penting agar tata laksana bisa diberikan segera sehingga dapat mengurangi angka kematian akibat sepsis. Kultur darah untuk diagnosis definitif sepsis memerlukan waktu yang lama sehingga dibutuhkan suatu marker yang bisa menentukan diagnosis sepsis secara dini. Tujuan Penelitian. Mengetahui sensitivitas, spesifisitas, nilai prediksi positif dan nilai prediksi negatif presepsin untuk diagnosis sepsis Metode. Penelitian ini merupakan suatu penelitian potong lintang (cross sectional) yang dilakukan di IGD dan Bangsal Anak RS Dr. M. Djamil Padang dari september-Desember 2018. Dilakukan pemeriksaan presepsin dan kultur bakteri darah dengan metode BacT/ALERT sebagai baku emas, kemudian dilakukan pengamatan klinis terhadap sampel penelitian saat masuk sampai pulang dari rumah sakit. Analisis statistik dilakukan untuk mendapatkan nilai cut off point presepsin. Hasil Penelitian. Pada penelitian didapatkan jenis kelamin laki-laki lebih banyak daripada perempuan. Usia rerata pada pnelitian ini 3,11 tahun. Rerata kadar presepsin lebih tinggi pada kultur bakteri darah yang positif (p=0,001). Cut-off point presepsin pada penelitian ini 2222,5 pq/ml dengan sensitivitas 76,1%, spesifisitas 74,3%, nilai prediksi positif 61,5% dan nilai prediksi negatif 85,2%. Nilai area under curve (AUC) pada penelitian ini 87,6%. Kesimpulan. Presepsin dapat digunakan sebagai parameter diagnostik pada sepsis. Kata Kunci presepsin, kultur bakteri darah, SIRS, sepsis

Early Childhood Education itself is an effort to provide guidance to children from the time they are born until they are six years old. This study aims to produce a song that is valid and effective in introducing the types and functions of the five senses to early childhood as a means of introducing the theme of myself, the sub-theme of the five senses by developing five aspects of child development. This research is aresearch that uses the development method, starting with determining the theme, selecting song lyrics and tones, then making arrangements to make a beautiful song. Follow-up validation was also carried out by the author to meet the criteria for songs that are in accordance with the musical components and aspects of early childhood development. Ponco Indro's song has also been tested by the author on TK Group B students at the Sholeh Assakiinah Children's Islamic Boarding School, Mantingan, Ngawi, East Java. The results showed that a song entitled "Ponco Indro" is a song that can be used as a learning medium that has educational value: it introduces the types and functions of the five senses, contains messages of motivation to learn, obeys teachers, instills gratitude and affection for family. Besides that, it can also improve 5 aspects of early childhood development: Religious and Moral Values, Language, Cognitive, Art and Social Emotional. It is said that because this work has gone through a validation process from experts who provide confirmation and validation of the quality and validity of the work, the revision process is also carried out by the author according to the validator's directions. The song "Ponco Indro" The song "Ponco Indro" is an interesting song, this can be seen from the number of views on YouTube, namely 262 views on the first upload, and 965 views on the second upload, as well as getting 155 likes and 35 comments. Based on research conducted by the author in developing the Ponco Indro song for Kindergarten B group students at the Sholeh Assakiinah Children's Islamic Boarding School, Mantingan, Ngawi, East Java, it can be concluded that the Ponco Indro song is a valid song because it has been validated by experts, namely music experts and music experts. material. So that this song is effective, as seen from the results of observations of group B students at the Sholeh Assakiinah Children's Islamic Boarding School, Mantingan, Ngawi, East Java, who can easily recognize and memorize the types and functions of the five senses through this song.     Spontaneous Intestinal Perforation in Neonate

    Spontaneous Intestinal Perforation (SIP) terjadi pada 8,4 persen bayi dengan berat badan lahir sangat rendah. SIP ditandai dengan onset mendadak dan tidak ditemukannya tanda infeksi. Presentasi klinis SIP pada bayi bervariasi, dan etiologinya belum diketahui secara pasti. Konsekuensi klinis SIP dapat menimbulkan komplikasi yang cukup parah sehingga dokter harus menyadari kemungkinan penyebab dan gejala. Terapi utama SIP yaitu: drainase peritoneum dan laparotomi dengan reseksi. Dilaporkan kasus SIP pada by perempuan usia 2 hari, yang didiagnosis berdasarkan gejala klinis dan rontgen abdomen.     High-Dose Sucralfate as a Mucosal-Protective Cornerstone in the Organ-Sparing Management of Pediatric Grade 2A Corrosive Gastroduodenitis: A Case Report and Pathophysiological Review

    Background: Accidental corrosive ingestion is a formidable pediatric emergency that can lead to severe gastrointestinal injury and long-term sequelae. Sulfuric acid, a common agent, induces coagulative necrosis, primarily affecting the stomach. The optimal management for moderate-grade injuries (Zargar Grade 2A) is debated, with a focus on preventing stricture formation. This report details a case managed with an aggressive pharmacotherapeutic protocol centered on high-dose sucralfate. Case presentation: A 2-year-10-month-old boy was admitted following accidental ingestion of battery acid. His presentation was atypical, with vomiting but no oropharyngeal lesions. Initial investigations revealed a significant systemic inflammatory response (leukocytosis: 19,220/mm³; thrombocytosis: 581,000/mm³) and aspiration pneumonitis. Despite a 12-day delay in endoscopy due to parental refusal, an aggressive conservative regimen was initiated upon admission. This protocol included high-dose, frequent-interval sucralfate (80 mg/kg every 2 hours), intensive intravenous acid suppression (omeprazole and ranitidine), and prophylactic antibiotics. The endoscopy on day 12 confirmed Zargar Grade 2A burns in the gastric fundus, pylorus, and proximal duodenum, with the esophagus spared. The patient improved rapidly, tolerated an oral diet by day 11, and was discharged on day 14. Conclusion: Follow-up endoscopy at 6 weeks and 6 months confirmed complete mucosal healing without any evidence of stricture or gastric outlet obstruction. This case suggests that an immediate, aggressive, non-surgical protocol featuring high-dose sucralfate can be effective in managing pediatric Grade 2A corrosive gastroduodenal burns, promoting complete healing and preventing long-term complications. The findings underscore the potential of this pharmacotherapeutic strategy and warrant further investigation

    Abstrak Latar Belakang: Pneumotoraks merupakan kelainan pada paru yang ditandai dengan terdapatnya udara pada rongga pleura. Terdapat beberapa jenis pneumotoraks salah satunya Pneumotoraks Spontan Sekunder (PSS). Pneumotoraks Spontan Sekunder (PSS) terjadi ketika penyakit dasar dengan target organ paru pada seseorang semakin memburuk. Penyakit paru tertinggi pada kasus PSS adalah Penyakit Paru Obstruktif Kronik (PPOK). Objektif: Mengetahui karakteristik pasien penderita pneumotoraks spontan sekunder yang disebabkan PPOK di Bangsal Paru RSUP Dr. M. Djamil Padang pada periode 2018 – 2021 berdasarkan usia, jenis kelamin, status merokok, keluhan utama, komorbid, dan sisi pneumotoraks pasien. Metode: Penelitian ini merupakan penelitian deskriptif retrospektif menggunakan rekam medis pasien pneumotoraks spontan yang disebabkan PPOK. Pengambilan sampel penelitian menggunakan teknik penelitian sensus sesuai kriteria inklusi. Hasil: Terdapat 26 pasien yang memenuhi kriteria inklusi dalam penelitian ini. Terdapat 50% pasien berada pada kelompok usia 55-64 tahun, 96,15% pasien berjenis kelamin laki-laki, 55,85% sebagai bekas perokok, 100% pasien dengan keluhan utama dispnea, komorbid pneumonia komuniti 34,09%, dan 53,85% pasien pneumotoraks sisi kiri. Kesimpulan: Kebanyakan pasien adalah kelompok usia 55-64 tahun, jenis kelamin laki-laki, merupakan bekas perokok, keluhan utama dispnea, komorbid pneumonia komuniti, dan mengenai sisi kiri dada.
