Reporte de un caso de obstrucción intestinal mecánica por fitobezoar
Bezoars are persistent deposits of indigestible material in the digestive tract, usually located in the stomach, although they can migrate to other parts of the intestine. Although most patients are asymptomatic, accumulation over time can produce symptoms associated with obstruction of the digestive tract, ranging from partial to total occlusion. We present the case of a 58-year-old male patient, with a history of severe mental retardation, who presents in a state of severe dehydration due to abundant dark vomiting and diarrheal stools and with a slightly distended abdomen. Physical examination reveals hypotension, tachycardia, dry mucous membranes and skin fold. A hydroelectrolytic imbalance, resulted from the clinical manifestations, and an acute gastric dilatation without a definite cause are suggested. 48 after being admitted to the intensive care unit and having corrected the hydroelectrolytic imbalance, he continues with great abdominal distention. Simple abdominal X-ray shows dilation of thin loops and air-fluid levels that persist despite the measures, which is why it is presented as a mechanical intestinal obstruction without being able to determine an apparent cause. An exploratory laparotomy is performed and mechanical intestinal obstruction caused by phytobezoar is found in the small intestine. Enterotomy and extraction is performed. The patients progresses favorably and is discharged 5 days after surgery without complications.Los bezoares son depósitos persistentes de material no digerible en el tubo digestivo, habitualmente localizados en el estómago, aunque pueden migrar a otras partes del intestino. Si bien la mayoría de los pacientes son asintomáticos, la acumulación en el tiempo puede producir síntomas asociados a la obstrucción del tracto digestivo, que va desde la oclusión parcial a la total. Se presenta el caso de un paciente de 58 años, masculino, con antecedentes de retraso mental severo, que acude en estado de deshidratación severa por presentar abundantes vómitos oscuros y deposiciones diarreicas, abdomen ligeramente distendido. Al examen físico hipotenso, taquicárdico, mucosas secas y pliegue cutáneo. Se le plantea un desequilibrio hidroelectrolítico producto al cuadro presentado y una dilatación gástrica aguda sin definir la causa. A las 48 horas de ingresado en sala de cuidados intensivos y corregido el desequilibrio hidroelectrolítico, continúa con gran distensión abdominal. La radiografía de abdomen simple muestra dilatación de asas delgadas y niveles hidroaéreos que persisten a pesar de las medidas, por lo que se anuncia como obstrucción intestinal mecánica sin poder determinar causa aparente. Se le realiza laparotomía explora y se encuentra obstrucción intestinal mecánica secundaria a fitobezoar a nivel de intestino delgado. Se realiza enterotomía y extracción del mismo. Evoluciona favorablemente, egresando sin complicaciones a los cinco días de operado
Reporte de un caso de lipomatosis pélvica
Pelvic lipomatosis is a rare condition defined as a non-malignant excessive growth of normal fat in the pelvis. Its first symptoms are usually atypical and some patients are not diagnosed until severe upper tract obstruction or even kidney failure occurs. We present the case of a 48-year-old female patient with a history of obesity, who presents complaining of an increase in volume in the lower abdominal region, as well as of some urinary incontinence. On physical examination a non-painful and somewhat mobile tumor mass in the hypogastrium of about 10 cm in diameter was palpated. Investigations, electrocardiogram and simple chest x-ray are performed. A computerized axial tomography of the hypogastrium shows a hypodense image, of fat density that displaces intestinal loops towards the periphery and the bladder and uterus downwards. An exploratory laparotomy is performed and a large, fatty-looking tumor mass is observed, occupying the entire hypogastrium, adhering to the serosa of the urinary bladder. Excision is performed, sending the specimen for histological study. It is concluded as a lipoma. The patient progresses favorably and is discharged the day after the operation without complications.La lipomatosis pélvica es una afección poco común, un crecimiento excesivo no maligno de grasa normal en la pelvis. Sus primeros síntomas suelen ser atípicos y algunos pacientes no son diagnosticados hasta que se produce una obstrucción grave del tracto superior o, incluso, una insuficiencia renal. Se presenta el caso de una paciente de 48 años, femenina, con antecedentes de ser obesa, que acude refiriendo aumento de volumen en la región inferior del abdomen, además de cierta incontinencia urinaria. Al examen físico se palpa una masa tumoral en hipogastrio de unos 10 cm de diámetro, no dolorosa y algo móvil. Se realizan exámenes complementarios, electrocardiograma y radiografía simple de tórax, estando todo normal. En la tomografía axial computarizada de hipogastrio se observa imagen hipodensa, de densidad grasa, que desplaza asas intestinales hacia la periferia y la vejiga y útero hacia abajo. Se le realiza laparotomía exploradora y se encuentra proceso tumoral grande de aspecto graso, que ocupa todo hipogastrio, adherida a la serosa de la vejiga urinaria. Se realiza exéresis de la misma, enviando la pieza a estudio histológico. Se concluye como lipoma. Evoluciona favorablemente y egresa al siguiente día de operada, sin complicaciones
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
